Substance P immunoreactivity in rat von Ebner's gland.

The present immunohistochemical study revealed substance P-immunoreactive neuronal elements in the von Ebner's gland of rats. Immunoreactive ganglion cells were observed as single cells or groups of several immunoreactive ganglion cells among intra-lingual muscles, at the base of the vallate papillae and near the von Ebner's gland. Very numerous substance P-immunoreactive varicose nerve fibres ran closely associated with the serous cells and excretory duct cells, and were seen to run along blood vessels in the gland. Since substance P-immunoreactive ganglion cells were present near the glands, the immunoreactive varicose nerve fibres in the von Ebner's gland may be partly derived from the intra-lingual ganglion cells. These substance P-immunoreactive varicose nerve fibres may have an effect on the secretory activity of the serous cells and duct cells, and on the vasodilation of blood vessels of the von Ebner's gland. Actin immunoreactivity was seen in numerous myoepithelial cells embracing serous cells and duct cells, and in the smooth muscle cells of blood vessels of the gland. By using a double immunolabelling technique with anti-substance P and anti-actin sera, substance P-immunoreactive varicose nerve fibres were found to be in close contact with myoepithelial cells.

The pathogenesis of decreased aspartate aminotransferase and alanine aminotransferase activity in the plasma of hemodialysis patients: the role of vitamin B6 deficiency.

The mechanisms responsible for the low plasma (p) AST and ALT activity in HD patients remain elusive. This prospective study was undertaken to clarify the relationship of AST, ALT and pyridoxal-5'-phosphate (PLP) levels (active form of vitamin B6) in these patients. A group of 52 HD patients were given oral pyridoxine HCl (30 mg daily), for 5 weeks. Prior to supplementation (Day 0), 17 (33%) patients had deficient pPLP levels (Group 1), the remainder (35 patients) had normal pPLP values (Group 2). A positive correlation was noted between pPLP and pAST (r = 0.57, p < 0.01) or pALT (r = 0.68, p < 0.01). The mean pAST (9.2 +/- 0.3 vs 13.4 +/- 0.7 U/l) and pALT (8.6 +/- 0.6 vs 11.4 +/- 0.9 U/l) were markedly lower in Group 1 than those in Group 2, respectively. These low AST and ALT levels increased to those seen in Group 2 where no change from basal values was seen despite vitamin B6 supplementation (Day 35). The administration of vitamin B6 resulted in a significant increase in pPLP in both groups (Day 35) but pPLP dropped to subnormal levels in 5 patients in Group 1 and 7 in Group 2 three months after supplementation was stopped (Day 125). The mean AST and ALT of these 12 pPLP deficient patients became lower than those of the remaining 40 patients with normal pPLP values (p < 0.05). In conclusion, low pAST and pALT levels in HD patients are in part due to a deficiency of pPLP which serves as a coenzyme for these transaminases.

[Desensitization immunotherapy on patients with mite-positive bronchial asthma using purified mite feces antigen fractions].

At present, we are performing desensitization immunotherapy on patients with mite-positive bronchial asthma using purified mite feces antigen fractions. We obtained the following results in 13 patients 4 to 12 months after the start of treatment. 1) The fraction showing the maximum reaction in the skin test was HM1-2 (molecular weight: 150-155 kD) in two patients, HM2 (30-40 kD) in nine patients and HM3 (10-20 kD) in two patients, and immunotherapy was performed using these fractions. None of the patients showed the maximum skin reactions or was treated with HM4 (less than 10 kD). 2) In nine patients with perennial asthma, the effects of treatment were excellent in four and moderate in three, while two cases remained unchanged. In four patients with seasonal asthma, the treatment was effective in all cases. 3) In patients in whom immunotherapy was effective, specific IgG antibody increased after the treatment, but it did not increase in the unchanged cases. Among the subclasses of specific IgG antibody increased by the treatment, no clear changes were seen in IgG4 antibody and no conclusion has been reached at present. 4) None of the patients had definite adverse reactions such as the occurrence of asthma attacks or anaphylaxis. These results suggested that purified mite feces antigens are safe and effective. We plan to continue this study with more patients in the future.

