Cytotoxicity induced by monoclonal antibody to alpha-fetoprotein coadministered with spleen cells of nude mice.

This paper describes an attempt to effectively induce antibody-dependent cell-mediated cytotoxicity (ADCC) in nude mice. A monoclonal antibody against alpha-fetoprotein, 80G, coadministered with spleen cells from other nude mice bearing HuH-7N (xenograft of human hepatoma cell line, HuH-7) significantly suppressed the growth of HuH-7N as compared to treatment with 80G alone. 80G with spleen cells from normal nude mice also had some suppressive effect. In contrast, no effect was observed with each spleen cells alone as well as 80G alone. These results suggest that further supply of effector cells could enhance ADCC activity in nude mice.

Inhibition by recombinant SLPI and half-SLPI (Asn55-Ala107) of elastase and cathepsin G activities: consequence for neutrophil-platelet cooperation.

1. The capacity of recombinant human secretory leukocyte proteinase inhibitor (SLPI) to inhibit human leukocyte elastase (HLE) and cathepsin G (Cat G) was investigated and compared with a recombinant truncated form (carboxyl-terminal domain, Asn55-Ala107) called 1/2 SLPI. 2. Both compounds were efficient when tested against enzymatic activities of purified HLE and Cat G indicating that the HLE- and Cat G-inhibitory sites were preserved in the truncated form. SLPI and 1/2 SLPI also affected platelet activation induced by 0.2 microM Cat G (IC50 = 112 +/- 13 nM for SLPI and 280 +/- 12 nM for 1/2 SLPI). 3. The effects of SLPI and 1/2 SLPI were then tested against polymorphonuclear neutrophil (PMN)-mediated platelet activation, a cell-to-cell interaction mediated by HLE and Cat G released from PMN. In this experimental system, addition of SLPI or 1/2 SLPI before N-formyl-Met-Leu-Phe (fMLP) led to the inhibition of the resulting platelet activation. As was the case for Cat G enzymatic activity and Cat G-induced platelet activation, SLPI was more efficient than 1/2 SLPI (IC50 = 676 +/- 69 nM vs 1121 +/- 150 nM). 4. The ratio of the IC50 against PMN-mediated platelet activation compared to purified Cat G-mediated platelet activation was 6.03 for SLPI and 4.32 for 1/2 SLPI. This difference may be due to the smaller size of the truncated form which could allow this molecule to diffuse more easily between PMN and platelets. 5. In conclusion, 1/2 SLPI could be a promising candidate in the treatment of pathological states linked to inflammation in which participation of HLE and Cat G has been evoked.

Successful trimethoprim-sulfamethoxazole therapy in a patient with hyperimmunoglobulin E syndrome.

A male patient with hyperimmunoglobulin E syndrome is described. Recurrent lymphadenitis and cutaneous staphylococcal abscesses were resistant to various antibiotics, and chemotaxis and hydrogen peroxide production of polymorphonuclear leukocytes were impaired. Following trimethoprim-sulfamethoxazole therapy, he was free from the above infections, and impaired polymorphonuclear leukocyte functions recovered and serum IgE decreased to approximately one-fifth of its initial level. Subsequent irregular medications, however, resulted in impairment of polymorphonuclear leukocyte functions and an increased serum IgE concentration, which recovered after regular resumption of trimethoprim-sulfamethoxazole treatment. From these results, the beneficial effects of trimethoprim-sulfamethoxazole in hyperimmunoglobulin E syndrome are clinically apparent, but in vitro studies failed to demonstrate the positive effect of trimethoprim-sulfamethoxazole on polymorphonuclear leukocytes and their mechanism still remains to be elucidated.

[Large scale outbreak of Yersinia pseudotuberculosis serotype 5a infection at Noheji-machi in Aomori Prefecture].

