Therapeutic application of phenylalanine immunoadsorbent with on-line regeneration.

To improve the adsorption capacity of the artificial reticuloendothelial system containing phenylalanine as a ligand, a regeneration method for the adsorbent during plasma perfusion was developed. The adsorbed rheumatoid factor, immunoglobulins, and complements were demonstrated to be elutable from the adsorbent with a 5% glucose solution in vitro. The regeneration method was applied to the treatment of a patient with rheumatoid arthritis. During each plasma perfusion, the adsorbent was regenerated, usually twice, with a 5% glucose solution at a flow rate of 50 ml/min. After each regeneration, the adsorption capacity of the adsorbent was found to be improved by determining the pre- and post-column plasma titers of a rheumatoid arthritis hemagglutination test.

Urinary prostaglandins and thromboxane in patients with chronic glomerulonephritis.

To evaluate the potential contribution of prostaglandins (PGs) and thromboxane (TX) to the development of chronic glomerulonephritis, we measured the urinary excretion of PGE, PGF2 alpha, 6-keto-PGF1 alpha and TXB2 by radioimmunoassay in 36 patients with chronic glomerulonephritis. In patients with nephrotic syndrome, urinary excretion of PGE and TXB2 was highly increased, whereas that of PGF2 alpha and 6-keto-PGF1 alpha remained normal. In patients with non-nephrotic chronic glomerulonephritis, urinary excretion of TXB2 was significantly increased, whereas that of PGE and 6-keto-PGF1 alpha remained normal and that of PGF2 alpha was significantly decreased. In patients with chronic renal failure, the urinary excretion of all PGS and TX was markedly decreased presumably due to a decrease in the number of cells which can metabolize arachidonic acid. These results suggest that TXA2 plays an important role as an exaggerating factor in the development of chronic glomerulonephritis, particularly that accompanying nephrotic syndrome, and that renal synthesis of PGE is compensatorily increased to maintain renal function in nephrotic syndrome.

Association of vesiculobullous eruptions with mycosis fungoides.

Mycosis fungoides (MF) is, on extremely rare occasions, associated with vesiculobullous eruptions. This report describes clinical, histological and immunohistochemical findings of a case of MF with vesiculobullous lesions. Characteristic features of our case included: bullous lesions evolving rapidly in 12-24 h with severe itch on erythematous and normal-appearing skin; a positive Nikolsky sign; an intraepidermal blister; direct and indirect immunofluorescence stainings were all negative with IgG, IgA, IgM and C3, and atypical lymphocytes in the infiltrates were of helper/inducer T cell type.

Suppression of luteinizing hormone secretion is removed at late lactation in ovariectomized lactating rats.

The effect of the suckling stimulus on pulsatile luteinizing hormone (LH) secretion in mid- and late lactation (days 10 and 20 of lactation) in rats was examined. Pulsatile LH secretion was strongly suppressed on either day 10 or 20 of lactation in intact rats in which the baselines of LH secretion were kept very low. In ovariectomized rats the baseline was kept as low as was observed in intact rats on day 10 of lactation, and pulsatile LH secretion was observed in 3 out of 6 rats. On day 20 the baseline secretion increased and pulsatile LH secretion was observed in 5 out of 6 rats, and the baseline for each rat showed various levels. These results clearly indicate that the pulsatile LH secretion was strongly suppressed until ovulation occurred on day 18-23 of lactation in intact rats and suggest that suppression of pulsatile LH secretion by the suckling stimulus at the hypothalamo-pituitary level is removed in late lactation and the time of the removal varies from animal to animal.

Photoperiodic modification of negative and positive feedback effects of oestradiol on LH secretion in ovariectomized goats.

