Action of the 1,4-dihydropyridine derivative, KW-3049, on the smooth muscle membrane of the rabbit mesenteric artery.

1. The effect of 1,4-dihydro-2,6-dimethyl-4-(3-nitrophenyl)-3,5-pyridine-dicarboxylic acid methyl 1-(phenylmethyl)-3-piperidinyl ester hydrochloride (KW-3049) on the smooth muscle membrane of the rabbit mesenteric artery was investigated using microelectrode and single electrode voltage clamp methods. 2. In whole tissue preparations KW-3049 did not alter the resting membrane potential of the artery, but did inhibit the action potential evoked by a depolarizing current in the presence of 10 mM tetraethylammonium (TEA). 3. Using the voltage clamp technique, the effects of KW-3049 on the inward current evoked in solution containing 100 mM BaCl2 were observed. When the membrane was held at -60 mV, KW-3049 inhibited the inward current in a concentration-dependent manner. The inward current evoked by a larger depolarizing pulse was inhibited to a larger extent than that evoked by a smaller one. 4. When the membrane was held at -80 mV, the inward current evoked at test potentials of -10 and 0 mV was enhanced by low concentrations of KW-3049 (below 100 nM). 5. KW-3049 accelerated the rate of inactivation of the inward current and shifted the voltage-dependent inactivation curve to the left. 6. KW-3049 has a long-lasting inhibitory action on smooth muscle cells, since the inhibition of the inward current persisted for over 1 h after the removal of KW-3049. 7. Our results suggest that KW-3049 has a selective and long-lasting action on the Ca channels of the smooth muscle cell membrane of the rabbit mesenteric artery. This agent has both facilitatory and inhibitory actions on the Ca channel, depending on the values of the holding and command potentials.

Clinical use of CA 125 and its combination assay with other tumor marker in patients with ovarian carcinoma.

The serum levels of CA 125 and CA 19-9 were determined by an immunoradiometric assay employing the monoclonal antibody OC 125 and anti-CA 19-9 antibody in 88 patients with ovarian carcinoma. When a cut-off value of CA 125 was set below 35 U/ml in the control group, serum elevated levels of CA 125 were found in 86.7% of the patients with surgically demonstrable ovarian serous cystadenocarcinoma, in 100% (4/4 cases) of clear-cell carcinoma, in 50% (2/4 cases) of endometrioid carcinoma, in 100% (5/5 cases) of undifferentiated carcinoma, and in 80% of the recurrent cases. Using a cut-off value of 37 U/ml, serum elevated levels of CA 19-9 were detected in 68.2% of mucinous cystadenocarcinoma, in 28.9% of serous cystadenocarcinoma, in 75% (3/4 cases) of metastatic ovarian carcinoma, and in 37.5% of the recurrent cases. A statistical analysis of the combination assay using CA 125, CA 19-9, tissue polypeptide antigen (TPA), immunosuppressive acidic protein (IAP), ferritin and CEA was carried out by multivariate method (discriminatory analysis) in 45 patients with ovarian carcinoma and 50 healthy subjects. As a result before treatment, positive rates of a single tumor marker were 79.7% with CA 125, 42.7% with CA-19-9, 73.1% with IAP, 61.7% with TPA, 64.3% with ferritin and 25.4% with CEA, respectively. A combination assay of these markers was useful for detecting identification of ovarian carcinoma, by which it gave a higher accuracy of ovarian cancer detection.

[A randomized trial comparing vindesine plus cisplatin with mitomycin C, vindesine plus cisplatin in patients with advanced non-small cell lung cancer].

Thirty-six evaluable patients with advanced non-small cell lung cancer were randomized to treatments involving vindesine (3 mg/m2 X 3) plus cisplatin (80-120 mg/m2) versus mitomycin C (8 mg/m2) plus vindesine (2 mg/m2 X 3) plus cisplatin (80-120 mg/m2). The response rate for the vindesine and cisplatin combination was 29%, versus 47% for the mitomycin C, vindesine and cisplatin combination. There was no evidence of improved duration of response in patients given mitomycin C, vindesine and cisplatin. The median survival for patients given mitomycin C, vindesine and cisplatin was 11.4 months, compared with 10.3 months for those given vindesine and cisplatin. Toxicity was almost comparable for the two treatments. The utility of addition of mitomycin C to vindesine and cisplatin should be evaluated in further investigations.

[A case of adult T-cell leukemia achieving complete remission with epirubicin, vincristine and prednisolone chemotherapy].

A 68-year-old man was referred to our hospital because of generalized lymphadenopathy. His abnormal findings were hepatomegaly, leukocytosis and elevated serum LDH. Anti-ATLA antibody was positive. As about half of the peripheral lymphocytes reacted to the monoclonal antibody CD25, these cells were considered to be compatible with ATL cells having an interleukin-2 receptor. Initially, recombinant beta-interferon was administered, but it was not effective. A combination of vincristine (VCR) and prednisolone (PDN) was partially effective against the lymphadenopathy, but the hepatomegaly and leukocytosis did not improve. 4'-epi-Adriamycin (Epirubicin), at a doses of 20 mg/body, weekly, was subsequently added to VCR + PDN therapy. The patient achieved complete regression 2 months later, and has been in continuous complete remission for more than 3 months.