In June 1991, there were large scale outbreaks of Yersinia pseudotuberculosis at 4 primary schools and 1 junior high-school in Noheji-machi in Aomori Prefecture. A total of 732 patients (725 pupils and school children, 7 teachers and personnel) were affected and 134 were hospitalized. Sex ratio of incidence was 1.1:1.0 without appreciable difference. Clinical symptoms (478 patients) were represented frequently by pyrexia (86.4%), eruption (73.8%), abdominal pain (66.7%), vomiting nausea (63.4%), etc., and were characterized by a strawberry tongue, pharyngeal redness, membranous desquamation of the fingers and arthralgia during convalescence. Yersinia pseudotuberculosis was isolated from 27 (81.8%) of 33 patients stool specimens, 1 waste water specimen and 2 (11.7%) of cooking employees' stool specimens. The isolates were confirmed serotype 5a, and positive for calcium-dependency and autoagglutination, and harboring 40-50 megadalton virulent plasmid. Restrictive endonuclease digestive pattern of plasmid proved to be identical. In many cases, patients' serum antibody titer showed a significant increase ratio to the isolated strain. In term of drug susceptibility, all the strains were sensitive to cefem, penicillin and amino-glycoside series and resistant to macrolide and sulfa series. The infectious source was limited to the school feeding, but the responsible food remained unknown. Mean latency and exposure day were presumed to be 6.5 days and May 30, respectively.

Cellular autoimmunity to retinal specific antigens in patients with Behçet's disease.

The notion that autoimmune mechanisms play a role in the pathogenesis of certain uveitic conditions in humans is supported by the observation that lymphocytes from such patients respond in culture against retinal specific antigens which are uveitogenic in animals. A large proportion of uveitis patients with Behçet's disease are reported to respond well to S antigen, to interphotoreceptor retinoid binding protein (IRBP) and to several of their uveitogenic peptides, in particular, the S antigen derived peptide M. Patients with Behçet's disease without ocular involvement were reported not to differ in their responses to S antigen from the responses in the control group, yet 35% of them responded to IRBP and approximately two thirds of them responded to the peptides (peptide M, peptide N, R-4, or R-14). The responses were inhibited by monoclonal antibodies to CD4 and to class II MHC HLA-DR molecules. The presence of lymphocyte responses to retinal antigens in patients with Behçet's disease without uveitis might indicate a preclinical stage of ocular involvement. Thus, these data support the idea that autoimmunity to retinal specific antigens may play a role in the ocular inflammation in Behçet's disease.

Colonic submucosal tumors: a new classification based on radiologic characteristics.

OBJECTIVE: Despite advances in gastrointestinal radiologic techniques, findings from barium enema studies cannot be used reliably to predict the histologic nature of a colonic submucosal tumor. Therefore, we generated a new and comprehensive classification system for all colonic submucosal tumors based on a correlation between their radiologic characteristics and their histopathologic features. MATERIALS AND METHODS: Between 1978 and 1990, 89 patients had 95 colonic submucosal tumors diagnosed on the basis of barium enema studies and confirmed by biopsy or surgery. Of these, 39 patients (44%) were symptomatic and 50 patients (56%) were asymptomatic. Single lesions (82 cases) and multiple lesions (12 cases of lymphoma and one case of blue rubber bleb nevus) were observed. The 95 lesions included 21 carcinoids, 27 malignant lymphomas, 17 lipomas, 10 lymphangiomas, six leiomyosarcomas, five leiomyomas, and nine others. We analyzed the radiologic findings and classified these lesions according to their appearance. RESULTS: All tumors were classified on the basis of their morphologic appearance into five types: (1) wide-based sessile lesion with gradually sloping margin and smooth surface (17 cases); (2) wide-based sessile lesion, more polypoid than the first type, with a smooth surface, with or without a central depression (49 cases); (3) wide-based sessile lesion with lobulated surface (six cases); (4) pedunculated lesion with smooth or granular surface (17 cases); (5) unclassified, not any of the previously mentioned types, may be diffusely stenotic or aneurysmal (six cases). The most common finding was the wide-based sessile lesion with a smooth surface and a possible central depression; next most common was the wide-based sessile lesion with gradually sloping margin and smooth surface; next was the pedunculated lesion with a smooth or granular surface. Eighty-six percent of the lesions were single; 14% were multiple. Histologic confirmation revealed mainly lymphoma (29%) and carcinoid (22%), of which the most common finding was a wide-based sessile lesion with a smooth surface, with or without a central depression. CONCLUSION: This new classification based on radiologic characteristics covers all varieties of colonic submucosal tumors. It can serve as a guideline for predicting a specific diagnosis of a colonic submucosal tumor on the basis of radiologic findings.

