Quantitative analysis of neonatal intrapersonal synchrony between crying vocalization and arm movement.

Neonatal microrhythms and their intrapersonal synchrony between crying vocalization and arm movement were preliminarily researched using a newly developed quantitative analyzing methodology aimed at developing a neurological assessment index. Subjects were twelve 1- to 17-day-old full-term appropriate-for-date neonates. Significant synchronous as well as lagged relationships between crying vocalization and arm movement were revealed. It was found that at the beginning of the cry, movement preceded the cry, while there was no dominant action seen at the end of the cry. It is suggested that this methodology is important as a possible indicator of the integrity of early neural mechanisms.

[Studies on HLA typing in a family line with spontaneous pneumothorax].

Our patient with spontaneous pneumothorax is the oldest brother of 3 siblings. Pneumothorax had occurred in his younger sister and in 3 of his mother's 4 siblings also. Namely, pneumothorax had occurred in 5 (including our patient) of 7 persons, family line on mother's side, and thoracotomy had been performed in 3 of them. Two cases in which operative findings were obtained were introduced. Next, HLA and alpha 1-antitrypsin (alpha 1-AT) were determined in this family line. As the result, the alpha 1-AT all remained in the normal range and HLA typing was not in a special relation to the occurrence of pneumothorax in this family line.

Mental disorders induced by carbamazepine.

We present here a case with various physical and neuropsychiatric symptoms caused by the administration of carbamazepine. The patient suffering from right ophthalmic neuralgia showed fever, eczema, erythema, lymphoadenopathy, eosinophilia, vomiting, headache, dizziness, nystagmus, and various mental disorders which consisted of emotional instability, personality change, delusions of reference and persecution, depressive state, and hyperventilation syndrome during the administration of carbamazepine. The physical symptoms in the present case were conformable to the side effect of carbamazepine. The mental disorders appeared in a few days from the start of carbamazepine administration and disappeared after the discontinuation of the administration of this drug without antipsychotic therapy and have never relapsed until now. The mental disorders and the physical symptoms were in parallel with their clinical course. This kind of mental disorders induced by carbamazepine has not yet been reported.

Estimating stature from the length of the lumbar part of the spine in Japanese.

In order to estimate the stature from the length of the lumbar part of spine (LLPS), studies were made on Japanese males (n = 42) and females (n = 29) autopsied in our laboratory during 1984-1987. Somatometry was performed on the stature and LLPS in centimetres, the latter being measured from the upper edge of the first lumbar vertebral body, to the promonitorium, along the anterior surface of the spine. LLPS were 19.9 +/- 1.19 cm in males and 18.6 +/- 0.84 cm in females (mean +/- S.D.). The regression equations calculated were as follows: stature in males = LLPS x 3.23 + 101.7; stature in females = LLPS x 2.31 + 110.8. The standard errors of estimate were 6.16 cm in males and 4.05 cm in females. This method is useful for estimating the stature of severely burned or mutilated bodies which have no limbs.

[Glomerular function and urine acidification in chronic renal diseases].

Intravenous sodium bicarbonate (NaHCO3) infusion test was performed in 26 patients with chronic glomerulonephritis (CGN) and 16 with distal renal tubular acidosis (dRTA) in order to evaluate urinary acidifying capacity in chronic renal diseases. Comparative studies with glomerular filtration were planned, so that the patients with CGN were divided by creatinine clearance (Ccr) into 3 groups (G-I greater than or equal to 70, 30 less than or equal to G-II less than 70, G-III less than 30 ml/min). Proximal tubular bicarbonate (HCO3) reabsorption rate increased in CGN as Ccr decreased. Urine to blood carbon dioxide tension gradient (U-B PCO2) was above 30 mmHg in controls and below 20 mmHg in dRTA. In patients with CGN, urine HCO3 concentration (UHCO3) did not increase during NaHCO3 loading as Ccr decreased. However, U-B PCO2 rose above 20 mmHg, when UHCO3 was above 50 ml/min. Fishberg concentrating test was also performed in 15 patients with CGN and 6 with dRTA so that the relationship between urinary concentrating ability and urine acidification might be evaluated. While both functions were decreased in dRTA, U-B PCO2 in alkaline urine remained above 20 mmHg in CGN associated with moderate renal dysfunction (Ccr greater than or equal to 30 ml/min) despite decreased maximal urine osmolality. Intravenous furosemide (FM) injection test was carried out in 8 patients with chronic renal failure (CRF) and 3 with dRTA. Minimal urine pH fell below 5.5 and net acid excretion (NAE) increased in controls, whereas these responses were not seen in dRTA. In CRF, urine pH generally decreased below 5.5 and those who had a similar response to FM as dRTA, seemed to have more severe disturbance of the distal acidification. In conclusion, U-B PCO2 in alkaline urine and lowered urine pH in FM loading appeared to be a useful index of distal tubular acid excretion in patients with renal dysfunction. In CGN with moderate renal dysfunction (Ccr greater than or equal to 30 ml/min), urinary acidifying capacity remained normal in comparison with decreased urine concentrating ability.

Neuropathological study on the nucleus basalis of Meynert in Pick's disease.

