[An assessment of nosocomial infections of methicillin-resistant Staphylococcus aureus based on coagulase typing and phage typing].

An epidemiologic investigation was carried out in Ogaki Municipal Hospital to clarify the status of nosocomial MRSA Infection between 1989 and 1991. In 1989, coagulase type IV, enterotoxin A-producing, and phage group I strains, which were highly resistant to multiple antibiotics and isolated in the internal wards, accounted for 43.4% of all MRSA strains clinically isolated in the entire hospital. In 1990, coagulase type II strains that were sensitive to GM but resistant to FMOX and IPM increased. There were significant differences in the frequency of detection of various strains among wards, suggesting an inter-ward variation in MRSA strains. Changes in environmental strains reflected those in clinical strains. The findings suggest the necessity of measures not only for long-hospitalized MRSA carriers themselves but also for the environment of patients, medical staff, and those taking care of patients.

[Studies on biodegradable microcapsules of anticancer drugs].

We made biodegradable microcapsules for the controlled release of an anticancer drug. The spherical capsules were designed to be infused repeatedly through an implanted angiographic catheter. The microcapsules were made by the Wurster method, and consisted of a core consisting of lactose and a shell consisting of lecithin, cholesterol, stearic acid, and polyvinylpyrrolidone. The capsules with diameters of 50 to 200 microns could be infused through a catheter. In blood plasma, the capsules absorbed water and disintegrated in 20-30 min. The amount of drug released from the microcapsules in a test of dissolution in a column could be controlled by changes in the thickness of the shell. These capsules will now be tested in rabbits with experimental tumors.

Angioscopy of the inferior vena cava: preliminary observations in cases with involvement by neoplasm. Work in progress.

Angioscopy of the inferior vena cava (IVC) was performed in 10 patients. Tumor thrombus extending into the IVC was suspected in six patients, and direct invasion of the IVC wall was suspected in four, based on computed tomographic and venographic findings. In each case, a fiberscope was directed to the area of interest in the IVC via a transfemoral 5-F catheter. Blood displacement by means of a saline infusion enabled angioscopic observation of the IVC wall. The neoplasms were seen clearly in all patients but one. The color and texture of the tumor thrombi surface made it easy to distinguish from the healthy IVC wall. In the four patients believed to have direct invasion of the wall, there were no angioscopic differences between the suggestive areas and adjacent areas of normal IVC wall. In these four cases, the absence of direct invasion was confirmed at surgery. Thus, angioscopy helps make an accurate nonoperative or preoperative diagnosis of tumor thrombi possible and may aid in the exclusion of direct invasion.

[Two routes chemotherapy by CDDP and STS against liver tumor].

Nine (eight males, one female) patients with unresectable liver tumor (seven HCC and two metastasis) were treated by two-routes chemotherapy using cis-diamminedichloroplatinum (CDDP) and sodium thiosulfate (STS). In these patients, 50-100 mg/body of CDDP was administered through the proper hepatic artery or right hepatic artery by one shot infusion or the balloon-occluded arterial infusion (BOAI) at 10 mg/min, and during administration, intra-inferior vena cava injection of STS (4 g/body) was given. None of 9 patients suffered nausea and vomiting during the treatment, 3 of 9 patients suffered nausea and vomiting to a mild degree after the treatment, none of 9 patients showed significant side effects, such as bone marrow suppression and/or renal disfunction. In conclusion, this study demonstrated the protection effect of STS injected in inferior vena cava against the toxicity of CDDP were well indicated.

Variations in intrathyroidal lithium content and their effect on the iodide uptake in mouse thyroid.

Lithium (Li) is accumulated in the thyroid but the mechanism of Li accumulation is not known. In the present study, the causes of variation in Li concentration in the thyroid and the relation between cellular Li and iodide were examined. This was done by using mice treated with Li (0.01% as Li2CO3) for 4 weeks, co-administered with propylthiouracil (PTU, 0.5 mg/ml daily p.o.) or thyroxine (T4, 0.5 micrograms per day i.p.) for last 10 days. The total content of Li in a whole thyroid (ng/thyroid) was not changed through treatment with PTU or T4. But the Li concentration in terms of mg/kg of the gland was reduced with PTU and was unchanged or slightly increased with T4, due to the change in the mass of thyroid. Furthermore, short-term (3 h) Li uptake in the thyroid was not affected by pre-treatment with PTU or T4. These results indicate that the variation in thyroidal Li concentration was not due to a direct effect of PTU or T4 on Li transport, but to a thyroidal condition caused by the secondary influence of drugs. A measurement of the thyroid: serum iodide concentration ratio (T/S) of 125I showed that the iodide uptake was reduced when intracellular Li concentration was high, and that PTU alone elevated the T/S, but Li + PTU brought it back to a normal level; whereas, T4 alone diminished the T/S, Li + T4 made it rise significantly more than did T4 alone.(ABSTRACT TRUNCATED AT 250 WORDS)

[Experimental study of transvascular direct current treatment for intraabdominal malignant tumors].

