[Pathological classification of malignant lymphoma].

The classification of malignant lymphoma is an important factor in treatment. Hodgkin's disease (HD) and non-Hodgkin's lymphoma (NHL) should be clearly differentiated from each other, since their clinical behavior and response to treatment are quite different. Although four histological subtypes of HD has been recognized, the subclassification is not so important clinically. Many classifications of NHL have been devised, but most of them are based on morphological features only and do not incorporate recent advances in understanding of origin, function and differentiation of lymphoma cells. Further improvement is necessary to establish a reproducible, histogenetically accurate and clinically meaningful classification.

The incidence of secondary tumors of the heart and pericardium: a 10-year study.

Secondary tumors of the heart and pericardium are much more common than primary tumors. During a 10-year period (1976-1985), only one instance of a primary tumor (malignant mesothelioma) was identified among 2,649 autopsies of malignant tumors at the National Cancer Center Hospital. In contrast, there were 407 cases in which heart and/or pericardium were secondarily involved with a malignant tumor from other organs. In 78 cases, the secondary tumors were present only in the pericardium, while in 329 cases, the tumors involved the heart itself with or without pericardial involvement. Among the primary tumors, three-quarters were carcinomas of various organs. In addition there were 53 hematologic malignancies, 19 melanomas and 17 sarcomas. In 31 cases, cardiac failure was the direct cause of death. The secondary tumors of the heart were often overlooked clinically because the cardiac dysfunction appeared to be part of the deteriorating general condition. Electrocardiography and echocardiography were often helpful in suggesting the presence of cardiac metastases. Appropriate diagnostic procedures and therapeutic intervention should be considered for the care of patients with advanced malignancy and sudden onset of cardiac failure.

Stopped-flow investigation of the reaction between vitamin E radical and vitamin C in solution.

Kinetic study of the reaction between vitamin E radical and vitamin C has been performed. The rates of reaction of vitamin C (ascorbic acid 1, 6-0-stearyl ascorbic acid 2, and 2,6-O-dipalmitoyl ascorbic acid 3) with vitamin E radical (5,7-diisopropyl-tocopheroxyl) in benzene-ethanol (2:1, v/v) solution have been determined spectrophotometrically, using stopped-flow technique. The second-order rate constants obtained are 549 +/- 30 M-1s-1 for 1, 626 +/- 53 M-1s-1 for 2, and 4.84 +/- 1.41 M-1s-1 for 3 at 25.0 degrees C. The result shows that the ascorbic acid ester 2 having a long-alkyl-chain at 6-position is 1.14 times as reactive as the ascorbic acid 1, whereas the ascorbic acid ester 3 substituted at 2-position is only 0.01 times as reactive as the ascorbic acid 1.

Hamartoma of the parotid gland: a case report with immunohistochemical and electron microscopic study.

A case of a solid parotid tumour in a 16-year-old boy is presented. Histologically, the tumour demonstrated some peculiar findings. An acinar pattern was predominant although every component seen in the normal salivary gland was present, namely, serous and mucous gland acini, ducts, myoepithelial cells, adipose and lymphoid tissue. Large eosinophilic granules were abundant in the large acinar cell cytoplasm. Immunohistochemically, the tumour demonstrated the proteins which are present in the normal parotid gland, for example, amylase, lactoferrin and lysozyme. Electron microscopic features were quite similar to those of normal parotid tissue except for accumulation of a large number of cytoplasmic granules in the acinar cells. There has been no previous report of a tumour with the same features as seen in this case. Our pathological diagnosis is hamartoma, although the possibility of hyperplasia or neoplasia can not be excluded.

Plasmacytoma of the lung associated with nodular deposits of immunoglobulin.

A case of extramedullary plasmacytoma of the lung associated with nodular immunoglobulin deposits is presented. The main tumor was located in the posterior basal segment of the lower lobe of the left lung and showed intrabronchial polypoid growth. Multiple metastatic tumors were observed in regional lymph nodes and the visceral and parietal pleura. Histologically, the tumor was composed of sheets of plasma cells with mild atypia. A large amount of amorphous material resembling amyloid was observed in the tumors, and was more marked in the metastatic lesions. Immunohistochemically, the tumor cells were shown to contain monoclonal IgG-kappa immunoglobulin. The amorphous deposits were not identical to amyloid since congo red stain was negative and amyloid fibrils were not observed ultrastructurally. By Western blotting, immunoglobulin (IgG, kappa) was identified as the major component in an extract from the nodular deposits. In the preoperative serum from the patient, M-protein (IgG-kappa) was detected by immunoelectrophoresis. Bone marrow examination revealed no abnormality.

[An immunohistochemical study using a double staining method for regional lymph nodes in gastric cancer].

We have performed a double immunofluorescent staining of two monoclonal antibodies, obtained from gastric cancer patients, that were directed against lymphocyte surface antigens on the regional lymph nodes. Through this method we were able to identify the lymphoid cell subset in detail. As a result of our double staining, most OKT8+ lymphocytes were also found to be Leu 15- lymphocytes and these OKT8+ Leu 15- lymphocytes were regarded as being cytotoxic effector cells or precursors. A few OKT8+ Leu 15+ lymphocytes (suppressor T lymphocytes) were obtained. More Leu 3a+ Leu 8- lymphocytes (helper T lymphocytes) were observed than Leu 3a+ Leu 8- lymphocytes (suppressor inducer T lymphocytes). To investigate whether these T lymphocytes were activated, we used an OKT 9 monoclonal antibody directed against the transferrin receptor and an OKIa1 monoclonal antibody directed against the HLA-DR framework. Such double stained lymphocytes (activated T lymphocytes) were few.

