Plasma motilin levels in patients with abdominal surgery.

Plasma motilin levels were measured by radioimmunoassay both pre- and postoperatively in 37 patients who underwent abdominal surgery. In 8 colorectal cancer patients with tumor removal and in 13 stomach cancer patients with total gastrectomy by Roux-en-Y anastomosis or subtotal gastrectomy of Billroth I anastomosis, the motilin levels decreased before the onset of postoperative peristalsis, and at the time when peristaltic sounds could be detected stethoscopically, the plasma motilin levels had increased by 200% of the preoperative level. Similar motilin levels were maintained until the time of the initial postoperative flatus and/or stool. Thereafter, plasma motilin levels decreased and returned to almost the same levels as the preoperative ones. The postoperative time course of plasma motilin in 10 gallstone patients was very similar to that in the 21 gastrointestinal cancer patients above. In 5 stomach cancer patients with subtotal gastrectomy of Billroth II anastomosis, however, the peak at the time of the initial postoperative peristalsis was not as remarkable as that in the 13 stomach cancer patients.

[Sensitivity test of antitumor drugs by the use of isotope].

A sensitivity test of antitumor drugs using primary shortterm culture with 3H-thymidine was performed on stomach cancer patients. Stomach cancer tissues, which were removed in the operating room, were cut into cubes of 1 to 2 mm. Thereafter, they were put on a stainless steel mesh which was placed in a small petri dish (40 X 15 mm) and they were moisted with the cultivating medium. An appropriate dose of antitumor drugs (MM-C, 5-FU, cytosine arabinoside) was added at the onset of cultivation and 3H-thymidine was further added 24 hours later. At the end of cultivation in a CO2 incubator for about 3 days, the specimens were homogenized in an ice-cold homogenizer with 5% trichloroacetic acid. DNA fraction of the specimens was extracted by Schmidt-Thannhauser method. The isotope activity in DNA fraction was measured in a scintillation counter and was represented as cpm/microgram DNA. Sensitivities of antitumor drugs can be determined by comparison between isotope incorporations into the specimens tested and those into control specimens. Bacterial and fungal contaminations were observed in primary stomach cancer tissues.

Comparative efficacy of antitumor treatment for liver metastases from colorectal cancer.

A retrospective study of 37 colorectal cancer patients with synchronous liver metastases was made. Of these patents, 6 who had undergone primary tumor removal were treated with considerable success by hepatic arterial infusion of 5-FU and mitomycin C. Ten patients who underwent primary tumor excision were treated by oral chemotherapy using fluorinated pyrimidines. These patients survived about 23 months. On the other hand, 12 patients after primary tumor removal without cancer chemotherapy survived for about 10 months. Six patients without antitumor treatment for both primary tumor and hepatic metastasis survived about 5.2 months. The overall results of this study suggest that intrahepatic arterial infusion is of practical importance for hepatic metastases from colorectal cancer and that oral chemotherapy is indeed effective for selected patients.

Minute gastric cancers found by gastric mass surveys.

Minute gastric cancer is defined in Japan as a cancer lesion measuring not more than 1 cm in maximum diameter in a freshly resected gastric specimen. The macroscopic findings, histopathological findings and courses to discovery have been presented in 14 cases of minute gastric cancer discovered in gastric mass surveys. The cases of minute gastric cancer consisted of mostly elevated or depressed lesions and multiple cancers; they were histopathologically classified as well differentiated tubular adenocarcinoma, and by depth invasion as mucosal carcinoma. Because of the particularly limited physical dimensions of such early tumors, discovery and diagnosis are of course extremely difficult, and are heavily dependent upon endoscopic examinations and the accompanying investigation of biopsy materials.

Evaluation of gastric mass survey.

The results of gastric mass survey during the 11-year period 1968-1978 were evaluated. The detection rate of gastric cancer was relatively high (0.22%) and that of early gastric cancer was surprisingly good (0.10%). The five-year survival rate (67%) accordingly was much higher than in Japanese out-patients in general. Gastric mass survey therefore is useful for the early diagnosis of gastric cancer. Although some questions of a practical nature remain to be solved, improvement and extension of surveys seem ot promise a reduction in the death rate from gastric cancer.

Evaluation on the recurrent cases of early gastric carcinoma.

In the First Department of Surgery, Faculty of Medicine, Chiba University, 213 cases with early gastric carcinoma have undergone an excision from 1955 to 1975. The observed 5-year, relative 5-year, observed 10-year, and relative 10-year survival rates of these cases amount to 89.3%, 99.8% and 98.0% respectively. The present investigation concerns 13 of these patients with diagnosed postoperative recurrent cases. The post-operative gastric carcinima recurrence might be classified to metastasis to liver and lymphnodes, peritoneal dissemination, residual gastric recurrence and recurrence in miscellaneous organs. The 13 cases involved 2 cases of liver metastasis, 9 cases of residual gastric recurrence and 2 cases of recurrence in miscellaneous organs. Nine of these cases underwent re-operations which were grouped to 6 cases of residual gastrectomy, 2 cases of excision of miscellaneous organs (cecum and rectum), and 1 case exploratory laparotomy. The development in the cases is characterized by 3 cases of stump persistent carcinoma at the residual stomach due to stump carcinoma positive in excision, 2 cases of residual gastric carcinoma due to lymphatic vessel invasion and 6 cases of multiple carcinoma.