[A case of traumatic diaphragmatic hernia accompanied with rupture of pericardium].

We experienced a case of left traumatic diaphragamatic hernia accompanied with rupture of pericardium after blunt trauma by traffic accident. A 55-year-old man who had suffered blunt chest and upper abdominal trauma because of smash of two cars when he had been turning to right in the corner was admitted to our hospital. On arrival at the hospital, the patient complained severe dyspnea that reduced by lying on his right. Another position couldn't be taken because of deterioration of dyspnea. Chest X-ray showed disappeared left diaphragmatic line and abnormal transparent mass that suggest alimentary canal in the left intrapleural space. Cardiac ultrasonography showed abnormal tubular mass close to the left side of mediastinum that deviated to right. The patient was diagnosed as traumatic diaphragmatic hernia and emergency operation was performed. Left diaphragma was lacerated sagitally toward its top for about 12 cm in length. Left pleuropericardium was seen to be lacerated longitudinally for about 8 cm in length in front of the phrenic nerve intraoperatively. Both lacerations were closed by direct suture. The patient discharged on the 37th postoperative day without phrenic nerve palsy. At making diagnosis of patient with chest and abdominal trauma, injure of diaphragma and/or pericardium also must be considered. If it is possible, operation must be performed without hesitation in order to get good prognosis.

Orthopaedic management of spinal metastases.

Spinal metastases result in severe spinal pain, neurologic deficits, or both. These symptoms usually are caused by spinal instability, in which conservative therapy can have no effect, and surgical treatment is required to restabilize the destroyed spinal segments. Surgical indications are instability of the spine, pain and/or paresis resistant to radiation therapy, acute progressing paresis, and unknown histologic diagnosis. There are 2 surgical approaches for vertebral metastases: prosthetic replacement and posterior stabilization. Single or 2 consecutive diseased vertebrae should be treated with replacement surgery. In this series, excellent surgical outcome was attained with this procedure, and surgical benefit was maintained until the terminal stage of each patient. Multiple vertebral metastases are treated with posterior stabilization using various instrumentation systems that provide rigid stabilization. To choose the most appropriate procedure for each patient, the local condition of the lesion and general status of the patient, including prediction of life expectancy, should be evaluated fully. Spinal metastases develop early and are not terminal events. Therefore, not only palliative treatment but also surgical intervention should be considered for spinal metastases when indicated.

[Prediction of postoperative EEG changes for intractable epilepsy through a multidimensional autoregressive analysis].

Preoperative EEGs were quantitatively analyzed by means of a multidimensional autoregressive model (AR model) in order to predict postoperative EEGs. Recorded preoperative EEGs were digitized at an interval of 10 msec. The AR model fitting was executed on each digitized data. As this model described a multichannel feedback system having a peculiar activity in each site under observation, the interstructural relations could be described distinctly in the direction. Namely, the independent and the projected activities through the feedback circuits could be separately described for each brain site. Therefore, postoperative EEG could be simulated by elimination of the component in the AR model corresponding to each operative region. In this report, we presented three cases and discussed usefulness of this method. Case 1 was a patient with post-traumatic epilepsy, who was treated with focal resection. Preoperative EEGs revealed spike and wave discharges mainly in the right frontal region. Simulated postoperative EEGs based on elimination of component from the right frontal region in the AR model, corresponding to focal resection, revealed disappearance of spike and wave discharges. These findings were quite similar to observed postoperative EEGs. Case 2 was a patient with posttraumatic epilepsy, who was treated with anterior callosotomy. Preoperative EEGs revealed diffuse multifocal slow spike and wave discharges. Simulated postoperative EEGs, based on elimination of interhemispheric feedback pathways in the AR model, corresponding to anterior callosotomy, revealed marked lateralization of diffuse spike and wave discharges to the left hemisphere. These findings were qualitatively similar to observed postoperative EEGs. These results suggested that postoperative EEG changes could be well predicted by multidimensional autoregressive analysis.(ABSTRACT TRUNCATED AT 250 WORDS)

High-molecular-size mite allergens with high carbohydrate content in mite extract containing mite feces of Dermatophagoides farinae.

