The use of intralesional steroids in the treatment of chalazion.

Intralesional injection of triamcinolone acetonide 5 mg/mL suspension was used in the treatment of 110 cases of chalazion; 92.3% of the cases resolved completely with one to three injections, irrespective of the duration and consistency of the chalazion. Recurrence and yellow deposits were seen at the site of injection in a minority of patients.

Mycobacterium senegalense from bovines in Eastern Nigeria.

Detailed characteristics of three mycobacterial strains sharing important properties of Mycobacterium senegalense are described. Their physiological properties were compared with those of a typical M. senegalense strain described by Chamoiseau (1979), six strains of M. senegalense and one typical strain of M. fortuitum from the culture collection of Institute of Microbiology, Rutgers. The three Nigerian strains exhibited minor variations in their physiological properties when compared with other strains of M. senegalense. Unlike the strain of Chamoiseau the Nigerian strains did not utilize benzoate or citrate. The strains were also different from the other six strains of M. senegalense by utilizing trehalose and in failing to produce acid in mannitol. Unlike earlier isolates of M. senegalense the Nigerian strains were not from cases of bovine farcy but from cases with pathological manifestations of pulmonary tuberculosis. They appeared to be intermediate strains between M. senegalense and M. fortuitum. These results raise doubts on the justification for giving specific rank to M. senegalense.

Systemic allergy to human (recombinant DNA) insulin.

Allergy to insulin is extremely rare and is, in part, due to the differences in amino acid sequence of animal and human insulin. We report here a 37-year-old, white, pregnant woman who was allergic to beef/pork, purified pork, as well as human insulin. She reacted with a wheal and flare reaction in response to intradermal human and purified porcine insulins. She was desensitized to human insulin in order to continue insulin therapy during pregnancy. The course of this patient emphasizes that some patients are allergic even to human insulin and may require desensitization prior to insulin therapy.

Brevibacterium sp. from poultry.

Two isolates of methanethiol producing coryneform bacteria sharing morphological and physiological similarities with Brevibacterium are described. They were isolated from bumble-foot-like manifestations of poultry but proved to be non-pathogenic for experimental animals.

Certain characteristics of Corynebacterium pyogenes infection.

Isolation, characterisation and antibiotic sensitivity of 18 strains of Corynebacterium pyogenes from different animals have been reported. All the C pyogenes isolates uniformly differed from the typical strains of this species by the utilisation of trehalose and their failure to acidify maltose. The majority of the strains failed to grow in the presence of 0.1 per cent potassium tellurite. Tuberculosis-like lung lesion in a seven-day-old calf yielded one of the strains of C pyogenes. Histopathological changes in the calf lung have also been described.

Histochemical study of the distribution of a few enzymes in the digestive system of Indian parrot.

Acid-and alkaline phosphatase, 5-nucleotidase and lipase have been localized histochemically in the gizzard, intestine liver and pancreas of Indian parrot, Psittacula krameri. In the gizzard and intestine, the mucosal epithelial cells are the main sites for the enzyme production. The tubular glands of the gizzard show intense reaction for all the enzymes tested. The hepatic sinusoid cells of the liver and the acinii of pancreas give positive reaction. Like pancreas, the intestine has also been found responsible for the production and secretion of lipase. Functional significance of phosphatases in the tissues tested has been discussed.

Successful correction of total anomalous pulmonary venous connection in a 2.5 kilogram premature neonate.

A 16 day old 2.5 kg premature infant with total anomalous pulmonary venous drainage to the coronary sinus was successfully treated with use of profound hypothermia and total circulatory arrest. To the best of our knowledge, this neonate is the smallest to successfully undergo correction of this anomaly. A plea is made for a more aggressive approach in early recognition and assessment of these critically ill infants, followed by early surgical correction.

Common childhood bacterial infections.

Acute bacterial infections are a common cause of pediatric visits to the emergency department. The diagnosis and treatment of pharyngitis, otitis media, pneumonia, urinary tract infection, acute bacterial lymphadenitis and gastroenteritis are reviewed. The authors propose the use of a limited number of antibiotic agents including penicillin, ampicillin, erythromycin, lincomycin, cephalexin, a sulfonamide and tetracycline to improve efficiency and quality of care and to allow physicians to become familar with the drugs' characteristics, indications, dosage and side effects.

Comparative in vitro studies of cinoxacin, nalidixic acid, and oxolinic acid.

Cinoxacin and nalidixic acid were found to be similar in in vitro activity against 138 Shigella isolates and somewhat less active than oxolinic acid on a weight basis. Cross-resistance developed when 10 shigellae were transferred on increasing amounts of the respective agent contained in Mueller-Hinton agar. Plate dilution studies of the effect of changes in agar pH on the minimum inhibitory concentration revealed that the antibacterial activity increased with decreasing pH. Protein binding investigations revealed a high degree of binding, with nalidixic acid > oxolinic acid > cinoxacin.

Fracture of the shaft of the femur in children.

Sixty-five cases of fracture of the femur shaft caused by trauma in children under age 12 were studied. All were treated by various conservative methods. Two patients later underwent open reduction and internal fixation. Children below 5 years of age were treated by Bryant's suspension traction method. Older children were treated by balanced skin traction. In a few cases where the position was not acceptable, the fracture was reduced under general anesthesia and fixed traction was applied. Patients were discharged with a hip spica when the fracture site was nontender (average three weeks). Undisplaced fractures were treated by a primary hip spica. Functional end results were good in 81.5%, fair in 15.4% and poor in 3.1%. Eight compound fractures in this series were treated conservatively.
