The existence of specific antibodies to cobalt in hard metal asthma.

Twelve workers with hard metal asthma diagnosed on the basis of peak flow diaries and positive bronchial reactions to cobalt chloride (CoCl2) were studied for sensitization by detection of specific antibodies to radioactive cobalt (57Co), cobalt-conjugated human serum albumin (Co-HSA) and cobalt-conjugated exchange resin (Co-resin). Their IgE titres ranged from 73 to 1500 IU/ml and eight were atopic individuals. Sixty serum samples from asthmatic patients with IgE titres of 14-4300 IU/ml were studied as controls in all tests. Eleven of twelve subject sera that selectively bound to 57Co after incubation with saturated ammonium sulphate (greater than 232 c.p.m., P less than 0.01) were divided into three groups: (1) six sera showing evidence of specific IgE antibodies to Co-HSA (greater than 673 c.p.m., P less than 0.01) without those to Co-resin; (2) one serum giving a positive radio-allergosorbent test (RAST) only to Co-resin (greater than 417 c.p.m., P less than 0.01), and (3) four sera that were negative for two antigenic agents (Co-HSA, Co-resin). These results suggest that the subjects had occupational asthma due to hard metal exposure from cobalt sensitivity. An immuno-allergic mechanism mediated by specific IgE antibodies to cobalt was confirmed to be responsible for the development of hard metal asthma, with the possibility of some role of the reaction without reagins.

Intraosseous epidermal cyst of the sacrum. A case report.

An unusual case of intraosseous epidermal cyst is reported. The patient, a 45-year-old Japanese female, had suffered from lumbago and dysuria for about 15 years. X-ray examinations and CT scan revealed an expanded osteolytic tumor without marginal sclerotic change within the sacrum, which anteriorly invaded the surrounding soft tissues at the S2/3 level. At this time, chordoma was suspected, but epidermal cyst with foreign body granuloma was finally diagnosed from biopsy and surgical specimens.

Osteosarcoma. Ultrastructural and immunohistochemical studies on alkaline phosphatase-positive tumor cells constituting a variety of histologic types.

The osteosarcomas were subclassified into osteoblastic, fibroblastic, chondroblastic and telangiectatic types and examined by electron microscopy. Their immunohistochemical reactions were also studied. In an overall survey of the above types, fibroblast-like cells revealed poorly developed cytoplasmic organelles with rather short, branching rough endoplasmic reticulum, mixed with osteoblast-like cells that were hardly distinguishable from the former. They appeared to be an early stage of an osteoblastic cell lineage from the distribution and development of their cell organelles and highly positive vimentin activity. The tumor cells in malignant cartilage varied in appearance from chondroblast-like to osteoblast-like cells. All types of tumor cells expressed alkaline phosphatase activity to a significant degree. Immunohistochemical staining showed a mixture of procollagen type I-positive cells among the cells positive for both procollagen type II and S-100 protein in the malignant cartilage. Irrespective of any ultrastructural differences between these various tumor cell types, they all revealed a significant degree of ALPase activity unlike other types of bone tumors, suggesting that the tumor cells which constitute the various types of osteosarcoma are derived from a common precursor cell.

Differences between the bony interfaces of titanium and hydroxyapatite-alumina plasma-sprayed titanium blade implants.

Histopathologic study was done to reveal the difference of bony interface in the mandibular bone between titanium blade implants and titanium implants coated with the mixture of hydroxyapatite-alumina by the plasma-spray technique, using the improved undecalcified histologic technique and the image analyzing system. In 4 adult shepherd dogs, the implantation was performed in the molar region of the mandible 2 months after tooth extraction. The coated implants and the non-coated ones were inserted respectively in the mandibles. They were sacrificed on the 5th, 7th, 14th and 28th day postoperatively. In 5-day implant after insertion, granulation tissues or fibrous connective tissues were observed around both implants in bone tissues. In 7-day implant, osteoid tissues regenerated around the apex of both implants, but attached a little to them. In 14-day implant, osteoid tissues or woven bone regenerated around both implants and attached more to the coated implant (approximately 13.7%) than the non-coated one (approximately 6.0%). In 28-day implant, regenerative woven or lamellar bone attached much more to the coated implant (approximately 63.4%) than the non-coated one (approximately 8.8%). Bony interface increased remarkably in the coated implants, chronologically. The results showed that the plasma-sprayed hydroxyapatite-alumina coating was effective for the initial fixation of endosseous dental implants.

[Clinical evaluation of cefpodoxime proxetil in the treatment of skin and soft tissue infections. A double blind comparison of cefpodoxime proxetil and cefaclor].

