A family of familial hypercholesterolemia with cerebral infarction and without coronary heart disease. An unusual case with corneal opacity, polyneuropathy and carpal tunnel syndrome in the family: therapy with probucol and tocopherol nicotinate.

A study is presented of a 48-year-old female patient and her three siblings with familial hypercholesterolemia. The family members had episodes of cerebral infarction and apparently had atherosclerosis of the internal carotid artery, but no coronary heart disease due to their almost normal level of cholesterol. The laboratory studies of the family members revealed the elevations of serum lipid peroxides, serum lipoprotein(a), leukotriene C4 in blood, the thromboxane B2/6-keto-prostaglandin F1 alpha ratio in plasma and serum hydroxyl radical. Therefore, it is suspected that these factors accelerating atherosclerotic process caused the cerebral infarction. The patient demonstrated corneal opacities, palpebral xanthomas, thickened Achilles tendons, polyneuropathy and the carpal tunnel syndrome. Laboratory studies revealed an elevation in the OKT4/8 ratio, monocyte dysfunction with respect to phagocytosis and chemotaxis, and the presence of the 46XX/45XO mosaic chromosome. Lipid deposits were observed in the Achilles tendon, the transverse carpal ligament, the Schwann's cells and axons of the sural nerve, and in the keratocytes and stroma of the cornea. Following the administration of tocopherol nicotinate and probucol, the patient's serum lipid peroxide normalized and there was improvement in her palpebral xanthomas, thickening of the Achilles tendons and polyneuropathy. We conclude that the lipid deposits in this patient were due to the abnormal oxidative metabolism of low-density lipoprotein and a disturbance of the scavenger pathway due to the monocyte dysfunction.

TRH-Gly, a precursor of TRH, alters GH secretion in acromegaly.

We explored effects of a precursor of thyrotropin (TSH)-releasing hormone (TRH), TRH-Gly, on growth hormone (GH) secretion in acromegaly. Intravenous injection of TRH-Gly produced a profound increase in GH secretion in eight, decrease in two, and no response in five out of a total fifteen patients. The magnitude of GH responsiveness to TRH-Gly was significantly correlated with that induced by TRH (r = 0.824, P less than 0.01). In contrast, TRH-Gly did not induce secretion of TSH or prolactin. The present data suggest that TRH-Gly may participate in regulating GH secretion in some patients with acromegaly and that TRH-Gly-induced GH secretion may be due at least in part to TRH-associated mechanisms underlying GH secretion.

C cell hyperplasia and carcinoma developing in sheep with experimentally-induced lymphosarcoma.

Two cases of C cell hyperplasia and one case of C cell carcinoma of the thyroid glands were bilaterally recognized in 11 sheep with experimentally-induced lymphosarcoma. The serum calcium concentration in the C cell carcinoma case was slightly increased above the normal concentration of around 9 mg per dl. Bilateral C cell hyperplasia also developed in the thyroid lobes of the C cell carcinoma case. Immunohistochemically, hyperplastic C cells and tumour cells were positive for calcitonin, calcitonin gene-related peptide, chromogranin A and neurone-specific enolase. No amyloid deposition nor multiple endocrine neoplasia was demonstrated in any of the cases. Ultrastructurally, many secretory granules were observed in the cytoplasm of neoplastic cells constituting the C cell carcinoma and in the hyperplastic C cells.

Protective immunity against bovine leukaemia virus (BLV) induced in carrier sheep by inoculation with a vaccinia virus-BLV env recombinant: association with cell-mediated immunity.

The effects of vaccination of sheep with a recombinant vaccinia virus (rVV) expressing the bovine leukaemia virus (BLV) envelope glycoprotein (gp60) were studied by determining BLV titres in peripheral blood leukocytes after vaccination and challenge. The proliferation of BLV was suppressed markedly, not only when rVV was inoculated prior to challenge with BLV, but also when it was inoculated after challenge. These results indicate that vaccination with rVV induces protective immunity that can suppress the growth of BLV in carrier animals. Since rVV induced a strong anti-BLV delayed-type hypersensitivity response without producing detectable levels of binding or neutralizing antibodies, and there was no apparent correlation between the humoral immune response and BLV proliferation, a cell-mediated immune response was assumed to play a major role in protective immunity.