Ovariectomized Shiba goats carrying an oestradiol implant (4-10 pg/ml) were kept under a short-day light regimen (10L:14D; Group 1, N = 4) or a long-day regimen (16L:8D; Group 2, N = 4). Plasma LH concentrations were lower (P less than 0.05) in Group 2 than in Group 1 between Days 40 and 200, suggesting an enhanced negative feedback effect of oestradiol on LH secretion under a long-day regimen. On Days 30, 60, 100, 149 and 279, an LH surge was induced by i.v. infusion of oestradiol for 48 h; the infusion rate was gradually increased from 0.5 (0 h) to 4.1 (48 h) micrograms/h, thereby mimicking the preovulatory increase of oestradiol secretion. The duration and magnitude of the induced LH surge were indistinguishable between the groups. The latency from the onset of oestradiol infusion to the LH surge was relatively constant in Group 1, 41.1 +/- 0.9 h (mean +/- s.e.m., n = 17) but was shorter in Group 2 (19.7 +/- 3.7 h, P less than 0.05) on Day 149; less oestradiol was therefore required for induction of the LH surge (27.4 vs 89.7 micrograms, P less than 0.01), suggesting an increased sensitivity to the oestradiol positive feedback under a long-day regimen. These results might be interpreted to indicate that the hypothalamic-pituitary axis of the goat becomes hypersensitive to the positive as well as the negative feedback effect of oestradiol under long-day conditions.

[Myocardial perfusion assessed by dynamic computed tomography before and after percutaneous transluminal coronary angioplasty].

A method for quantitatively and visually assessing myocardial perfusion using a new transmission computed tomography scanner and contrast media injection was devised. Myocardial perfusion was assessed before and after percutaneous transluminal coronary angioplasty (PTCA). Six patients with left anterior descending coronary artery stenosis undergoing PTCA were successfully studied. Dynamic scans, each with a scan time of one sec, were performed at the mid left ventricular level using bolus injections of contrast medium from the inferior vena cava. Regions of interest (ROIs) were located in the left ventricular cavity and in the myocardium perfused by the left anterior descending coronary artery, and gamma-variate fitted time density curves were recorded within the first pass phase, excluding recirculation. Cardiac outputs calculated from the ventricular curves of the ROIs in the left ventricular cavity correlated well with the thermodilution measurements using Swan-Ganz catheters (r = 0.90, p less than 0.01). The ratios of myocardial blood flow (F) to the tissue element volume (V) were calculated (F/V) from the myocardial curves of the ROIs in the myocardium as parameters of myocardial perfusion, according to the Zierler's principle. The F/Vs in the myocardium perfused by the left anterior descending coronary artery before PTCA were significantly less than those of the control subjects (4.4 x 10(-2) +/- 1.2 x 10(-2) vs 8.1 x 10(-2) +/- 2.9 x 10(-2): p less than 0.05).(ABSTRACT TRUNCATED AT 250 WORDS)

[Evaluation of rIFN-gamma in the treatment of lymphoma and melanoma of the skin by systemic and intralesional administration].

This study evaluated the clinical effect of rIFN-gamma for the treatment of lymphoma and melanoma of the skin by systemic intravenous and intralesional administration. By intravenous drip infusion, one of two cases of mycosis fungoides (stage IV) showed CR of skin lesions by administration of a total of 160 X 10(6) units, 4 or 8 X 10(6) units, every other day for 3 months. The remaining case was, however, in a state of PD. Intralesional administration of rIFN-gamma to six cases of cutaneous lymphoma, including three cases of mycosis fungoides, two cases of cutaneous T cell lymphoma, and one case of adult T cell lymphoma, resulted in three cases of CR, two cases of MR and one case of PD. In contrast, four cases of malignant melanoma, two of which received systemic administration and two intralesional administration, did not show any obvious clinical response, one showing PR and the other three PD. Histopathological examination of skin lesions before and after administration of rIFN-gamma indicated that the cutaneous lymphoma lesions were initially infiltrated by lymphocytes and macrophages, and that later on, they were free from tumor cells. Our study indicates that rIFN-gamma appears to provide a new modality for the treatment of cutaneous lymphoma by systemic and intralesional administration.

Evaluation of fetal development and well-being with ultrasonic Doppler fetal actocardiogram.

A new ultrasonic technique for objective, continuous and simple recordings method of fetal movements on CTG chart has been introduced. Fetal activity and heart-rate will be correlated. This allows a differentiation of fetal active and resting states. The duration of the first one was 24 to 150 minutes, that of the last one 10 to 36 minutes. Fetal actogram is useful in the prediction of imminent fetal distress.