In order to objectively evaluate the effectiveness, safety and usefulness of the new oral cephem cefpodoxime proxetil (CS-807, CPDX-PR) for the treatment of skin and soft tissue infections, a double-blind comparative study was undertaken using cefaclor (CCL) as the control drug. CPDX-PR and CCL were administered for 7 days at daily doses of 400 mg (divided into 2 portions) and 750 mg (divided into 3 portions), respectively. A total of 243 patients (118 in the CPDX-PR group and 125 in the CCL group) was treated in this study. The effectiveness, safety and usefulness were evaluated in 222 (106 in the CPDX-PR group and 116 in the CCL group), 234 (113 in the CPDX-PR group and 121 in the CCL group) and in 223 patients (107 in the CPDX-PR group and 116 in the CCL group), respectively. There were no differences in patients' backgrounds between the 2 groups, except for the presence or the absence of surgical treatments. The results we obtained are summarized below: 1. In the evaluation of clinical efficacy by the subcommittee, excellent, good, fair and poor efficacy were observed in 36, 43, 17 and 10 patients in the CPDX-PR group, respectively; the efficacy rate was, therefore, calculated to be 74.5%. As for the CCL group, respective results were observed in 50, 39, 17 and 10 patients, indicating an efficacy rate of 76.7%. There was no significant difference between the 2 groups. Improvement rates judged by physicians in charge were 80.2% in the CPDX-PR group and 88.8% in the CCL group. Moreover, no significant difference in diseases or severity were found between the 2 groups. 2. As for the bacteriological efficacy, the 2 groups showed high elimination rates, as 90.1% and 91.6% of the disease causing bacteria were eliminated in the CPDX-PR group and in the CCL group, respectively. Elimination rates in single infections with Staphylococcus aureus were determined to be 85.7% in the CPDX-PR group and 85.0% in the CCL group. 3. Although 6 patients in the CPDX-PR group and 2 patients in the CCL group developed side effects, which were mainly gastrointestinal symptoms, there was no significant difference in the incidence of side effects between the 2 groups. Abnormal laboratory values were found in 5 patients in the CPDX-PR group and 1 patient in the CCL group. 4. There was no significant difference in the usefulness between the 2 groups.(ABSTRACT TRUNCATED AT 400 WORDS)

Determination of 4,4'-methylenebis(2-chloroaniline) in urine by liquid chromatography with ion-paired solid-phase extraction and electrochemical detection.

This highly specific and sensitive method for measuring urinary 4,4'-methylenebis(2-chloroaniline) (MBOCA) involves liquid chromatography with electrochemical detection. Before chromatography, urine samples are prepared by ion-paired solid-phase extraction on a disposable octadecylsilica column with acidic methanol solution containing 1-heptanesulfonic acid. This enhances the specificity of the method. Mean overall recovery ranged from 97.1% to 99.5% at added MBOCA concentrations of 20 and 100 micrograms/L in urine. Sensitivity for urine was 1 microgram/L. The intra-assay CV was 2.2% at a MBOCA concentration of 100 micrograms/L. We believe that this method is acceptable for routine measurement of MBOCA in urine from individuals exposed to this industrial chemical.

[S-adenosyl-L-methionine concentration in cerebral interstitial fluid monitored by intracerebral microdialysis].

Ketamine-anesthetized gerbils were implanted with 0.5 mm diameter microdialysis tubing through the cortex, hippocampus and thalamus with a stereotaxic instrument. The cannula was perfused with artificial cerebrospinal fluid at a flow rate of 2 microliters/min. Samples of the perfusate representing the cerebral interstitial fluid were collected and subsequently analyzed for their content of S-adenosyl-L-methionine (SAM) with high performance liquid chromatography (HPLC). In vitro tests using the dialysis cannula gave SAM recovery which varied from 3.44 +/- 0.31% (n = 3) at a perfusion flow rate of 8 microliters/min to 43.8 +/- 3.81% (n = 3) at 0.5 microliters/min (mean +/- SEM). The in vivo SAM content of the perfusate was studied and the endogenous SAM level in cerebral interstitial fluid of gerbil was 3.49 +/- 1.52 nmol per ml (n = 3). Intraperitoneal injection of SAM (250 mg/kg) produced 10-fold increases in the perfusate SAM concentration. After the experiment the brains were checked by immunopathological study using anti-albumin antibody to examine the degree of damage to the blood-brain barrier permeability. These results show that intracerebral microdialysis offers several potential advantages for studies of extracellular neurochemistry.

[Atrial septal defect diagnosed and quantitatively-evaluated by transesophageal two-dimensional Doppler echocardiography].

Transesophageal two-dimensional (2-D) echocardiography (TEE: horizontal sector scan) combined with the pulsed Doppler method was performed to determine the size of atrial septal defect (ASD) and to measure directly shunt flow signals through the defects in six ASD patients (32 +/- 16 yrs) confirmed by cardiac catheterization. The size of the defect in the horizontal dimension was measured by TEE; the vertical dimension of the defect was determined from the distance of the transesophageal probe positions of the rostral and caudal margins of the defect. The size of defect determined by TEE was compared with that observed at surgery. Shunt flow signals were recorded by the pulsed Doppler technique by setting the sample volume at the center of the defect. Left-to-right shunt flow was determined as follows: shunt flow volume (ml/min) = mean velocity (cm/s) x the area of a defect (cm2) x 60. The shunt flow was compared with that obtained by the Fick's method. In all patients, TEE accomplished unequivocal visualization of a defect. The defect sizes by TEE correlated well with measurements obtained at surgery (r = 0.66). A clear laminar shunt flow with its peak in late systole and atrial systole was observed. Significant correlations were obtained between shunt volumes by TEE and those by the Fick's method (r = 0.91, p less than 0.05). Thus, TEE proved a useful method for diagnosing ASD and for evaluating shunt flow volumes.

[Surgical treatment for malignant soft tissue tumors of the hip].

Surgical treatment for malignant soft tissue tumors has resulted in a marked improvement in the grading of lesions, utilizing the surgical staging system introduced by Enneking. We attempted to divide malignant soft tissue tumors of the hip into four compartments; the anterior compartment, medial compartment including the neovascular bundle, posterior compartment including the sciatic nerve, and intra-bony lesions with tumors. A lesion localized in the anterior compartment would be resected with a wide margin. For resection of a lesion localized in the medial compartment, vascular reconstruction must be performed with replacement using artificial vessels for the femoral vein and the saphena magna femoral artery. As for the posterior compartment, for the lesions are easily resected with or without the sciatic nerve. In the last compartment of intra-bony lesions, disarticulation or endoprosthesis replacement after wide-resection surgery must be performed. In such cases, musculocutaneous reconstruction may be needed.
