[The use of 99mTc-dimercaptosuccinic acid renoscintigraphy in the evaluation of differential renal function].

We studied the total and differential renal function by 99mTc-dimercaptosuccinic acid (DMSA) renoscintigraphy and present a formula to estimate the renal depth for the Japanese and the attenuation coefficient which influenced renal uptake. Total renal uptake of 99mTc-DMSA correlated well with creatinine clearance and with the PSP test, and there was a close correlation between its relative uptake and relative function as determined by 99mTc-diethylenetriaminepentaacetic acid (DTPA) renography. Therefore differential renal function test with 99mTc-DMSA renoscintigraphy was found to have clinical utility. We also demonstrated 99mTc-DMSA renoscintigraphy provided useful morphological information.

[Clinical efficacy of aztreonam in pelvic peritonitis].

Aztreonam (AZT) was evaluated for its clinical efficacy in 6 patients with pelvic peritonitis. In all the cases, AZT was administered by intravenous injection, and the duration of treatment ranged from 4 to 12 days. Daily dose was 2 g in 4 cases, and 4 g in the remaining 2 cases. Surgical treatment was necessary in 2 cases, whereas in another 2 cases, either LMOX or ABPC was administered in addition to AZT. The clinical results was excellent or good in 5 out of 6 cases. Side effects possibly attributed to AZT were diarrhea in 2 cases and slight increase of GOT and GPT in 1 case, although they disappeared promptly afterward.

[Use of cefminox in infections in the field of obstetrics and gynecology].

A total of 15 cases of obstetrical and gynecological infections was treated with cefminox (CMNX, MT-141), a new cephamycin antibiotic, with following results. The subjects consisted of 3 cases of salpingitis, 2 cases of parametritis, 5 cases of endometritis, and 1 case each of puerperal fever, inflammation of the pelvic dead space, Bartholin's pyocele, vulvar abscess and suppurative mastitis. In 2 cases of endometritis, pelveoperitonitis and adnexitis were complicated, respectively. As a rule, CMNX was administered intravenously at a dosage of 1 g each twice a day by drip infusion route. The clinical results were rated as excellent in 8 cases, good in 6 cases and poor in 1 case, with an efficacy rate of 93.3%. No subjective or objective side effects were seen nor any abnormal laboratory test results were found.

[Clinical efficacy of cefpimizole in inflammatory diseases in the field of gynecology].

Clinical study to evaluate the usefulness of cefpimizole (AC-1370) was made in the 6 patients with female genital organ infections. Two grams or 4 g of AC-1370 was administered a day by intravenous drip infusion ranging from 5 to 18 days. Responses were excellent in 2 cases, good in 3 cases and poor in 1 case, parametritis after hysterectomy. The efficacy rate was 83%. Neither general side effect nor abnormal laboratory finding was observed. AC-1370 showed a satisfactory clinical efficacy in treatment of the infections in the field of gynecology, and it has been concluded that AC-1370 will be an useful antibiotic for these infections.

[The use of dimethyl sulfoxide in the treatment of intractable urinary frequency].

Intravesical instillation of dimethyl sulfoxide (DMSO) was used in the treatment of patients with intractable urinary frequency due to chronic prostatitis, chronic cystitis, tuberculous contracted bladder and interstitial cystitis. Before the application of this therapy, all 4 patients were examined carefully to rule out cases of acute infectious diseases of the urinary tract, active urinary tuberculosis, neurogenic bladder and carcinoma in situ of the bladder. Three of the four patients achieved an excellent response both subjectively and objectively. In the United States, intravesical instillation of DMSO had already been established as the specific method in the treatment of interstitial cystitis and no side effects have been reported so far. Therefore, we recommend the use of intravesical instillation of DMSO more commonly in various forms of intractable urinary frequency.

[Effect of Robaveron tablet (KN-7) in the long-term therapy for urinary disturbances].

Long-term therapeutic effect of Robaveron tablet (KN-7) was studied on 10 female patients of middle and old age with ptosis of urinary bladder and 9 patients with neurogenic bladder. The patients had mainly complained of such subjective symptoms as pollakisuria, difficulty of urination, sense of residual urine, lower abdominal discomfort and urinary incontinence. Robaveron tablet was administered at 2 tablets t.i.d. for 3-26 months. And the drug efficacy was evaluated by residual urine, cystometric findings and subjective symptoms. A significant decrease in the residual urine rate and a significant increase of pressure amplitude were obtained, and improvement of subjective symptoms was seen with an effective rate of about 70%. Overall effectiveness, rated slightly improved or better was 89.5%. No cases of side effects or abnormalities in laboratory tests were observed. Robaveron tablet is safe and effective for patients with urinary disturbance accompanied by ptosis of urinary bladder, as a myogenic disorder, as well as neurogenic bladder in the long-term therapy.

