[Augmentation of chemotherapeutic efficaciousness of UFT by oral l-leucovorin--growth-inhibitory activity of combination against human tumor xenograft].

Combination chemotherapy with FUra and LV has been reported as a useful treatment for patients suffering from colon carcinoma. Usually, both FUra and LV are administered by intravenous infusion, but not orally. UFT, an anti-neoplastic agent consisting of FT and uracil, is widely used for oral administration in Japan. Using human tumor xenografts of 10 cell lines, we evaluated the efficacy of UFT combined with l-LV, which is the active form of LV, by oral administration. Combined treatment of UFT with l-LV was more effective than UFT alone on the growth suppression of colon carcinoma (KM 20 C, Col-1) and mammary carcinoma (H-31, MX-1). When 1.85 mg/kg (5.55 mg/m2) of LV was given to tumor bearing mice, the antitumor activity of UFT was augmented and at a dose of 5.56 mg/kg (16.7 mg/m2) of LV, it was significantly augmented. Among various 5-FU derivatives, such as UFT, 5'-DFUR or FUra, combined treatment using UFT with l-LV was the most effective by oral administration. l-LV did not improve the anti-tumor efficacy or toxicity of 5'-DFUR. l-LV seemed to augment the anti-tumor activity of FUra, but not significantly. These results suggest that combination chemotherapy of UFT with LV is a promising approach for the clinical treatment of human colon cancer.

[Clinical analysis of ureteral injuries].

Six cases of ureteral injuries were analyzed. Two cases were male and 4 cases were female. The right side was affected in 1 case and the left in 5 cases. The affected portion was the uretero-pelvic junction in 1 case and lower iliac vessels in the other cases. Five cases were iatrogenic injuries in surgical or gynecological pelvic operation, and the other was due to blunt trauma. Intraoperative diagnosis was made in 4 cases. Drain leakage led to diagnosis in one case and pyelonephritis in another. Ureteroureterostomy was performed in 2 cases, pyeloureterostomy in 1 case, ureteroneocystostomy with psoas hitch in 1 case, Boari bladder flap in 1 case and simple suture in 1 case. 4-0 or 5-0 Dexon with an atraumatic needle was used for all anastomoses. Except for case 1, who died of the recurrence of carcinosarcoma, renal functions were preserved in all patients during the follow-up period ranging from 5 months to 7 years 8 months.

[Surgical treatment and long-term results of cardiac myxoma in 38 patients].

Long-term results of thirty-eight patients with cardiac myxoma, who underwent resection at National Cardiovascular Center between May, 1978 and December, 1993, were reviewed focussing on embolism, cerebral complication, tumor recurrence and metastasis. The mean age was 49.6 years (range 0 to 74) in 20 female and 18 male. Overall follow-up were known for 35 of 38 patients (92.1%), and an average of follow-up period was 6.7 years (range 0.5 to 15.9 years). 15 of 38 patients (34.2%) had embolism (brain 10, peripheral artery 4, coronary 1) preoperatively. All patients survived operation and the hospital mortality rate was 0%. Long-term results were almost satisfactory (myxoma related-death in one patients). However, distant metastasis occurred in two patients, and tumor recurrence in one patient irrespective of a presence of preoperative embolism. Cerebral embolism occurred in one patient and cerebral arterial lesins were noticed in five patients postoperatively. Cardiac myxoma has a malignant potentiality and a close follow-up is necessary about recurrence, metastasis and cerebral artery disease.

[Interleukin-2 activity in monocrotaline-treated rat lungs].

To study the relationship between cytokines and the development of monocrotaline-induced pulmonary hypertension, monocrotaline was given and interleukin-2 was measured. The amount of interleukin-2 produced by pulmonary cells obtained by bronchoalveolar lavage was high only on the 14th day after monocrotaline injection (p < 0.01). This suggests that many kinds of cells in the monocrotaline-treated rat lung were activated by cytokines, and that these interactions play an important role in the development of monocrotaline-induced pulmonary hypertension in rats.