Central hypothyroidism due to isolated TRH deficiency in a depressive man.

A depressive man was evaluated for developing chronic fatigue and cold intolerance, in whom laboratory findings showed decreased thyroid hormone levels (T4, 2.7 micrograms dl-1; T3, 0.76 ng ml-1) with normal blood levels of TSH. A single bolus injection of TRH (500 micrograms) significantly stimulated prolactin secretion, but did not cause an increase in blood TSH levels (basal level, 1.2 microU ml-1 vs. 1.3 microU ml-1 30 min after injection). By contrast, TRH-induced TSH stimulation occurred after repeated injection of TRH for 4 consecutive days (basal level, 1.5 microU ml-1 vs. 5.6 microU ml-1 30 min after injection). Blood thyroid hormone concentrations were restored to normal levels after long-term administration of TRH. Other pituitary functions remained unchanged. A diagnosis of central hypothyroidism due to isolated TRH deficiency was made in this case, and the data presented here indicate that partial resistance of pituitary thyrotrophs to TRH may be associated with depression.

Epstein-Barr viral genomes in carcinoma metastatic to lymph nodes. Association with nasopharyngeal carcinoma.

Lymphoepithelioma of the nasopharynx is a neoplasm known to have a strong association with Epstein-Barr virus (EBV). Using the Southern blot method, polymerase chain reaction (PCR), and/or in situ hybridization, we examined lymph nodes containing metastatic carcinoma, including metastatic lymphoepithelioma, for the presence of EBV genomes in order to determine whether EBV was associated exclusively with lymphoepithelioma. All of six lymph nodes from patients with lymphoepithelioma in the neck were found to have EBV genomes using the above methods. In four of the six cases, the primary site was the nasopharynx, and in the other two no primary site was found. Four of 12 squamous cell carcinomas and one of 18 adenocarcinomas expressed the EBV genome only by PCR, but not by Southern blotting or in situ hybridization, probably due to the presence of latent EBV in lymphocytes. These results indicate that metastatic carcinoma in lymph nodes showing EBV genomes revealed by Southern blotting or in situ hybridization is lymphoepithelioma, and that the nasopharynx is very likely the primary site.

Histopathology and distribution of cells harboring bovine leukemia virus (BLV) proviral sequences in ovine lymphosarcoma induced by BLV inoculation.

Six sheep with lymphosarcoma induced by hypodermic inoculation of bovine leukemia virus (BLV) materials were examined to elucidate the relation between pathologic lesions and integration of BLV provirus in cellular DNAs. Antibodies to BLV gp-antigens had been detected since the 3rd week after the inoculation, and BLV was positive when checked 3 months later. Lymphosarcomas followed the leukemic phase in 4 sheep. The other 2 sheep showed initial lesions of lymphosarcoma and were aleukemic clinically. Five animals were killed by enthanasia and autopsied at 2.5 to 3.5 years postinoculation (pi) because of their diseased condition. One animal died 10 years pi following the 4th leukemic episode. Sarcomatous lesions were confirmed grossly and histologically, and the proliferating neoplastic cells were classified into lymphocytic, prolymphocytic, lymphoblastic and histiocytic types. Integration of BLV provirus in cellular DNAs of the peripheral blood lymphocytes (PBL) and neoplastic cells of sarcomatous lesions was examined by Southern blotting technique. BLV provirus was demonstrated in the PBL of all infected animals and in most of the sarcomatous lesions of the spleen, kidney and lymph nodes except 4 lymph nodes showing slight neoplastic infiltration. The results indicated that ovine lymphosarcoma could be caused by BLV and the cells carrying proviral information seemed to be disseminated and proliferate in the lesions.

Epidemiological and pathological studies on congenital diffuse hyperplastic goiter in calves.

