A 42-year-old female visited our hospital because of left breast tumor and left arm swelling with severe pain. She had had right radical mastectomy and bilateral oophorectomy at 27 and 29 years of age, respectively. On admission, she had a hard mass, which seemed to be a severe invasion of the chest wall, on her left breast with a severe nipple ulcer. We inserted a catheter operatively through the thyrocervical truncus to the subclavian artery for the arterial infusion therapy. She was administered 250 mg of 5-FU daily, and 10 mg of ADM, 10 mg of CDDP, 10 KE of OK-432, every other week. During 70 days, 10,000 mg of 5-FU, 50 mg of ADM, 50 mg of CDDP and 50 KE of OK-432 were administered. As soon as the breast tumor became smaller, showed some mobility and the nipple ulcer healed, we carried out left mastectomy and axillary lymph node dissection. Pathological findings showed severe degeneration and necrosis of cancer cells. Lymphocytes surrounded necrotic tissue, and there was a follicular pattern of invasion. This phenomenon was considered to result from the promotion of cellular immunological reaction by OK-432.

Terodiline HCl was administered in a long-term study to 20 patients with neurogenic bladder and pollakisuria. Its efficacy on urinary frequency and urinary incontinence was studied together with its safety and changes in blood concentration. The dosing period extended from 2 through 53 weeks (21 weeks on the average). The drug was found effective in 62% of diurnal urinary frequency patients, 71% of nocturnal urinary frequency, 73% of urinary incontinence, and 69% of nocturnal enuresis. Side effects of dizziness and nasal obstruction were seen in only one case. The drug was judged to be useful in 75% of the patients studied. Terodiline HCl showed no further increase in plasma concentration due to the long-term administration, and it disappeared from plasma within one to two months after the last dosing.
We have previously reported that in culture, rabbit serum inhibits the growth of the epithelial cell line from Buffalo rat liver (BRL) lower than that of the tumorigenic one transformed by Rous sarcoma virus (RSV-BRL). Here, the serum was fractionated by several different methods. The findings are: 1) the growth inhibitor present (GI) existed as large complexes with non-inhibitory proteins; 2) the complexes were dissociated by 1 M NaCl plus 6 M urea; 3) the dissociated GI did not pass through membrane filter with Mr cutoff 10k; 4) it was stable in 8.5 M urea and 1 M acetic acid (pH 2.5), but labile against either dithiothreitol and trypsin; 5) it was separable into two species with pI 7.5 and 9.5; 6) both species were more effective on RSV-BRL than on BRL.
Several amino beta-lactam antibiotics, including ampicillin, amoxicillin, cyclacillin, cephalexin, cephradine and cefadroxil, were found to bind in vitro to specific components in 105,000 g supernatant of homogenate obtained from rat intestinal mucosa. The major binding component (fraction b) was purified by chromatography on DEAE-cellulofine and by gel filtration on Sephadex G-50. The molecular weight of fraction b was determined by SDS polyacrylamide gel electrophoresis (15,000 Da). The binding behaviour of these amino beta-lactam antibiotics to fraction b were estimated by equilibrium dialysis. There were significant high affinities of all tested amino beta-lactam antibiotics which were well absorbed from intestine, but there was not a good correlation between binding and absorption of these drugs. It was also found that poorly absorbed cephalosporins which lack aminobenzyl group in their structure, cefazolin and cephaloridine, did not bind to fraction b.
The characteristics of binding of amino beta-lactam antibiotics including ampicillin, amoxicillin, cephalexin and cephradine to fraction b obtained from the 105,000 g supernatant of rat small intestinal mucosa was investigated. The mutual inhibition of binding among these antibiotics was observed, and these were dependent on the concentration of inhibitors. It was found that dipeptides such as L-carnosine and glycylglycine significantly reduced the binding of cephalexin to fraction b, but the binding of cephradine was only slightly decreased by these dipeptides. Furthermore, the binding of cephalexin and cephradine was not influenced by amino acids (L-phenylalanine, glycine). Although ANS(1-anilino-8-naphthalenesulfonic acid magnesium salt), which is a hydrophobic probe, bound to the fraction b, there was no competitive inhibition in binding between ANS and amino beta-lactam antibiotics. The Scatchard plot of binding data of cephalexin gave two dissociation constant (Kd) values (1.37 and 15.7 microM). On the other hand, one Kd value (11.2 microM) was obtained for ampicillin.
An unusual case of a coexistence of pancreas divisum and intestinal malrotation in a patient with cholecystolithiasis is described herein. The diagnosis of pancreas divisum was established by endoscopic retrograde pancreatography, and the intestinal malrotation was diagnosed by duodenography and barium enema. An operation was performed for the cholecystolithiasis. The pancreas was soft, and its shape was almost normal. Cholecystectomy and prophylactic appendectomy were performed, however nothing was done to the pancreas.