[Changes in smooth muscle cell proliferation in pulmonary arteries of rats given monocrotaline].

To study the relationship between proliferation of smooth muscle cells and structural changes in arteries of rats given monocrotaline, light microscopy with immunohistochemistry for bromodeoxyuridine (BrdU) was done after a single subcutaneous injection of monocrotaline. In increase in DNA synthesis in smooth muscle cells was seen from 3 days until 14 days after administration. Since this increase was seen before pulmonary hypertension and medial thickness developed some mechanisms that induce proliferation of pulmonary atrial smooth muscle may have been triggered soon after monocrotaline injection.

Classification criteria for polymyositis and dermatomyositis.

OBJECTIVE: The establishment of classification criteria for polymyositis (PM) and dermatomyositis (DM). METHODS: Questionnaires inquiring about patients with DM, PM, systemic lupus erythematosus, progressive systemic sclerosis and noninflammatory neuromuscular diseases were distributed to the main medical institutes in Japan. Data were collected and analyzed by computer. RESULTS: Among skin lesions of DM, heliotrope rash, Gottron's sign and erythema or purpura on the extensor surfaces of the extremity joints were shown to be distinguishing criteria. In both DM and PM, proximal muscle weakness, muscle grasping and spontaneous pain, nondestructive arthritis or arthralgia, elevated CK or aldolase level, presence of systemic inflammatory signs, myogenic changes on EMG, positive and anti Jo-1 antibody and pathologic findings compatible with inflammatory myositis were distinguishing criteria items. CONCLUSION: When a patient satisfies one of 3 skin lesion items and at least 4 other items, he or she shall be classified as having DM, sensitivity 94.1%. When a patient satisfies at least 4 items other than skin lesion items, he or she shall be classified as having PM, sensitivity 98.9%. Specificity of DM and PM is 95.2%.

[Regional work of the left ventricle before and after CABG].

To evaluate the effects of CABG on regional function of the left ventricular wall, regional work of the left ventricular anterior wall was calculated before and after CABG in 8 patients with multivessel coronary artery disease. In all patients, including CABG for LAD branch, complete revascularization was performed. Regional work of the left ventricular wall was calculated as RW (mJ/cm3) = - integral of sigma d epsilon, where sigma = wall stress and epsilon = regional strain (1n (1/H): H = wall thickness). A major and a minor axis, and anterior wall thickness during cardiac cycle were obtained from the left ventriculogram. Preoperatively, the RW varied widely (from -0.9 to 13.3 mJ/cm3). In 4 cases, the RW decreased and in the other cases, it increased. The RW converged in the tight range (from 2.5 to 6.7 mJ/cm3), postoperatively. These results suggest a uniformalization of the RW in the different regions of the left ventricle was established after the complete revascularization. We conclude that the RW obtained from the analyses of sigma-epsilon relationship is a useful index for evaluating the effects of CABG on the treated region of the left ventricle.

[Saphenous vein to saphenous vein bypass grafting in four redo CABG patients].

In four redo CABG patients, new saphenous vein grafts were anastomosed to the angiographically normal part of the old, and partially narrowed or occluded saphenous vein grafts in an end-to-side fashion. Inner surface of the old saphenous vein was confirmed to be smooth and free of atherosclerotic change during anastomosis. Postoperative course was uneventful and angiography demonstrated the good patency of new saphenous vein grafts in all patients. Although this technique is not acceptable if the old saphenous vein graft shows extensive atherosclerotic change, this method may be a useful alternative in case of limited saphenous vein graft disease in terms of less difficulty and operation time.

Hepatic resection for hepatolithiasis and long-term results.

