[The clinical effect of low-dose Ara-C in patients with refractory acute nonlymphocytic leukemia and myelodysplastic syndrome].

Twenty-one patients with refractory acute nonlymphocytic leukemia (ANLL) and myelodysplastic syndrome were treated with low-dose cytosine arabinoside (LDARC). LDARC was administered by subcutaneous injection every 12 hours at a dose of 10 mg/m2 (regimen A) or at a dose of 20 mg/m2 by continuous intravenous infusion (CIV) or continuous subcutaneous injection (CSC) (regimen B). Among 22 courses, two elderly patients with ANLL (M4 and M5) and one M4 patient with myelofibrosis obtained complete remission (CR) and three elderly patients with ANLL (one M1 and two M2) and one patient with ANLL developed from RAEB in transformation obtained partial remission (PR). The overall remission rate (CR + PR) was 31.8%. The CR durations were 2.5, 5 and 2 months, respectively. The plasma concentration of Ara-C determined in regimen B was 8.26 +/- 4.12 ng/ml. There was no difference in the plasma concentration of Ara-C between CIV and CSC. We consider that the major mode of action of LDARC lies in its cytotoxic effect, since all patients who obtained remission exhibited pancytopenia and bone marrow hypoplasia. However, in several patients, we observed transient monocytosis and granulocytosis which were considered to be suggestive evidence of differentiation of leukemia cells.

[Clinical effect of human lymphoblastoid interferon in patients with multiple myeloma].

Five patients with multiple myeloma were treated with human lymphoblastoid interferon (HLBI). HLBI, 3 X 10(6) IU/day, was administered daily for more than two weeks by intramuscular injection. Out of four evaluable patients, a minor response was obtained in 3 patients. In these responders, one patient developed pleural effusion due to the infiltration of myeloma cells during the administration of HLBI, and drug resistance was observed in another patient during the re-administration of HLBI. Therefore, out of six evaluable courses, a minor response was obtained in 3 courses of HLBI treatment. No severe side effects were observed. Thrombocytopenia, general malaise, liver dysfunction and anorexia were the main reasons for discontinuation of HLBI administration. On the basis of the preliminary study, it is concluded that HLBI is worth trying in the management of refractory multiple myeloma.

[Studies on the diagnosis of uterine tumor by 201-thallium-chloride (author's transl)].

Cases of gynecological tumor mainly uterine benign and malignant tumors were evaluated using 201Thallium-Chloride (201Tl-Cl) after intravenous injection. The positive scans of 100% in 42 cases of uterine myoma including 29 cases of leiomyoma, 8 cases of adenomyosis and 5 cases of combined type myoma, also 100% in 8 cases of body cancer indicating adenocarcinoma and 25% in 12 cases of cervical cancer indicating squamous cell carcinoma were observed. The smallest focal lesion for the positive scans was needed approximately more 5 cm in diameter in the cases of leiomyoma. The most accumulation of activities was showed in adenomyosis or adenocarcinoma with much adenoid structure in the pathological study. Accumulation of 201Tl in uterine benign and malignant tumors was suggested the analog of potassium. The mechanism of tumor affinity was correlated to perfusion volume and the acceleration of the potassium metabolism in a tumor. On the other hand, we suggested the diagnosis of the malignant uterine tumor should be attended due to 201Tl was accumulation also in the benign uterine tumor. The uterine scanning using 201Tl-Cl was acknowledged to be very useful and safe indicating early detection at 20 minutes after the injection, (non-traumatic procedure and non-visualized bone tissues). In conclusion, this radioisotopical procedure was indicated high value of screening examination in the diagnosis of uterine tumor.

Detection of plasma (1 --> 3)-beta-D-glucan in patients with Fusarium, Trichosporon, Saccharomyces and Acremonium fungaemias.

(1 --> 3)-beta-D-glucan, a characteristic fungal molecule, is known to increase in blood in invasive candidiasis, aspergillosis and cryptococcosis. This report shows that the plasma glucan concentration was also elevated in four patients infected with Fusarium, Trichosporon beigelii, Saccharomyces cerevisiae and Acremonium. In three of the patients, the elevation preceded positive blood cultures by 5-17 days, and in one of them it even preceded the onset of fever by 6 days. In a fourth patient, the glucan level decreased with clinical improvement. Plasma (1 --> 3)-beta-D-glucan determination appears to be useful also for diagnosis and follow-up of these unusual deep mycoses.