[Combination chemotherapy with mitomycin C, adriamycin and cisplatin (MAP) in advanced adenocarcinoma of the lung: clinical effect and renal toxicity].

Fourteen patients with advanced adenocarcinoma of the lung were treated with a combination chemotherapy of mitomycin C, adriamycin and cisplatin (MAP). The overall response rate was 43% with 6 partial responses. The median duration of response was 5.5 months. The median survival for responders was 17 months, versus 9 months for non-responders. Toxicity included nausea and/or vomiting, alopecia, myelosuppression and renal toxicity. The serial measurement of renal tubular function was found to be useful in detecting renal damage caused by cisplatin containing chemotherapy.

[Antiemetic effects of combinations of metoclopramide, droperidol and dexamethasone for the prevention of cisplatin-induced gastro-intestinal toxicity: a randomized crossover trial].

Thirty-two patients with primary lung cancer receiving combination chemotherapy including cisplatin at a dosage of 80-120 mg/m2 were entered into an antiemetic randomized crossover trial. Patients received metoclopramide (2 mg/kg i.v. every 2 h X 5), droperidol (0.5 mg/kg i.v.) and dexamethasone (30 mg i.v.) on day 1, and metoclopramide (1mg/kg i.v. every 8 h X 3) (Regimen A) or metoclopramide and droperidol (2.5 mg i.v. every 8 h X 3) (Regimen B) on days 2 to 5. No vomiting occurred within the first 24 hours after cisplatin administration in 75% of patients. Regimen B was found to be more effective than regimen A with respect to the mean duration of vomiting (p less than 0.1), the mean duration of nausea (p less than 0.05), the mean duration of anorexia (p less than 0.05), and the mean score of the patients' opinions (p less than 0.1). When the patients were asked for their opinion of the two regimens, 39% preferred regimen B (p less than 0.05). There were no major side effects with either regimen.

[Videodensitometric assessment of right and left ventricular functions by digital subtraction angiography].

Right and left ventricular volumes and systolic indices were determined by intravenous digital subtraction ventriculography in 50 patients with various heart diseases. Using a constant injection speed of 35 ml Renografin-76 contrast medium, serial right and left ventriculograms were obtained in the 30 degree right anterior oblique projection. Right ventriculograms were also obtained in the 60 degree left anterior oblique projection. The videotape recordings of subtracted images were continuously digitized into 128 X 128 eight bit (256 gray scales) pixel matrices using an image-processing computer. The endocardial outlines of the right and left ventricles were drawn manually using a joystick, frame by frame, for each cardiac cycle. By integrating overall the pixel densitometric counts within this outline for each frame, the computer generated a time-density curve with maxima and minima represented in the end-diastolic and end-systolic frames, respectively. Systolic indices including ejection fraction (EF), one-third ejection fraction (1/3 EF) and the peak ejection rate (PER) were derived from the time-density curve. Right ventricular volume was determined by the single-plane or biplane mathematical formulae of Ferlinz et al., and left ventricular volume was calculated by the area-length method in the 30 degree right anterior oblique projection. Results by the two geometric methods correlated well for right ventricular volume (r = 0.84) and for EF (r = 0.80). Right ventricular densitometric counts correlated closely with single-plane volume (r = 0.91). Right ventricular ejection fraction (RVEF) determined by videodensitometry also correlated satisfactorily with that by single-plane angiography (r = 0.74).(ABSTRACT TRUNCATED AT 250 WORDS)

Granulocytic sarcoma of the intestine as a primary manifestation nine months prior to overt acute myelogenous leukemia.

A case of granulocytic sarcoma is reported. A 51-year-old male was found, upon laparotomy, to have a tumor mass in the terminal ileum. Nine months later, acute myelogenous leukemia (AML) was diagnosed upon a peripheral blood and bone marrow examination showing an increased number of abnormal myeloblasts. Scattered lesions of granulocytic sarcoma were found in the intestine, colon, testis, skin, gingiva and bone marrow. Chemotherapy, with multiple regimens for leukemia in combination, did not affect the tumor and the patient died 10 months following his development of overt leukemia. Detailed pathological findings are presented because of the difficulty in differentiating granulocytic sarcoma from non-Hodgkin's lymphoma.

[A case of non-Hodgkin's lymphoma manifesting an eyelid tumor as a major symptom].

It is well known that a majority of non-Hodgkin's lymphoma (NHL) in the head and neck region arises from the Waldeyer's ring and neck lymph nodes. This report covers a rare case of a 69 year-old male with a symptom like the Mikulicz syndrome, who was diagnosed as having the NHL of B-cell type. This patient has been in complete remission following local radiotherapy for the tumor of the eyelid.
