An adult case of lymphangiomatosis of the mediastinum, pulmonary interstitium and retroperitoneum complicated by chronic disseminated intravascular coagulation.

Pulmonary lymphangiomatosis is a rare clinical and pathological entity which is distinct from lymphangiomyomatosis and from pulmonary lymphangiectasis. We report a case of a 20 year old man with diffuse lymphangiomatosis involving the mediastinum, lungs and retroperitoneum. The patient's intrathoracic lymphangiomatosis produced restrictive and obstructive impairment of his pulmonary function, but did not result in chylothorax or chylopericardium, which are two common manifestations of the disease. The patient's clinical course was remarkable for the concomitant development of chronic disseminated intravascular coagulation (DIC). Lymphangiomatosis involving both the mediastinum and pulmonary parenchyma is rare, and DIC is a very unusual complication of lymphangiomatosis.

Tumor angiogenesis as a predictor of recurrence in gastric carcinoma.

PURPOSE: We investigated the correlation between tumor angiogenic activity and progression of gastric carcinoma using immunohistochemical staining with antifactor VIII-related antigen (F-VIII RAg) antibody. MATERIALS AND METHODS: One hundred twenty-four specimens resected from patients with gastric carcinoma were investigated by staining with a monoclonal antibody against F-VIII RAg. Correlations between the microvessel count (the mean number of microvessels in the five areas of highest vascular density at 200 times magnification), various clinicopathologic factors, and prognosis were studied. RESULTS: The microvessel count increased with histologic stage. The microvessel count was significantly higher in patients with lymph node metastases than in those without such metastases. Moreover, in patients with a high microvessel count (> or = 16), prognosis was significantly poorer than in those with low count (< 16). Multivariate analysis indicated that the microvessel count is an independent prognostic factor in patients with gastric cancer. According to the mode of recurrence, the frequency of hepatic metastases was significantly increased in patients with a high count. CONCLUSION: Microvessel count may be a good prognostic indicator and may be useful as a predictor for the mode of recurrence in patients with gastric carcinoma.

The phylogenetic relationships of methanol-assimilating yeasts based on the partial sequences of 18S and 26S ribosomal RNAs: the proposal of Komagataella gen. nov. (Saccharomycetaceae).

Twelve strains of methanol-assimilating yeast species were examined for their partial base sequences of 18S and 26S rRNAs. In the partial base sequencings of 18S rRNA (positions 1451-1618, 168 bases), P. kodamae had the same partial base sequence as Ogataea minuta, and P. finlandica, P. pini, P. trehalophila, C. maris, and C. methanolovescens were the same in the partial base sequences as O. glucozyma. Candida boidinii and C. methylica were similar to O. glucozyma, but somewhat different from O. minuta (the base differences, one and four, respectively). Pichia naganishii had seven, four, six, and nine base differences with O. minuta, O. glucozyma, P. anomala, and P. membranaefaciens, respectively, and C. methanosorbosa did four, three, five, and twelve base differences. Pichia pastoris was quite different (the base differences among the methanol-assimilating yeasts, 32-31), and no bases were found in the fingerprint segment. In the partial base sequencings of 26S rRNA (positions 1611-1835, 225 bases; positions 493-622, 130 bases), P. finlandica, P. kodamae, P. pini, P. trehalophila, C. maris, C. methanolovescens, and C. methanosorbosa were similar to O. minuta and O. glucozyma. Pichia naganishii, C. boidinii, and C. methylica had somewhat different partial base sequences. The base differences and the percent similarities of P. pastoris were quite high (101-89) and quite low (40-47). Based on the sequence data obtained, the methanol-assimilating yeast species are discussed taxonomically and phylogenetically. A new genus, Komagataella was proposed for P. pastoris with a new combination, Komagataella pastoris.

The feline herpesvirus type 1 ICP4 down-regulates feline immunodeficiency virus long terminal repeat (LTR)-directed gene expression via the C/EBP site in the LTR.

We investigated effects of feline herpesvirus type 1 (FHV-1) ICP4 on feline immunodeficiency virus (FIV) long terminal repeat (LTR)-directed gene expression by transient transfection assay in Crandell feline kidney cells. We demonstrated that FHV-1 ICP4 significantly stimulates the FIV LTR after introduction of site-specific mutation of the C/EBP site in the LTR, and the C/EBP site is sufficient to confer inhibitory effects by FHV-1 ICP4 on a heterologous promoter. These results indicate that FHV-1 ICP4 possesses both ability to transactivate FIV LTR-directed gene expression and to down-regulate the FIV LTR via the C/EBP site.

Comparisons among feline herpesvirus type 1 isolates by immunoblot analysis.