A series of mite allergens with high carbohydrate content was fractionated from mite extract containing mite feces by ultrafiltration, and successive chromatography on Ultrogel AcA 54, Sepharose CL-6B, and DEAE Toyopearl 650S. The final fractions isolated were termed HM1-2-Eff, -0.1A, -0.1B, -0.3A, -0.3B, and -1M according to the eluted salt concentration. These six fractions were stained weakly with Coomassie Brilliant Blue but strongly with periodate-Schiff's reagent on SDS-PAGE. Their molecular weight were estimated to be 150-155 KD on Sepharose CL-6B. All of these six fractions provoked strong erythema in skin of mite-sensitive and asthmatic patients. HM1-2-Eff, -0.1A, -0.1B and -0.3B reacted with 73% of mite-sensitive patients' IgE by the nitrocellulose dot blotting. One ng/ml of 4 fractions (HM1-2-0.1A, 0.3A, 0.3B and -1M) released histamine from mite allergic patient's blood cells. HM1-2-0.3A induced proliferation of T cell from immunized mouse by mite extract containing mite feces (Dff).

Analysis of pedestrian injuries in traffic accidents (2)--Especially on leg injury.

A deep case study on 118 pedestrians struck to fronts of vehicles or vehicle derivatives was carried out. In this paper we investigated a leg injury which occurs the most frequently in a vehicle-pedestrian collision. The data obtained from the case study were compared with those of autopsied cases. Influence of impact speed and point, and of an accident pattern were considered. The leg injury showed higher AIS (Abbreviated Injury Scale) than that of the arm injury caused by direct impact against the front exterior of vehicles. The impact speed and ISS (Injury Severity Score) showed strong correlation, but the former and AIS did not show significant correlation. The higher impact speed and the run-over are likely to cause a bilateral leg fracture.

[The catecholamine concentrations of collected autologous blood during adrenalectomy for pheochromocytoma].

We studied the catecholamine concentrations in collected autologous blood of a patient undergoing adrenalectomy for pheochromocytoma. In the preoperative laboratory data, plasma concentrations (normal ranges) of epinephrine, norepinephrine and dopamine were 60180 pg.ml-1 (< 100), 11090 pg.ml-1 (100-450) and 104 pg.ml-1 (< 20), respectively. The catecholamine levels of collected blood were epinephrine 2490000 pg.ml-1, norepinephrine 352300 pg.ml-1 and dopamine 6100 pg.ml-1 before wash. Wash of collected blood with 1000 ml saline diluted the catecholamines to epinephrine 212000 pg.ml-1, norepinephrine 18700 pg.ml-1 and dopamine 4900 pg.ml-1. Platelet activation by contact with tissue collagen or thrombin results in the release of catecholamine concentrated in the dense body. The mechanical stimulation by suction, roller pump and centrifugation during blood collection may accelerate the catecholamine release from platelets. Thus, saline wash hardly reduces catecholamine concentrations of collected blood from a patient with pheochromocytoma. In this particular case, plasma catecholamines seem to exceed the potential capacity of platelets in amount. The dilution effect for epinephrine and norepinephrine probably reflects the washout of greater amount of plasma catecholamines. However, saline wash was unable to reduce catecholamines contained in the collected blood to a safe level, and hypertension following autotransfusion was predictable. We conclude that hemodynamic change should be monitored carefully during intraoperative autotransfusion in a case of pheochromocytoma.

[Mechanism on formation of new ipsilateral corticospinal tract following neonatal unilateral cortical ablation in rats].

The corticospinal tract in the rat after neonatal ablation of the unilateral cerebral cortex was studied morphologically using the antegrade horse-radish peroxidase (HRP) tracing method. An aberrant ipsilateral tract was observed 7 days after the operation. Formation of the aberrant neuronal pathway has been confirmed to be contributed mainly by new axons, which ramified at the level of the pyramidal decussation from healthy corticospinal fibers. This ramified axon ran toward the ipsilateral dorsal funiculus. A few fibers also contributed to the aberrant pathway by changing their direction on the way of extension at the level of the pyramidal decussation. These results indicate that the ramification and change of the direction of extending axons play an important role for formation of a new ipsilateral corticospinal tract.

