Effects of bed rest on bone metabolism in patients with femoral neck fracture.

The effect of continuous 1 week bed rest during traction on bone metabolism were studied in patients with femoral neck fracture. Bone demineralization was assessed by measuring serum calcium (Ca), phosphorus (P), and alkaline phosphatase (AlkP) before, during, and after 1 week-bed rest with skeletal traction. In addition, 24-hour urinary excretion of calcium and phosphorus were measured. Serum Ca did not change significantly before, during, and after bed rest, whereas serum P increased after 1 week bed rest and postoperatively remained significantly elevated. Serum AlkP did not increase with bed rest, but became elevated postoperatively. Urinary Ca excretion increased from 105.8 +/- 15.2 to 138.0 +/- 15.4 mg/24h after 1 week of bed rest, and remained elevated postoperatively. In contrast, urinary P excretion initially increased 495.1 +/- 65.0 to 610.8 +/- 87.7 mg/24h, and subsequently decreased to a level significantly less than the admission level. These results suggest that 1 week bed rest for the patients with femoral neck fracture resulted in bone atrophy and demineralization.

[A case of X-linked recessive bulbospinal muscular atrophy with demyelinating neuropathy and hypertrophy of the calves].

We report a 53-year-old man of X-BSMA with neuropathy. The patient developed slowly progressive muscular weakness and wasting over a 2-year period with an accompanying numbness in the finger tip. He can run normally but not so fast. When he was aged 52-year old, difficulty in running progressed. General physical examination revealed nothing particular except for hypertension and gynecomastia. He showed muscular weakness and atrophy in the tongue, shoulder girdle, and upper and lower limbs. Calf muscle hypertrophies were prominent on both sides. The tendon reflexes were absent. Slight sensory impairment for vibration and pin-prick was present distally in all limbs. Autonomic nerve dysfunction was not observed. Hyperglycemia, elevated HbA1c and elevated serum CK (1,242 IU/l) were seen. The computed tomographic analyses on skeletal muscle showed hypertrophic changes in the calf muscles with a few fatty infiltrations. Electromyography showed a systemic neurogenic pattern. Motor nerve conduction velocities were slightly delayed in the lower limits. Sensory nerve action potentials were not elicited in all nerves tested. Sural nerve biopsy disclosed marked reduction of myelinated fibres for that of large diameter with thin myelin. Teased fibre studies showed a definite increase in the incidence of fibres with segmental demyelination and remyelination. In electron microscopic examination, typical or atypical onion bulb formation was observed on individual fibres. Axonal changes were minimum. We believe that segmental demyelination observed in this patient is not secondary to axonal damage. We, also, investigated AR gene abnormality by polymerase chain reaction (PCR) in this patient.(ABSTRACT TRUNCATED AT 250 WORDS)

[Reproductive and developmental toxicity study of prednisolone farnesylate (PNF)--study of subcutaneous administration of PNF during the perinatal and lactation periods in rats].

The effects of Prednisolone farnesylate (PNF), a newly synthesized corticosteroid, administrated during the perinatal and postnatal periods were studied in Sprague-Dawley rats. This compound was injected subcutaneously to female rats at dose levels of 0(control), 0.05, 0.5 and 5 mg/kg/day, once a day, for the period from day 17 of pregnancy to day 21 after parturition. Twenty-two to 25 dams in each dose group were allowed to litter naturally, and observations were made on the postnatal growth and development of their offspring. 1. In the dams treated at doses of 0.5 and 5 mg/kg, decreased body weight gains, atrophy of the thymus and retention of the substance at the injected site were noted. However, general signs, food consumption, parturition, lactation and nursing behaviors were not affected by the administration of PNF. 2. In the F1 newborns, the postnatal growth, development, responses, behaviors, learning ability and reproductive ability were not influenced. Additionally, no embryonic or fetal abnormalities of their fetuses (F2) were detected. From these results, the no-effect dose levels of PNF on the parental general states, the parental reproductive ability and those of the F1 offspring are thought to be 0.05 mg/kg/day, 5 mg/kg/day or more and 5 mg/kg/day or more, respectively, under the experimental conditions of this study. Moreover, the F2 fetuses are not affected by doses up to 5 mg/kg/day of PNF.

[A biochemical study on the effects of interleukin-6 for premature rupture of membranes].