BACKGROUND: Hepatic resection is an established procedure for treating patients with hepatolithiasis. The long-term results after hepatic resection for hepatolithiasis were appraised. STUDY DESIGN: Of 127 patients with hepatolithiasis seen between 1973 and 1993, 43 patients underwent hepatic resection, and constituted the basis of this study. RESULTS: There was one operative death (operative mortality rate of 2.3 percent) and three deaths as a result of concomitantly associated cholangiocellular carcinoma. Of the remaining 39 patients, the complete stone clearance rate was 67 percent with operation alone, but reached 87 percent when cholangioscopy was used. Operative morbidity was recorded in five patients (12 percent), but they recovered with conservative therapy. Stones recurred in 15 percent of patients after a mean follow-up period of four years (range of three to six years). Eleven (28 percent) of 39 patients died after hepatectomy as a result of related diseases after a mean follow-up of 6.2 years. These 11 patients had associated biliary drainage procedures. They experienced a higher mortality rate (p < 0.05) than patients who did not have biliary drainage. CONCLUSIONS: We conclude that hepatic resection is adequate treatment for hepatolithiasis. The patients having additional biliary drainage procedures had a higher mortality, but one cannot reach conclusions about the negative effect of the drainage procedures because the drainage procedures were done in a selected and not a randomized manner.

[Results of coronary artery bypass grafting in dialysis patients].

To determine the short- and medium-term results of coronary artery bypass grafting (CABG) in dialysis patients, we analyzed a group of 14 patients with chronic renal failure who underwent CABG between May 1990 and October 1994. Two patients had concomitant valve repair for mitral regurgitation. Hospital mortality was 14% (2 out of 14). These two patients died of ileus due to ischemic colitis and agranulocytosis respectively. There was one late death from stroke. The four significant postoperative complications (morbidity 29%) were composed of two sternal dehiscence, one cardiac tamponade because of bleeding, and one perioperative myocardial infarction. Graft patency rate was 97% (34 out of 35 in 13 patients) within one month. Actuarial survival was 86% at one 1 to 3 years, and 43% at 3 and a half years. This rate is not significantly different from all dialysis patients, but night be better than dialysis patients with coronary artery disease who had not undergone CABG in the previous reports. Left ventricular size is larger in patients who died or who had significant complications in hospital than in patients with uneventful postoperative course. Cardiac arrest time, cardiopulmonary bypass time, chest tube output, and the amount of transfusion might be also related to mortality and morbidity though statistically not significant.

[Evaluation of surgical procedures based on the types of hypertrophied obstructive cardiomyopathy].

The surgical procedures were reviewed based on the types of hypertrophied obstructive cardiomyopathy in 20 patients. The patients were classified in three groups: (1) Asymmetric septal hypertrophy (ASH), (2) Atypical or diffuse concentric hypertrophy (DIF), (3) Mid ventricular obstructive hypertrophy (MID). Among 11 patients with ASH, the ventricular septal myotomy (My) or the myotomy-myectomy (Mye) were performed in 9 patients and MVR was performed in 2 patients. Among 6 patients with DIF, My or Mye were performed in 4 and MVR was performed in 2. All three patients with MID were treated with MVR. The intraventricular systolic pressure gradient greater than 40 mmHg was still existent postoperatively in one ASH patient on whom Mye was performed and in 3 DIF patients on whom My or Mye were performed. The four patients had III degree mitral regurgitation preoperatively and were successfully treated. The postoperative NYHA functional class became II or less in all patients. For ASH, the My was effective in 9 patients except one earlier patient. For DIF, further consideration of surgical procedure might be required. For MID, a MVR is recommended.

Role of histamine produced by macrophages in mouse bone marrow.

