Rectal bioavailability of 6-mercaptopurine in children with acute lymphoblastic leukaemia: partial avoidance of "first-pass" metabolism.

Plasma levels and the area under the plasma concentration-time curve (AUC) values of 6-mercaptopurine (6-MP) were determined in a balanced crossover study of oral (powder) and rectal (macrogol suppository) administration to 5 children with acute lymphoblastic leukaemia (ALL). The AUC (538.6 ng.h.ml-1) after the rectal dose of 30 mg/m2 was approximately 1.5-times of that (365.5 ng.h.ml-1) after the oral dose of 87.5 mg/m2. The coefficients of variation of interindividual variability of the AUCs were 21.5% and 32.3%, respectively. The relative bioavailability of the macrogol suppository compared to the powder was approximately 4.39. These findings indicate that rectal administration of 6-MP could avoid the first-pass effect of this drug in the alimentary canal and/or liver, resulting in a large AUC of 6-MP, and so could reduce interindividual variability in plasma 6-MP concentrations. Rectal administration of 6-MP may be more effective than empirical oral dosing for the treatment of children with ALL, especially for patients with nausea and/or vomiting.

Direct injection analysis of melphalan in plasma using column-switching high-performance liquid chromatography.

An automated column-switching high-performance liquid chromatographic (HPLC) method with ultraviolet detection is described for a simple and rapid determination of melphalan, an alkylating agent, in human plasma following direct sample injection. The system consists of a pretreatment column and an analytical column connected in series via a switching valve. Plasma samples are loaded onto a pretreatment column with an aqueous mobile phase, with which the sample to be analyzed is retained and the solubilized plasma proteins are flushed to be discarded. The retained compound is eluted from the pretreatment column onto the analytical column by using the chromatographic mobile phase with a higher elution capacity. The column-switching technique can be used to achieve an automated assay. Analytical recovery of the compound was 99.1%, and the coefficients of both intra- and interday variations did not exceed 3.5%. The detection limit was 10 ng/ml for the compound. Recovery of melphalan in plasma was only 2.1% after 4 weeks at 25 degrees C, as compared to 24.5% at 5 degrees C and 100.1% at -20 degrees C.

Determination of 6-mercaptopurine in acute lymphoblastic leukemia patients' plasma by high-performance liquid chromatography.

A simple, selective, sensitive, and rapid high-performance liquid chromatography (HPLC) method is described for the quantitation of 6-mercaptopurine (6-MP) in plasma of patients with acute lymphoblastic leukemia (ALL). Pharmacokinetic data are presented for seven children with ALL receiving 6-MP therapy. A sensitivity of greater than or equal to 2 ng/ml in plasma was achieved on a reverse-phase octadecylsilane column using an HPLC system following a cleanup step with a solid-phase extraction cartridge. The chromatogram was monitored at 325 nm. Analytical recovery of 6-MP was 90%. The coefficients of variation for intra- and interday variabilities were 2.51 and 4.23%. This assay method is clinically useful for pharmacokinetic studies of 6-MP in ALL patients.

[Epidemiological and therapeutic study on urethritis of male and cervicitis from viewpoint of STD--a study using NY-198. Sapporo Clinical Research Group for STD].

Urethritis in males and cervicitis in females, which were sexually transmitted diseases, were treated with NY-198, a new quinolone antibiotic, and its efficacy was studied. Seventeen male patients with gonorrheal urethritis were administered a single 300 mg dose of NY-198. The efficacy rate on the 3rd day after administration was 100%, but it was 85.7% on the 7th day due to recurrence in 1 patient. The results of treatment of non-gonorrheal infections were as follow. In this treatment, NY-198 was administered in a daily dose of 600 mg in 3 divided doses for 14 consecutive days. In the treatment of chlamydial urethritis of males, the efficacy rate in 26 patients was 84.6% on the 7th day and 84.0% on the 14th day in 25 patients. In the treatment of chlamydial cervicitis, the efficacy rate was 100% on both the 7th (3/3) and 14th (6/6) days. In the treatment of non-gonorrheal and non-chlamydial cervicitis, the efficacy rate was 100% on the 7th day (1/1) and 50% (1/2) on 14th day. The efficacy rate in all 40 males with non-gonorrheal urethritis was 85.0% on the 7th day and 88.9% for 36 patients on the 14th day, while that in all 4 females with cervicitis was 100% on the 7th day and 87.5% on the 14th day. No side effects were seen in any of the patients. Overall, NY-198 had an efficacy rate of 80% in the treatment of chlamydial infections. NY-198 was found to be a useful drug which is efficacious in the treatment of all STD-related microbes such as gonococci and chlamydia.

4-Quinolones

Histamine content and mast cells in human gastric and duodenal mucosa.

A high histamine content was found in all areas of human gastric and duodenal mucosa. In single subjects there was no general preference of histamine being located predominantly in fundus and corpus. In biopsy specimens of human corpus mucosa many histamine-containing O-PD reactive cells could be identified. In the sections treated previously with O-PD also many toluidine blue staining cells (mast cells) could be visualized. As shown by a transcription technique their place and number was the same as those of O-PD reacting cells. It is concluded that histamine in human corpus mucosa is localized exclusively in mast cells which seem to participate in acid secretion and in duodenal ulcer disease in man.

From Diplazium donianum, makisterone A, makisterone D, and an unidentified stereoisomer of makisterone B have been isolated. The presence of two other unidentified phytoecdysones has been noted.

Surgical treatment of vascular lesions of the spinal cord.

Paravertebral block and resection of upper thoracic sympathetic ganglions were performed on cases in which vascular disturbance of the spinal cord was considered partly responsible. Block was performed in 14 cases and clinical improvement was seen in 10 cases out of them while resection was considered effective in 2 out of 3 cases. The evoked EMG of patients was assumed recovery of a part of synaptic function in the ischemic cord after the block. On the other hand, the skin temperature of the lower extremity did not show considerable change and this supports the view that the restoration of clinical picture was not due to the improvement of the periphral circulation of extremities. From these observations, it would be well presumed that favorable effect of sympathectomy consists partly in the improvement of vascular disturbance of the spinal cord.
