Knowledge, attitudes and behavior related to sexuality in adolescents with chronic disability.

This study explored the level of knowledge, attitudes and activity related to sexuality in a group of adolescents and young adults with myelomeningocele, a group of matched controls and a group of adolescents with cystic fibrosis. Slightly more than half of the study sample were female, the mean age was 17.2 years, and their backgrounds were mainly middle-class. 28 per cent of the myelodysplastic group reported previous sexual activity, compared with 60 per cent of the control group and 43 per cent of the group with cystic fibrosis. Most of the disabled adolescents expressed a desire to marry and have children, but fewer than 20 per cent had sought information regarding their sexual or reproductive function from their physician. Among those sexually active, 60 per cent of the controls and 67 per cent of those with cystic fibrosis had used contraception, compared with 16 per cent of the myelodysplastic group. The authors conclude that sex education and screening for contraceptive need is indicated for adolescents with chronic disabilities.

Ultrastructural cytology of the cyclic corpus luteum of the cow.

Corpora lutea (CL) from cows on day 12 of the oestrous cycle were studied by electron microscopy to investigate whether, and how, different subpopulations of luteal cells can be identified in tissue sections. Tissues from 6 CL were examined, and nucleated profiles of luteal cells were classified as large, medium or small on the basis of their areas in electron micrographs. Cut-off points for area categories for large, medium and small-sized cells were based on diameters of greater than 25, 20-25 and less than 20 microns, respectively, if the measured areas were converted to a circular shape after correction for shrinkage. The only qualitative features which distinguished cells of large size from those of small size were the presence of clusters of secretory granules, and of exocytosis of these granules, in large cells only. However, these features were observed in only 59% of large cells, probably primarily due to sampling limitations in single sections. Other qualitative features which have been regarded as diagnostic of large or small luteal cells were observed in cells in all size categories. It was concluded that large and small luteal cells in the cyclic CL of the cow are distinguishable by their ultrastructural features. However, these data do not support the recent suggestion that the mid-cycle CL of the cow contain two subpopulations of large luteal cells in approximately equal numbers.

An acid protease secreted by transformed cells interferes with antigen processing.

The major excreted protein of malignantly transformed mouse fibroblasts (MEP), which is the precursor to lysosomal cathepsin L, was used to study the effect of exogenous acid proteases on antigen processing. When MEP and native pigeon cytochrome c were added to Chinese hamster ovary (CHO) cells expressing transfected major histocompatability complex class II gene products, the antigen-specific T-cell hybridoma 2B4 did not respond to the antigen. MEP appears to destroy the antigen in an acid compartment of the presenting cell because: (a) MEP is only active as a protease under acid conditions; (b) mannose 6-phosphate inhibited the internalization of MEP and blocked its effect on antigen processing; (c) the destruction required the simultaneous entry of the antigen and MEP into the cells; and (d) cytochrome c fragment 66-104 which does not need to be processed stimulated 2B4 in the presence of MEP. These results support the hypothesis that antigen processing requires internalization of the antigen into an acidic compartment, and they provide a new model for the investigation of the contribution of acid proteases to the reduced immunocompetence of tumor-bearing animals.
