[Analysis of the result of thyroid microsomal particle agglutination test by ELISA].

No accurate method to detect thyroid microsomal (MC) antibody (Ab) in serum has been generalized. In this study, the titer of MC Ab obtained by the method of MC autoantibody particle agglutination (MCPA) was analyzed by enzyme linked immunosorbent assay (ELISA). MC and thyroglobulin (Tg) were prepared from Graves' thyroid. ELISA was done by coating the plate with MC, adding Tg to buffer and using peroxidase-conjugated anti-h IgG. 1) The titer of MCPA correlated with the MC Ab ELISA index in serum without Tg Ab, but it did not in serum with Tg Ab. MC Ab was negative by ELISA while it was positive by MCPA in some of the sera with Tg Ab. 2) When ELISA was done using buffer without Tg, the amount of IgG bound to MC was greater in serum with TGPA: + and MCPA: - than in serum with MCPA: + and TGPA: -. 3) The zone phenomenon observed in MCPA did not always indicate an excess of MC Ab. 4) MC Ab was positive by ELISA in some of the negative MCPA sera obtained from patients with Hashimoto's disease in which diagnosis was confirmed by biopsy. In conclusion, the result obtained in MCPA now in use is strongly influenced by Tg Ab. Furthermore, since binding of Ab to MC is judged by agglutination of particles in MCPA, ELISA is superior in sensitivity and accuracy in detecting MC Ab.

[Two cases of Mycoplasma pneumoniae pneumonia with obstructive ventilatory disturbance].

Mycoplasma pneumoniae (M. pneumoniae) pneumonia is usually considered as a benign disease, but in rare cases, the disease is fatal. Fatal cases requiring mechanical ventilation and many extrapulmonary complications have been reported. Cases accompanied with obliterative bronchiolitis also have been reported. Two cases of M. pneumoniae pneumonia with obstructive ventilatory disturbance were reported. In both cases clinical diagnosis of M. pneumoniae pneumonia was confirmed by a rise in the complement fixation antibody titer, a rise of the cold hemagglutinin titer and isolation of M. pneumoniae from sputum and pharynx. In one case we performed transbronchial lung biopsy (TBLB). The lung biopsy specimen showed bronchiolitis, so histological findings were thought to be compatible with ventilatory function. In both cases arterial oxygen pressure (PaO2) was below 60 Torr, by administration of erythromycin (EM) and adrenocorticosteroids, chest X-ray findings, atrial blood gas analysis findings and ventilatory function were improved.

[A case of proliferative sparganosis associated with PIE syndrome and pulmonary embolism].

A 48-year-old man was admitted because of ischialgia. The CT scanning revealed tumors in the right pelvic cavity and the gluteal muscle. The bodies of Sparganum proliferum and granulation tissues were revealed by the tumor biopsy. In this hospital course, the chest roentgenogram revealed patchy infiltrative shadows in both lungs and left pleural effusion while chest CT scanning revealed multiple nodular shadows. Eosinophils increased in the peripheral blood and transbronchial lung biopsy (TBLB) showed non-destructive vasculitis with eosinophilic infiltration. The pulmonary lesion was diagnosed as intrapulmonary migration of Sparganum proliferum and PIE syndrome caused by this worm. Thiabendazole and prednisolone were administrated, but he died of pulmonary embolism.

Effect of hyperglycemia on hepatic extraction of insulin in totally pancreatectomized dogs.

Using the glucose clamp technique, we investigated the effect of blood glucose levels on hepatic extraction of insulin in pancreatectomized dogs. A primed continuous infusion of regular insulin was administered at a rate 1.5 mU/kg body wt./min into the portal or peripheral vein and the blood glucose level was clamped at hyperglycemia (over 200 mg/dl) or euglycemia (below 100 mg/dl) by a variable infusion of glucose through the saphenous or portal vein. Plasma insulin concentrations were measured during portal or peripheral insulin infusion, as were the metabolic clearance rates of insulin during insulin infusion via each route. Hepatic extraction of insulin was calculated from the two metabolic clearance rates using the insulin kinetic model (Pilo' method). Hepatic extraction of insulin during the hyperglycemic clamp was 45.8 +/- 2.5%, significantly higher than that during the euglycemic clamp (35.7 +/- 2.8%: p less than 0.01). When glucose was infused into the portal instead of the peripheral vein during the euglycemic clamp, hepatic extraction of insulin increased (44.7 +/- 3.4 vs 37.6 +/- 2.9%: p less than 0.05). It was concluded that hyperglycemia increases the hepatic extraction of insulin. Moreover, portal hyperglycemia during the euglycemic clamp also increases the hepatic extraction of insulin. It is suggested that portal glucose concentration may affect the hepatic extraction of insulin.

[Productive aspergilloma on the inner wall of a cavity].