Feline herpesvirus type 1 (FHV-1) isolated from cats suspected for feline viral rhinotracheitis could be divided into 3 genotypes (C7301, F2 and C7805 types) by digestion pattern with a restriction enzyme M1uI[16]. Variation of immunogenic products of 77 isolates were examined by immunoblot analysis using a polyvalent cat serum against C7301 strain, Japanese prototype strain of FHV-1. The result showed that a 36 kDa band was detected in 31 out of the 63 isolates belonging to C7301 type. However the band was not observed in the remaining 32 isolates of C7301 type and 14 isolates belonging to either F2 or C7805 type. In one isolate (91-58) of C7301 type, approximately 75 kDa band instead of 70 kDa band was observed, although the other bands were commonly observed in the remaining 76 isolates. From the results of the present and previous studies, FHV-1 field isolates could be divided into at least 4 groups and this grouping might be applicable for epidemiological studies of FHV-1 infection in the field.

Construction of the recombinant feline herpesvirus type 1 deleted thymidine kinase gene.

We constructed a recombinant virus containing defined deletion within thymidine kinase (TK) gene from feline herpesvirus type 1 (FHV-1) C7301, which was isolated in Japan, by standard rescue procedures with thymidine arabinoside (araT) selection. The araT resistant recombinant virus was designated as C7301dlTK. Southern blot analysis, together with polymerase chain reaction, revealed a deletion with the expected size, based on agarose gel electrophoresis, of the TK gene region in the C7301dlTK. Growth kinetics of the C7301dlTK in CRFK cells was similar to that of ther parent C7301 that possesses TK activity. However, plaques produced by the C7301dlTK were significantly smaller than those of the parent virus. This report migth be applicable for the development of a new genetically engineered FHV-1 vaccine.

Effects of upper limb exercise on thermoregulatory responses in patients with spinal cord injury.

We evaluated changes in body temperature, heart rate, and oxygen uptake during arm cranking exercise (20 watts, 30 min) in an artificial climate room at a temperature of about 25 degrees C or 35 degrees C (relative humidity, about 50%) in 5 patients with paraplegics due to spinal cord injury (SCI). The tympanic temperature (Tty) was significantly higher from rest to recovery at 35 degrees C than at 25 degrees C. The mean Tty after 10 minutes of rest was 36.68 degrees C +/- 0.396 (mean +/- SD) at a room temperature of 25 degrees C and 37.25 degrees C +/- 0.253 at 35 degrees C, showing a difference of about 0.5 degrees C. This difference was maintained during exercise and recovery. The Tty at the end of exercise was higher than that at the start of exercise by 0.66 degrees C +/- 0.218 (mean +/- SD) at 25 degrees C and by 0.59 degrees C +/- 0.210 at 35 degrees C. The skin temperature (Tsk) in each measurement area (the head, arm, chest, thigh, shin, and calf) was significantly higher at 35 degrees C. Oxygen uptake did not differ between 25 degrees C and 35 degrees C. The heart rate was significantly higher at 35 degrees C than at 25 degrees C during rest and at the start of exercise but did not differ during exercise and recovery. Thus, mild-moderate exercise in this study did not cause marked changes such as increases in body temperature and heart rate that affect biological function in SCI.(ABSTRACT TRUNCATED AT 250 WORDS)

Adjustments of cardiac function during prolonged exercise relative to lactate threshold.

Alterations in cardiac function during prolonged submaximal exercise relative to lactate threshold (LT) were evaluated on 7 normal healthy males aged 30.3 +/- 5.7 years. Systolic time intervals were analyzed through simultaneous recordings of electrocardiogram, phonocardiogram, and impedance cardiogram at a paper speed of 50 mm/s. Determination of stroke volume (SV) was based upon the method described by Kubicek et al. ANOVA followed by the Scheffe post-hoc comparison revealed that SV and myocardial contractility indices (MSER and PEP/LVET) remained relatively unchanged throughout 40-min cycling exercise, although the changes in heart rate and oxygen uptake were statistically significant. In addition, systolic blood pressure remained almost unchanged during the exercise. These results may be interpreted as evidence of the "contractility reserve", i.e., the ability of maintaining heart muscle contractility during prolonged exercise at LT intensity.

Smoldering adult T-cell leukemia with B-cell lymphoma and early gastric cancer.

We report a case of smoldering adult T-cell leukemia (ATL) with B-cell lymphoma and early gastric cancer. A 64-year-old man was admitted to our hospital because of proteinuria and hypergammaglobulinemia. Systemic lymphadenopathy, "flower cells" in peripheral white blood cells, and hypergammaglobulinemia with monoclonal gammopathy (IgA, lambda type) were found. As Southern blot analysis revealed monoclonal integration of human T-lymphotrophic virus type I proviral DNA in peripheral blood mononuclear cells, he was diagnosed as having smoldering ATL. The tissue specimen of an inguinal lymph node showed proliferation of abnormal lymphocytes which were stained with anti-lambda antibody, indicating B-cell lymphoma. A polypoid lesion in the stomach was histologically diagnosed as early gastric cancer.

Significance of urinalysis for subsequent kidney and urinary tract disorders in mass screening of adults.

