Ultrastructure of the myocardium in acute febrile mucocutaneous lymph node syndrome.

A fine structure of the myocardium was examined in the acute febrile mucocutaneous lymph node syndrome, which was recently prevailing in Japan. The right ventricular myocardial cells in ten cases were observed through endomyocardial biopsies. On light microscopy, accumulation of histiocytes was demonstrated in some cases. Electron microscopic examinations revealed myofibrillar changes, increased glycogen contents and other degenerative changes. On the myocardium of a case in which a biopsy was performed during the active phase of the disease, these changes were prominent. However, these changes were also observed for some months to some degree.

Sipple's syndrome with peculiar changes in pancreatic islets--an autopsy case.

A diagnosis of the heritable disorder Sipple's syndrome was made in a Japanese male aged 28 years. The coexistence of bilateral phenochromocytomas, bilateral medullary thyroid carcinomas and secondary hyperplasia of parathyroid was confirmed at the time of autopsy. Pancreatic islets were hyperplastic with marked proliferation of A and D cells. Transition of the ductal cell to the islet, i.e. "nesidioblatosis" was observed. There was no proliferation of B cells, but a retention of B cell granules, a manifestation of suppressed secretion of insulin attributed to the overproduction of catecholamines was evident. In the stomach, numerous petechial hemorrhages and proliferation of gastrin cells were found. The pathogenesis of changes in the pancreatic islets and stomach is discussed from the viewpoint of hormonal disorders induced by pheochromocytoma and medullary thyroid carcinoma such as are found in Sipple's syndrome.

Hypokalemia and metabolic alkalosis resulting from overuse of magnesium oxide.

A 23 year old woman had taken large doses of magnesium oxide (up to 20 to 30g per day) initially for habitual constipation and later for idiopathic edema instead of diuretics, until the concealed abuse was discovered. During the abuse she showed hypokalemia and metabolic alkalosis. When the ingestion was stopped, the electrolyte imbalance recovered within two weeks.

Congenital left atrial enlargement. A case report with special reference to myocardial fine structure.

A case of intrapericardial congenital left atrial enlargement was reported. The patient is an asymptomatic 3 year-old girl with abnormal cardiac silhouette seen on a chest roentgenogram. Angiography showed marked left atrial enlargement, and the diagnosis of intrapericardial congenital left atrial enlargement was confirmed on operation. Specimens of the atria were electron microscopically examined. Myocardiocytes of the left atrium showed marked increase of the mitochondria and decrease of myofibrils. Abnormal Z bands and disarrangement of myofibrils were often observed. Atrial specific granules in the left atrium were increased in size and number. The right atrium also showed same changes. The pathogenesis of this condition was discussed.

Cardiac involvement and prognosis in acute mucocutaneous lymph node syndrome.

Cardiac involvement and prognosis were studied in the acute febrile mucocutaneous lymph node syndrome which recently has been found among Japanese infants and younger children. The reason for its particular predilection for the main coronary arteries and the pathologic process are discussed in relation to the pathogenesis and nutrient of the main coronary arteries.
