Skin imaging with high frequency ultrasound - preliminary results.

This study presents the detailed construction and the principle of performance of high frequency (HF) ultrasound scanner for skin examination. The aim of this study was to show a difference between diseased and healthy skin and to evaluate the usefulness of the scanner in monitoring of therapeutic efficacy of morphea and lichen sclerosus et atrophicus (LSA). We examined 48 patients aged between 15 and 64 years; 25 patients with plaque-type morphea, nine patients with linear morphea and 14 patients with LSA. In the course of 18 months all patients were examined before, during and after treatment. In 29 patients ultrasonographic evidence of regression (decreasing of the skin thickness) was observed, in eight patients ultrasound examination showed progression and in 12 patients no difference in the ultrasound scan before and after treatment could be shown. Our study shows that HF ultrasound scanner is suitable to differentiate between the healthy and diseased skin in morphea and LSA as well as to evaluate the treatment efficacy of these diseases.

[Magnetic resonance arteriography as a method for diagnosing arteriosclerosis of the lower extremities].

The purpose of our study was the evaluation of usefulness of Magnetic Resonance Angiography in diagnostic process in patients with arteriosclerosis of lower extremities and complications such as aneurysms and pseudoaneurysms. 105 patients were examined using 2D TOF method. We concluded the results were very similar as in radiological examinations. The MRA is a very helpful method in evaluation of constriction and occlusion of iliac and femoral arteries and aorta.

[False aneurysms of lower extremity arteries: noninvasive preop diagnosis with magnetic resonance angiography and Doppler ultrasonography].

The purpose of this study was to work out a diagnostic algorithm to be used in patients surgically treated because of a false aneurysm of arteries supplying the lower limb. The study includes preoperative vessel evaluation as well as treatment results assessment with MRA and US methods. Examinations included until now 16 patients with clinical false aneurysm suspicion. Most cases presented false aneurysms being complications after arterial graft implantation, in two patients, who were not previously operated, diagnosed false aneurysms were the result of injury. We performed 23 examinations (7 of which were postoperative treatment result evaluation examinations). Examinations were performed on MR equipment with a 1.5 T field induction. The 2D TOF with traveling presaturation method was used. Primary data and reconstructed (MIP) angiograms were analyzed by a radiologist having no access to clinical information and previous examination results. Ultrasound duplex doppler examinations were performed with 7 MHz linear probe. The MRA examinations visualized 7 false aneurysms of the lower Y-graft junction. Clinical verification, ultrasonography examinations and surgery confirmed the existence of 6 aneurysms of the Y-graft as well as 5 out of 6 diagnosed in the ileofemoral graft junctions area. The post-traumatic aorta aneurysm and the postarteriographic femoral aneurysm were also confirmed. Two false positive MRA diagnoses were due to dilatation of the artery below the graft junction, in one case it was a dilatation of the vessel below a stenosis. Furthermore, the MRA helped diagnosing stenoses and occlusions in femoral arteries and common iliac artery.

[Results of treatment of upper gingiva and palate carcinomas].

From 1981 to 1986 in the Department of Maxillofacial Surgery, Institute of Stomatology, Medical Academy in Warsaw 85 patients were treated for carcinoma of the upper gingiva and palate. The follow-up of the patients after treatment was from 3 to 7 years. The obtained results of the treatment were analysed depending on the degree of differentiation of neoplasms and their biological aggressiveness and the therapeutic method used. It was found that the results of the treatment of upper gingiva carcinoma were not satisfactory. The best results were obtained using surgical treatment followed by radiotherapy. A positive result of the treatment without symptoms of recurrence was achieved in 21 cases (24.7%).

[Comparison of the effectiveness of conventional methods of radiological examinations and computed tomography in the diagnosis of lower gingiva tumours].

