The heart as the primary site of lesion in malignant lymphoma is extremely rare. We experienced 2 cases of malignant lymphoma whose initial presentation was massive pericardial effusion with cardiac tamponade. The first case was a 75-year-old man who had shortness of breath for 1 week. Chest X-ray showed cardiomegaly (CTR 65%), and his condition was diagnosed as congestive heart failure at first and thereafter echocardiogram revealed pericardial effusion. The second case was a 76-year-old man who complained of exertional dyspnea which worsened over 2 weeks. His condition was diagnosed as congestive heart failure at first and echocardiogram revealed pericardial effusion. Pericardial drainage tapped bloody fluid and cytological examination revealed malignant lymphoma. After treatment, the first case lived for eleven months with no recurrence of pericardial effusion. The second case has lived for 4 years. We present these 2 cases can be defined as primary cardiac lymphoma according to McALLISTER and FENOGLIO, i.e., a lymphoma that involves only the heart and pericardium. More cases of primary cardiac lymphoma will be found in the future because of the ease with which the echocardiogram can detect pericardial effusion.

A novel human hepatoblastoma cell line (HuH-6KK) with a high growth rate in a serum-free medium without extracellular matrix was developed from an original one, HuH-6 c15 (HuH). The original HuH cells (38 passages) did not proliferate well in RPMI 1640 medium containing 20% fetal calf serum (FCS). The HuH cells (HuH-6KK) with a high growth rate were selected by culturing them in an enriched RDF containing 20% FCS and 0.01% mucous polysaccharide (spirulinan) isolated from a blue-green alga, Spirulina subsalsa. The HuH-6KK cells showed a rapid growth in serum-free eRDF medium containing insulin, transferrin, ethanolamine, and selenite (eRDF-ITES medium) without fibronectin. The proliferation of the original HuH cells was also observed in the eRDF-ITES medium, but the growth was slow compared with the HuH-6KK cells. In the medium without ITES, the growth of the HuH-6KK and original HuH cells was slow. Among the ITES ingredients, insulin promoted the growth of HuH-6KK cells the most.

The rate x pressure product is a product of the systolic blood pressure and heart rate and is widely used in clinical medicine as an index of the cardiac oxygen supply-demand balance. Employing 20 healthy male subjects, we studied the changes in the rate x pressure product due to a body position change from the sitting position to the supine position in a dental chair. The following results were obtained: 1. The change from the sitting position caused a 10% decrease in the rate x pressure product, and this value did not return to the initial sitting position value even after 5 min in the supine position. 2. The heart rate tended to decrease due to the position change. 3. Immediately after the position change, the systolic blood pressure decreased significantly (p less than 0.05), but it tended to return to the initial value with the lapse of time. The lower rate x pressure product in the supine position suggests that the supine position is more advantageous than the sitting one in treatment of the elderly and patients with cardiovascular diseases from the viewpoint of the systemic cardiovascular kinetics.
Dermoid cyst or epidermoid cyst of the oral cavity usually occurs in the floor of the mouth. We encountered a 21-year-old woman who had developed epidermoid cyst in the oral floor. Since infancy she had been aware of a swelling in the oral floor. Recently, the swelling began to grow larger gradually. The lesion was excised under general anesthesia by intraoral approach. The swelling was spherical in shape, measured 50 x 45 x 40 mm in size, and weighed 40 g. Histopathological examination revealed keratinized, stratified squamous epithelium on the cyst wall and marked deposition of melanin at the base of the epithelium.
An anti-human cytomegalovirus (HCMV) antibody in 330 sera from patients and normal subjects was examined by ELISA. The test results were compared with the results of CF test and there was 99.1% agreement. Specific anti-HCMV IgM antibody was searched for and only 4 sera were positive. This makes it unlikely that screening for IgM anti-HCMV will effectively prevent posttransfusion HCMV infections.

Glucagon and insulin metabolism was studied in 9 cirrhotic patients and 4 non-cirrhotic controls. Net output of glucagon and insulin into portal circulation was calculated from difference between portal venous and systemic arterial concentrations multiplied by portal plasma flow. Metabolic clearance rate was also calculated as the ratio of the output to systemic concentration. Portal blood flow was measured by the continuous local thermodilution method. The results were as follows: 1) Arterial glucagon concentration was elevated in liver cirrhotics compared with non-cirrhotic-controls. Glucagon output in cirrhotics was higher than in controls [46.3 +/- 11.8 vs. 13.2 +/- 2.5 ng/min (mean +/- SEM), p less than 0.05]. Metabolic clearance rate of glucagon was not significantly different between the two groups. 2) There was a significant correlation between glucagon output and portal venous concentration of norepinephrine (r = 0.625, p less than 0.05). 3) Insulin levels in systemic arterial blood were higher in cirrhotic patients than non-cirrhotic subjects. Insulin output was not significantly different between the two groups, however, metabolic clearance rate of insulin in cirrhotics was reduced in comparison with the controls (274.5 +/- 44.3 vs. 557.7 +/- 108.6 ml/min, p less than 0.05).
