[Pharmacokinetics of ceftizoxime suppository in experimental animals].

Ceftizoxime suppository (CZX-S) was administered rectally to mice, rats and dogs, and the pharmacokinetics were studied in comparison with those after intravenous, intramuscular and subcutaneous administration of ceftizoxime (CZX). Absorption of CZX given rectally was rapid in all animals, similar to intramuscular or subcutaneous administration. The peak serum levels of CZX in mice, rats and dogs when administered rectally at a dose of 25 mg/kg were 23.1 micrograms/ml at 7.5 minutes, 23.5 micrograms/ml at 15 minutes and 25.2 micrograms/ml at 15 minutes, respectively. These values were about 76%, 68% and 42% of the values for subcutaneous or intramuscular administration in mice, rats and dogs at the same respective doses. Urinary recoveries of CZX after rectal administration of 25 mg/kg were 44.2% (0-12 12 hours) in rats and 27.7% (0-6 hours) in dogs, and 2.7% (0-6 hours) of the dose was excreted into bile fluid in rats. Organ distribution of CZX when administered rectally to rats was similar in distribution pattern to that of muscular administration, although its concentrations in various organs were slightly lower than those for intramuscular administration, as was the case for serum concentration. Serum concentrations of CZX were proportionately elevated with dose when dogs were rectally administered CZX-S in doses of 12.5, 25 and 50 mg/kg. In the case of multiple administrations (t.i.d. for 10 days) of CZX-S to dogs, no remarkable difference was found in serum concentrations of CZX in comparison with single doses, and no accumulation of CZX was demonstrated.(ABSTRACT TRUNCATED AT 250 WORDS)

[Recent advance in diagnostic imaging: NMR].

Application of NMR, whether in imaging or in spectroscopy, is evolving rapidly and its clinical feasibility is to be fully assessed. It is not clear if speculation today will hold true tomorrow, but here present status of NMR in medicine, especially in its roles in research of the central nervous system are discussed as follow; NMR imaging in brain tumors, cerebrovascular diseases, degenerative diseases, trauma, spinal cord diseases, Imaging by surface coil, Contrast material in NMR imaging, Blood flow measurement by NMR, Chemical shift imaging, Studies of metabolism by 31P NMR spectroscopy, Recent advances and possible future role of NMR in medicine.

[Prolonged action preparation of cefaclor].

This study was conducted to develop a prolonged action preparation of cefaclor (CCL) which can offer, with the twice-a-day administration, as much effectiveness as its conventional preparation (Kefral capsule) with the 3 times-a-day administration. Absorption site of CCL in gastrointestinal tract, preparation form (enteric coated granules) which slowly release CCL, dissolution property of the form, and mixed ratio of the form and rapid release form (nonenteric coated granules) were studied and complex granules consisting of 40% of nonenteric coated granules and 60% of enteric coated granules which dissolve at pH 6 were chosen as a prolonged action preparation of CCL. Bactericidal activity of the prolonged action preparation (S6472) was confirmed to be the same as that of the conventional preparation by comparative viable cell count study in which concentrations of CCL simulated to plasma concentrations following the administration of S6472 at the dosage of 375 mg b.i.d. and the conventional preparation at the dosage of 250 mg t.i.d. were used. From the above, S6472 is considered to be a prolonged action preparation of CCL which serve our purpose. Since S6472 can be given with the twice-a-day administration, its daytime administration is not necessary. Therefore, S6472 is considered to be much useful preparation for the patients.

[A case of complete regression of gastric carcinoma and liver metastases by treatment with tegafur].

The patient was a 70-year-old man who had complained of epigastric discomfort since March 1980. In April his liver was found to be enlarged on examination by his family doctor, and he was referred to our hospital. By X-ray and endoscopic examination a carcinoma of Borrmann type 2 was found in the lesser curvature of the body of the stomach. Biopsy of the tumor disclosed papillotubular adenocarcinoma. multiple liver metastases were demonstrated by computed tomography. From May 1980 tegafur (FT-207, 750 mg) in the from of a suppository was given daily. During the course of endoscopic follow-up the shape of the gastric carcinoma changed to one of an irregular shallow ulceration and biopsy failed to reveal cancer cells in August 1980. In April 1981 only an ulcer scar remained in the stomach. In February 1981 computed tomography showed small low-density areas in the liver, but these had completely disappeared by June 1982. Starting in August 1980, tegafur suppository was given on alternate days until June 1984. By this time there had been no recurrence of the carcinoma either in the liver or in the stomach, and the chemotherapy was discontinued. By the WHO standard for cancer chemotherapy this patient therefore corresponds to the class of complete response.

Intravenous lidocaine as a suppressant of coughing during tracheal intubation.

