[Combination chemotherapy with PAP (peplomycin, adriamycin and cisplatin) in recurrent uterine cervical cancer with pulmonary metastasis].

Six patients with recurrent carcinoma of the uterine cervix were treated with a combination of peplomycin, adriamycin and cis-platinum (PAP). The PAP regimen consisted of peplomycin at a dose of 5 mg/day, continuous i.v. drip on days 1-7, adriamycin at a dose of 40 mg/m2 i.v. on day 1, and cis-platinum at a dose of 40 mg/m2, continuous i.v. drip on day 1, repeating at five to six week intervals. Three complete responses and three partial responses were obtained in six evaluable patients. Toxicities included mild to moderate nausea and vomiting, alopecia, nephrotoxicity, and myelosuppression, were generally manageable. We have concluded that PAP regimen is one of the useful regimens for recurrent uterine cervical carcinoma with pulmonary metastasis.

[Combined chemotherapy and elemental diet in advanced lung cancer patients].

The nutritional and immune status were studied in advanced lung cancer patients receiving combined chemotherapy and an elemental diet to test the effectiveness of the latter. Parameters such as skin tests, lymphocyte count, total protein and albumin values, body weight, and nitrogen balance were improved or maintained by the administration of the elemental diet. A better response to chemotherapy was achieved in patients receiving the elemental diet. Elemental diet support is suggested as a useful addition to chemotherapy in advanced lung cancer.

[Efficacy of intrapleural treatment with aclacinomycin combined with closed tube thoracostomy for malignant pleural effusion].

Intrapleural treatment with aclacinomycin combined with closed tube thoracostomy was used in 7 patients with malignant pleural effusion. Five patients had no recurrence of effusion 3 months after the treatment. Aclacinomycin levels were much higher in blood cells than in plasma, and metabolites were present as the active form. We posit that the local instillation of aclacinomycin is indicated in the management of malignant pleural effusion.

Cryoglobulin removal with cryofiltration in the treatment of cryoglobulinemia.

The technique of cryofiltration has been applied in the treatment of cryoglobulinemia. Good clinical improvement has been demonstrated accompanied by decreases of cryoglobulin levels. Three treatments in a week produced transient improvement of clinical symptoms and reduction of cryoglobulin levels. Follow-up treatments performed at adequate frequency and volume processed maintained relief of symptoms and serologic improvement. The cryofilter demonstrates good selectivity for cryoglobulin removal in cryogel, confirmed by electrophoretic analysis of both the cryogel filtrate from the cryofilter used clinically. Plasma cryofiltration appears to be a safe alternative to plasma exchange, requiring no replacement fluids.

Efficacies of whole blood and plasma cross circulation on galactosamine induced hepatic failure rats.

Galactosamine induced hepatic failure rats were treated by whole blood and plasma cross circulation. While no significant improvement in survival rate was achieved by means of these 2 procedures, oxygen consumption in state 3 and ATP synthesis of mitochondrial function were significantly improved as compared to the sham circulation groups. Maintenance of mitochondrial functions as by extracorporeal perfusion techniques may be advantageous in acute hepatic failure.

[Remission maintenance therapy of non-Hodgkin's lymphoma with CHOP protocol].

We have retrospectively studied the problems concerning duration of maintenance therapy and chemotherapy regimens for non-Hodgkin's lymphoma treated at Shikoku Cancer Center Hospital. Four out of 15 complete remission cases had been in complete remission more than 3 years. Maintenance chemotherapy consists of CHOP protocol administering every 4 weeks and the median maintenance duration was 14 months with a median cycle of 14. Thereafter, patients have been followed up without receiving any therapy. The prospective study on maintenance therapy should be designed considering conditions such as 1) sufficient drug doses for cytocidal effects, 2) shorter maintenance period as possible, and 3) minimal side effects and complications to be expected.

[Long-term cross-circulation on galactosamine induced hepatic failure rats].

Cross-circulation was carried out in between galactosamine treated hepatic failure rats and normal rats and its efficacy was mainly evaluated by measuring mitochondrial respiratory function of the liver. Twenty-four hours after the injection of galactosamine, cross-circulation was performed at a blood flow rate of 1 ml/min for 8 hours. Survival rate of the rats in 48 hours and 60 hours after the injection of galactosamine was 86% and 57% in a cross-circulation group and 66% and 23% in a sham-circulation group, respectively. In a cross-circulation group, there showed significant increases of ATP synthesis per 100 gr. body weight, turnover number of phosphorylation and Cyt a (+a3) which were markedly reduced in a sham-circulation group. These appear to suggest that cross-circulation provides augmented mitochondrial energy supply in the liver and contributes higher survival rate of the rats.

[Homologous plasma cross-circulation on galactosamine induced hepatic failure rats].

Plasma cross-circulation which resembles plasma exchange clinically was carried out in between galactosamine (GalN) treated hepatic failure rats and normal rats. Twenty-four hours after the injection of GalN, plasma cross-circulation was performed at a plasma flow rate of 0.1 ml/min for 6 hours. In eighteen hours after the completion of a procedure, survival rate was 75% (6 out of 8 survived) in GalN injected rats treated with plasma cross-circulation, as compared to 40% (4 out of 10 survived) in GalN injected rats treated with sham-circulation. State 3 oxygen consumption and ATP synthesis in the liver mitochondrial respiratory function, and ATP and total adenine nucleotide contents in the liver in the former group were significantly higher than those in the latter group at the time of sacrifice of the survived animals. These appear to suggest that plasma cross-circulation enhances mitochondrial phosphorylative activity in the liver and may contribute higher survival rate of the rats. Plasma exchange using a large amount of fresh plasma, therefore, would be effective for the treatment of acute liver failure in its early stage.

Effect of human fibroblast interferon on the cytotoxic activity of natural killer cells and lymphocytes against autochthonous and allogeneic tumor cells.

An ovarian cancer patient was treated with human fibroblast interferon (HFIF) given daily by iv infusion, and the lymphocyte natural killer (NK) activity against K562 and the cytotoxicity against autologous tumor cells under various experimental conditions were investigated. After HFIF treatment, NK activity against K562 increased significantly, whereas the autologous tumor cell kill by lymphocytes was not increased with target tumor cells that were fresh, frozen then thawed, or frozen, thawed and cultured for 4 to 5 days before use. In vitro HFIF-stimulated lymphocytes also did not enhance cytotoxicity against the various autologous tumor cells. However, lymphocytes from allogeneic healthy persons did show enhanced cytotoxicity against cultured tumor cells of this patient after cultivation with HFIF, but had no significant effect against frozen tumor cells. Mixed lymphocyte-tumor cell reaction (MLTR) and MLTR-induced lymphocyte cytotoxicity against autologous tumor cells were not enhanced by HFIF. Thus, tumor-specific immune reactions directed toward autologous tumor cells were not enhanced by HFIF treatment under the conditions tested in this study.
