Small-intestinal involvement in familial polyposis diagnosed by operative intestinal fiberscopy: report of four cases.

Operative intestinal fiberscopy, in which the duodenal fiberscope was introduced during laparotomy for colectomy in familial polyposis via the enterotomy opening, permitted the demonstration of small intestinal polyps in six of seven consecutive cases. Four of the six patients had adenomatous polyps in the proximal jejunum, including one patient with the concomitant presence of ileal adenomas. Polyposis due to lymphoid hyperplasia in the terminal ileum was found in three patients. Preoperative upper gastrointestinal surveys revealed adenomas in the duodenums of all seven patients, adenomas in the gastric antrum in three, and multiple hamartomas in the gastric corpus in two. Thus, in familial polyposis or Gardner's syndrome, more or less the entire gastrointestinal tract seems to be involved and the term "gastrointestinal polyposis" seems to describe these conditions.

The role of disseminated intravascular coagulation in shock induced by transfusion of human blood in dogs.

Hemolytic blood transfusion reactions were produced in 20 dogs. Animals transfused with human blood twice in a two-week interval demonstrated a significant increase in anti-human red blood cell agglutination titer at the time of the second transfusion. Eighteen dogs that survived from the first transfusion reaction were divided into two groups with or without heparin treatment. The hemolytic transfusion reaction in the sensitized animals was characterized by shock and multiple coagulation abnormalities. Heparin failed to prevent shock, hemoglobinemia and thrombocytopenia, and decreased the mortality rate in heparinized animals. Pathologic examination revealed that the thrombi in the splanchnic organs were less common in the heparinized dogs.

Complete atrioventricular block following radiation therapy for malignant thymoma.

Complete atrioventricular block following radiation is very rare. We present a case which developed after radiation therapy for malignant thymoma. The etiology of conduction disturbances due to radiation is unknown. In our case, serial electrocardiograms showed stepwise progression of the conduction disturbance, and His bundle electrocardiograms revealed new prolongation of the H-V interval. Endomyocardial biopsy specimens demonstrated occlusion in small arteries and diffuse degenerative changes in the myocardium. We therefore attributed the complete atrioventricular block in our patient to secondary damage to the conduction system, caused by radiation-induced occlusive changes in the small arteries supplying the conduction system.

Regression of gastric polyps in Gardner's syndrome with use of indomethacin suppositories: a case report.

Patients with familial adenomatous polyposis most often have multiple polyps in the large bowel and also in the gastroduodenal region. There are reports of regression or disappearance of colorectal and duodenal polyps while on sulindac therapy. We report here what seems to be the first case of regression of gastric polyps while on indomethacin suppository treatment.
