An autopsy case of malignant pheochromocytoma arising in the organ of Zuckerkandl is reported, of which distant metastases were found in the liver and lymph nodes. The biochemical and electron microscopic findings from the tumor tissue showed noradrenaline-secreting type. It was revealed that sex ratio in malignant pheochromocytoma was more prominent in the female, and the frequency of extra-adrenal origin was higher in malignant pheochromocytoma than in benign one from comparative study of reported cases of malignant pheochromocytoma.
Triazolam was 4 to 5 times as potent as diazepam in reducing hyperemotionality of either septal-lesioned or olfactory bulbectomized rats (O.B. rats), and in suppressing muricide in O.B. rats. This agent was equipotent with diazepam in inhibiting fighting behavior of long-term isolated mice, but was longer in duration of action. Triazolam was approximately 4 times more potent than diazepam in preventing pentetrazol convulsion, but was 10 times less potent in inhibiting maximal electroshock convulsion in mice. The muscle relaxant effect of triazolam as assessed by the inclined screen test was 34 times, and the effect on rotarod performance was 17 times more potent than that of diazepam in mice. Triazolam (0.2 approximately 0.5 mg/kg i.v.) changed the EEG to a drowsy pattern in unanesthetized rabbits with a chronic electrode implant, and suppressed the EEG arousal response to auditory stimulation and electrical stimulation given to either the mesencephalic reticular formation or posterior hypothalamus. The limbic afterdischarges induced by either hippocampal or amygdaloid stimulation were also markedly inhibited by triazolam. These EEG effects of triazolam were qualitatively similar to, but were 4 to 5 times more potent than those of diazepam. These results indicate that triazolam is a potent tranquilizer with a longer duration of action, and the muscle relaxant effect is considerable as compared with diazepam.

A successful free groin flap transfer to the arm in a 3-month-old baby is described. The vessels were large enough for microvascular anastomosis and we believe that there is no minimum age restriction for free flap transfer.
Electroencephalographic (EEG) effects of flurazepam were investigated in unanesthetized, unrestrained rabbits with chronic electrode implants and compared with those of diazepam. Flurazepam, at doses of 0.5 approximately 5 mg/kg i.v., induced a drowsy EEG pattern, i.e. high voltage slow waves in the cortex and amygdaloid complex and desynchronization of the hippocampal theta waves. In addition, low voltage fast waves were superimposed, especially on the cortical EEG. Flurazepam suppressed the EEG arousal responses induced not only by auditory stimulation but also by electrical stimulation of the mesencephalic reticular formation, posterior hypothalamus and centromedian thalamus. The EEG arousal response induced by i.v. injection of physostigmine was suppressed by flurazepam. Flurazepam depressed the photic driving response and the augmenting response. The recruiting response was slightly enhanced by flurazepam. The limbic afterdischarges elicited by either hippocampal or amygdaloid stimulation were suppressed by flurazepam. Flurazepam caused reductions of pressor responses to stimulation of the posterior hypothalamus and the mesencephalic reticular formation in anaesthetized rabbits. There was little or no effect on pressor responses to the injection of noradrenaline, carotid artery occulusion and asphyxia with flurazepam. In general, these effects of flurazepam were similar to those of diazepam, but the drug induced actions which differed from those of diazepam.

An unusual presentation of polyp of the ampulla of Vater is described. The patient initially had a chronic relapsing pancreatitis. Pancreatocholangiography revealed a tumor in the main pancreatic duct that proved to be a pedunculated polyp of the ampulla of Vater by operation. Transduodenal excision and sphincteroplasty may cure the lesion and relieve the symptoms of this disease.
A clinical operative technique for free muscle transplantation by microneurovascular anastomoses is presented. Two cases of free transfer of the gracilis muscle for dynamic reconstruction of facial paralysis are described, including a follow-up study with electromyography, light microscopy, and electron microscopy. We feel this new technique will have a wide range of application in reconstructive surgery.
We report the transfer of a free musculocutaneous compound flap, including the gracilis muscle, in one stage by microvascular surgery. The advantages of this free flap are discussed.
Since 1973 we have transferred various kinds of free flaps by microvascular surgery. In hand reconstruction, the free flap is less often required than in repairs of other sites, such as the leg. However, the importance of restoring sensibility in the hand is evident. Recently, we have transferred thin dorsalis pedis flaps as sensory free flaps in 5 cases of hand reconstruction, and we describe our experiences with them.

The effect of tryptophan hydroxylase inhibitor p-CPA and 5-HT precursor L-5HTP on conditional avoidance response and 5-HT content in rat brain was examined with the following results: Continuous administration of p-CPA improved acquisition of conditional avoidance response and retarded the loss acquired response. Body weight gain was suppressed at the low level. When the conditional avoidance response of the rats reached a certain level, it was suppressed by giving L-5HTP at the doses of 25 and 50 mg/kg in the rats given p-CPA, but no effect was observed in the rats given CMC. The 5-HT content in brain was reduced to 22% of control value by a single administration of p-CPA 316 mg/kg, but it rapidly recovered to a normal level with L-5HTP administration. From the above results, it was demonstrated that the presence or absence of p-CPA loading causes a significant difference in the appearance of behavior alteration in a rats following administration of L-5HTP.
Using the single radial immunodiffusion method, the serum levels of IgG, IgA, Ig M, transferrin, haptoglobin, alpha2-macroglobulin, alpha1-antitrypsin and alpha1-acid glycoprotein were estimated in healthy subjects and patients with liver diseases consisting of chronic active and inactive hepatitis, incipient cirrhosis, cirrhosis and primary liver cancer. The results obtained from the statistical analysis of the data were as follows: i) Immunoglobulins and alpha2-macroglobulin in all diseases were higher than those of healthy subjects. ii) The increased transferrin levels were found in chronic active and inactive hepatitis, and the increased alpha1-antitrypsin levels were observed in chronic inactive hepatitis, in incipient cirrhosis in cirrhosis and in primary liver cancer was higher than those of the other liver diseases. iii) Haptoglobulin levels in all diseases except for chronic inactive hepatitis were decreased. iv) alpha1-acid glycoprotein in chronic active hepatitis, in incipient cirrhosis and in cirrhosis were lower than that of healthy subjects. The evaluation of significance for difference of each protein level among disease groups clarified that the decrease of haptoglobin in cirrhosis and the increase of alpha1-antitrypsin in primary liver cancer were characteristic change respectively.
