[Five cases of rectal cancer showing remarkable response to suppository administration of tegafur].

Five cases of rectal cancer, one male and 4 females, were preoperatively treated with Tegafur suppositories at 1,500 mg per day, for 8 to 21 days (mean, 14 days). These tumors were 2 early cancers and 3 advanced cancers, the configuration of which were one IIa type, one IIa + IIc type and 3 Borrmann 2 types. Their size ranged from 1.3 to 4.2 cm in diameter. Curative resections were performed in all cases, and the examination of the resected specimen revealed marked effects of Tegafur as follows: 1) Reduction of tumor size, 2) disappearance of cancer wall, 3) degeneration of cancer cells and destruction of cancer glands accompanied by fibrous stroma with infiltration of lymphocytes and plasma cells, 4) formation of xanthogranulomatous tissue with foamy histiocytes, which were characteristic findings showing that cancer tissue had been present there. As a result, rectal cancer clearly showed remarkable response to suppository administration of Tegafur for only about two weeks, if the tumor cells were sensitive to the drug.

[A case of the successful surgical treatment of mediastinal giant neurogenic tumor, and the chest wall reconstruction by the autologous ribs].

A mediastinal tumor, particularly a neurogenic tumor, sometimes develops into a giant tumor; this large tumor mass then puts pressure on the heart and lungs, and may cause severe complications. In our department, 164 mediastinal tumors were experienced, the largest one was an intrathoracic fibromyoma which weighed 3,760 g. This report deals with our recent experience in which we found a neurogenic tumor weighed out 4,500 g. This tumor was excised in an emergency operation, after which the patient's life was saved by twenty-two days of artificial respiration. Since this tumor was so large, four entire right ribs had to be removed in order to create enough space to excise it. In order to reconstruct the chest wall, three of the removed ribs were retransplanted in their original sites. An extremely good prognosis resulted.

DNA-binding protein(s) interacts with a conserved nonameric sequence in the upstream regions of wheat histone genes.

A nuclear protein(s), HBP-2, that binds to the upstream region of the wheat histone H4 gene was identified from a fractionated nuclear extract of wheat germ by DNase I footprinting. The DNase I-protected region contained the conserved nonameric motif, CATCCAACG. Cross-competition experiments that used the mobility shift assay showed that this nuclear protein(s) binds specifically to the upstream sequence that has been postulated to be a cis element of the wheat H3 gene. Our findings suggest that this DNA-binding protein(s) may be a trans-acting factor in the regulation of the transcription of wheat histone genes.

Functional differences in cholesterol ester hydrolase and acyl-coenzyme-A/cholesterol acyltransferase between the outer and inner zones of the guinea pig adrenal cortex.

Previous studies clearly demonstrated that cells isolated from the chromatically distinct inner (primarily zona reticularis) and outer (zona fasciculata and zona glomerulosa) zones of the guinea pig adrenal cortex have vastly different steroidogenic capabilities; the outer zone produces far more cortisol than the inner zone, and mitochondrial cholesterol side-chain cleavage activity in the inner zone is not modulated by ACTH. Thus, in this study we have investigated the characteristics and properties of cholesterol ester hydrolase (CEHase) and acyl-coenzyme-A/cholesterol acyltransferase (ACAT) in the outer and inner zones of the guinea pig adrenal to clarify the zonal difference in cholesterol metabolism. CEHase showed two pH optima at around pH 4.5 and pH 7.5 in the outer zone, while optimum activity in the inner zone was found only around pH 4.5, suggesting a lack of neutral CEHase in the inner zone. The acid CEHase was found mainly in the lysosomal fraction, and the neutral CEHase was located mainly in the microsomal fraction. The neutral CEHase activity in the outer zone was significantly enhanced by ether stress, while the acid CEHase activity was not changed in either of the two zones by stress. The basal activity of ACAT was significantly higher in the outer zone than in the inner zone. The present experiments also demonstrated that the inner zone always showed lower ACAT activity than the outer zone even when added cholesterol exogenously. It is suggested that the relatively low activity of ACAT may not always be explained by the reduced content of cholesterol in the microsomal fraction of zona reticularis. ACAT activity in the outer zone was significantly decreased by ether-induced stress, but that in the inner zone was not changed. The content of esterified cholesterol in whole homogenate and the microsomal fraction was significantly decreased in the outer zone, but not in the inner zone, by ether stress. Thus, the decrease in esterified cholesterol after ether stress seems to relate to increased neutral CEHase activity and decreased ACAT activity in the outer zone. The present observation clearly indicates that the lack of neutral CEHase and the lesser activity of ACAT in the inner zone than in the outer zone may contribute to the impaired cholesterol metabolism in the inner zone.