Direct current treatment (DCT) is a treatment for malignant tumors suggested by Nordenström in 1978. In DCT, two platinum electrodes, an anode and a cathode, are introduced percutaneously. Usually the anode is inserted into the tumor, and the cathode is inserted into healthy tissue near the tumor. Passage of a direct current acidifies the surroundings of the anode and causes necrosis of this part of the tumor. DCT has been used to treat lung cancer and superficial tumors clinically, but it has not yet been used for intra-abdominal tumors. Here, basic and experimental studies were made of the possibility of DCT of intra-abdominal tumors. A modification, called transvascular DCT, was done with the anode inserted into the liver of rabbits and the cathode placed within a blood vessel by vascular catheterization. First, tests were done to identify a suitable material for the cathode, from the standpoint of safety. DCT is usually done with platinum electrodes. However, in transvascular DCT, platinum could not be used for the cathode because with its use, hydrogen gas was produced in the vessel, causing embolisms. So, a cathode made of AgCl and coated with Ag was developed to prevent embolism. With this cathode, gas was not generated. Next, pathological changes in the walls of the vessel that contained the cathode were observed after DCT. The cathode was put into a tube placed in the inferior vena cava (IVC) or the aorta. a. IVC: A histological study done immediately after DCT showed an area of necrosis in the wall of the IVC in all 19 rabbits. A study done 7 days after DCT showed damage in the IVC wall of all five animals examined. b. Aorta: A histological study done immediately after DCT showed a small edematous area in the wall of the aorta in seven of the 22 animals, and no evidence of any damage in the other 15. A study done 7 days after DCT showed no damage in any of the 16 animals. The aorta was more suitable for use as the vessel to contain the cathode than the IVC, because injury of its wall was minor and reversible. Thus, transvascular DCT could be done safely by use of an Ag-coated cathode made of AgCl and placed in the aorta.

Effect of lithium carbonate administration singly or in combination with some psychotropic drugs on the radioiodide uptake by mouse thyroid.

The present study dealt with two objects; the first object was to examine whether or not lithium uptake in thyroid is modified by the thyroid state and how the intrathyroidal Li affects iodide uptake by the thyroid. Male and female mice were given lithium carbonate (Li) with propylthiouracil (PTU) or thyroxine (T4). Li was measured by a flameless atomic absorption spectrometry. The total Li content in thyroid was unaffected with PTU or T4, however, Li concentration per unit mass was reduced by PTU but unaffected by T4. The thyroid: serum ratio (T/S) of 125I resulted that the T/S became higher when Li concentration per unit mass was lower and vice versa, suggesting that Li uptake is controlled by thyroid states and Li in the gland interferes with the iodide uptake. Serum triiodothyronine (T3) and T4 by radioimmunoassay showed that PTU alone and in combination with Li lowered serum T4, while a high level of T4 by its supplement was suppressed by co-administration of Li. T3 level was lowered by Li alone, but not severely affected by other drugs. The results suggest that Li enhances T4 clearance without T4-T3 conversion. The second object was to examine the effect of combined psychotropic drugs on thyroid function. Carbamazepine (CBZ), haloperidol (HLP) and imipramine (IPA) were given singly or in combination with Li to examine their effects on the T/S of 125I. Only CBZ reduced the T/S but CBZ plus Li had no summative effect. Neither HLP nor IPA affected the T/S, singly or in combination with Li, suggesting that HLP or IPA does not interfere with an iodide pumping machinery. No distinct sex difference was observed in drug effects.

[Adrenal arterial embolization and radiotherapy of a case with metastatic adrenal tumor and its tumor thrombus in inferior vena cava from hepatocellular carcinoma].

