[Functional treatment of palmar edge fractures of the middle phalanx and palmar capsule ruptures of the interphalangeal joint].

Rupture of the palmar plate of the PIP joint is frequently followed by flexion contracture. The results of operative or immobilizing treatment are disappointing and therefore 46 patients have been treated by a Capener splint. 35 of these joints could be reviewed for the functional result. Only in five cases was extension reduced by five to ten degrees, the remaining being normal in this respect. By contrast flexion was limited in 27 patients, in 19 of them by between five to ten degrees, in the remaining eight by not more than 35 degrees.

Peripheral nerve injuries resulting from common surgical procedures in the lower portion of the abdomen.

Twenty-three patients had a painful ilioinguinal and/or iliohypogastric nerve entrapment syndrome following common surgical procedures in the lower portion of the abdomen (appendectomy, repair of inguinal hernia, and gynecologic procedures through transverse incision). The diagnostic triad of nerve entrapment after operation comprises (1) typical burning or lancinating pain near the incision that radiates to the area supplied by the nerve, (2) clear evidence of impaired sensory perception of the nerve, and (3) pain relieved by infiltration with anesthetic for local effects at the site where the two nerves leave the internal oblique muscle. Surgical repair of the scar with resection of the compromised nerve is the most effective treatment. Sixteen patients became symptom free after neurectomy, seven still suffer chronic pain in the scar.

[Changes in the drug resistance of bacteria from peripheral infections after 5 years].

The reports of outpatients with surgical infections of the years 1972 and 1977 are compared to demonstrate changes in composition of the spectrum and resistance to antibiotics. In the first period 425 cases could be used for study, in the second only 132 cases could be traced. In the second period phlegmonous infections, caused by streptococci were twice as frequent as in 1972. In the same year resistance of staphylococci to Penicillin was found in 43% of the cases. Five years later no more resistance was present and in 28,8% Penicillin even proved to be the most efficient drug. Likewise the streptococci had become more sensitive to Penicillin in the second period, but Cephalosporin remained the treatment of choice for this type of infections. Gram negative germs as B. coli were found much less in 1977 than five years before and their sensibility to sulfamethoxazol-trimethoprim was remarkably higher. The study demonstrates the changes in antibiotic resistance of a given germ population as well as the need for close observation of these changes. Standard antibiotic treatment has to be adapted to the modifications of germ behavior.

[Perilunar, transscaphoid, transcapital, transstyloid fracture-dislocation of the wrist. Operative reconstruction].

Report of a new type of fracture dislocation of the wrist and the intercarpal joints, which has not been published until now. A perilunar dislocation was associated with fractures of the scaphoid and capitate bones as well as a fracture of the radialstyloid. Open reduction and internal fixation was followed by fracture healing and slight posttraumatic arthritis. Loss of function was minimal and working capacity not reduced.

[Intravenous regional anesthesia (author's transl)].

Experiences, with about 1500 cases of intravenous regional anesthesia in outpatient surgery of the limbs over 10 years are reported. In 1975, 158 operations out of 5960 were done using this technique. Intravenous regional anesthesia is suitable for surgery of the limbs, but time of operation should not exceed 90 min, nor should hemostasis be a major problem to consider and the course of surgery should be predictable. Contraindications for this type of anesthesia are hypertonia, lack of accessible veins, heart failure, children, as well as surgery of undefinite extent or for local sepsis. When these rules were followed, no serious complications were seen.

[Advances in osteosynthesis of hand fractures].

In recent years stable internal fixation of hand fractures has become more commonly used though the majority of fractures of the hand bones are still treated successfully by other orthopedic means. We feel that in selected fractures internal fixation achieves better results than conservative treatment. Operative methods with less traumatic skin incisions for the fixation of multiple metacarpal fractures are proposed. A retrograde screw fixation technique is presented which permitts application of compression in very small bone fragments, which risk to be crashed by standard screwing. Results of 60 internal fixations of carpometacarpal fractures of the first metacarpal are reported. Follow-up examination of 41 patients between 1-7 years following surgery, revealed posttraumatic arthritis only in 3 joints, although remaining small articular irregularities were frequently seen on X-ray. Secondary angulation occured in four cases in spite fixation. This was due to poor cortical fixation on the palmar side of the fracture, thus in such cases temporary plaster cast should entirely be applied.
