Ileal neobladder. Principles of function and continence.

Since April 1986, total bladder substitution, by the ileal neobladder in cases of radical cystoprostatectomy or bladder augmentation, proved to be a reliable alternative method of urinary diversion in 81 patients. The operative technique is standardized, comparably simple and safe to prevent upper urinary tract deterioration, reflux, as well as incontinence. The neurovascular bundle can be preserved, so potency might not be compromised. As preservation of the external urethral sphincter is possible, total day and night time continence is a result of residual sphincter function and abolishment of high pressure waves of the intestinal reservoir. Follow-up of the patients is between 1 and 28 months (mean 12.2 months). 64 patients had a follow-up of more than 3 months postoperatively and the evaluation included private micturition behavior and urodynamic investigation. Stress incontinence, which has to be corrected by an artificial sphincter, was found in 3 and night time incontinence needing some external device in 2 patients. There was no perioperative mortality. The special technique of creating the ileal neobladder by folding the ileal segment 4 times with complete detubularization, besides preserving urethral sphincter function, seems to be the most important reason why total continence during the day and night is achieved in more than 90% of the patients.

[The ileum neobladder--technic and results of bladder augmentation].

Since April 1986, total bladder substitution (ileal neobladder) and bladder augmentation proved to be a reliable alternative method for urinary diversion in 81 patients. The operative technique is standardized, comparably simple and safe to prevent upper urinary tract deterioration, reflux, as well as incontinence. 11/81 patients (27-73 years, mean 52.5) underwent bladder augmentation (7 female, 4 male). 2/11 patients (1 female, 1 male) underwent undiversion (ileum conduit, neurogenic bladder). 10/11 patients are totally continent day and night. One female patient remained totally stress incontinent despite bladder neck suspension. Two female patients empty their bladder by intermittent catheterization.

A new case of pericentric inversion 20.

A prenatal diagnosis from a chorionic villi biopsy revealed a pericentric inversion of chromosome 20. The child's father also carries the inverted chromosome. The breakpoints p13q11.2 differ for 4 of the 5 published cases of inversion 20.

[Microcomputer systems in urology: hardware--mole's hole or useful equipment?].

The basic concepts and elements of micro- and minicomputer hardware components frequently used in urology are described. For advanced users, the details and trends are discussed in more detail. The interdependence of hardware and operating systems is emphasized. In addition, a method for the evaluation of personal computer performance is presented. Finally, proposals are made concerning purchasing procedures and selection criteria.

Potentiation of nitrosourea cytotoxicity in human leukemic cells by inactivation of O6-alkylguanine-DNA alkyltransferase.

The HL-60 promyelocytic leukemia cell line is resistant to nitrosoureas and contains high levels of the DNA repair protein O6-alkylguanine-DNA alkyltransferase (alkyltransferase). We examined the protective role of the alkyltransferase in the nitrosourea resistance observed in this myeloid leukemia cell line to determine whether inactivation of the alkyltransferase with the modified base, O6-methylguanine (O6mGua), could sensitize these cells to nitrosoureas. The HL-60 cells were sensitized approximately 3.0-fold to five different nitrosoureas when the alkyltransferase was inactivated by 88% following a 24-h preincubation in 0.5 mM O6mGua. No effect of O6mGua preincubation was observed in the K562 chronic myelogenous leukemia cell line which is sensitive to nitrosoureas and has low levels of alkyltransferase activity. When regeneration of HL-60 alkyltransferase activity after exposure to nitrosoureas was prevented by maintaining cells in O6mGua, HL-60 became even more sensitive (3.7- to 8.5-fold) to nitrosoureas but remained slightly more resistant than K562. Next, we compared the dose of methyl- and chloroethylnitrosoureas which were cytotoxic in HL-60 with the dose which caused repair-induced inactivation of the alkyltransferase. Both methyl- and chloroethyl-nitrosoureas caused the dose-dependent inactivation of the alkyltransferase and with both, cytotoxicity was increased with O6mGua exposure. However, chloroethylnitrosoureas, which form a variety of O6 alkylation adducts, some of which are poorly repaired, exhibited 7-12 times more cytotoxicity relative to repair-induced inactivation of the alkyltransferase whereas methylnitrosoureas became cytotoxic only when the alkyltransferase had been inactivated. These data suggest that leukemic cells are sensitized to both methyl- and chloroethylnitrosoureas when O6mGua is used to persistently inactivate the alkyltransferase. However, the alkyltransferase provides more efficient protection from methylnitrosoureas than chloroethylnitrosoureas most likely because the latter form adducts which are poorly repaired by the protein and which if unrepaired may become cytotoxic cross-links.

Clinical evaluation of the Cavitron Biotronics tonometer.

The Mackay-Marg tonometer has been shown to be an accurate tonometer for the measurement of intraocular pressure in eyes with both normal and diseased corneas. This tonometer is no longer manufactured and has been replaced by the Cavitron Biotronics tonometer that works on the same principle but utilizes a different recording display. The Cavitron Biotronics tonometer's accuracy was compared with that of the Goldmann applanation tonometer in 75 eyes. The Cavitron Biotronics tonometer is not as accurate as the original Mackay-Marg tonometer for individual readings. However, if four readings are taken, the value of the Cavitron Biotronics tonometer that correlated best with Goldmann applanation tonometry was the one reading of the four with the highest value. Using this one value, the Cavitron Biotronics instrument underestimates the true intraocular pressure below 27 mm Hg and overestimates it above this value.

The DiGeorge syndrome. I. Clinical evaluation and course of partial and complete forms of the syndrome.