Chorioamnionitis was thought to be a cause of premature rupture of membranes (PROM). Recently it has been accepted that IL-6 is one of the important cytokines in inflammation. To investigate the correlation between PROM and IL-6, IL-6 production in chorionic cells and amniotic cells was examined. Moreover, the response of these cells to IL-6 was also examined. The results were as follows. 1. Chorionic cells and amniotic cells obtained from human membranes were cultured. 2. Comparing IL-6 production, chorionic cells produced a larger amount of IL-6 (425pg/ml/24 hrs) than amniotic cells (4pg/ml/24 hrs). 3. IL-1 treatment (10U/ml) caused a significant increase in IL-6 release from both chorionic and amniotic cells (43.2ng/ml/24hrs vs. 50.3pg/ml/24hrs, respectively). 4. Lipopolysaccharide (LPS) (10 micrograms/ml) stimulated the release of IL-6 in chorionic cells, but did not affect it in amniotic cells at all. 5. By staining with F actin, it was demonstrated that IL-6 expanded the cellular gap in amniotic cells. 6. After amniotic cells were co-cultured with chorionic cells, IL-1 treatment caused actin-polymerization in amniotic cells. These results suggest that chorionic cells produce a large amount of IL-6 as a result of stimulation by IL-1 or LPS, and the IL-6 produced constricts the amniotic cells. The phenomena may participate in the mechanism of PROM.

Course before and after treatment of a patient with Budd-Chiari syndrome monitored by iodine-123-iodoamphetamine scintigraphy per os and per rectum.

A 55-yr-old woman with Budd-Chiari syndrome was treated by percutaneous transluminal angioplasty with a balloon catheter. Before and after treatment, portal scintigraphy was performed by the administration of [123I]iodoamphetamine per os and per rectum. An enteric capsule was used for the oral administration. Before treatment, the portal shunt index via the superior mesenteric vein was 40.5%; two weeks after treatment, it was 50.2%; and five months after treatment, it was 41.2%. Before treatment, the portal shunt index via the inferior mesenteric vein was 86.0%; two weeks after treatment, it was 87.6%; and five months after treatment, it was 21.8%. The treatment improved the portal circulation through the inferior mesenteric vein only, with little effect on the portal circulation through the superior mesenteric vein.

[Apolipoprotein AI levels in cord blood].

Lipid is known to increase during pregnancy but the factors responsible for the change have not been established. In addition, the lipid concentration in preeclamptic pregnancy is significantly higher than in normal pregnancy. The apolipoproteins are an important determinant of metabolism and the structure of plasma lipoproteins. The 26 healthy pregnant women, the levels of cord apolipoprotein AI were determined by TIA methods. The cord and plasma apolipoprotein AI were 76.12 +/- 20.04 mg/dl (n = 26, mean +/- S.D.) and 190.50 +/- 18.84 mg/dl, respectively. Cord apolipoprotein levels correlated to maternal age (r = -0.12, p less than 0.05), maternal weight (r = -0.11, p less than 0.01), the gestational week (r = +0.42, p less than 0.01), infant weight (r = -0.01, p less than 0.05), placental weight (r = -0.03, p less than 0.05), and diastotlic blood pressure (r = +0.06, p less than 0.05). These data suggest that the measurement of cord apolipoprotein AI may be a useful factor in evaluating preeclamptic pregnancy.

[Apolipoprotein levels in preeclamptic pregnancies].

Lipoprotein is known to increase during pregnancy but the factors responsible for the change have not been established. In addition, the lipoprotein concentration in preeclamptic pregnancy is significantly higher than in normal pregnancy. The apolipoproteins are an important determinant of metabolism and the structure of plasma lipoproteins. In normal pregnancies, non pregnancies and preeclamptic pregnancies the levels of blood apolipoproteins AI, AII, B and E were determined by TIA methods. (1) In normal pregnancies, the concentrations of apolipoproteins AI, AII, B and E were 182.6 +/- 20.9 mg/dl (n = 12, mean +/- S.D.), 33.3 +/- 5.7 mg/dl, 128.6 +/- 20.8 mg/dl, and 6.8 +/- 1.9 mg/dl, respectively. (2) In the pregnancies, the concentrations of apolipoproteins AI, AII, B and E were 135.6 +/- 9.3 mg/dl (n = 5), 30.8 +/- 1.9 mg/dl, 76.0 +/- 19.7 mg/dl, and 4.4 +/- 0.7 mg/dl, respectively. (3) In the preeclamptic pregnancy, the concentrations of apolipoproteins AI, AII, B and E were 181.0 +/- 27.6 mg/dl (n = 22), 33.2 +/- 4.8 mg/dl, 145.7 +/- 41.6 mg/dl and 5.8 +/- 1.4 mg/dl, respectively. The concentration of apolipoprotein B in preeclamptic pregnancy was significantly higher (p less than 0.001) and apolipoprotein E was significantly lower (p less than 0.01) than in normal pregnancies. These data suggest that the measurement of apolipoprotein is useful for the evaluation of preeclamptic pregnancy.