Morphological changes of myoepithelial cells of mouse lacrimal glands during postnatal development.

To reveal the correlation between myoepithelial cell configuration and size/shape of glandular endpieces (acini), we observed postnatal developmental changes of myoepithelial cells in the lacrimal glands of mice. Glandular and myoepithelial cells were examined in paraffin sections and in isolated acini. In newborns, rudiments of acini showed no clear lumina and glandular cells had few secretory granules. There were no myoepithelial cells with actin. At 3 days after birth, some rudiments showed lumina; however, secretory granules were not salient. Round cells in the periphery of the acini showed immunoreactivity for actin. In 1-week-old mice, glandular cells were polarized: luminal cytoplasm contained some secretory granules, and nuclei were located basally. Most myoepithelial cells were flattened and sometimes projected thin processes in various directions. At 2 weeks, the glandular cells increased their size and contained numerous secretory granules, and the myoepithelial cells were almost stellate. In 4-8 week-old mice, acini increased their size, and myoepithelial cells were very thin and processes were prolonged in length and increased in number. During postnatal development, their distribution of myoepithelial cells was more scarce, while the size of acini increased. This reciprocal relation of myoepithelial cell distribution and acinar size may indicate that the changes of myoepithelial cell configurations depend on the change of acinus size.

A visual telemedicine system: the integration of ordinary TV and HDTV still image transmission.

This paper describes a project to develop and test a new telemedicine system using image transmission. The system was implemented at Aomori prefecture, the northernmost prefecture of Japan, to provide medical consultation to remote hospitals from a medical center located in Aomori city, capital of the prefecture. The characteristics of the system are the combination of an HDTV still image transmission system and an ordinary tele-conference system using 65 kilobit telephone line i.e., an ordinary telephone line available in almost any part of Japan. We also developed a disk storage system that stores transmitted images automatically without human intervention. The system was assessed in real clinical settings and has proved to be effective in various areas of medical consultation, including radiology, pathology, dermatology, etc. We hope that this system becomes a standard telemedicine system in the future.

Beta 2-microglobulin has a strong affinity to bone marrow and synovium: an experimental study.

This experimental study was undertaken to clarify whether beta 2-microglobulin (B2M) deposits are the cause or merely the consequence of arthritis or osseous changes in chronic renal failure. Twenty-one heminephrectomized, male inbred DBA/1J mice were divided into 3 groups of 6 animals and a group of 3 mice. Collagen arthritis was produced by the intradermal injection of an emulsion of bovine collagen type II and complete Freund's adjuvant in groups 1, 2 and 4. Human urine derived B2M, 100 micrograms and 300 micrograms was then given subcutaneously to group 1 for up to 10 days and also to the 6 mice in group 3 without arthritis. Three mice in group 4 received horse myoglobin 100 micrograms daily for 1, 3 and 5 days as a control. Regardless of the presence of arthritis or osseous changes, B2M was deposited predominantly in the bone marrow and synovium of the B2M treated mice in groups 1 and 3. On the other hand, group 2 which had not been given B2M had no deposits. Deposits of myoglobin were not seen in group 4 mice. These findings suggest that B2M is deposited in the bone marrow and synovium as a primary event.

Contamination of soil with parasite eggs in Surabaya, Indonesia.

