[A case of cholangiocarcinoma treated by transcatheter arterial embolization].

A case history with a 2 year and 10 month survival, involving a peripheral cholangiocarcinoma that had been treated 3 times by transcatheter (TAE) and radiotherapy is discussed. The main cholangiocarcinoma markedly decreased in size and was found to contain many small calcified deposits after the 3 episodes of TAE.

[Relationship between angiographical manifestations and operative findings in 100 cases of hemifacial spasm].

Relationship between angiographical manifestations and operative findings of hemifacial spasm was studied in 100 cases. Vertebral angiography was performed, and Towne, straight AP, and lateral projections were routinely studied. The anterior inferior cerebellar artery (AICA) directly compressed the facial nerve root exit zone in 54 instances, the posterior inferior cerebellar artery (PICA) in 38, and the vertebral artery (VA) in 11. Compressions by multiple vessels were observed in 3 cases. Anatomical variations of the AICA and the PICA were classified into 3 groups according to their origins and their distributions of blood supply: Type I, normal distribution of AICA and PICA; Type II, common trunk anomaly with dominant AICA (basilar artery origin); and Type III, common trunk anomaly with dominant PICA (vertebral artery origin). In our cases, 35% of them showed normal distribution, 34% dominant AICA, and 35% dominant PICA. Analyses of the angiograms revealed significantly increased numbers of common trunk anomalies when compared with normal angiograms studied by Takahashi. In 60 of the 65 cases with common trunk anomalies, facial nerves were compressed by the main trunk or the branches of the dominant artery. There were 35 cases which belonged to Type I anatomical classification. They were subdivided into 2 groups according to the size of the AICA and PICA: 1. AICA greater than PICA, and, 2. PICA greater than AICA. In the AICA greater than PICA subgroup, the AICA was the offending artery in all but one case. In the PICA greater than AICA subgroup, the PICA was responsible in 9 of 17 cases. In 31 cases, angiograms showed a redundant VA with lateral elongation into the cerebellopontine angle.(ABSTRACT TRUNCATED AT 250 WORDS)

[Endoscopic resection with local injection of hypertonic saline epinephrine for the treatment of early gastric cancer].

We have developed a new method of endoscopic resection of early gastric carcinomas and other tumors. Characteristics of the new method are summarized as follows. 1. In order to prevent bleeding after endoscopic resection of the tumors, local injection of hypertonic saline epine phrine (HSE) solution was applied to the lesion. Even a depressed lesion like II c type early gastric carcinoma was artificially protruded by local injection of HSE solution. 2. Instead of a single fiberscope, double fiberscopes were employed simultaneously for endoscopic resection of the lesion. 3. Prior to resection of the lesion by high frequency current snare method, incision of the mucosa along the marking line around the lesion performed, enabling the area to be resected precisely. 4. Because tissue damage is mild, resected materials removed are suitable for pathological study. Endoscopic resection was performed on a total of 113 cases in early gastric carcinomas; 103 cases among them (91.2%) were curative resections. Ten cases of the rest underwent surgical gastrectomy for the endoscopic non-curative resections. Rate of five-year survival in 103 cases was 85.2%. Endoscopic resection by our method seems to be effective for the treatment of early gastric carcinomas in any lymph node metastasis and in patients at high surgical risk.

Localization of myoglobin in human muscle cells by immunoelectron microscopy.

The localization of myoglobin in human skeletal muscle cell was studied by immunoelectron microscopy. The Fab'-horseradish peroxidase (HRP) conjugate was prepared by the maleimide method recently developed by Ishikawa et al. This method gave better recovery and less polymerization of HRP than the periodate method. By the direct immunoperoxidase method using this conjugate, myoglobin was shown to be localized in the I-band region of skeletal muscle cells. The outer membrane of mitochondria and triads stained intensely, but A-bands were not stained. Myonuclei and intermyofibrillar spaces were also not stained. The localization of myoglobin in the I-band implies its function in energy generation.

Computed tomography and angiography in MELAS (mitochondrial myopathy, encephalopathy, lactic acidosis and stroke-like episodes); report of 3 cases.

Among mitochondrial encephalomyopathies, MELAS (mitochondrial myopathy, encephalopathy, lactic acidosis and stroke-like episodes, Pavlakis et al. 1983) is recognized as a distinct syndrome characterized by generalized convulsions and recurrent stroke-like episodes. The neuroradiological findings of three patients with MELAS are reported here. Retrospective review shows that MELAS should be included in the differential diagnosis of infarct-like lesions of the cerebrum.

Oligodendrogliomas of the lateral ventricle: computed tomography and angiography.

Computed tomographic (CT) and angiographic findings in four cases of oligodendroglioma of the lateral ventricle are reported. CT showed a midline nodular intraventricular mass containing punctate calcifications and cysts. The tumors were slightly to markedly enhanced after the infusion of contrast media. Angiography showed slightly increased vascularity in the mass, and displacement of subependymal veins near the tumor. Oligodendrogliomas of the lateral ventricle are rare but should be included in the differential diagnosis of intraventricular tumors near the foramen of Monro.

Leg contracture in mice after single and multifractionated 137Cs exposure.

This is a report of studies of time-dose relationships for post-irradiation leg contractures in mice. The isoeffect doses for various degrees of contracture, measured 250 days after irradiation, increased with the number of fractions, but not with the overall treatment times, throughout 30 days. The isoeffect curves relating the total doses for given levels of responses to the doses per fraction were steeper for leg contractures than for acute skin reactions. The alpha/beta ratios ranged from 1.4 to 5.0 Gy, depending on the degrees of contracture. They were less than the 7.5 to 50 Gy for acute skin reactions as determined in previous experiments using the same animals and irradiation systems. Thus, the data resembled those from other slowly-responding normal tissues such as the spinal cord, kidney and lung. The leg contracture consisted of dermatogenic, myogenic, and arthrogenic components; after the mice were sacrificed there was residual contracture following removal of the skin and muscle. Inhibition of bone growth accounted for only a small proportion of the contracture. The overall response reflected responses of several tissue types.
