Effects of calmodulin inhibitor on histamine release from rat peritoneal mast cells induced by concanavalin A and ionophore A23187.

The involvement of calmodulin in stimulus-secretion coupling of histamine release from rat mast cells was examined using a specific calmodulin inhibitor, W-7 [N-(6-aminohexyl)-5-chloro-1-naphthalenesulfonamide]. W-7 at low concentrations inhibited histamine release induced by either concanavalin A or ionophore A23187 and at high concentrations enhanced the release. But its congener, W-5 [N-(6-aminohexyl)-1-naphthalenesulfonamide], which has virtually no calmodulin-inhibiting activity, did not affect the release. It is concluded that W-7 modulates histamine release by inhibiting some functions of calmodulin in the cells.

A case of manic state in which lactation occurred after Li2CO3 administration.

Li2CO3 alone was administered to a 21-year-old female, who was in a manic state. As a consequence, lactation occurred on about the 50th day after Li2CO3 administration started, and the volume of lactation changed in parallel with the serum Li2CO3 level. The basal plasma PRL concentration and the responses to TRH (500 micrograms) were elevated during Li2CO3 administration. However, both of them returned to within the normal range after the suspension of Li2CO3 administration. From the above-mentioned findings, it is suggested that lactation is closely related to the Li2CO3 administration.

Aneurysm of the left aortic sinus causing acute myocardial infarction.

A 30-year-old female had an acute myocardial infarction complicated by congestive heart failure. Angiography demonstrated an aneurysm in the area of the left aortic sinus. The aneurysm compressed and displaced the main trunk of the left coronary artery and the proximal portion of the left anterior descending artery. This aneurysm was considered to be the cause of the infarction. Aortic valve replacement and removal of the aneurysm were performed. Postoperative studies revealed good function of the replaced valve, good antegrade filling of the left coronary artery, and improved left ventricular function.

[A case of ovarian dermoid cyst with bladder perforation].

A 60-year-old female presented with the main complaints of hematuria, pain upon urination, and pollakiuria. Simple abdominal X-rays revealed a dumb bell-shaped calculus shadow, and a calculus was detected in the right posterior wall by cystoscopy. At surgery, a left ovarian dermoid cyst accompanied by a calculus that was adhesive and perforating in the right posterior wall of bladder was detected. Twenty-eight cases of ovarian dermoid cyst with bladder perforation including this case are reported.

Primary oxalosis with cardiac manifestations.

This 48-year-old male developed renal insufficiency, peripheral sensitivity to cold, occasional ventricular extrasystoles and atrio-ventricular block. He died suddenly after ventricular fibrillation. Autopsy showed extensive deposits of oxalate crystals in the kidney, myocardium, arterial walls of the extremities, and in many other visceral organs. The atrio-ventricular (A-V) node was also severely involved and marked fibrosis was found in the common A-V bundle (His) and in both bundle branches. The A-V node artery showed mural crystals and the lumen was narrowed by 50 per cent. Features supporting the diagnosis of primary oxalosis with cardiac manifestations include the onset of urolithiasis at 10 years of age, and 5 other siblings all having died of "renal disease" in infancy.
