[Experience with gestodene-containing hormonal contraceptive].

An oral contraceptive containing gestodene (Minulet) was examined in collaborating with the Richter-Wyeth Pharmaceutical Factory. The authors present their experiences of monitoring of 591 cycles of a hundred women between 18 and 35 years of age. There were no pregnancy and severe side effects during that period. Irregular bleeding occurred in 17.5% of women in the beginning of the treatment, however it gradually decreased and ceased by the fifth cycle. Both the length and the quantity of the withdrawal bleeding decreased by the end of the sixth cycle. During the observation there was no amenorrhoea and the dysmenorrhoea presented a decreasing tendency, expressing in per cent of the cycles. Their own data support, that the oral contraceptives containing gestodene meet requirements of today's medical science, and beyond the low hormone content they also fulfil the next demands: reliable contraceptive effect, efficacy, excellent cycle control, good tolerability and limited side effects.

Studies on the antibodies to human herpesvirus type 6 among Hungarian patients with asymptomatic HIV infection.

The occurrence and the possible role in promoting HIV infection by human herpesvirus type 6 (HHV-6) have not yet been revealed in Hungary. In different groups of patients, serum titre of IgM and IgG antibodies, as well as avidity of IgG were quantitated by indirect immunofluorescence and an enzyme-linked immunosorbent assay, using isolate U1102 of HHV-6 variant A as antigen. In 60% of HIV-seronegative adult controls, high avidity IgG antibodies were found in low titre suggesting childhood infection. In HIV-seronegative persons with high risk behaviour for HIV-infection, both IgM and low avidity IgG were frequently found in higher titre, representing either primary or frequent reinfections, or reactivation of latent HHV-6. In asymptomatic HIV-seropositive patients, high titre of high avidity IgG antibodies was predominant, proving virus infection in the near past. These results indicate the contribution of HHV-6 to immunosuppression prior to AIDS, predisposing the organism to HIV infection.

Identification of a soluble interleukin-8 inhibitor in the supernatant of polymorphonuclear leukocytes.

Interleukin-8 (IL-8) plays a crucial role in the pathogenesis of inflammatory and hyperproliferative diseases in various organs. The purpose of the present investigation was to establish whether there is any naturally occurring inhibitor of IL-8. Here we demonstrate that an IL-8 inhibitor (IL-8INH) is present in the supernatant of polymorphonuclear (PMN) leukocytes. The release of IL-8INH could be increased by stimulating the PMN leukocytes by concanavalin A. IL-81NH blocks the IL-8-induced chemotaxis and Candida albicans killing activity of PMN leukocytes and epidermal cells in vitro, and IL-8-induced neutrophil infiltration in the mouse ear in vivo. The mechanism of action of IL-8INH involves blocking of 125I-IL-8 binding to the IL-8 receptor. Binding of 125I-IL-8 to neutrophils could not be displaced by the IL-8INH, however, preincubation of 125I-IL-8 with IL-8INH increased binding inhibition, suggesting an interaction between IL-8 and the inhibitor. Crosslinking of 125I-IL-8 to IL-8INH shows that IL-8INH binds specifically to 125I-IL-8, and the IL-8INH protein has an apparent molecular weight of 52 kDa in sodium dodecyl sulfate-polyacrylamide gel electrophoresis. The partial purification of the IL-8INH on DEAE-Sephadex anion-exchange chromatography column also suggests a 50-60-kDa inhibitor protein which blocks IL-8-induced effects on neutrophils by binding to IL-8.

Iron status in a healthy population of Hungarian secondary school boys and girls.

Iron status was assessed in a sample of 103 male and 282 female students aged from 15 to 18 years attending secondary schools in Budapest. Using a ferritin model, in boys the prevalence of iron depletion, iron-deficient erythropoiesis and iron-deficient anemia was 3.9, 2.9 and 1.0%, respectively. In girls these values were 9.6, 8.2 and 2.8%, respectively. Males had better eating habits than females. Data for nutrients intake connected with iron status are discussed. The authors recommend the improvement of iron status by dietary means and oppose the indiscriminate iron supplementation.

Reduction of apoptosis of in vitro cultured lymphocytes of HIV-positive persons by N(G)-hydroxy-methylated-L-arginine and 1'-methyl-ascorbigen.

