Growth and health status of preschool Karen highlanders.

The physical growth and health status of preschool (< 6 years) Karen highlanders belonging to the Pwo (212 males, 227 females) and Sgaw (107 males, 101 females) ethnic groups, and residing in northwest Thailand are described in the present report. The growth and health status of Thai tribal groups, including the Karen, is generally assumed to be substantially poorer than those of Thai lowlanders, who are primarily of Thai ancestry. However, the Karen highlanders in the present report fell within the range of variation in growth and health status found in lowland southeast Asians. Also, the growth and health status of Pwo preschool children tended to be significantly poorer than those of Sgaw children, despite the fact that both groups reside in isolated villages within the same ecozone and utilize similar food production techniques. The cause of these differences cannot be determined at the present time, but several potential factors are discussed.

Fine needle aspiration cytology of an intraductal papilloma originating in a sublingual gland. A case report.

BACKGROUND: Intraductal papilloma of the salivary gland is a very rare tumor; only eight cases have been reported to date. Only histopathology has been used to investigate this tumor to our knowledge. There has not been any reported case of this tumor in a sublingual gland. CASE: A 3-cm oral floor mass with a lanulalike lesion in a 72-year-old female was aspirated. The cytologic features included various cellular smears containing compact clusters of papillary, radial, palisading, trabecular, tubular and buddinglike projected patterns; monomorphic columnar cells with oval-to-spindle nuclei basally located; abundant, finely vacuolar cytoplasm; indistinct nucleoli; and no squamous differentiated cells. A sublingual glandectomy was performed. Cytology, histology, immunohistochemistry and electron microscopy were performed with standard methods. The intraductal papilloma seems to have arisen within an excretory duct and differentiated into acinar cells. CONCLUSION: The cytologic findings of intraductal papilloma are unique and may allow its specific diagnosis on fine needle aspiration.

Discoid lupus erythematosus developing in areas where fragments of windshield glass had become embedded in the skin.

We observed a 26-year-old female patient with discoid lupus erythematosus on her left cheek skin, where fine fragments of windshield glass had been embedded in an automobile accident 8 years previously. She gradually developed general fatigue, morning stiffness of fingers and anti-nuclear antibody. As an etio-pathogenesis of this patient, we speculate that a long exposure to quartz (silica) could give rise to discoid lupus erythematosus in only local damaged areas and at the same time induce systemic immunological changes in some genetically restricted persons; such as production of anti-nuclear antibody. Silica has multi-potential biological effects, especially on immunological functions.

Experience with photodynamic therapy (endoscopic laser therapy) for the treatment of early gastric cancer.

BACKGROUND/AIMS: Photodynamic therapy has been developed as an endoscopic laser therapy for gastrointestinal malignant tumors. The targets for curative upper gastrointestinal endoscopic therapy are carcinomas that are considered statistically unlikely to be accompanied with metastases to the lymph nodes. Endoscopic mucosal resection is the therapy of first choice for such carcinomas. In the application of photodynamic therapy, we narrow down its practical indications to patients who are not indicated for curative endoscopic treatment by preoperative examination or those with histologic findings of endoscopic mucosal resection specimens who reject surgical treatment or are at high risk in surgical treatment. METHODOLOGY: The effect of photodynamic therapy using Porfimer sodium and an Excimer dye laser was evaluated endoscopically in 8 lesions of 7 patients with early gastric cancer. RESULTS: Complete responses were obtained in all patients. As side effects, mild photosensitivity was seen in 6 patients and lasted for several months. CONCLUSIONS: Photodynamic therapy was safety employed, with success in 7 patients with early gastric cancer. We conclude that photodynamic therapy can be a useful palliative method with high tumor selectivity in the treatment of early gastric cancer.