[Five-year clinical statistics on operations at our department].

A five-year statistical observation was made on operations and patients examined and treated surgically at our Department. Methods and numbers of operations are shown and discussed briefly. The total number of operations was 967, and the male/female ratio was 7.0:1. The most frequent operation was TUR-P (114, 11.8%) and the second was TUR-Bt (89, 2.2%).

Natural killer cell activity during pregnancy.

Natural killer cell activity was found to be depressed from the early period of pregnancy to the third trimester, as compared to that in the nonpregnant state. Natural killer cell activity in the puerperium tended to be depressed slightly more than during pregnancy. There were no differences in activity between toxemic and normal pregnant women in the same gestational week. A retrospective survey showed a negative correlation between natural killer cell activity during pregnancy and the birth weight of the baby. Although the role of natural killer cells during pregnancy needs to be more clearly elucidated, they may be involved in fetal growth.

Evaluation of left ventricular posterior wall movement after open mitral commissurotomy by echocardiogram: with reference to the effect of papilloplasty.

Left ventricular posterior wall movement in 20 patients with mitral stenosis (MS) was measured using M-mode echocardiogram in order to evaluate the improvement of myocardial function after open mitral commissurotomy (OMC) and compared between the cases with (10 patients) and without (10 patients) papilloplasty. The maximum left ventricular end-diastolic posterior wall velocity (LVPEVdmax) was increased from 71 +/- 12 to 90 +/- 16 mm/s in OMC patients (p less than 0.01) and from 59 +/- 19 to 101 +/- 28 mm/s in OMC + P patients (p less than 0.001). The maximum left ventricular systolic posterior wall velocity (LVPWVsmax) showed an increase from 51 +/- 9 to 62 +/- 10 mm/s in OMC patients (p less than 0.02) and from 48 +/- 10 to 69 +/- 8 mm/s in the OMC + P group (p less than 0.001). The mean LVPWVs increased from 35 +/- 8 to 48 +/- 8 mm/s in the OMC + P group (p less than 0.01). These parameters correlate fairly well with stroke volume index (SVI), ejection fraction (EF), and fractional shortening (FS) derived from internal LV dimensions. Thus, the posterior wall movement may prove to be useful as an index for evaluating the improvement of LV function after OMC.

Changes in NK cell activity during the estrous cycle and pregnancy in mice.

Changes in NK cell activity during the estrous cycle and pregnancy in mice were studied by a 4 h 51Cr-release assay using YAC-1 cells as a target. NK activity both in the spleen and peripheral blood mononuclear cells showed a single fluctuation with a peak at met-estrous-2 during the estrous cycle. Splenic NK activity was suppressed in the early to mid-stages of pregnancy but increased sharply in the late stage. The activity declined thereafter, reaching the estrous level post-partum. NK activity in the peripheral blood showed a decrease throughout the entire pregnancy, but increased on Day 1 post-partum, returning to the estrous level thereafter. To elucidate the mechanisms involved in these variations of NK activity, NK-enriched and non-adherent cells were prepared from the spleens of mice at estrous and met-estrous-2 and those at early and late stages of pregnancy and then examined for NK activity. The results showed that there was no difference in the cytotoxic activity among these purified NK cells. Adherent cells purified from the spleens of mice in the early stage of pregnancy when co-cultured with the non-adherent NK cell fractions suppressed the NK cytotoxicity. These results strongly suggest the possibility that phagocytic and/or adherent cells may be involved in the regulation of splenic NK activity during the estrous cycle and pregnancy.

Electrophoretic and ultrastructural analysis of the rabbit's striated external urethral sphincter.

The male rabbit's external urethral sphincter was investigated with O'Farrell's 2-dimensional electrophoretic analysis of myosin light chains and electron microscopy. Its pattern of myosin light chains was different from that of the soleus (predominantly slow twitch muscle) but was very similar to that of the psoas (predominantly fast twitch muscle). Ultrastructurally it was shown to be red muscle resembling the soleus. Therefore the fiber type of the rabbit's external urethral sphincter was determined to be the red (fast) type.