Perinatal diseases such as weak calf, stillbirth, or abortion were found in 86 calves from about 600 cows in a farm (A Farm) in 1987 and 1988. Such perinatal diseases were decreased to 36 calves in 1989 and 1990 after an additional supply of seaweed was made to the maternal feeds. From these cases, 23 stillborn and weak calves were pathologically examined. The mean weight with standard deviation of the 18 thyroid glands was 36.3 +/- 28.6 g before feeding of the seaweed supplement, and 12 (67%) of the glands showed moderate to marked degree of diffuse hyperplastic goiter in histological criteria. On the other hand, 5 glands weighed 12.0 +/- 3.4 g and revealed no such histological lesions after the additional supply. These results suggest that the goiter had been associated with birth of weak calves. Further study was performed on 37 newborn calves affected with Akabane disease in 1986 in A Farm and the neighbouring B Farm located. Seven (54%) of 13 thyroid glands from A Farm and 1 (4%) of 24 glands from B Farm showed histologically moderate to marked goitrous lesions. There were dotted "Inugarashi", Rorippa indica, family Cruciferae in both pastures. The mean iodine contents of the orchard grasses were 87 and 121 micrograms/kg on dry basis in A and B Farms. Marginal deficiency or lower limit of iodine and possible thiocyanate content of "Inugarashi" may have been responsible for the goiter in the farms.

Immunohistochemical localization of serotonin in the pineal gland of the chicken during post-hatching development in relation to light-dark cycle.

In the present immunohistochemical study using an antiserum against serotonin, the localization of serotonin was demonstrated in the pineal gland of the chickens from 1 day to 3 years of age. Serotonin-like immunoreactivity was located in the supranuclear region of the follicular cells and in the perikaryon of the parafollicular cells in the pineal gland, exhibiting distinct changes during post-hatching development. The reactivity reached its peak in 1-year-old chicken. Moreover, diurnal changes in immunoreactivity also existed after 1 day of age, being high in the light period and low in the dark period. Occasionally in 1-3-year-old chickens, a part of granular-shaped reactive products observed in the light period appear to be diffused in the cytoplasm in the dark period. In addition, a number of immunoreactive fibers were shown in the pineal parenchyma, exhibiting a slight increase in number and in reactivity at 40 days of age.

Modified CHOP in an aged patient with non-Hodgkin's lymphoma and probable extensive pulmonary involvement.

A 75-year-old female was admitted to our hospital because of shortness of breath and palpitations. Chest roentgenogram together with a CT scan showed multiple nodular shadows in both lung fields, with hilar lymphadenopathy. The cervical lymph nodes were enlarged, and were biopsied to reveal diffuse large T-cell non-Hodgkin's lymphoma. Extensive pulmonary involvement was suspected and treatment with a modified CHOP regimen was begun. Following therapy, pulmonary symptoms were remarkably improved, and chest roentgenogram revealed virtually complete clearance of lung lesions. No severe side effects were observed as a result of the therapy.

[Histopathologic diagnosis of bone marrow in leukemia and related disorders].

Histopathologic diagnosis of the bone marrow in leukemia is usually a supplementary method to the cytological in acute and chronic leukemia. However, for patients with MDS and MPD and with dry tap bone marrow biopsy is very important. Important morphological findings and useful immunohistochemical methods for differentiation and characterization of leukemia are reported and the usefulness of sequential examination of bone marrow in leukemia during and after chemotherapy is emphasized. In addition to leukemia, histological features and differential points of myelodysplastic syndrome (MDS) and myeloproliferative disorders (MPD) are mentioned. The proliferating megakaryocytes differed in size and shape between MDS and MPD. The difference in proliferating rate of the cells examined by PCNA was also useful to differentiate the two disorders histologically.

[Gene analysis in human hematopoietic malignancies].

