Amino acids and peptides. XXIX. Synthesis of peptide fragments related to active center of eglin c and studies on the relationship between their structure and their inhibitory activity against cathepsin G and alpha-chymotrypsin.

H-Ser-Pro-Val-Thr-Leu-Asp-Leu-Arg-Tyr-OMe, corresponding to the sequence 41-49 of eglin c, inhibited human leukocyte cathepsin G and alpha-chymotrypsin. In order to gain further insight into the relationship between the structure and the inhibitory activity against cathepsin G and alpha-chymotrypsin, peptide fragments related to the above nonapeptide were synthesized by a conventional solution method and their inhibitory activities were examined. The smallest peptide which exhibited inhibitory effects on the above enzymes was H-Pro-Val-Thr-Leu-OMe, corresponding to the sequence 42-45 of eglin c.

[Morphologic transformation of renal lesions by steroid treatment and its therapeutic implication in a patient with lupus nephritis].

When we perform renal biopsy in patients with lupus nephritis (LN), we use the histopathological findings in glomerulus for the choice and dosage of steroid and immunosuppressive therapy. Recently, we experienced a case of LN who underwent three times of renal biopsy and had received various dosages of steroid. Our therapy resulted in her renal function deteriorated over 5 years. In detail, the patient was a 45-year-old female and the first renal biopsy was performed at the active stage of LN with nephrotic syndrome and serological activities. The biopsy specimen showed membranous LN accompanied by mesangial proliferation, necrosis, cellular crescents and intramembranous deposits. Thus, large dose of steroid including pulse therapy was given, and subsequently massive proteinuria and serological activities subsided gradually. However, 4.0 g of proteinuria per day remained in the patient. Accordingly, the second biopsy was done at one and half years later and it showed the regression to nearly pure membranous LN with thickening of glomerular basement membrane and only mild increment of mesangial matrix. Thus, she remained on 15 mg of daily prednisolone. Eventually, she progressed to 2.3 mg/dl of serum creatinine in renal function. The third biopsy was undertaken 5 years later and it showed sclerosing LN with tubular atrophy. Based upon these serial morphologic alterations in LN, we presumed that steroid treatment per se was not adequate for the control of this type of LN and new additional therapy including cytotoxic drugs is probably needed for the improvement of these glomerular lesions.

[Anti neutrophil cytoplasm antibody in Wegener's granulomatosis].

Autoantibodies against human neutrophil cytoplasma (ACPA) were assayed in 50 serum samples from patients with systemic vasculitic syndrome by indirect immunofluorescence method. ACPA titer was higher than 32x in 5 patients with active Wegener's granulomatosis (WG) although ACPA of low titer was found in 2 patients with AGA, 1 patient with temporal arteritis. It was confirmed that serial changes of ACPA titer in 5 patients with WG proved to be the selective marker of disease activity and the severity of renal injury in WG. It was concluded that the detection of ACPA is useful both for the early diagnosis and monitoring the activity of WG.

Synthesis of peptide fragments related to eglin c and examination of their inhibitory effect on human leukocyte elastase, cathepsin G and alpha-chymotrypsin.

Various kinds of peptide fragments related to eglin c were prepared by the conventional solution method and their inhibitory effects on human leukocyte elastase, cathepsin G and alpha-chymotrypsin were examined. Peptide (31-40) inhibited cathepsin G (Ki = 2.3 x 10(-4) M), peptide (41-49) potently inhibited cathepsin G and alpha-chymotrypsin (Ki = 4.2 x 10(-5) M and 2.0 x 10(-5) M, respectively), and peptide (60-63) inhibited leukocyte elastase (Ki = 1.6 x 10(-4) M), whereas, peptide (31-35) weakly inhibited both elastase and cathepsin G (Ki = 2.1 x 10(-3) M and 7.3 x 10(-4) M, respectively).

Coexistence of papular mucinosis and systemic amyloidosis associated with lambda-type IgD paraproteinemia.

The patient is an obese, 55-year-old woman. She noticed purpura at several sites when she was 49 years old. She visited our clinic with a chief complaint of exertional dyspnea at 51 years of age. Physical examination revealed localized edema in the left chest wall and lower abdomen with translucent papules in the center. Macroglossia, hemorrhagic macules, loss of axillary and pubic hairs, and goose-egg sized swellings of submaxillary lymph nodes were also found. Laboratory data were within the normal range except an increase of IgD and low ECG voltage. Bone-marrow puncture revealed an increase (27%) of plasma cells with some atypicality. Serum immunoelectrophoresis clarified IgD lambda-type paraproteinemia and lambda-type Bence Jones proteinemia. Histologically, edema with an increase in GAG was conspicuous in the circumscribed areas of the middle dermis. Amyloid deposition was clarified in the walls of arterioles in the deep dermis.

[Therapeutic effects and influence on the urinary enzyme activity of human urinary trypsin inhibitor (urinastatin) in cases with acute renal failure--changes in the urinary activities of NAG and AAP].

We studied the therapeutic effects of human urinary trypsin inhibitor (UTI) in 5 cases with acute renal failure, resulting from traumatic shock in 1 case, post-operative shock in 2 cases, septic shock in 1 case, and dehydration in 1 case. We administered 300,000 u/day of UTI intravenously at the initial phase of acute renal failure for 7 days. We measured the activities of urinary N-acetyl-beta-D-glucosaminidase (NAG), alanine aminopeptidase (AAP) and the activities of serum beta-glucuronidase, PMN-elastase serially. As a control, we also studied same markers in 5 cases with acute renal failure without the administration of UTI. We could obtain the following results. 1) Urinary activities of NAG and AAP were already elevated markedly at the onset phase of acute renal failure. 2) The administration of UTI caused a significant decrease of the activities of NAG and AAP in the urine as compared with those in the controls. 3) The administration of UTI caused also the significant suppression of the activities of beta-glucuronidase and PMN-elastase in the serum. These results suggested that UTI has the protective effects on the tubular epithelial cell injuries in cases of acute renal failure.

Antibodies to rheumatoid arthritis nuclear antigen (RANA) in Japanese patients with rheumatoid arthritis.

We studied antibodies to rheumatoid arthritis nuclear antigen (RANA) by the Ouchterlony method in 0.5% agarose plates, using soluble antigen extracted with 0.25 M sucrose solution from cultured Raji cells. Anti-RANA antibody was found in sera from 24 to 40 (60%) patients with rheumatoid arthritis (RA), from 4 of 20 (20%) patients with systemic lupus erythematosus (SLE), and from 2 of 30 (7%) healthy controls. When sucrose extracts from BJAB, Ramos, and JM cells were used as the cellular antigens, no anti-RANA precipitin lines were detected. Indirect immunofluorescence study, using Raji cells or human B lymphocytes transformed by EB virus as substrate tissues, demonstrated anti-RANA antibody as fine speckled nuclear staining. Although RA patients with positive anti-RANA antibody usually had high titers of anti-Epstein-Barr nuclear antigen (EBNA) and anti-viral capsid antigen (VCA) IgG antibodies, the Wilcoxon ranks sum test showed no close statistical correlation between the presence of anti-RANA antibodies and the titers of anti-EBNA or anti-VCA (IgG) antibodies. These data showed that the incidence of positivity of anti-RANA antibodies in Japanese RA patients is almost the same as that in American and European RA patients.