TV game play and obesity in Japanese school children.

A survey conducted in 9 cities in Japan indicated for 1517 boys and 1546 girls there was more use of TV games for longer periods when children were classified as obese than when classified as moderate or lean. More boys than girls played although most children had experience and many owned TV game machines.

[Stimulus time dependence of cerebral blood flow response to photic stimulation].

The time dependency of rCBF response to photic stimulation in the primary visual cortices was studied using H215O bolus-injection and PET. The subjects consisted of 5 normal volunteers aged from 25 to 40 years old. Their regional CBF was measured at 2 min and 7 min after the start of photic stimulation (frequency, 10 Hz). The percentage increases of rCBF were 19.6 +/- 7.9% (mean +/- SD, range: 8.9-27.6%) in the primary visual cortex on the left side and 23.2 +/- 13.6% (2.5-35.8%) on the right side at 2 min, respectively, while, in comparison, they were 25.3 +/- 20.0% (1.8-49.7%) on the left side and 25.3 +/- 16.4% (3.5-44.5%) on the right side at 7 min, respectively. No significant difference was found in the rCBF responses between at 2 min and 7 min. Therefore, the taming effect does not need to be considered when carrying out a photic stimulation study.

Current trends in cataract and refractive surgery in Japan--1992 survey.

A survey was carried out by mail in January 1993 to investigate the current trends in cataract and refractive surgery in Japan. A questionnaire was sent to members of the Japanese Intraocular Lens Implant Society, and replies were received from 59.2% prior to the cut-off date of February 15. Results were cross-analyzed and compared with the data from American and British surveys.

Double-masked comparative study of UF-021 and timolol ophthalmic solutions in patients with primary open-angle glaucoma or ocular hypertension.

A double-masked comparative clinical study of UF-021 (Rescula), a unique prostaglandin-related compound, was carried out at 18 centers with timolol maleate 0.5% ophthalmic solution as an active reference drug. After a wash-out period, UF-021 (0.12%) or timolol (0.5%) was given topically twice a day for 12 weeks to 158 patients with primary open-angle glaucoma or ocular hypertension, in a randomized double-masked manner. Both groups showed a significant reduction in intraocular pressure from the second week to the end of the study. In overall improvement rating, 91.4% of the cases (64/70) in the UF-021 group and 88.3% (68/77) in the timolol group were judged to be "Extremely improved" or "Improved". Five and 4 cases reported having side effects in the UF-021 and timolol groups, respectively, but none of the cases required any change or discontinuation of treatment. While UF-021 did not affect blood pressure, both systolic and diastolic blood pressures in the timolol group were significantly decreased. These results suggest that UF-021 (0.12%) has the potential to lower intraocular pressure equivalent to timolol (0.5%), while having no effect on cardiovascular functions.

Effects of topical application of UF-021, a novel prostaglandin-related compound, on aqueous humor dynamics in rabbit.

The mechanism underlying the intraocular pressure (IOP) lowering effect of a novel prostaglandin-related compound, (+)-isopropyl Z-7-[(1R,2R,3R,5S)-3,5-dihydroxy-2-(3-oxodecyl)-cyclopentyl]hept-5 -enoate (UF-021), and the influence of this new hypotensive agent on the permeability of the blood-aqueous barrier (BAB) were studied in albino rabbits. A single instillation of 0.06% UF-021 significantly lowered the IOP with a peak reduction of 5.2 mmHg without changing the aqueous flow rate. Uveoscleral outflow and outflow facility to general blood circulation were both increased. The BAB permeability evaluated with fluorescein isothiocyanate-labeled rabbit albumin (FITC-RA) showed approximately sixfold increase after a single instillation of 0.06% UF-021, which suggested a drug-induced compromise of the rabbit BAB.

Ocular hypotensive effect of PhXA41 in patients with ocular hypertension or primary open-angle glaucoma.