Promyelocytic HL-60 cells differentiated into mature cells when they were cultured in the presence of dimaprit (10(-4) M), a histamine H2 agonist. An injection of Escherichia coli lipopolysaccharide increased the activity of histidine decarboxylase in bone marrow cells in C3H/HeN mice to a much greater extent than in C3H/HeJ mice, which are resistant to various effects of lipopolysaccharide. Histamine production increased concomitantly. In WBB6/F1 (W/W(v)) mice, which are genetically deficient in mast cells, histidine decarboxylase activity increased more than in C3H/HeN mice. Pure (>99% nonspecific esterase, CD14 and Mac-1 positive) macrophage populations were obtained from long-term culture of the bone marrow cells (bone marrow-derived macrophages, BMDM). Culture of the cells in the presence of lipopolysaccharide caused a slight, but dose-dependent increase in histidine decarboxylase-associated histamine synthesis. Granulocyte/macrophage colony-stimulating factor (rmGM-CSF) or interleukin 3 (rmIL-3) potently increased lipopolysaccharide-induced histamine formation.

Synthesis of neoglycolipids containing a mucin-type core unit.

The unnatural glycolipids O-beta-D-galactopyranosyl-(1-->4)-O-(2-acetamido- 2-deoxy-beta-D-glucopyranosyl)-(1-->3)-O-(2-acetamido-2-deoxy-alpha-D- galactopyranosyl)-(1-->1)-ceramide (1), O-beta-D-galactopyranosyl-(1-->4)-O-(2-acetamido-2-deoxy-beta-D-glucopyr anosyl) - (1-->6)-O-(2-acetamido-2-deoxy-alpha-D-galactopyranosyl)-(1-->1)-ceramid e (2), and O-beta-D-galactopyranosyl-(1-->4)-O-(2-acetamido-2-deoxy-beta-D- glucopyranosyl)-(1-->3)-O-[O-beta-D-galactopyranosyl-(1-->4)-(2-acetamid o-2- deoxy-beta-D-glucopyranosyl)-(1-->6)]-O-(2-acetamido-2-deoxy-alpha-D- galactopyranosyl)-(1-->1)-ceramide (3), and their beta-(1-->1)-linked isomers, were synthesized. The precursor oligosaccharides for 1, 2, and 3 were made by coupling O-2,3,4,6- tetra-O-acetyl-beta-D-galactopyranosyl)-(1-->4)-3,6-di-O-acetyl-2- deoxy-2-phthalimido-alpha,beta-D-glucopyranosyl trichloroacetimidate with tert-butyldiphenylsilyl 2-azido-4,6-O-benzylidene-2-deoxy-beta-D-galactopyranoside, tert-butyldiphenylsilyl 2-azido-3-O-benzoyl-2-deoxy-beta-D-galactopyranoside, and tert-butyl-diphenylsilyl O-(2,3,4,6-tetra-O-acetyl-beta-D-galactopyranosyl)-(1-->4)-O-(3,6-di-O-a cetyl-2 - deoxy-2-phthalimido-beta-D-glucopyranosyl)-(1-->3)-2-azido-2-deoxy-beta- D- galactopyranoside, respectively. These oligosaccharides were converted into their trichloroacetimidates, which were coupled with 3,2'-di-O-benzoyl ceramide. Deprotection of the coupling products gave the title compounds 1, 2, and 3.

Descending projections from the superior colliculus to the reticular formation around the motor trigeminal nucleus and the parvicellular reticular formation of the medulla oblongata in the rat.

We observed by the anterograde and retrograde tracing techniques in the rat that the lateral part of the superior colliculus (SC), where the nigrotectal fibers from the dorsolateral part of the substantia nigra pars reticulata (SNr) terminated, sent projection fibers to the reticular region around the motor trigeminal nucleus (RFmt) and parvicellular reticular formation (RFp) of the medulla oblongata, where many premotor neurons for the orofacial motor nuclei were known to be distributed. The SC neurons sending their axons to the RFmt and RFp were mainly located in the stratum griseum intermedium, and additionally in the stratum griseum profundum. Our results suggest that neuronal signals conveyed through the nigro-tecto-bulbar pathway to the RFmt and RFp may exert control influences upon oral behavior.