We analyzed the T-cell receptors and immunoglobulin genes in 140 cases of hematopoietic malignancies, including ALL/LBL, malignant lymphomas, AILDs, Hodgkin's disease, ATL and CLL. In many cases, the findings obtained by immunoassociated gene analysis were in parallel to the phenotypic findings. Gene analysis is useful to determine the clonality and the lineage of malignant cells. However, 10-30% cases appeared bigenotype. In some cases, oligoclonalities were recognized. The relationship to the prognosis in these cases was discussed. The development of gamma delta- and alpha beta-cells on T-cell ontogeny was elucidated. Some T-cell leukemic lines were found to be pre-alpha beta T-cells with some consensus characteristics. We also analyzed the V beta family of 15 alpha beta T-cell leukemia cell lines using six V beta cDNA probes. This approach may be useful to determine the clonality of T-cell malignancies. The method of using a PCR system on immunoassociated genes has been described. The PCR method, using the super variable region as a tumor specific fragment, is valuable in the detection of the residual cells of lymphoma and leukemia at a concentration of 1/10(5) cells.

[A case of lacrimal gland malignant lymphoma, associated with Sjögren's syndrome].

A 64 year-old female complained of left upper lid swelling. Painless mass lesions were found in her bilateral lacrimal gland areas. She also complained of a dry sensation in both eyes and her mouth. The rose bengal test, Schirmer's test and fluorescein dye test were positive. Lip biopsy revealed chronic sialoadenitis and rheumatoid factor titer was elevated. Anti-nuclear and anti-RNP antibodies were positive. Anti-SS-A and anti-SS-B antibodies were both negative. Left pre-and post auricular, cervical and inguinal lymph nodes were palpable. Excisional biopsies of bilateral orbital lesions and an inguinal lymph node revealed the presence of reactive lymphoid hyperplasia in the right orbit, and B cell type, diffuse, large cell lymphoma, in the left orbit and inguinal lymph node. Intensive chemotherapy resulted in complete remission. Lacrimal gland lymphoma must be considered when examining cases of Sjogren's syndrome.

Photosensory elements in the pineal gland of the Japanese quail, with special reference to the paraboloid.

In 1-day-old Japanese quails, Coturnix coturnix japonica, photosensory elements of the pineal gland were examined by means of histochemical demonstration of glycogen and transmission electron microscopy. The parenchymal cells of the quail show immature structure, especially, small apical protrusion with a few mitochondria in the pinealocyte and rare appearance of the dense body in the supporting cell. The paraboloid is found in cytoplasmic processes projecting into the follicular lumen and in perikarya of the follicular cells. It is composed of a well-developed network of tubular smooth-surfaced endoplasmic reticulum and abundant glycogen granules, but there is not a higher concentration of glycogen in its central region. The paraboloid-containing cell is considered as a type of the pinealocyte, because of occasional appearance of the synaptic ribbon in its perikaryon. In addition to lamellar whorls, regular stacks of lamellar disks are observed in the pineal lumen, being adjoining to the paraboloid-containing cytoplasmic process. The present results may provide further evidence for the direct photoreception by the avian pineal.

[Intestinal T-cell lymphoma (so-called malignant histiocytosis of the intestine) complicated by multiple perforations].

We describe a case of intestinal T-cell lymphoma which was histologically diagnosed of malignant histiocytosis of the intestine. A 47-year-old man was admitted to our hospital because of fever and generalized lymphadenopathy. Mild anemia, leukocytosis, positive CRP and a high level of LDH were noted. Pathological finding of the lymph node was compatible with dermatopathic lymphadenopathy with a slight increase in atypical lymphoid cells. At the 14th day after admission, he suffered from abdominal pain and was diagnosed as having perforative peritonitis. In laparotomy, the infiltration of histiocyte-like atypical cells were found around a site of small perforation of the terminal ileum. The findings were compatible with that of malignant histiocytosis of the intestine (MHI). He had recurrent perforations of the small intestine and died of peritonitis and sepsis at the 42nd day. Southern blot analysis of the biopsied lymph node showed TCR-beta gene rearrangement. Some patients diagnosed clinically and pathologically as having MHI may have a T-cell lymphoma like our case.