[Clinical trial of SM-108 in myeloproliferative disorders].

SM-108 (4-carbamoylimidazolium-5-olate) is a new purine antagonist which blocks IMP dehydrogenase, the key enzyme of de novo GMP biosynthesis. Eleven patients were entered into a clinical study of SM-108 for the treatment of myeloproliferative disorders. These included 10 cases of chronic myelogenous leukemia (CML) and one case of essential thrombocythemia (ET). Two cases of CML had received prior chemotherapy. SM-108 was given orally at a dose of 400-600mg/m2 daily. Of nine evaluable cases, four cases of CML achieved complete responses and two cases of CML achieved partial responses (response rate = 66.7%). The duration of drug action was short, and the daily dosage required adjustment at frequent intervals. Long-term response without therapy has not been seen with SM-108 in CML. There was no response in the case of ET. The side effects included constipation (1/11), diarrhea (1/11) and mild hepatic toxicity (1/11). No cardiac or renal toxicity was observed.

[A case report of leiomyoma of the urinary bladder].

A 69-year-old male patient developed gross hematuria. Cystoscopy revealed a very small mass growing from the dome. The patient underwent transurethral resection of the mass under epidural anesthesia. Pathological examination of the resected specimen revealed submucosal leiomyoma of the bladder. The pathology, clinical manifestations, radiological and cystoscopic findings, treatment and prognosis of vesical leiomyoma are reviewed.

[A case of Cushing's syndrome caused by an adrenal black adenoma].

A black adenoma of the adrenal cortex was surgically removed from an 18-year-old female who had clinical and laboratory findings characteristic of Cushing's syndrome. The electron microscopic study revealed typical features of steroid-producing cells, including a prominent smooth and rough endoplasmic reticulum, and mitochondria. In addition, many of the adenoma cells contained numerous pigmented granules which were lipofuscin. Three months after the surgery, the hormone levels returned to the normal range. The Japanese literature of functional black adenoma is reviewed.

Mediastinal malignant lymphoma complicated with esophagobronchial fistula: successfully treated case.

A 64-year-old man presented with mediastinal and bilateral hilar adenopathy, and a biopsy of a scalene node revealed non-Hodgkin's lymphoma. One week after a cycle of combination chemotherapy, he developed an esophagobronchial fistula. Following a resolution of pneumonia by antibiotics, a cervical esophagostomy was made and, after the improvement of his general condition with parenteral hyperalimentation, he was given one course of combination chemotherapy which was continued until the mediastinal lymph node shadow completely disappeared. A subcutaneous bypass operation was performed on the stomach. After receiving one more cycle of intensification chemotherapy, he was discharged. Three months later, a bronchoscopy showed healing of the fistula. Nine months postoperatively, there is no evidence of the lymphoma or the esophagobronchial fistula recurring. Esophageal involvement is rare in malignant lymphoma and this is only the sixth reported case of esophageal fistula of the respiratory tract in association with lymphoma, and just the second to be treated successfully.

Pituitary adenomas in patients under 20 years old. A clinicopathological study of 12 cases.

The clinical manifestations, pathological features and follow-up data on 12 cases of pituitary adenoma in patient less than 20 years old were evaluated. This group represented 8.5% of our 142 cases of pituitary adenoma from all age groups during the period of study. There were five males and seven females whose ages ranged from 13 to 19 years at diagnosis. Immunocytochemical staining demonstrated the presence of prolactin in 10 tumors, ACTH in one tumor and the remaining neoplasm was negative for the five major pituitary hormones (prolactin, hGH, ACTH, TSH, gonadotrophin). The results of the immunocytochemistry correlated appropriately with the clinical manifestations. Extension beyond the sella turcica at presentation was a common feature as evidenced by the high incidence of visual defects (75%). A complete excision was accomplished in only two patients. The aggressive behavior of these tumors was demonstrated by a recurrence rate of 50% and only a single long-term cure. Early detection and therapy, if possible, are essential for the successful management of pituitary adenomas in younger patients as implied by our study and other reports in the literature.

A case of simultaneous bilateral germ cell tumors arising from cryptorchid testes.

We report a rare case of simultaneous bilateral testicular germ cell tumors arising from uncorrected cryptorchid testes. Each side had a different histological type, which consisted of pure high grade seminoma on the left side, and teratocarcinoma with choriocarcinoma and yolk sac tumor elements in addition to seminoma on the right side. Patients with cryptorchidism are known to have a higher risk of germ cell tumors. Genetic factors also may have a role in the oncogenesis in our patient, since his older brother had had a seminoma in the left cryptorchid testis previously. Both patients had the HLA-Aw24 antigen. The characteristics of familial testicular tumors are discussed.

Mucinous adenocarcinoma of prostate: a case report and review of the literature.

Mucinous adenocarcinoma of the prostate is rare and its biological behavior is not well known. We report a case of mucinous adenocarcinoma of the prostate, which was treated successfully with castration. Positivity for prostatic specific antigen by immunohistochemistry confirmed the prostatic origin of this tumor. A review of the literature revealed 30 authentic cases. Prostatic mucinous adenocarcinoma has been said to be different clinically from ordinary prostatic adenocarcinoma. It is insensitive to hormonal therapy, rarely produces acid phosphatase and rarely metastasizes to the bone. However, our case, together with the frequent presence of coexisting acinar elements in mucinous adenocarcinoma, indicates no significant difference in the clinical behavior between mucinous and ordinary acinar carcinomas.