Some formaldehyde generating chemicals due to reduction of apoptosis in lymphocytes may slow down the progress of immune decline of HIV-infected individuals. N(G)-hydroxy-methylated-L-arginine (MAX) and 1'-methyl-ascorbigen (MeAsc) could enter this way the biochemical pathway of cells and affect the apoptotic process. Separated peripheral blood lymphocytes of five asymptomatic HIV-positive persons were cultured. Unstimulated, IL-2 stimulated and IL-2 stimulated plus 0.1, 1.0, 10.0 microg/ml MAX or MeAsc treated lymphocytes were investigated for apoptosis morphologically (HE) and by flow cytometrical DNA fragmentation method. IL-2 stimulation lowered the apoptotic rate in lymphocytes of HIV-positive persons related to unstimulated ones. MAX and MeAsc reduced the apoptotic activity of stimulated lymphocytes in the least or the middle doses while in the higher dose did not. MAX and MeAsc reduced the apoptotic activity of stimulated lymphocytes originated from HIV-positive patients in vitro. This compounds may have the same effect in vivo and may prolong the symptomless period of HIV-infected patients. The role of methylation and production of formaldehyde in this process is discussed.

Lipid status in adolescents born with low birth weight.

The study compared anthropometric parameters and lipid status of adolescent boys and girls (14-16 y) born with LBW or ABW. Values for body weight and height of LBW adolescents were lower than those for ABW's. BMI differed significantly between LBW and ABW boys. In LBW boys, serum LDL-C level was somewhat lower, and the ApoA1 concentration was higher than in ABW's. Other lipid parameters did not differ among the corresponding groups. These data suggest that hypercholesterolemia, which is seen in LBW adults, must develop in older ages, and the irregular lifestyle including improper nutritional habits may promote its occurrence in this vulnerable group.

[Cardiovascular disease-related serum lipid parameters in a Hungarian subpopulation].

The blood samples of 229 volunteers (100 men, aged 35-59 y and 129 women, aged 35-54 y) were randomly selected and analysed for serum lipids (triglyceride, total, HDL- and LDL-cholesterol), apolipoproteins (apolipoprotein A1, A2 and B) and Lp(a). The mean serum level of triglyceride was in the high risk category in males (2.50 +/- 2.27 mmol/L) and in normal range in females (1.31 +/- 0.82 mmol/L). Concentrations of total cholesterol (male: 5.93 +/- 1.16, female: 5.66 +/- 0.99 mmol/L) and LDL-cholesterol (male: 3.72 +/- 0.94, female: 3.55 +/- 0.81 mmol/L) were in the borderline risk category, and up to the 75th percentiles both total cholesterol (male: > 6.71, female: > 6.25 mmol/L) and LDL-cholesterol (male: > 4.38, female: > 4.10 mmol/L) were in the high risk categories. The mean values of HDL-cholesterol and apolipoprotein A1, A2 and B were in the desirable range in both sexes, but apolipoprotein B in males and females (male: > 1.3, female: > 1.29 g/dl) were in the high risk category up to the 75th and 90th percentiles, respectively. The mean levels of serum Lp(a) were 19.5 +/- 25.0 and 21.0 +/- 25.2 mg/dl for men and women, respectively, and up to the 75th percentiles were in the high risk category (> 30 mg/dl) both in males and females.

[Postoperative percutaneous endoscopic gastrostomy/jejunostomy].

In order to cure complications appeared in the postoperative period two patients were treated with percutaneous endoscopic gastrostomy/jejunostomy (PEG, PEGJ) with the purpose of long-lasting enteral feeding and decompression. The indications of PEG/PEGJ were the following: external gastric fistula in one case and anastomotic leakage in one case. In the patients the PEG was located by intraoperative X-ray examination, this method was not published earlier. Regarding complications of the early postoperative period the PEG and the PEGJ are considered useful and expedient procedures with the aim of lasting enteral feeding and decompression.

[Rehabilitation of patients with head and neck tumors, with special attention to psychotherapy].

Few people are more severely handicapped than patient with facial deformities. The face forms a centre point of attention in human relationships with particular emphasis placed on cosmetic acceptance. Surgical ablation of head and neck cancer often creates significant functional and aesthetic problems. In addition to the numerous physiologic problems that can befall the patient, devastating changes in appearance may occur with serious psychologic sequel. Our purpose here is to discuss the numerous physiologic and psychological problems occurring after major ablative surgery of the head and neck. A team effort is essential to the effective treatment of those patients, including: surgeon, oncologist, radiotherapist, prosthodontist and last but not least psychologist, who has a main role in strengthening the patients and helping them to lead an acceptable quality of life.

Nutritional survey of pregnant women in Hungary.

