Reliability of HbA1 assay in the mass survey for diabetes mellitus, with special reference to the preservation and transportation of blood samples.

Studies on the reliability of the HbA1 assay in mass surveys for diabetes mellitus were carried out with special reference to the preservation and transportation of blood samples. It is essential to confirm that the preservation and transportation of samples have no effect on values for HbA1, since most of the mass surveys are carried out as field works. In our experience the levels of HbA1 remained unchanged for one week, both in samples kept at 4 degrees C, and in frozen samples kept at -40 degrees C or -80 degrees C and transported on solid CO2. The levels of HbA1 in the samples transported from Manila to Wakayama by the above-mentioned methods did not differ from those obtained in corresponding fresh samples. There was a good correlation between levels of HbA1 and levels of plasma glucose obtained 1 or 2 hr after breakfast (PPG). It was concluded that the use of both criteria (HbA1 of more than 8.00% and PPG of more than 120 mg/100 ml) in the diagnosis of diabetes mellitus is more reliable than use of either one alone.

Proposal of the diabetic diet load test (DLT) as a new diagnostic.

The diet loading test (DLT), which measures the change in the blood sugar (BS) level after a loading diabetic diet (5 units = 400 kcal, including 50 g of carbohydrate), has been devised as a new diagnostic for diabetes. Twenty-seven and 253 patients classified as borderline type (G-B) and diabetic type (G-DM), respectively, from the results of 50 g-OGTT (GTT) were subjected to DLT to evaluate its clinical usefulness. Diagnostic criteria of DLT were established from the BS levels (mean +/- 2S.D.) in 46 normals as follows: normal type (D-N), lower than 100 mg/100 ml at baseline and than 120 mg/100 ml both 1 and 2 hr after loading; diabetic type (D-DM), higher than 120 mg/100 ml at both 1 and 2 hr; borderline type (D-B), neither of these patterns. According to these criteria, 253 patients of G-DM were divided into 249 D-DM and 4 D-B; 27 G-B were subclassified into 3 groups: 7 D-N (all of them were above 70 years old), 13 D-B and 7 D-DM (all of them were in a remission stage of diabetes). Furthermore, the results of DLT were more closely correlated with HbA1 levels, the daily profile of BS and the degree of retinopathy than those of GTT. The reproducibility of DLT was also better than that of GTT. In conclusion, DLT is a new system which makes it possible to differentiate physiological glucose intolerance in the aged and in patients in a remission state of diabetes from borderline cases diagnosed with GTT. Furthermore, it was proved that a better correlation existed between the state of BS control and the results of DLT than those of GTT. The usefulness of DLT in a population survey was also proved.

Status of antitoxic immunity against diphtheria in Japan. A report of the National Epidemiological Surveillance for Important Communicable Diseases.

Antitoxic immunity against diphtheria was surveyed during the period from 1962 to 1980. The survey was done by Schick test in the first decade and then by antitoxin titration of sera by the cell culture method in the recent six years. The data show clearly that the successful control of diphtheria has been accomplished in Japan as the result of active immunization. Both single (D) and combined (DP or DPT) vaccines were effective to convert Schick reaction negative. In addition, immunity was greatly improved by the introduction of the combined vaccine and one additional booster injection. However, such subsequent changes in the immunization schedule introduced in 1976 that giving the primary vaccination at older age and the omission of one booster injection at the preschool, has resulted in appearance of a high risk group below three years of age as well as in the lower immunity levels among school children. The data indicate that the continuous surveillance for diphtheria is required in this country.

Neuraminidase-sensitive erythrocyte receptor for enterovirus type 70.

Enterovirus type 70 (EV70) agglutinated human 'O' erythrocytes at 4 degrees C as well as 22 degrees C, but visible agglutination was lost when warmed at 37 degrees C although the virus remained attached to the surface of the erythrocyte. The receptor sites for the virus were neuraminidase-sensitive. A direct involvement of sialic acid on the cell surface in virus-cell interaction was confirmed by the fact that the presence of fetuin or free N-acetylneuraminic acid inhibited the haemagglutinating activity of EV70. Similar numbers of virus particles were required for 1 haemagglutinating unit (HAU) of EV70 and 1 HAU of mengovirus, whereas 2.6-fold or more of virus particles of echovirus type 7 and type 11 gave the same activity. On the other hand, the number of receptor sites on the cell surface for EV70 was found to be sevenfold more than for mengovirus. Therefore, the erythrocyte receptor for EV70 is different from that for common enteroviruses and similar, though not identical, to the cardiovirus receptor. However, serological tests such as neutralization, complement fixation or haemagglutination inhibition did not reveal any common antigen between EV70 and cardiovirus.

Characterization of a temperature-sensitive defect of enterovirus type 70.