A study was carried out to assess the clinical efficacy of PhXA41, a new phenyl-substituted prostaglandin F2 alpha-isopropyl ester analogue, using a single administration in 35 subjects with ocular hypertension or primary open-angle glaucoma. PhXA41 caused a dose-dependent intraocular pressure (IOP) reduction which continued 24 hours or more after administration. The mean IOP reduction 8 hours after treatment compared with the baseline IOP was 3.4, 4.9 and 5.9 mmHg for the doses of 25, 50 and 100 micrograms/ml, respectively. Although slight conjunctival hyperemia occurred in some patients, it disappeared by the next day with no treatment. No aqueous flare or cells were detected, no significant change in pupillary diameter was found, and no systemic symptom was reported. Thus, PhXA41 was well tolerated in subjects with ocular hypertension or primary open-angle glaucoma. Furthermore, its IOP-reducing effect was so long-lasting that a once daily application may suffice for clinical use.

[Difference in the distribution of 123I-IMP and 99mTc-HMPAO in the striatum--comparison with 15O-PET].

Brain perfusion SPECT studies of 123I-IMP and 99mTc-HMPAO were performed in 10 patients with various brain disorders and the difference in the distribution of 123I-IMP and 99mTc-HMPAO in the cortices and striatum was compared with the cerebral blood flow by 15O-PET method. The ratios of the radioactivity in 14 ROIs of the cortices, striatum and cerebellum to that of the reference region or the right frontal cortex were compared among the 3 studies. The coefficient variance between 123I-IMP and PET was 0.71 and that between 99mTc-HMPAO and PET was 0.74 in the regions excluding the striatum. The striatal activity ratio in 123I-IMP was slightly less than those in 99mTc-HMPAO and PET which were close to each other. The visual activity in the striatum in 123I-IMP was also slightly less than those in 99mTc-HMPAO and PET. All the striatal lesions of decreased perfusion diagnosed by PET were observed in 123I-IMP, however 2 lesions were missed in 99mTc-HMPAO. This seems to be relevant to the difference in the distribution of 123I-IMP and 99mTc-HMPAO.

Identification of alpha 1C-adrenergic receptor mRNA in bovine retinal pigment epithelium.

PURPOSE: To examine the localization of a novel alpha 1-adrenergic receptor subtype of alpha 1C receptor in the eye, we compared the amount of alpha 1C receptor transcripts in bovine retinal pigment epithelium (RPE) and in neural retina by employing reverse transcription of mRNA and the polymerase chain reaction (RT-PCR) assay. METHODS: RT-PCR assay with specific primers for the alpha 1C-adrenergic receptor and for beta-actin showed linear relationships between input quantity of RNA and the amount of amplified PCR products for the alpha 1C-adrenergic receptor and also for beta-actin, when PCR was conducted for 24 and 15 cycles, respectively. RESULTS: The RT-PCR assay demonstrated that the spontaneous expression level of the alpha 1C-adrenergic receptor mRNA was much higher in bovine RPE than in neural retina; the alpha 1C-adrenergic receptor/beta-actin ratio from RPE was 0.33 +/- 0.15 (n = 4), whereas that from neural retina was virtually zero. CONCLUSIONS: The RT-PCR assay is a sensitive semiquantitative method for a low abundance mRNA in a limited number of cells. Using the alpha 1C receptor as a model, we demonstrated the usefulness of this assay by showing the uneven distribution of the alpha 1C receptor transcripts in bovine RPE cells and neural retina.

Fluorine-18-fluorodeoxyglucose positron emission tomography in technetium-99m-hydroxymethylenediphosphate negative bone tumors.

We have encountered two cases of bone tumors with high 18F-fluorodeoxyglucose (FDG) uptake and negative 99mTc-HMDP bone scintigraphy, including a patient with myeloma and a patient with a metastatic bone tumor from esophageal cancer. Bone scintigraphy with a 99mTc-phosphate complex reflects osteoblastic activity in the bone tissue surrounding the tumor, whereas the accumulation of FDG is associated with the metabolic activity of the tumor itself. An FDG-PET study can therefore be used as a complementary study for the detection and follow-up of bone tumors when a 99mTc-phosphate bone scintigram is negative.

[MR imaging of pericallosal lipomas].