A nutritional survey of a Hungarian group of pregnant women was carried out. One hundred and twenty nine women aged 25.9 years, entered the study, but only 70 completed all the protocol. Average body weight gain was 12.4 kg and the mean birth weight of the newborns was 3,386 g. Mean energy and nutrient intakes of pregnant women showed similar patterns as in Hungarian non-pregnant women of the same age. The mean energy intake was high (11 MJ), being 10% higher than for non-pregnant women. The mean protein and lipid intakes were also high, 91.9 g and 108.4 g, respectively, exceeding by 7% and 6% the intakes of the non-pregnant women. Dietary intakes of saturated fatty acids (SFAs) and monounsaturated fatty acids (MUFAs) were close to 12% of energy, and the intake of polyunsaturated fatty acids (PUFAs) was 7.6% of total energy. Palmitic acid (16:0), oleic acid (18:1 n-9) and linoleic acid (18:2 n-6) made the greatest contribution to the total peak area of SFAs, MUFAs, and PUFAs, respectively. The ratio of P/S (polyunsaturated/saturated fatty acids) was appropriate: 0.65; however, the ratio of linoleic acid (18:2 n-6), to linolenic acid (18:3 n-3) was high: 16. The cholesterol intake was somewhat lower in pregnant than in non-pregnant women. Complex carbohydrate intake was low (245 g) but it was still by 75% higher than in non-pregnant women. The excess sodium intake (6.3 g) was very similar to that of non-pregnant women. Mean values for retinol, tocopherol, ascorbic acid, cobalamin and copper intakes were higher than the Hungarian Recommended Dietary Allowances (RDA). Thiamine, riboflavin, pyridoxine, niacin, calcium, iron and zinc intakes were insufficient. Data showed an imbalance in the energy and nutrient intakes of Hungarian pregnant women, and this could be harmful for both the mother and pregnancy outcome.

Clinicopathologic aspects of 8 Kaposi's sarcomas among 1009 renal transplant patients.

In 8 of 1009 patients receiving renal grafts and consecutive immunosuppressive therapy during the last 20 years in Budapest, Hungary, Kaposi's sarcoma developed 6.5 months (average) after transplantation. Of 8 cutaneous cases of this disease, 5 also presented visceral manifestations. Seven patients died. Those with visceral tumors died because of the tumor itself. One patient affected by cutaneous tumor has survived for 5 years post-treatment. The relatively high incidence of Kaposi's sarcoma may be explained by a high infection rate with Herpes virus 8, which was shown to be positive in all three cases investigated.

[Neurofibromatosis with abdominal and severe central nervous system complications].

A young male patient suffered in cutaneous neurofibromatosis was followed during four years. He developed tissue proliferations of various histological structures manifested in the skin, central nervous system and different splanchnic organs. The classification criteria of neurofibromatosis as well as the diagnostic and therapeutic recommendations are discussed.

[The bone-marrow transplantation program in Hungary: report from the period 1990-1995].

The article summarises the statistical data of bone marrow transplantation (BMT) carried out in Hungary between 1990-1995 in yearly distribution. Since the first BMT up to the end of 1995, 168 BMT were performed. The number of transplantations since 1990 to our days was gradually increasing. As a result of this activity in the three transplantation centers (National Institute of Haematology and Immunology, St. László Hospital and County Hospital in Miskolc) 36 allogeneic and 12 autologous BMT were performed in 1995. Out of the allogeneic BMT cases, 14% of them were completed with unrelated, donors in the last three years. The most frequent indications for allogenic BMT are: chronic myeloid leukaemia (CML), acute lymphoid leukaemia (ALL), acute myeloid leukaemia (AML), myelodysplasia, severe aplastic anaemia. Child allogenic BMTs are carried out on pediatric patients in St. László Hospital in leukaemia, severe aplastic anaemia cases and children born with immunodeficiency. Autologous BMTs started in an organised way in 1995 for adult patients in cases of non-Hodgkin lymphoma, Hodgkin lymphoma and for children with solid tumour indication in the Miskolc Centre. BMT activity in Hungary compared with international data, especially within Europe, shows a significant drop behind. To calculate for ten million inhabitants, the optimal BMT activity should be between 100-200 transplantations (allogeneic and autologous BMT together) in 1994. Among the Central European countries Hungary and Poland fall most behind. Autologous BMTs in most countries of Europe are above of allogeneic BMTs in numbers as indication in cases of lymphoma and solid tumours (first of all mamma carcinoma) comes into focus. This summary emphasises the most important difficulties in connection with the development of the National BMT program.

[Polycytemia vera in an 11-year old girl].

The case of an 11 year old girl with three line type of polycythaemia vera, with 4 cm splenomegaly and a plethoric complexion is presented. Peripheral blood values were as follows: RBC: 7.75 x 10(12)/l, Hb: 18.8 g/l, WBC: 15.2 x 10(9)/l, platelets: 920 x 10(9)/l. Serum erythropoietin level: < 1 mU/ml. In vitro, erythroid colonies developed from the bone marrow in the absence of added erythropoetin. For three years the haematocrit value has been controlled by regular venesections. Since extreme thrombocytosis developed, the treatment was continued with interferon alpha. Different treatment protocols are discussed.
