[A benign tumor of the cavum: Thornwald's cyst].

The non neoplastic diseases of the nasopharynx are uncommon especially in adults. Cyst of pharyngeal bursa or Thornwald cyst are located in the supero-posterior nasopharynx, in the location of the normal pharyngeal bursa which open into the pharyngeal mucosa. When it persists, a cyst can develop if the orifice becomes occluded. The have a characteristic and distinctive C.T. and N.M.R. aspects, as presented in this paper, well circumscribed round midline mass, sparing muscles lateral as well as posterior, with an high signal in T1 and T2 N.M.R. sequences. No discussion with other pathologic conditions is useful because carcinoma, adenoid and pharyngeal polyp, or Killian's polyps haven't this configuration.

[Use of X-ray computed tomography in cervical infections].

Infections of face and neck represent serious and potentially life threatening conditions that are sometimes difficult to differentiate from neoplastic tumours, especially in subacute clinical forms. Conventional radiographic techniques offer interest for cervical masses, except Ultra-Sonographic exam, in sites regarding vascular axes, but carries little value for evaluating their spread into the different cervical spaces. On the other hand, C.T. is valuable to precise the location and the extent and to determine its inflammatory nature by studying the fats and the aponeurosis around it. It help in the analysis of associated adjacent signs: soft tissue swelling, extensive obliteration of adjacent fats, swelling of cervical aponeurosis, thickening of adjacent muscles. These findings are documented by the study of fourteen patients, admitted in St-Antoine hospital. All abscesses, except one, were easy to diagnose because of their low central attenuation. False negative cases are possible and noted by other authors. So, in absence of response to appropriate therapy, surgery is necessary to eliminate a misdiagnosed abscess. Furthermore, it's sometimes possible to suspect an etiology (foreign body, tuberculosis).

[Cysts of Tornwald's bursa. Apropos of 3 cases].

Thornwald's cyst is a benign bursa formation of the posterior superior wall of the rhinopharynx due to occlusion of the pharyngeal bursa, this entity has been known for more than a century but it is only exceptionally observed. A series of 3 cases have been diagnosed at Hôpital Saint-Antoine over the last 6 months and the perfect visualization of the cysts by CT scan and MR imaging wall no doubt allow their fortuitous detection more frequently.

[Testicular lymphoma in echography. Apropos of 2 cases].

Lymphomas of the testis are uncommon disease. Their ultrasonographic pattern is not well known. We report two cases: the first has sonographic findings soon described which appear typical enough: It's a small area, intratesticular, well-limited and hypoechogenic. On the other hand, the second is an original case report (heterogeneous scrotal mass in which it is impossible to recognize the different structures of the scrotal sac), with a bulky involvement of the lymphoma. The scrotal sonogram appears as the choice investigation for the staging and also for the remaining testicle which can be often affected.

[Leydig cell tumors in adults. Value of ultrasonic diagnosis. Apropos of 7 tumors].

Leydig cell tumors arise from the gonadal stroma and represent two or three per cent of all testicular tumors. They may occur at any age in contrast with germinal tumor. In adults, about 20 per cent occur with endocrine disturbance: gynecomastia or impotence. Ten per cent are malignant and few cases are bilateral. They are small, yellow, brown tumor. Reinke's crystals are observed in 50 per cent and are pathognomonic. They present no necrosis no hemorrhage, no cyst. A benign tumor is difficult to distinguish from a malignant tumor. A vascular and capsular invasion, an infiltrative margin, a bilateral or multiple tumors must evoke a malignant case. Cryptorchidism, even corrected seems to be partly responsible in tumor induction. Ultrasonography can help for a precocious and reliable diagnosis of tumors, specially in case of occult tumors (2 cases). It also helps to survey the controlateral testis (bilateral tumor: 1 case).

[Pitfalls in scrotal echotomography].

We reviewed 500 scrotal sonograms with a gray-scale equipment using a 7 Mega Hertz transducer. 7 cases are presented in which there are the two principal traps which must be avoided in the practice of scrotal ultrasound: To find as pathological, some scrotal or parascrotal structures which are anatomical. To confuse pathological images. These diagnostic problems are almost resolved by three conditions: A perfect knowledge of normal ultrasound anatomy, a good experience of the operator in this scope and a strict management of the ultrasonic investigation. There, the scrotal sonogram is highly reliable.

[Bone metastases of a hemangiopericytoma].

Hemangiopericytoma is a rare intra-cranial tumour. Extra-cranial metastasis to bone are much less common. Hemangiopericytoma must be differentiated from angioblastic meningioma, basis on morphological features and biological potentials. (Pena-Popoff).

[Lymphomatoid granulomatosis. Value of standard radiography. Apropos of a case].

A new french observation of Lymphomatoid Granulomatosis of Liebow is reported. The case concerns a thirty-five old white woman presenting a cutaneous localisation and a severe pulmonary infection with acute febrile dyspnea and macronodular opacities in chest radiography. Ten months later, encephalic damage led to death. In this observation, the diagnosis is based on skin and chest biopsies showing the characteristic angiocentric and angiodestructive, lymphoreticular granulomatous disease, defined by Liebow. In our knowledge, 152 cases have been reported in North America (5), four cases in Great Britain. This observation is interesting by its rarity, its nosologic discussion it permits.