Soil was examined for contamination by parasite eggs in Surabaya Indonesia. Surveys were carried out on three occassion; July, 1993 (dry season), March, 1994 (rainy season), and August, 1994 (dry season). Throughout the study, five species of nematode eggs (Ascaris lumbricoides, Toxocara cati, Trichuris trichiura, Physaloptera sp, Capillaria sp), two species of cestode eggs (Hymenolepis diminuta, Spirometra erinacei), and one species of protozoa oocyst (Isospora felis) were detected. The contamination rate and number of species found from the soil were significantly different in the dry and rainy seasons. In the dry season, the prevalence was 8-20%, with two to four species detected. During the rainy season, this rate was 83% with eight species, suggesting parasite infection to possibly occur mainly in this season. The reason for this seasonal difference may be that, in spite of constant temperature around 27 to 29 degrees C throughout the year, rainfall in the dry season in only a few percent of that of the rainy season. We concluded that parasite eggs die during the dry season owing to dryness of the soil. Contamination of soil with parasite eggs and the number of species found were greater in alley-ways and at communal water supply sites around residential areas than in open-air parks or sandy beaches. The method used in the present study proved extremely effective for ascertaining the actual dynamics of parasite infection in a certain region.

Esophageal cancer treated with radiotherapy: impact of total treatment time and fractionation.

PURPOSE: Local control rate and survival rate of esophageal cancer treated with radical radiation therapy (RT) were analyzed with special respect to total treatment time and fractionation. METHODS AND MATERIALS: Between 1979 and 1992, 88 patients with Stages I-III esophageal cancer were treated radically with RT at Kyoto University Hospital and Wakayama Red Cross Hospital. Of the 88 patients, 52 patients were treated with conventional fractionation (1.7-2.0 Gy/day, five times/week), and the remaining 36 patients were treated with accelerated hyperfractionation (AHF). In 1989, we started AHF regimen for esophageal cancer. Daily fractionations were 2.0 Gy and 1.2 Gy (field-in-field), or 1.5 Gy and 1.5 Gy at 5- to 6-h interval. Most of the patients treated with AHF received the total radiation dose of 64-68 Gy. Twenty-seven patients were treated with intraluminal brachytherapy (IBT) as boost therapy following external RT. Fourteen patients were treated with IBT following AHF. RESULTS: The median of treatment time of AHF was approximately 2 weeks shorter than that of conventional fractionation. Local control rate at 1 year were 47% for AHF, which was significantly higher than that for conventional fractionation (22%, p < 0.05). The improvement of local control by AHF was responsible for a trend to an improved cause-specific survival (p = 0.07). Local control rates at 1 year were plotted as a function of total treatment time. The slope of the linear regression line was -2.3 +/- 0.5% per day (p < 0.025) for patients treated with external RT alone, indicating a 2.3% per day loss in local control. Pretreatment and treatment parameters were evaluated in a multivariate analysis for the end point of local control. T stage (T1, 2 vs. T3, 4; p = 0.003) and fractionation schedule (p = 0.03) were independent of prognostic significance. Patients could tolerate the AHF well, although esophageal stenosis was noted frequently as a late toxicity. CONCLUSION: Accelerated hyperfractionation was the most important treatment-related variable in this patient population. Total treatment time may have a significant impact on the treatment outcome for esophageal cancer.

Three-dimensional treatment planning for maxillary cancer using a CT simulator.

PURPOSE: The results of three-dimensional treatment planning using a computed tomography simulator were evaluated in patients with maxillary cancer. METHODS AND MATERIALS: Treatment planning was done in 25 patients using an x-ray simulator and plain x-ray films (1979-1982, group 1) in 34 patients using an x-ray simulator and computed tomography films (1983-1987, group 2), in 24 patients using a computed tomography simulator (1988-1992, group 3). The number of patients with Stage IV disease increased in the order of group 1 to group 3. RESULTS: The average radiation field was smallest in group 3 (66.5 cm2) followed by group 2 (67.4 cm2) and group 1 (72.9 cm2). A radiation dose of more than 30 Gy to the lens of the effected side was delivered to 13% of group 3, 44% of group 2, and 44% of group 1. The dose to the lens on the uneffected side was zero in 56% of group 1, 74% of group 2, and 96% of group 3. A long-term decrease in visual activity on the effected side occurred in 11% of group 3, 32% of group 2, and 44% of group 1. However, a significant increase in survival was only noted between groups 1 and 2, because the three population of patients were different. CONCLUSION: The three-dimensional treatment planning results in a better treatment than two-dimensional treatment planning as measured by complication rates and field sizes.