Nuclear protein(s) binding to the conserved DNA hexameric sequence postulated to regulate transcription of wheat histone genes.

Nuclear protein(s) that specifically bind(s) to the upstream hexamer motif, ACGTCA, of wheat histone H3 and H4 genes has (have) been identified. Sequences homologous to this hexamer are found to be conserved in the upstream region of not only wheat histone genes but also other plant and animal histone genes. This suggests a possible role(s) for the hexamer and the nuclear protein(s) in the transcriptional regulation of the wheat histone genes. This hexamer is homologous to the upstream core sequence, TGACGTCA, which is highly conserved in some animal genes whose expression is regulated by cAMP.

Cyclic AMP-dependent protein kinase activity and protein phosphorylation in zones of the adrenal cortex.

Steroidogenesis is not stimulated by ACTH in the inner zone of the guinea pig adrenal cortex; adenylate cyclase is normally stimulated. To further explore the lack of a steroidogenic response to ACTH in the inner zone, cAMP-dependent protein kinase activity and protein phosphorylation were examined in the outer and inner adrenocortical zones. To summarize: total cAMP-dependent protein kinase activity was 40% higher in the outer zone than in the inner zone; of the total cAMP-dependent protein kinase activity, cytosol contained 80% for the outer and 70% for the inner zone. In both zones only the type II isozyme was present. Qualitative and quantitative differences in protein phosphorylation were noted for the two zones.

Sensorineural hearing loss associated with systemic lupus erythematosus.

Because our recent clinical experiences suggested a possible association between hearing loss and systemic lupus erythematosus (SLE), we prospectively studied the hearing of 30 patients hospitalized because of exacerbation of SLE. Twenty-nine of the 30 patients were receiving immunosuppressive therapy at the time of testing. We found an 8% incidence of substantial, previously undetected hearing loss without attributable cause. Hearing loss could not be correlated to age, sex, disease activity, organ-system involvement, laboratory test abnormalities, or duration of symptoms of SLE. Otolaryngologists treating patients with unexplainable hearing loss--particularly if it is sudden, fluctuating, or rapidly progressive--are alerted to the possibility of underlying systemic autoimmunity. We advise physicians who frequently treat patients with SLE to include questions on hearing in the review of systems and to refer the patient with hearing complaints for thorough otologic evaluation.

[Adenoid cystic carcinoma of the esophagus].

A 56-year-old man was admitted with the complaint of dysphagia. X-ray studies and endoscopy revealed a protruding tumor at the middle third of esophagus. Under the diagnosis of esophageal cancer, subtotal esophagectomy was performed. A Borrmann type I like tumor measuring 6.7 X 3.8 X 2.2 cm was identified on the resected specimen. The surface of the tumor was irregular, nodular and covered with thin compressed esophageal mucosa. The histology of the tumor was consistent with adenoid cystic carcinoma. Incidentally, small foci of invasive squamous cell carcinoma were found adjacent to this tumor. There was no lymph node or remote organ metastasis.

[Leiomyosarcoma of the remnant stomach--a case report].

A case of leiomyosarcoma of the remnant stomach is reported. The patient was a 57-year-old woman whose chief complaint was easy fatigability. Twenty six years earlier, she had undergone gastrectomy with the diagnosis of duodenal ulcer. By X-ray examination of the stomach, a Borrmann type I gastric cancer was suspected on admission. Endoscopic study of the stomach was performed, and a diagnosis of leiomyosarcoma was made from the biopsy specimens. Total gastrectomy was performed. On the resected specimen, a Borrmann type I like tumor measuring 4.5 X 5.5 X 3.2 cm was identified in the anterior part of the body of the remnant stomach. Histologically, the lesion was confirmed as leiomyosarcoma of the stomach.