The mechanism of the failure of enterovirus type 70 to replicate at a nonpermissive temperature (39 degrees C) was investigated, and the following results were obtained. (i) Viral RNA synthesis was not observed at 39 degrees C in LLC-MK2 cells, in accordance with our previous findings with primary monkey kidney cells (Miyamura et al., Intervirology 9:206-213, 1978). (ii) Shutoff of host cell macromolecular synthesis by virus infection was as efficient at 39 degrees C as at a permissive temperature (33 degrees C). This inhibitory effect similarly occurred even in the presence of guanidine hydrochloride. (iii) Viral protein synthesis proceeded in vivo at the nonpermissive temperature, and the rate of the protein synthesis was higher than that at the permissive temperature under the conditions in which sufficient viral mRNA had been accumulated. This was also confirmed by analyzing the intracellular proteins synthesized at the nonpermissive temperature by sodium dodecyl sulfate-polyacrylamide gel electrophoresis, which identified them as virus-specific proteins. (iv) When infected cells were incubated at 39 degrees C and then transferred to 33 degrees C, viral RNA synthesis took place even in the presence of cycloheximide. (v) Furthermore, in experiments performed with an in vitro cell-free assay system, viral polymerase activity was found in the membrane-bound preparation extracted from infected cells which had been incubated at 39 degrees C in the presence or absence of guanidine hydrochloride. These results indicate that early translation of mRNA proceeds normally at the nonpermissive temperature and that the temperature-sensitive defect resides in the transcriptional stage.

Development of parathyroid adenoma in a case of chronic renal failure suggesting tertiary hyperparathyroidism.

A case of tertiary hyperparathyroidism with a parathyroid adenoma and hyperplasia was described as the first report in Japan. A 24-year-old man manifesting hypercalcemia had been treated with hemodialysis for 9 years. On admission, clubbed fingers richitic rosaries and a palpable node in the neck were the major physical findings. Laboratory examinations revealed hypercalcemia had elevated levels of plasma parathyroid hormone (PTH) in a c-terminal assay. Nevertheless the levels of PTH found by a n-terminal assay disclosed an only small increase. The PTH levels were scarcely decreased after a bolus injection of calcium gluconate. A large tumor corresponding to the palpable node was resected. Histologically, the tumor was an adenoma with thick fibrous capsule and interparenchymal bands. Other parathyroid glands showed secondary clear cell hyperplasia without signs of cellular hyperactivity.

Enterovirus type 70-neutralizing IgM in animal sera.

The neutralizing activity against human enterovirus type 70 was found in serum samples from normal cattle, horses, sheep, goats, swine and chickens raised in Japan. The frequency was variable depending upon animal species and the year of bleeding. The neutralizing activity in bovine sera was shown to reside in IgM by sucrose gradient centrifugation and immune gel electrophoresis. These findings suggested that the neutralizing substance in domestic animal sera is the antibody of IgM class elicited by unidentified viruses antigenically related to human enterovirus type 70.

Seroepidemiologic studies of acute hemorrhagic conjunctivitis virus (enterovirus type 70) in West Africa. II. Studies with human sera collected in West African countries other than Ghana.

Human sera were collected in Senegal, Nigeria, Ivory Coast, Dahomey, Liberia, Gabon and Togo during the pre-epidemic period of acute hemorrhagic conjunctivitis (AHC) from 1965 to 1969, and tested for virus neutralizing (VN) antibody to enterovirus type 70 (EV70). Of these, 1109 (91%) were antibody negative (less than equal to 1:4), 116 (9%) neutralized at a dilution of 1:8 or over, and 45 (4%) at dilutions of at least 1:16. The distribution pattern is not significantly different from that of sera collected from Kenya in 1967 or from army recruits in the United States, Argentina, Brazil and Colombia in the 1960s. Sera collected during the post-epidemic period (1970 to 1977) in Senegal, Sierra Leone, Mali, Upper Volta, Chad, Niger and Gabon were also examined; 1573 (68%) were VN antibody negative (less than or equal to 1:4), while 733 (32%) and 433 (19%) had titers of 1:8 or greater and 1:16 or over, respectively. There is a significant difference in distribution between pre- and post-epidemic antibody titers (p less than 0.001), although the incidence of AHC was lower in these countries than in Ghana and Southeast Asia. The prevalence of VN antibodies tends to be lower in the dry, hot inland areas and thus humid coastal monsoonal climates and dense populations seem to favor the spread of AHC.

Prevalence of antibody to hepatitis A virus in Sri Lanka.

Antibody to hepatitis A virus was studied by immune adherence hemagglutination using 287 serum specimens collected in 1975 by random sampling from healthy individuals living in Colomobo, Sri Lanka. The overall prevalence of antibody to the virus was 76.9% and the prevalence between males and females was approximately the same. The age-specific prevalence of the antibody indicated that hepatitis A in the area is mainly an infection during the early childhood.