Experimental studies on permeability of tracers into the spinal cord.

For investigation of the intramedullary penetration of metrizamide, the intramedullary movements of fluorescein, Evans Blue, Horseradish peroxidase (HRP) and ionic lanthanum in the normal cord and experimental syringomyelia of rats were studied. Syringomyelia was induced by injection of kaolin suspension into the cisterna magna. Subsequently HRP or ionic lanthanum was perfused into the subarachnoid space and its distribution was examined by electron microscopy. In normal rats, HRP mainly became localised in the basement membrane between the pia mater and glial cells and only a few vesicles migrated into the extracellular spaces (ECS) between glial cells. Ionic lanthanum, however, migrated deep into the ECS. In rats with experimental syringomyelia many vesicles of HRP migrated deep into the ECS of glial cells and the white matter. These findings indicated that the ECS of the marginal glia and the neuropil constitute part of a pathway through which metrizamide migrates into the intramedullary cavity.

Clinical and experimental studies on permeability of tracers in normal spinal cord and syringomyelia.

Delayed CT Myelography (CTM) demonstrated intramedullary cavities in 18 patients, most of which appeared on the 6-hour scan. The attenuation values of the normal spinal cord increased progressively with time, and recorded a peak at 6 hours. The CT numbers of the gray matter of metrizamide were significantly high, as compared with those of iotrol. In the tracer study reported, lanthanum penetrated through the marginal glia to the normal and pathologic spinal cord. Reaction products of horseradish peroxidase (HRP) were mainly in the extracellular space of the normal spinal cord. The pathologic spinal cord, however, allowed the passage of HRP into the parenchyma through the extracellular space of the marginal glia. Consequently, the extracellular space of the spinal cord surface constitutes a pathway for cerebrospinal fluid solutes. Either metrizamide or iotrol thus diffuses into the spinal cord from the subarachnoid space.

Crystallization and preliminary X-ray diffraction study of ferrocytochrome c2 from Rhodopseudomonas viridis.

Crystals of ferrocytochrome c2 from a non-sulphur purple photosynthetic bacterium, Rhodopseudomonas viridis, have been grown from ammonium sulphate solution at pH 8.5 by the sitting-drop vapour-diffusion procedure. The crystals belong to the trigonal system, space group P3(1)21 (or its enantiomorph P3(2)21) with unit-cell dimensions of a = b = 75.8 A and c = 40.1 A, and diffract to at least 2.0 A resolution. Assuming that an asymmetric unit contains one protein molecule (approx. 12,300 Mr), the solvent content of the crystal is approximately 54.5% (v/v).

Purification of rat retinol-binding protein using Blue Sepharose CL-6B.

Studies were conducted to isolate rat plasma retinol-binding protein, the specific transport protein for vitamin A in the rat. Retinol-binding protein was isolated from rat plasma by a new simple procedure using Blue Sepharose CL-6B, and highly purified retinol-binding protein was obtained. This procedure included DEAE-cellulose chromatography at pH 6.0, Sephadex G-75 gel filtration in the presence of 3.0 M urea, Blue Sepharose CL-6B affinity chromatography at pH 7.0 and finally Sephadex G-100 gel filtration at pH 7.4. The third step completely accomplished the dissociation and separation of retinol-binding protein from its complex with prealbumin and plasma albumin. This procedure is a significant improvement over previously published procedures, in which sample recycling and preparative polyacrylamide gel electrophoresis are necessary. The molecular weight, electrophoretic behaviour, ultraviolet and fluorescence spectra of the retinol-binding protein were similar to those appearing in other reports.