Eight cases of pericallosal lipomas were studied by MR imaging at 1.5 Tesla. All were located around the splenium of the corpus callosum, and were classified as the curvilinear type. Corpus callosal abnormalities were detected in the six of these eight cases, and included two cases of short corpus callosum and four of thin splenium. The larger lipomas tended to have greater abnormalities of the corpus callosum. T1-weighted sagittal images were the most useful pulse sequences for diagnosing pericallosal lipomas and corpus callosal abnormalities. On review of the literature we found that the curvilinear type of pericallosal lipoma is more frequently observed by MR imaging than the tubulonodular type.

Identification and isolation of common and tissue-specific geranylgeranylated gamma subunits of guanine-nucleotide-binding regulatory proteins in various tissues.

Heterotrimeric guanine-nucleotide-binding regulatory proteins (G proteins) have been classified into several subtypes on the basis of the properties of their alpha subunits, though a notable multiplicity of gamma subunits has also been demonstrated. To investigate whether each subtype of alpha subunit is associated with a particular gamma subunit, various oligomeric G proteins, purified from bovine tissues, were subjected to gel electrophoresis in a Tricine buffer system. All G proteins examined were shown to have more than two kinds of gamma subunit. Of the brain G proteins, GoA, GoB, and Gi1 contain the same set of three gamma subunits, but Gi2 contains only two of these subunits. Lung Gi1 and Gi2 and spleen Gi2 and Gi3 had similar sets of two gamma subunits, one of which was distinct from the gamma subunits of brain G proteins. These observations indicate that each subtype of alpha subunit is associated with a variety of beta gamma subunits, and that the combinations differ among cells. For analyses of the structural diversity of the gamma subunits, beta gamma subunits were purified from the total G proteins of each tissue and subjected to reverse-phase HPLC under denaturing conditions, where none of the beta subunits were eluted from the column. Three distinct gamma subunits were isolated in this way from brain beta gamma subunits. In contrast, lung and spleen beta gamma subunits contained at least five gamma subunits, the elution positions and electrophoretic mobilities of which were indistinguishable between the two tissues. Among several gamma subunits, two subspecies appeared to be common to the three tissues. In fact, in each case, the partial amino acid sequence of the most abundant gamma subunit in each tissue was identical, and the sequences coincided exactly with that of 'gamma 6' [Robishaw, J. D., Kalman, V. K., Moomaw, C. R. & Slaughter, C. A. (1989) J. Biol. Chem. 264, 15758-15761]. Fast-atom-bombardment mass spectrometry analysis indicated that this abundant gamma subunit in lung and spleen was geranylgeranylated and carboxymethylated at the C-terminus, as was 'gamma 6' from brain. In addition to abundant gamma subunits, other tissue-specific gamma subunits were also shown to be geranylgeranylated by gas-chromatography-coupled mass spectrometry analysis of Raney nickel-treated gamma subunits. These results suggest that most gamma subunits associated with many different subtypes of alpha subunit are geranylgeranylated in a variety of tissues, with the single exception being the retina where the G protein transducin has a farnesylated gamma subunit.

[MR imaging of stereotactic thalamotomy using radiofrequency methods].

Sixty-eight stereotactic radiofrequency (rf) thalamic lesions in 57 patients with movement disorders were evaluated by magnetic resonance (MR) imaging. Postoperative periods ranged from 5 days to 4 years and 9 months. All 68 rf lesions were clearly detected on T2-weighted images (T2WI). Changes in signal intensity on T2WI were classified into five patterns, as follows: Pattern I: lesions with three concentric zones consisting of an inner hypointense, middle hyperintense and outer hypointense zone (31 lesions); Pattern II: lesions consisting of an inner hypointense and outer hyperintense zone (4 lesions); Pattern III: lesions consisting of an inner hyperintense and outer hypointense zone (27 lesion); Pattern IV: lesions of a hyperintense area alone (2 lesions); Pattern V: lesions of a hypointense area alone (4 lesions). The outer hypointense rim in Patterns I and III is thought to represent hemosiderin deposition. The abnormal signal intensity on T2WI caused by rf ranged from 2 to 12 mm in diameter, and lesions in the late phase were smaller than lesions in the early phase.