To evaluate dipstick urinalysis as a predictor of subsequent kidney and urinary tract disorders in apparently healthy adults, we designed a cross-sectional, prospective, and retrospective study within a cohort. The severity of proteinuria was significantly (p < 0.01) associated with the amount of pathological casts, whereas hematuria without proteinuria was not. The frequency of subsequent serum creatinine increase (0.3 mg/dl/5 years or more) was significantly enhanced (from 0.4% to 7.3% along with the severity of proteinuria, whereas it was not related to the severity of hematuria. Patients who subsequently developed renal failure and glomerulonephritis exhibited hematuria (11.1-32.1%) less frequent than proteinuria (62.3-83.3%). Even those with renal tumors or stones showed infrequent (14.3-27.9%) hematuria. Thus, urine protein and occult blood have different implications in mass screening.

Severity of alexithymia is related to psychosocial factors in patients with peritoneal dialysis.

We examined the relationship between alexithymia and psychosocial factors in 72 peritoneal dialysis patients. The 72 patients had significantly higher scores on alexithymia and anxiety than did 73 healthy volunteers. The alexithymia scores were significantly and positively correlated with anxiety scores, suggesting that alexithymia may be related to anxiety derived from the stress associated with dialysis therapy. After 3 yr. of follow-up consultations, patients were still showing higher scores on alexithymia and anxiety; however, alexithymia scores were not correlated with anxiety scores but rather were significantly associated with poor social support. Alexithymic characteristics may be related to psychosocial factors such as the availability of social support.

A protective role of gamma delta T cells in primary infection with Listeria monocytogenes in autoimmune non-obese diabetic mice.

We investigated the host defense mechanism in primary infection with Listeria monocytogenes in non-obese diabetic (NOD) mice at pre-diabetic stage showing an impaired responsiveness of the alpha beta T cells to T-cell receptor (TCR) triggering. The NOD mice showed a deteriorated resistance at the late stage after an intraperitoneal infection with L. monocytogenes compared with BALB/c and C57BL/6 mice as assessed by bacterial growth in organs. Consistent with our previous findings, a prominent increase in number of gamma delta T cells was evident at the early stage after infection, while generation of Listeria-specific alpha beta T cells was impaired in these mice. In vivo administration of anti-TCR gamma delta monoclonal antibody (mAb) allowed L. monocytogenes to grow exaggeratedly in the NOD mice. These results imply that gamma delta T cells may be mainly involved in protection against primary infection with L. monocytogenes in NOD mice.

[Tubulointerstitial changes in some hematological disorders].

This paper describes tubulointerstitial changes of the kidney in association with a) pathophysiology of paraproteinuria and related disorders, and b) the management of leukemia and malignant lymphoma. Various forms of tubulointerstitial changes might be provoked following the accumulation of "abnormal macromolecules" In a), multiple myeloma, light chain cast nephropathy (myeloma kidney), AL amyloidosis, light chain deposition disease, macroglobulinemia, cryoglobulinemia etc. are briefly reviewed. In the majority of cases, leukemia and lymphoma do not manifest themselves as tubulointerstitial disorders. However, a large number of patients suffer from tubulointerstitial abnormalities in the course of and/or after receiving chemotherapy. Thus it is explained, in b), why treatment for hematological malignancy is apt to induce tubulointerstitial complications. In this context, drugs responsible for the development of tubulitis and/or interstitial fibrosis are briefly reviewed.

[A case of interstitial pneumonia in polymyositis difficult to distinguish from gold pneumonitis].

Rheumatoid arthritis was diagnosed in a 48-year-old woman. She received a gold compound, and 4 weeks after the start of that therapy, interstitial pneumonia appeared. Findings from a muscle biopsy, and high serum CPK and LDH levels indicated that she suffered from polymyositis rather than rheumatoid arthritis. The result of a drug lymphocyte stimulation test (DLST) for the gold compound was more than 200%. Because the usefulness of the DLST for the gold compound in the diagnosis of gold pneumonitis is not thoroughly established, the DLST was also done in patients with rheumatoid arthritis who were receiving the gold compound without side effects, and in normal subjects. Many of the rheumatoid arthritis patients and some of the normal subjects had a positive response to the gold compound. Therefore a positive response on the DLST for the gold compound does not always support the diagnosis of gold pneumonitis.

Evaluation of disease activity in patients with moderately active ulcerative colitis: comparisons between a new activity index and Truelove and Witts' classification.

OBJECTIVES: We have recently proposed a new activity index (AI) for ulcerative colitis. The aim of this study was to examine how AI would reflect the effect of medical therapy in comparison with Truelove and Witts' criteria. METHODS: In this trial, we studied 35 patients with extensive colitis who persisted with moderate disease for 2 wk. They showed no change in disease activity according to Truelove and Witts' classification despite 2 wk of corticosteroid therapy. Nine of the 35 patients underwent surgery (operated group), and 26 responded to the medical therapy and went into remission (nonoperated group). RESULTS: In the nonoperated group, AI values significantly decreased at 1 and 2 wk after treatment compared with pretreatment values, whereas there was no significant change in AI in the operated group during the clinical course. Remission occurred in all patients indicating below 180 of AI values 2 wk after therapy, whereas approximately 65% of patients indicating above 180 of AI values subsequently needed colectomy. CONCLUSIONS: This study suggests that AI will be useful in evaluation of response to medical therapy and prediction of remission and colectomy in patients with moderately active ulcerative colitis.