Effects of intravenously administered lidocaine on cough suppression during tracheal intubation under general anesthesia were evaluated in two studies. In study 1, 100 patients received either a placebo or 0.5, 1.0, 1.5, or 2.0 mg/kg lidocaine intravenously 1 min before tracheal intubation. All visible coughs were classified as coughing. The incidence of coughing decreased as the dose of lidocaine increased. A dose of 1 mg/kg or more of intravenous lidocaine suppressed the cough reflex significantly (P less than 0.01). Coughing was suppressed completely by 2 mg/kg of intravenous lidocaine. In study 2, 108 patients received 2 mg/kg lidocaine intravenously or a placebo 1, 3, 5, 7, 10, or 15 min before intubation. The same criteria for determining whether a patient did or did not cough during tracheal intubation were used as in study 1. The incidence of coughing decreased significantly (P less than 0.01) when 2 mg/kg of lidocaine was injected intravenously between 1 and 5 min before our attempting intubation. Cough reflex was suppressed completely by plasma concentrations of lidocaine in excess of 3 micrograms/ml.

Macrophage activation with pantethine and pantetheine-4'-phosphate.

Adjuvant activities of pantethine (PaSS) and pantetheine-4'-phosphate (PSH-4'-P) were investigated in mice. By the multiple intraperitoneal administration, both PaSS and PSH-4'-P activated the functions of mouse peritoneal adherent cells and splenic natural killer cells. PSH-4'-P was also effective for the activation of natural killer cells by single injection. In in vitro, PaSS induced interleukin-1 (IL-1) secretion at a low concentration but PSH-4'-P did not. Both PaSS and PSH-4'-P could neither induce interleukin-2 (IL-2) secretion, nor could enhance IL-2 secretion by Con A.

[A case of cavernous hemangioma of the testis].

Non-germinal cell tumor of the testis is a rare disease, and vascular tumor of the testis is a still rarer disease. Herein, a case of cavernous hemangioma of the testis is reported. A 75-year-old man consulted our department with the complaint of a painless left intrascrotal tumor. Laboratory findings revealed slight leukocytopenia and thrombocytopenia of unknown origin. In chest X-ray, a diffuse reticular shadow was shown and it was considered due to pulmonary fibrosis, but, alpha-fetoprotein and CEA were normal. Left radical orchiectomy was performed under spinal anesthesia. The tumor existed under the tunica albuginea, and the cutting surface of the tumor was brown and irregular. There were hemorrhagic portions in some places. The left epididymis and the left spermatic cord were normal. Histologically, the tumor was diagnosed as cavernous hemangioma. The vascular tumor of the testis is a very rare disease, and only 17 cases have been reported including this case. This case was the 7th case of cavernous hemangioma of the testis, and the first case in Japan. This patient died of respiratory failure due to pulmonary fibrosis and pneumonia. In the autopsy, there was no abnormal finding that was considered to be related to the cavernous hemangioma of the testis.

Molecular species of monoglycosylceramides in embryonic and adult Japanese quail intestine.

Molecular species of monoglycosylceramides from embryonic and adult Japanese quail intestine were separated without derivatization by high performance liquid chromatography and analyzed by fast atom bombardment-mass spectrometry. The reversed-phase chromatography on an octadecyl silica beads column separated the lipids by the difference in their ceramide moieties which contain considerable amounts of hydroxy fatty acids and 4D-hydroxysphinganine (phytosphingosine). The phytosphingosine and sphingosine in the ceramide moieties of the isolated monoglycosylceramide fraction gave characteristic fragment ions of (M - 162)H+ for the phytosphingosine, and (M - 18)H+ for the sphingosine components. The presence of phytosphingosine and hydroxy fatty acids in the ceramide moieties of the monoglycosylceramide early in the embryonic stage necessarily excludes the possibility of their exogenous origin such as bacterial flora and nutrients.

Ibuprofen in the prevention of experimentally induced postoperative adhesions.

Using a five-dose 70 mg/kg regimen, of ibuprofen significantly reduced both overall adhesion formation and the severity of adhesions when given to rabbits in the immediate postoperative period. This reduction was apparently time- and dose-related, as animals given smaller doses of ibuprofen at subsequent intervals had a tendency toward more adhesions and also more severe adhesions. A positive correlation was found between the severity of adhesions and the formation of new glycosaminoglycans and collagens.

Studies on relationship between cysts and granulomas in murine cryptococcosis.

The histopathology of murine cryptococcosis was observed until the 55th day and particular attention was paid to whether or not cysts, which had been formed in the brain, could change to granulomas. Cryptococcus neoformans RIB-12M was used in this experiment. As experimental animals, five-week-old male BALB/c mice, weighing 20-22 g, were used. An infective inoculum was prepared by adjusting the number of cryptococci to 10(6) or 5 X 10(6)/0.2 ml. Each mouse was inoculated intravenously with 0.2 ml of the cell suspension, and the colony forming unit of the brain and liver, and the histopathological findings in various visceral organs were investigated. 40 X 10(4) colonies grew from 100 mg of the brain tissue of the eighth day. Thereafter, the number increased gradually. It reached 500 X 10(4) on the 20th day. The colony forming unit from the liver reached a peak on the 12th day (250 X 10(4] and thereafter the number decreased gradually. Histopathologically, the brain and liver were severely affected with the fungus. In the brain cysts with cryptococci continued to increase until the end of the experiment. On the other hand, in the liver several purulent foci appeared on the second day. On the eighth day numerous mononuclear cells accumulated at the foci and their lesions changed to granulomatous ones with cryptococci. The number of granulomatous lesions reached a peak on the 16th day in the mice inoculated with 5 X 10(6) cryptococci, and thereafter showed a tendency to decrease gradually.
