Hemodynamic and plasma renin responses to treatment of a renal arteriovenous malformation.

We report a case of a right renal arteriovenous malformation with high output cardiac failure and hypertension which was cured by nephrectomy. Hemodynamic findings pre- and post-operatively are presented. Plasma renin activity (PRA), both from peripheral venous and renal vein samples, was assessed. The fall in PRA associated with nephrectomy lends support to the suggestion that the renin-angiotensin system plays a role in the hypertension in these patients.

Duplex scanning in the assessment of deep venous incompetence.

A noninvasive method to evaluate deep venous incompetence by duplex scanning is presented. For this test, it was decided to have the patient standing so as to make the test less dependent on the need for patient cooperation and to allow gravity to produce reflux. Results were validated against ambulatory venous pressure measurements. The method described had a sensitivity of 84% and specificity of 88%. Duplex scanning is a useful screening test for detecting the presence and site of incompetence in patients with deep venous disease.

Acute viral encephalitis complicating a first manic episode.

The diagnosis of viral encephalitis in a patient with acute mania was difficult because of symptom overlap and inconclusive laboratory evaluations. Final differentiation was made clinically. Despite the generally assumed usefulness of diagnostic tests for encephalitis, only the electroencephalogram showed consistent sensitivity in this patient and in similar previously reported cases. Diagnosis was further complicated by the potential side effects of neuroleptic medications, which may mimic infection through extrapyramidal symptoms, fever, or altered blood counts. In addition, legitimate physical complaints were discounted as a result of the patient's psychiatric status.

Does long-term low-dose corticosteroid therapy cause hypertension?

1. One hundred and ninety-five patients undergoing low-dose prednisone or prednisolone therapy were investigated. Blood pressure, weight, serum urea, sodium and potassium were recorded before therapy and again after at least 1 year of therapy. 2. The rise in both mean systolic and mean diastolic blood pressure was paralleled by an increase in the prevalence of arbitrarily defined hypertension. 3. There was no relationship between change of blood pressure and either dose of corticosteroid or duration of therapy. Blood pressure before therapy was the main determinant of the change in blood pressure. 4. Mean serum sodium levels rose slightly but serum potassium levels did not change during the follow-up period. There was no significant weight gain. 5. These results indicate that treatment of asthma and rheumatoid arthritis with prednisolone or prednisolone in low dose does not cause hypertension or biochemical features suggestive of mineralocorticoid excess.

Below-knee amputation in peripheral arterial disease.

A below-knee amputation will heal in most patients with atherosclerotic peripheral arterial disease, using no anterior flap and a long posterior myoplastic flap. The technique used by the authors is presented in detail, with their experience in 32 cases.