In 22 patients aged 34 to 87 years treated in the Department of Maxillofacial Surgery, Institute of Stomatology, Medical Academy in Warsaw for malignant tumours of the lower gingiva the usefulness was assessed of the conventional radiological methods and computed tomography for the evaluation of malignant infiltrations. Computed tomography was found to be an auxiliary radiological method determining best the morphology and syntopy of malignant neoplasms of the lower gingiva infiltrating the soft tissues of the oral floor.

[Late results of treatment of mandibular fractures].

In a group of 1246 patients treated in the Department of Maxillofacial Surgery-Institute of Stomatology, Medical Academy in Warsaw, in a period of 3 years (1984-1986) late results of treatment of mandibular fractures were assessed. In the assessment the range of mandible abduction, function of temporomandibular joints, occlusal abnormalities, the sensory function in the area innervated by the mental nerves and the condition of posttraumatic scars changing the morphology and aesthetic appearance of the face were considered. Limitation of mandibular abduction after treatment was found in 39 cases. Functional disturbances of temporomandibular joints developed in 58 cases, and various occlusal complications were found in 130 out of 1246 patients. In 56 patients various sensory loss was noted in the innervation area of the mental nerves. Posttraumatic scars in 89 patients affected in various degree facial morphology and aesthetic appearance.

[Position of the connective tissue attachment and the image of alveolar process bone in pantomograms in a selected group of adults].

In 20 subjects aged 20-29 years the position of the connective tissue attachment was determined and the atrophy of the alveolar process bone was assessed by means of an analysis of pantomograms. The radiological image of the paradontium was compared with the result of clinical examination. It was found that radiological diagnosis by means of pantomography may be a valuable supplementation of clinical examination in case of advanced parodontitis with loss of connective tissue attachment by at least 6 mm.

[Introduction of a new technique of partial pulp amputation in permanent teeth in children after mechanical injuries. Preliminary communication].

The study material comprised 56 permanent teeth with traumatic pulp exposure complicated in 15 cases by partial tooth luxation in 52 children aged from 7 to 15 years. In each case routine clinical and radiological examinations were done. The time between trauma and procedure ranged from several hours to 70 days. In all teeth partial pulp amputation was done cutting off by the gentle technique a part of the coronal pulp and covering of the remaining pulp with calcium hydroxide, zinc oxide with eugenol or with Dycal, carboxyl cement and permanent filling. The analysis of the studied material showed that the greatest group comprised children aged 8-11 years (40 cases), and fractures of one-half of the crown prevailed (32 cases), point pulp exposure (32 cases), developmental stage Rc and A 1/2 of root (40 cases). The short-lasting follow-up made possible clinical and radiological control of only 39 teeth after 6 months, and 7 teeth after 12 months. After 6 months good results were obtained in 84.6% of cases.

[Use and evaluation of X-ray films produced by WZF Foton for occlusal radiograms].

Sensitimetric properties are described of X-ray films for taking occlusal radiograms: Dental DX, RF-42, Ultra-Speed, XR-11 KS (XM). The film Dental DX was estimated on the basis of examination of a skull specimen and clinical material. The sensitivity of the film Dental DX was 400% higher than that of the generally used RF-42 film.

[Radiological estimation of interradicular space of primary molar teeth].

Radiological estimation of interradicular space of primary molar teeth has been performed. Radiograms taken from the department's archives on which primary molar teeth without dental caries were visible have been used for the purposes of the examinations. Roentgenograms had been made for exposition conditions required for endodontic therapy needs. Periodontium, lamina dura of the alveolus, bone texture of the interradicular space and disposition of permanent tooth buds have been estimated according to the radiograms. From the examination results it followed that proper estimation of interradicular space of primary molar teeth according to radiograms made for endodontic therapy needs is not possible. Radiological image of interradicular space of primary molar teeth in various both with reference to bone structure and disposition of permanent tooth buds. In most cases the interradicular space of primary molar teeth demonstrates thin bone structure on radiograms that should be considered to be physiological state when the continuity of lamina dura of the alveolus is preserved.
