Further application of VM-plasty.

The use of VM-plasty in the repair of web contracture and its application for other regions are presented. We refined VM-plasty so that both ends remain in the "dog ear" form, eschewing the conventional excision and instead making flaps that are set in a zigzag shape. This procedure results in an increase in the effect of extension to the lateral direction, in addition to preventing recontracture. This method is used not only to repair finger web contracture but also for other contracted scars. No recontracture has been observed, and good results have been obtained.

The ulnar recurrent fasciocutaneous island flap: reverse medial arm flap.

A new island fasciocutaneous flap raised on the inner medial surface of the upper arm has been used for reconstruction of soft-tissue defects of the elbow. The blood supply to this flap comes from the fasciocutaneous perforators of the ulnar recurrent vessels. This unique vascular arrangement allows for safe transference of the upper medial skin to the elbow region. This flap has been used to cover nine defects in eight patients, and results have been good. Except for one case of sensory disturbance, there were no complications or loss of overlying skin. It is a relatively quick and simple procedure involving only one stage that adequately corrects the skin defect around the elbow region and does not require prolonged splinting.

[Postoperative complications of abdominal aortic aneurysm--with special reference to postoperative renal hypofunction].

Effects of various factors before and during operation on postoperative renal hypofunction were studied in 34 preoperatively evaluated surgical cases of non-ruptured abdominal aneurysm. The cases were divided into two groups, with and without postoperative renal hypofunction. The two groups showed no differences with respect to age, sex, preoperative serum Crn or Ccr level, presence or absence of treatment with antihypertensives or diabetes mellitus, duration of intraoperative aortic clamping, or the volume of blood loss or transfusion. However, the group with postoperative renal hypofunction included many cases of preoperative hypertension. The incidence of postoperative renal hypofunction was high among cases which exhibited preoperative hypertension, but was low among those with preoperative normal blood pressure. Further, renal hypofunction occurred in few of the patients whose blood pressure had been controlled by the use of antihypertensives. The possibility was suggested that in palliative operations, preoperative blood pressure control, was useful for the prevention of postoperative renal hypofunction.

Cutaneous and fascial vasculature of the leg: anatomic study of fasciocutaneous vessels.

In order to understand the fasciocutaneous circulation of the leg more clearly, the vasculature of the skin and deep fascia was investigated. We mapped the vascular plexus of the deep fascia and found that the vessels and fascial plexus have their own axes. The vasculature of the deep fascia was categorized into six types. This classification should help in the planning of various fasciocutaneous flaps of the leg.

Cutaneous and fascial vasculature around the rectus abdominis muscle: anatomic basis of abdominal fasciocutaneous flaps.

Macroscopic and radiographic observations of the area around the rectus abdominis muscle were made by cadaver dissection to confirm the vascular patterns of the superior and inferior epigastric arteries. The superior epigastric artery was observed to penetrate the posterior sheath at the middle of the xiphoid process and the anterior sheath at the middle of the upper third of the upper abdomen. The inferior epigastric artery penetrates the posterior sheath near the middle of the lower abdomen. It also penetrates the anterior sheath in an area ranging from the upper third of the lower abdomen to the umbilicus. These arteries anastomose at the lower third of the upper abdomen. This information is useful for planning and implementing vertical fasciocutaneous flaps in the abdomen or for any other flaps considered in that area.

Free compound rib-latissimus dorsi osteomusculocutaneous flap in reconstruction of the leg.

The use of a free compound rib-latissimus dorsi osteomusculocutaneous flap is described, for reconstruction of the tibia and soft tissue of the leg. The entire flap depends on the thoracodorsal artery for its blood supply. Dye injection studies of the artery demonstrated continuity between it and the intercostal artery, which is the periosteal vessel of the rib, through a dense network of vascular communications. The longest follow-up reported is three years and ten months. Follow-up showed that the use of this compound flap produced uniformly well-contoured soft tissue coverage and bony structure that thickened almost to the same dimensions as the tibia. The transferred tissue was examined by x-ray and bone scintigraphy, and was found to be viable.

Blood pressure and sympathetic nervous function in spontaneously hypertensive rats derived from breeders on low sodium diet.

We investigated the effect of dietary salt restriction in spontaneously hypertensive rat (SHR) breeders to determine if the development of hypertension of offspring would be blunted. Weaning SHRs (F-0) were divided into 3 groups and were given a diets containing sodium of 0.4% (G 1), 0.05% (G 2), or 0.4% plus mefruside 0.001% (G 3) with distilled water. Potassium content was 0.75% and the other ingredients except chloride were identical in all the diets. Offsprings (F-1) were derived from selective inbreeding within each group. Systolic blood pressure rose over 160 mmHg by 8 weeks of age in all the male rats of all the groups in both F-0 and F-1. There was no difference in blood pressure between G 1 and G 2 at any age, while in G 3 blood pressure was significantly lower than in G 1 at 12 and 20 weeks of age in F-1. The findings in pressure were ascertained by heart-body weight ratio determined at autopsy, which was similar between G 1 and G 2, and was smaller in G 3 than the others at 20 weeks of age. Aldostrone excretion rate was markedly higher in G 2 than G 1 and G 3 at all study points, whereas plasma renin content was similar between G 1 and G 2, and higher in G 3. Plasma concentrations of noradrenaline and adrenaline did not differ among the 3 groups at any age in both F-0 and F-1.(ABSTRACT TRUNCATED AT 250 WORDS)

Reconstructing chest walls with vertical abdominal fasciocutaneous flaps.

Our experience with the vertical fasciocutaneous flap in the reconstruction of chest wall defects is described. The vascular supply of this vertical abdominal fasciocutaneous flap comes from perforating vessels stemming from superior epigastric vessels. The flap can be elevated in the same way as a standard fasciocutaneous or vascular island flap with a small part of the rectus muscle and superior epigastric vessels attached. This type of flap was used in 16 patients with major anterior chest wall excisional defects. The average size of the flap was approximately 10 by 33 cm. Superficial necrosis was observed in 2 patients and distal 2 cm total skin loss was observed in one patient. All other flaps survived completely.

[Capacity for IgE antibody production in ASK mice].

The capacity for IgE anti body production in ASK mice, which are highly sensitivity to anaphylactic shock, was compared with that in C3H and AKR mice. Three strains of mice were immunized with DNP-Ascaris mixed with aluminum hydroxide gel. IgE antibody to DNP in the sera was titrated by the rat 48-hour passive cutaneous anaphylaxis test. Maximum IgE titers of each strain of mice were 1: 2560 in C3H, 1: 1280 in ASK and 1: 640 in AKR. IgE antibody was detected in the sera until 170 days in C3H, 290 days in ASK and for not less than 320 days in AKR. These results suggest that the ASK mouse is a high responder strain for IgE antibody.

[Effect of human recombinant beta-interferon on natural killer activity of peripheral lymphocytes in cancer patients].

After human recombinant beta-interferon (ReIFN-beta) was administered to 13 cancer patients intrathoracoperitoneally and by intravenous drip infusion every day or 3 times a week, the effect of peripheral lymphocytes on natural killer (NK) activity was studied. When ReIFN-beta was given every day or 3 times a week, the NK activity was notably enforced, and was maintained throughout the period of administration. NK activity was also enforced regardless of whether ReIFN-beta was given intravenously on intrathoracoperitoneally. By the administration of ReIFN-beta, pleural effusion and ascites in three patients were depleted, and in one patient, Douglas' cul-de-sac tumor disappeared. It was therefore suggested that ReIFN-beta was effective against the cavity of carcinomatosis.