This study describes clinical signs and symptoms in 16 patients with the DiGeorge syndrome (DGS). Diagnosed on the basis of typical facial stigmata, a broad spectrum of severity is seen with respect to congenital heart disease, hypoparathyroidism and immunologic parameters. A simple index of severity is introduced that clearly differentiates complete forms of the syndrome (cDGS) with poor prognosis from partial forms of the syndrome (pDGS). Of 13 pDGS patients, 12 are still living; 8 underwent corrective heart surgery without infectious complications. Moderate to severe mental retardation is seen in all pDGS patients. Due to the lack of thymus function, immunodeficiency is a result of cDGS, whereas immunoregulatory disturbances (hypergammaglobulinaemia, high titres of specific antibody production) prevail in pDGS patients.

[Value of the Miller-Abbott tube in the treatment of postoperative ileus].

This comparative study reports about therapeutic procedures in postoperative ileus. A group of 51 patients was treated with the Miller-Abbott-tube, placed by endoscopy, another comparable group, covering 41 patients, was attended by primary relaparotomy. The present study describes indications, technique, and problems of the Miller-Abbott procedure; success and failure in both groups are compared and discussed. The results of the Miller-Abbott-group enable us to reestimate conservative treatment of postoperative ileus.

Spontaneous sister-chromatid exchange frequencies in human B and T lymphocytes at BrdU borderline concentrations for sister-chromatid differentiation.

Human peripheral blood B and T lymphocytes, highly purified by immunologic methods, were supplemented with gamma-irradiated unseparated autologous mononuclear cells to restore helper functions and stimulated with pokeweed mitogen and phytohemagglutinin, respectively. Spontaneous sister-chromatid exchange (SCE) frequencies were investigated in proliferating B and T lymphocyte cultures labeled with the cell-type-specific borderline concentrations of 5-bromodeoxyuridine (BrdU) for sister-chromatid differentiation (SCD). B lymphocytes from 6 different donors showed mean values of 3.28-3.72 SCE events/cell. In T lymphocytes, mean values of 6.30-7.28 SCEs/cell were observed. The differences between the SCE distributions of the cell populations are highly significant. The results show that the differences in the spontaneous SCE frequencies between human B and T lymphocytes were not due to a difference in the uptake of BrdU.

Fetal heart rate response to intrauterine saline solution flush.

Response of the fetal heart rate to instillation of normal saline solution 5 ml through an intrauterine pressure catheter was observed on 50 occasions. The test was performed only in those patients in whom an intrauterine pressure catheter had been placed. When the fetal capillary pH was greater than 7.24, accelerations of fetal heart rate by 15 beats/min were observed in 39 of 50 occasions. On the other hand, no fetal heart rate accelerations were observed when the fetal capillary was less than or equal to 7.24.

The ileal neobladder.

An ileal neobladder for total bladder replacement was created in 11 patients. To achieve a low pressure system, disruption of directional bowel peristalsis with a longitudinal incision at the antimesenteric border of a 70 cm. ileal segment is performed. A spherical pouch, the neobladder, is fashioned and anastomosed to the urethra. The ureters are implanted according to the method of Le Duc and Camey. Videourodynamic studies during various postoperative phases demonstrate this neobladder to be a urinary reservoir with a capacity approximating that of a normal bladder, good compliance during filling by maintaining pressure lower than 30 cm. water and no reflux. Of the 11 patients with the neobladder 8 are completely dry day and night, while 3 have grade I stress incontinence. All 11 patients had recognizable sensations of bladder distension closely simulating those of normal bladders. The use of this ileal neobladder in male patients undergoing radical cystectomy offers an alternative free of a stoma to urinary diversion, resulting in a highly compliant, low pressure bladder.

Proprietary physical therapy service in entry-level physical therapy curricula.

The purposes of this study were to 1) assess the value of including an expanded business administration unit that would include proprietary practice issues in an entry-level physical therapy curriculum and 2) organize topics by priority that could be included in such a unit. Twenty-seven physical therapists (100%) in private practice in Montana responded to the questionnaire. Ninety-three percent of the respondents stated that such an expansion would be valuable in the entry-level physical therapy curriculum. Topics of greatest priority were 1) Self-analysis of Resources, 2) Reimbursement, and 3) Contracting Services and Referral Sources. Topics of least priority were 1) City and State Tax and Licensure Requirements, 2) Computers and Computer Programs, and 3) Insurance Planning. Further research and development are needed to formulate methods and objectives for such an expansion.

Monosomy, trisomy, fragile sites, and rearrangements of chromosome no. 1 in a mentally retarded male with multiple congenital anomalies.

A 17-year-old male was referred for evaluation because of short stature and severe mental retardation. Major clinical findings included microphthalmia, micrognathia, low-set ears, a prominent beaked nose, clubbing of digits, and premature graying of hair. Cytogenetic analysis revealed a 45,XY,-1/46,XY/47,XY,+1 mosaicism in lymphocytic culture, a 45,XY,-1/46,XY mosaicism in skin fibroblasts, and fra(1p) sites in 2% of the metaphases from lymphocyte, fibroblast and bone marrow cultures. Post-zygotic non-disjunction causing this mosaicism is believed to be responsible for the patient's phenotype.

Evaluation of the sensitising potential of 4 polyamines present in technical triethylenetetramine using 2 animal species.

Contact sensitivity to 4 polyamines present in technical grade triethylenetetramine was investigated using the 'VAA mouse' assay and the guinea pig maximization test (GPMT). The criteria used to assess sensitivity in the GPMT classed all substances as having the same degree of sensitising potential. The mouse assay ranked N-(2-piperazin-1-ylethyl)ethylenediamine as having a significantly greater sensitising potential than the other compounds.