The method of human gallbladder epithelial cell culture has been developed successfully with active mucus secretory function. Human gallbladder epithelial cells were dissociated by Dispase digestion from the specimens obtained by cholecystectomy for uncomplicated gallbladder stone cases. The dissociated cells formed a monolayer in Eagle's minimum essential medium supplemented with 10% fetal bovine serum within 24 h after the inoculation. These cells were maintained for at least 2 wk without fibroblastic overgrowth. Cultured cells contained periodic acid Schiff-positive material in cellular cytoplasm for 3 d. On transmission electron microscopy these materials were identified as mucous secretory granules. Mucous secretory function was determined by [3H]glucosamine incorporation. Sixty percent of the secreted glycoproteins labeled with [3H]glucosamine was eluted in excluded fractions of Sepharose 4B gel filtration, which were considered to be mucous glycoprotein, because they were found to be resistant to proteoglycan-specific enzymes such as hyaluronidase, chondroitinase ABC, heparitinase, and heparinase. The mucous glycoprotein secretion was maintained for 3 d and found to be inhibited in a dose-dependent manner by monensin (10(-7) to 10(-5) M) which is a known blocker of secretory function.
We examined diurnal changes of pancreatic exocrine secretion and plasma levels of cholecystokinin (CCK) and human pancreatic polypeptide (hPP) in men with postoperative transient external pancreatic drainage (3 with pancreatico-jejunostomy: PJ, and 5 with pancreato-duodenectomy:PD). Plasma CCK levels increased and the pancreatic exocrine secretion showed a corresponding increase after meals in both groups. In the PJ group hPP levels increased after meals and there was a significant correlation between the plasma levels of CCK and hPP. In the PD group the plasma hPP levels were not detectable throughout the study and there was a better correlation between the exocrine secretion and the plasma CCK levels than in the PJ group. Plasma CCK levels showed rhythmic changes associated with pancreatic exocrine secretion, with no significant changes in plasma glucose and insulin in the night (24:00-07:00). These results suggested that (1) the resection of the pancreatic head which is rich in hPP contents led to a complete loss of anti-CCK effects on CCK, and that (2) fluctuation in plasma CCK levels, in the fasting state, might initiate pancreatic exocrine secretion.
The non-malignant epithelial cell line from Buffalo rat liver (BRL) and its malignant transformant obtained by Rous sarcoma virus infection (RSV-BRL) were used as the indicators. A survey was made in animals for growth inhibitors which could inhibit the growth of BRL, but not the growth of RSV-BRL in culture, under the consideration that the loss of the sensitivity to such inhibitors might be the most dominant cause for acquisition of tumorigenicity. From human platelets, a growth inhibitor was extracted with acid-ethanol and prepared to a homogeneous purity, separated from the major TGF-beta activity, which stimulated the anchorage-independent growth of NRK cells in the presence of EGF. The purified inhibitor (Mr 25-27k; pI 8.7-9.1) showed 50% inhibition on the growth of BRL at 20 pg/ml (0.7 pM), but hardly influenced the growth of RSV-BRL. Its inhibition was specific to epithelial cell lines, thus called epithelial growth inhibitor (EGI). It consisted of two subunits with practically identical Mr, which were cross-linked with disulfide bonds. Its sequence of N-terminal 10 amino acids was consistent with that of TGF-beta. In rats, serum contained a growth inhibitor with Mm 220k. The inhibitor, when treated with high concentrations of urea or guanidine-HCl, liberated an active component with Mr 30k and pI 5.2. When the growth inhibitor with Mm 220k was treated with ethanol-acetone or 1 M acetic acid, its activity increased more than 10-fold. Growth inhibitors were extracted with acid-ethanol from various tissues of rats. The activities of the extracts to inhibit the growth of BRL cells were high in spleen, lung and tumors with relatively high mitotic activities, but low in muscle, heart and brain. Acid-ethanol extract from BRL cells showed growth inhibitory activity toward BRL cells themselves significantly higher than that from RSV-BRL cells. The serum-free media conditioned with BRL and RSV-BRL cells showed a little growth inhibitory activity toward BRL cells. On treatment with acetic acid, their activities increased significantly, comparable for both types of cells. The media thus activated inhibited the growth of BRL cells, but not that of RSV-BRL cells. All the growth inhibitors described above including human platelet inhibitor, whether present in extracts or purified, resembled one another in stability against acid, heat and reducing reagents.
A comparative study was done on the pancreatic endocrine (insulin (IRI), human pancreatic polypeptide (hPP] and exocrine secretion (fluid volume, amylase output, bicarbonate output), before and after administration of proglumide. At the time of the test meal loading, plasma hPP and cholecystokinin (CCK) were also measured. During continuous i.v. administration of proglumide 750 mg and 1650 mg/h 30 min before CCK-8 (20 ng/kg) stimulation, no significant difference in the rise of plasma hPP level was observed, as compared with a single stimulus of CCK-8, nor were there any significant differences in the exocrine secretion. The values of hPP and CCK-8 under a load of test meal significantly elevated after a food load. Although hPP showed a significant inhibition with the administration of proglumide 1650 mg/h, plasma CCK-8 and blood sugar levels were not significantly different from control groups. Therefore, while proglumide has a very weak effect on the action of pancreatic endocrine function, it does have an inhibitory effect on pancreatic function, during physiological stimulation.