Right adrenal arterial embolization and radiotherapy were performed in a patient to treat the metastatic tumor and its tumor thrombus in inferior vena cava from hepatocellular carcinoma. This case developed no complication following the embolization and radiotherapy. This therapy necrosed the metastatic tumor and made inferior vena cava recanalization. Adrenal arterial embolization and radiotherapy were effective treatment for the metastatic adrenal tumor and its tumor thrombus.

Diagnosis of thyroid mass due to intravenous digital subtraction angiography--does it play an important part in diagnosis of thyroid mass?

33 patients with thyroid mass were studied by intravenous digital subtraction angiography (IV-DSA), thyroid scintigraphy (TS), ultrasonography (US), and computed tomography (CT). The detective rate of lesion by each diagnostic imaging was as follows: IV-DSA 71%, TS 70%, US 94%, and CT 76%. US was the most excellent and most sensitive diagnostic imaging of both benign and malignant lesions. As regards the thyroid cancer by IV-DSA, malignant findings were heterogeneity, an irregular contour, and an unclear contour of tumor stain. Specific diagnostic rate of each imagings was as follows: IV-DSA 88%, TS 100%, US 86%, and CT 75%. US was not so good imaging of a specific diagnosis for the thyroid mass. As regards the thyroid mass, influence of the common carotid artery and the internal jugular vein were investigated by the degree of compression of both vessels on IV-DSA. However IV-DSA could not reveal tumor invasion to these vessels. We learned present the limit of diagnosis of the thyroid mass due to IV-DSA, because IV-DSA was unable to reveal to us exactly information of both a sensitive and a specific diagnosis. We, however, think that IV-DSA is a useful diagnostic imaging as a mapping study which reveals a coronal image of a relationship between the thyroid mass and the common carotid artery and/or the internal jugular vein in surgery.

Two cases of venous aneurysm of the intrahepatic portal vein branch: MR demonstration.

Two cases are reported of congenital intrahepatic portal vein aneurysm in women who had chronic hepatitis. Ultrasound (US) and computed tomography (CT) investigations revealed vascular communication between intrahepatic portal vein and venous aneurysms in both cases. These portal vein aneurysms were fusiform in shape and two and one centimeter in diameter in each case. On angiography portal-hepatic venous shunt was observed in one case. The MR image of two cases were presented. We believe this is the first report of MR image of portal vein aneurysm.

Effects of administration of dopamine and L-DOPA to dogs on their plasma level of dopamine sulfate.

The effect of intravenous administration of dopamine (DA) or L-3,4-dihydroxyphenylalanine (L-DOPA), its immediate precursor, on the level of DA sulfate in dog plasma was examined, to clarify the source and physiological significance of DA sulfate which is present at high level in the plasma. After DA administration, the plasma level of free DA increased markedly, but the level of DA sulfate did not change. However, after administration of L-DOPA, the levels of both free DA and DA sulfate increased greatly. After a single injection of L-DOPA, increase in the level of free DA was transient, but that of DA sulfate persisted for a long time. These results suggest that some of the DA sulfate in dog plasma is formed from circulating L-DOPA, not from circulating DA, and that formation of DA conjugate may play a role in regulating the plasma level of free DA.

[The availability of digital subtraction angiography using continuous intra-arterial infusion tubes founded various malignancy].

DSA was employed using continuous intraarterial infusion tubes for various malignancies (73 cases) which were examined a total of 135 times. In head and neck malignancy (50 cases), the general position of the infusion tube had been determined beforehand by dye infusion, but DSA from the tube showed that the tubes in 24 cases (48%) were located in the wrong position, especially in tongue cancer (21 cases) where many tubes were discovered to be in an erroneous position (71%) such as the common carotid artery. We were unable to determine the effect of chemotherapy and radiation using DSA only. In 9 cases of breast cancer for which fixation of the tube was not attempted under X-ray fluoroscopy, 7 (78%) showed an unusual tube position such as the intraaortic arch. In 5 cases of abdominal malignancy, only the tube position for sigmoid colon cancer was unusual. We were able to observe the effect of chemotherapy by DSA in 2 cases. For DSA in one out of 3 hepatomas using a Port-A-Cath, we observed that infusion of anticancer drug with degradable starch microspheres caused a reduction in tumor size. However, in the two remaining cases, we were unable to observe any effect of infusion of these drugs by DSA for various mechanical reasons. DSA from an infusion tube revealed not only the location of the tube accurately and promptly, but also the effect of chemotherapy.