The pathological changes in the nucleus basalis of Meynert (nbM) in 10 autopsied cases with Pick's disease were studied in comparison with 15 age-matched controls. Both the number and density of nerve cells and the degree of fibrillary gliosis were examined at the anterior, intermediate, and posterior divisions of the nbM. In only 2 of the 10 Pick cases was the number of neurons significantly reduced, with moderate fibrillary gliosis at the anterior division (case 5: reduced by 64%, case 8: reduced by 46%). This neuronal loss in the anterior nbM division may not be a retrograde degeneration secondary to the cortical lesions, since there were some cases without neuronal loss in the nbM despite the presence of much more severe cortical lesions than in these two cases. In all Pick cases except case 1, mild to moderate fibrillary gliosis was found without an apparent neuronal loss at the posterior division of the nbM. This change in the posterior nbM division may be due to secondary retrograde degeneration, since there was a parallel relationship between the degree of fibrillary gliosis in the posterior nbM division and the severity of cortical lesions in the temporal tip and the superior temporal gyrus, to which the cholinergic fibers are known to project from the posterior division of the nbM. The present study suggests that the mild degeneration of the nbM in most cases with Pick's disease may be secondary, but that there are also some cases of Pick's disease with a primary degeneration in the nbM.

Neuropsychiatric studies of the "pellucidumsyndrom".

Twenty-nine cases with the cavum septi pellucidi were examined by the brain X-ray CT comparing with the reported cases of "Pellucidumsyndrom" in the previous literatures. As to the clinical diagnoses of the 29 cases with the cavum septi pellucidi, there is no specificity of the clinical disorder in relation to the cavum septi pellucidi. But one case had an extremely large cavum septi pellucidi showing various types of mental and neurological symptoms similar to the "Pellucidumsyndrom" described by Wilder. It is suggested that the extremely large cavum septi pellucidi of the present case may produce the neuropsychiatric symptoms according to the direct action to the neighboring regions of the cerebrum.

A clinical case of Klinefelter's syndrome with various psychiatric symptoms.

We present here a clinical case of Klinefelter's syndrome with various psychiatric symptoms. A 60-year-old male showed delusions of persecution, poisoning, and reference, auditory, visual and somatic hallucinations, depressive state and memory disturbance. These clinical symptoms except for memory disturbance were the characteristic clinical features of psychiatric symptoms in some cases of Klinefelter's syndrome reported so far. There has been no reported case of Klinefelter's syndrome showing memory disturbance. The memory disturbance of the present case is regarded as an amnestic organic brain syndrome (DSM-III) due to Klinefelter's syndrome.

[Traumatic diaphragmatic hernia].

Five cases of traumatic diaphragmatic hernia were reported and 495 cases reported in Japan were reviewed. Analysis of cases of hernia following blunt trauma in the present study provided a simple classification of fissure of the diaphragm, i.e., "tension type (indirect type)" and "impact type (direct type)". In the former type, the fissure occurs in the central tendon or its vicinity in the diaphragm as a result of distortion of the bony thorax without direct force on the diaphragm, while in the latter type, the fissure occurs in the diaphragm close to the struck bony thorax as a result of blunt force on the bony thorax to which the diaphragm is attached.

[Angiographic findings in a patient with spontaneous hemopneumothorax].

We experienced one patient with spontaneous hemopneumothorax in whom angiography was performed preoperatively. The patient was a 23-year-old female and her plain chest X-ray revealed an index-finger-tip-sized bulla at the left apex and a narrow restiform shadow connecting the pleural cupola with the bulla. Left subclavian arteriography revealed preoperatively that the restiform shadow consisted of aberrant vessels branched from the left costocervical trunk and distributed in and around the bulla at the apex. In the present study very rare angiographic findings and excised specimens obtained from the patient are shown, with reference to surgical indication of spontaneous pneumothorax.

A Graves' disease of identical twins with mental disorder.

We present here the clinical cases of identical twins with Graves' disease associated with various mental disorders. They showed restlessness, hyperexcitability, emotional instability, anxiety, depressive state, obsessive-compulsive manifestations, and delusions of persecution and reference. In the case of twin A, the psychiatric symptoms and Graves' disease were in parallel with their clinical course, and his psychiatric symptoms disappeared after a subtotal thyroidectomy. In the case of twin B, however, the psychiatric symptoms had almost disappeared during the period of hypothyroidism for two months after a subtotal thyroidectomy, but the symptoms recurred in spite of the data of euthyroidism in blood examinations after that period of time. It was suggested that the psychiatric symptoms of twin B were due to the euthyroid Graves' disease. The close resemblance of the psychiatric symptoms in both twin A and B might be based on the same genetic disposition due to their being identical twins.

The mechanism of experimental adipocere formation: substrate specificity on microbial production of hydroxy and oxo fatty acids.

Studies are reported on microbial conversion of various unsaturated fatty acids to 10-hydroxy and/or 10-oxo fatty acids by Micrococcus luteus. Four fatty acids possessing cis-9-unsaturation produced 10-hydroxy and 10-oxo fatty acid products, but three enoic acids possessing trans-9-unsaturation or double bond(s) in other than the 9-carbon position were inactive as substrates. 10-Hydroxy palmitic and stearic acids were converted to the corresponding 10-oxo fatty acids, but the 10-oxo compounds were inactive as substrates. This indicates that the metabolic sequence of cis-9-enoic fatty acid by the microbial enzyme(s) is first converted to 10-hydroxy fatty acid and then to its 10-oxo compound.