Fungus ball type pulmonary aspergilloma is easily recognizable on chest X-ray. This is a report on "productive aspergilloma on the inner wall of a cavity" (PAIC), named from our clinical experience in 1980, which grew to form aspergillus strata on partial or overall inner walls of the cavity. Our 5 cases with this type supported the existence of this type and revealed an 18% incidence in patients with pulmonary aspergilloma, characteristic clinical features and immunological reactions, thus pointing to the necessity of early diagnosis and treatment for this type of aspergilloma. The species of aspergillus cultured from the patients with this type of disease was Aspergillus fumigatus.

[Successful treatment of a patient with fulminant psittacosis].

A previously healthy 40-year-old woman was admitted with severe dyspnea, cough and slight fever. Chest X-ray film revealed bilateral widespread opaque infiltration with ground glass shadows around it. The laboratory examination showed moderate hepatic and muscular injury with disseminated intravascular coagulation. In addition her arterial blood gas showed severe hypoxemia (PaO2: 25 Torr under room air). Moreover, about 1 week prior to admission, 2 baby budgerigars she had been raising for half a year died. Because of this history and multi-organ injuries, this disease was considered to be acute pneumonia owing to fulminant psittacosis causing acute respiratory failure. On the first day of admission, she was intubated and ventilated mechanically with an oxygen concentration (FIO2) of 100%. Subsequently, treatment with intravenous minocycline (400 mg/day), heparin for D.I.C. and corticosteroid were started. Abnormal findings in both chest X-ray and several laboratory parameters improved gradually though fever continued for a week. On the 14th day of her hospital stay, she was weaned from the ventilator successfully and the administration of corticosteroid and heparin tapered. On the 41st day, she was discharged without any symptoms. Results of complement fixation (CF) antibodies against chlamydia on paired sera showed a significant rise from 1:32 to 1:256. Moreover, both IgG and IgM antibodies for Chlamydia psittaci with microplate immunofluorescent antibody technique (MFA) showed an 8 times' rise during 10 days after admission. The definitive diagnosis was made with positive isolation of C. psittaci from both the throat swab of this patient and the spleen and liver of the dead budgerigar by the cell culture method. Psittacosis should always be borne in mind as a possible cause of fulminant pneumonia with acute respiratory failure, and such a situation can be handled successfully if emergency care including mechanical ventilation is available.

[Successful treatment with a small dose of N4-behenoyl-1-beta-D-arabinofuranosylcytosine in a case of chronic myelomonocytic leukemia in blastic crisis].

A 56-year-old female was admitted for the examination of leukocytosis in May, 1982. The hematological examination showed hemoglobin 12.0g/dl, platelets 14.5 X 10(4)/microliters and leukocytes 18,000/microliters with 28% of monocytes. A bone marrow aspiration revealed granulocytic hyperplasia. Granulocytes showed nuclear abnormalities, such as folding or lobulation. From these data, a diagnosis of chronic myelomonocytic leukemia (CMMoL) was made and followed with no treatment for 2 years. However, fever, bone pain, anemia, thrombocytopenia and the increase of monoblasts in the peripheral blood and bone marrow were observed in May, 1984. These findings indicated that she was in the blastic phase. She was treated by intensive combination chemotherapy (BHAC-AMP), but did not attain any remission. Therefore, a small dose of N4-behenoyl-1-beta-D-arabinofuranosylcytosine (BH-AC; 1 mg/kg/day) was administered for 70 days. As a result, complete remission was obtained and continued for 37 months with the same therapy. The experience of this case suggests that small dose of BH-AC could be applied to a case of CMMoL in blastic crisis.

[Changes in serum RNase activities in chronic phase and acute crisis of chronic myelocytic leukemia].

We investigated serum acid and alkaline RNase activities in 16 cases of acute crisis of chronic myelocytic leukemia (CML), 21 cases of untreated CML, and 13 cases of treated CML, to clarify clinical significance of the determination of RNase activities in acute crisis of CML. We obtained results as follows; the ratio of acid to alkaline Rnase activities (Ac/Al ratio) of chronic phase of CML was 1.64 +/- 0.47 (mean +/- SD) in the untreated cases, and was 1.32 +/- 0.16 in the treated cases. The Ac/Al ratio always indicated over 1.0 in the chronic phase of CML without any relationship to treatments. On the other hand, in the cases of acute crisis, the Ac/Al ratio was significantly lowered (0.94 +/- 0.22) as compared to the chronic phase of CML (p less than 0.001), and was similar to that of acute leukemia. The acid and alkaline RNase activities of the blast from the patients with acute crisis of CML showed remarkably lower value than those of leukemic cells from patients with chronic phase of CML. Therefore, it was Suggested that the return to normal range of Ac/Al ratio in acute crisis of CML depended on marked decrease of RNase activities of blasts. Thus, serial determinations of the enzyme activities are considered to be one of useful tools for prediction of acute crisis of CML.

A case of left atrial automatic tachycardia successfully treated with aprindine.

Electrocardiogram in a 15-year-old girl showed persistent supraventricular tachycardia at rates of 130 to 140 bpm. Electrophysiological study confirmed left atrial automatic tachycardia, which was transferred to sinus rhythm by intravenous infusion of aprindine (100 mg/10 min). Therefore, aprindine (60 mg/day) was administered orally, and neither recurrence of left atrial automatic tachycardia nor side effects were observed during subsequent follow-up period of 16 months.

High-performance liquid chromatographic determination of formate as benzimidazole in biological samples.

Formate was determined as benzimidazole by high-performance liquid chromatography after reaction with o-phenylenediamine at 130 degrees C for 2 h in 1 M perchloric acid. The useful concentration range was 1.6-40 mumol/l and the determination limit was 20 pmol. The recoveries from rat liver homogenate and human urine were 90.3 +/- 2.9 and 89.4 +/- 2.5%, respectively. Using this method, the activity of formaldehyde dehydrogenase in biological samples could be measured, and also the formate concentration in the liver and urine of rats to which methanol had been administered.

Augmented antitumor effect of recombinant human interleukin-1 alpha by indomethacin.

In murine syngeneic tumor models, the antitumor effect of recombinant human interleukin-1 alpha (rHu IL-1 alpha) was significantly augmented by oral coadministration of indomethacin (IND). The augmentation was more or less observed by various routes of rHu IL-1 alpha (i.m., i.v., and intratumoral routes), against various tumors (Meth A sarcoma, colon 26 adenocarcinoma, B16 melanoma, and Lewis lung carcinoma) and irrespective of administration timings (in early and late stages of tumor growth). This results suggests that prostaglandin E2 produced by host cells in response to rHu IL-1 alpha and/or by tumor mass might interfere with the antitumor activity of rHu IL-1 alpha and also that cyclooxygenase inhibitors such as IND might counteract such interference. In the combination of rHu IL-1 alpha with IND, its efficacious doses (5-50 micrograms/kg) against murine tumors were at least 300-3000 times lower than its median lethal dose (more than 15 mg/kg). In addition, IND partially prevented the loss of body weight attributed to rHu IL-1 alpha injections at relatively high doses. Combined use of rHu IL-1 alpha with IND seems to be desirable from both therapeutic and toxicological viewpoints.

Interaction of nitrite with catalase in the perfused rat liver.

The interaction of nitrite with catalase was investigated spectrophotometrically in the perfused rat liver. Real-time spectral changes were obtained using a reflectance scanning spectrophotometer in the liver perfused with a haemoglobin-free medium in a non-recirculating system. Administration of sodium nitrite caused a specific pattern of spectral change indicating the decomposition of catalase compound I to free catalase. The spectral change due to the interaction with nitrite did not occur during potassium cyanide or ethanol infusion, nor in the aminotriazole-pretreated rat liver. The spectral change was observed at concentrations of nitrite in the perfusate over 0.01 mM, and the K0.5 value (the concentration producing half the maximum spectral change) was 0.06 mM. It was concluded that relatively low concentrations of nitrite caused decomposition of catalase compound I in the physiologically functioning liver cell.

Discriminant analysis of promoter regions in Escherichia coli sequences.

We have previously developed a general method based on the statistical technique of discriminant analysis to predict splice junctions in eukaryotic mRNA sequences [Nakata, K., Kanehisa, M. and DeLisi, C. (1985) Nucleic Acids Res., 13, 5327-5340]. In order to evaluate further applicability of this method, we now analyze the promoter region of Escherichia coli sequences. The attributes used for discrimination include the accuracy of consensus sequence patterns measured by the perceptron algorithm, the thermal stability map, the base composition and the Calladine-Dickerson rules for helical twist angle, roll angle, torsion angle and propeller twist angle. When applied to selected E. coli sequences in the GenBank database, the method correctly identifies 75% of the true promoter regions.

In vitro activities of T-3262, NY-198, fleroxacin (AM-833; RO 23-6240), and other new quinolone agents against clinically isolated Chlamydia trachomatis strains.

The in vitro activities of three newly developed quinolone drugs (T-3262, NY-198, and fleroxacin [AM-833; RO 23-6240]) against 10 strains of clinically isolated Chlamydia trachomatis were assessed and compared with those of other quinolones and minocycline. T-3262 (MIC for 90% of isolates tested, 0.1 microgram/ml) was the most active of the quinolones. The NY-198 and fleroxacin MICs for 90% of isolates were 3.13 and 62.5 micrograms/ml, respectively.

Biosynthesis of leukotriene B4 in bovine lens.

Ocular allergic reactions and inflammatory cascades in the eye involve various arachidonic acid metabolites. One such metabolite, leukotriene B4 (LTB4) was detected in bovine lenses after incubation in Tyrode's solution and analyzed by reverse phase and normal phase high-performance liquid chromatography (HPLC) and gas chromatography-mass spectrometry (GC-MS). LTB4, a product of the lipoxygenase pathway has a chemotactic potency, and may play an important role in the inflammatory cascade. Our results demonstrate the capacity of the bovine lens to synthesize LTB4. It is possible that LTB4 synthesized in lenses under some conditions plays an important role in ocular inflammatory and allergic reactions.
