Activity of fludarabine in previously treated non-Hodgkin's low-grade lymphoma: results of an Eastern Cooperative Oncology Group study.

PURPOSE: Fludarabine (2-fluoro-arabanoside-monophosphate) is a new antimetabolite chemotherapeutic agent. We performed a multicenter, phase II study of this drug in previously treated patients with refractory or relapsed non-Hodgkin's lymphoma (NHL) to determine its response rate by histologic classification. PATIENTS AND METHODS: Sixty-two assessable patients were given 18 mg/m2 by intravenous (IV) bolus injection daily for 5 days, every 28 days. Forty-eight percent had previously had one chemotherapy regimen, and the remainder had had two regimens; 42% had had radiation. RESULTS: Patients received 273 cycles of fludarabine chemotherapy, with a median of two cycles and ranging up to 25 cycles. Sixty patients were assessable for response, including nine complete responses (CRs; 15%) and nine partial responses (PRs; 15%). The response rate for patients with lower-grade histology was 52% (13 of 25); the greatest response rate was seen in those with follicular small cleaved-cell lymphoma, including seven of 11 treated. Five responders remain in unmaintained remission; the median survival of responders is greater than 30 months. Toxicity included mild neutropenia and a 10% incidence of grade 3 neurologic toxicity with occasional reversible visual and auditory changes. CONCLUSION: Fludarabine is active in patients with previously treated NHL (particularly low-grade histologies). Future studies will examine its activity in combination with other chemotherapeutic agents in previously untreated patients.

Differentiation of a human eosinophilic leukemia cell line (EoL-1) by a human T-cell leukemia cell line (HIL-3)-derived factor.

Differentiation of a human eosinophilic leukemia cell line, EoL-1, induced by the culture supernatant of a human ATL cell line, HIL-3 (HIL-3 sup) was compared with differentiation induced by defined cytokines. HIL-3 sup induced EoL-1 cells to express eosinophilic granules and segmented nuclei after 6 to 9 days of incubation. HIL-3 sup also induced the expression of Fc epsilon receptor II (Fc epsilon RII/CD23) and an eosinophil differentiation antigen EO-1 mainly on eosinophilic granule (+) cells. Furthermore, HIL-3 sup induced EoL-1 cells to respond to an eosinophil chemotactic factor, platelet activating factor. HIL-3 cells express messenger RNA (mRNA) of interleukin-5 (IL-5), macrophage colony-stimulating factor (M-CSF), and IL-3 but not granulocyte CSF (G-CSF). Granulocyte-macrophage CSF (GM-CSF) and tumor necrosis factor-alpha (TNF-alpha) were detected in the HIL-3 sup. Recombinant IL-2 (rIL-2), rIL-3, rIL-4, rIL-5, rM-CSF, and rGM-CSF did not induce eosinophilic granules. rG-CSF induced a few eosinophilic granule (+) cells, and TNF-alpha, which did not induce eosinophilic granules by itself, enhanced the ability of G-CSF to induce them. However, G-CSF and TNF-alpha did not induce the expression of Fc epsilon RII and EO-1 antigen. Moreover, anti-G-CSF, anti-TNF-alpha, anti-GM-CSF, anti-IL-3, and anti-IL-5 antibodies did not suppress the effect of HIL-3 sup on the differentiation of EoL-1 cells. All the data suggest that HIL-3 sup contains an unidentified factor that induces differentiation of EoL-1 cells, and that EoL-1 cells and HIL-3 sup provide an important model for the examination of differentiation mechanisms and functions of eosinophils.

Growth regulation of a human mature B cell line, B104, by anti-IgM and anti-IgD antibodies.

An EBNA- human B lymphoma cell line, B104, was established. B104 cells express IgD as well as IgM on their surface, which is thought to be a basic characteristic of mature B cells. The growth of B104 cells was inhibited by treatment with a panel of anti-IgM antibodies. Cell cycle analyses revealed that the transition of B104 cells from the G2/M to the G0/G1 phase of the cell cycle was markedly inhibited by treatment with anti-IgM antibodies. Progression of B104 cells to the M phase of the cell cycle was found to be suppressed in the presence of anti-IgM antibodies. In contrast, both the entrance of G0/G1 phase cells into the S phase and the progression of S phase cells to the G2/M phase of the cell cycle did not seem to be inhibited significantly by treatment with anti-IgM antibodies. These results indicate that the mechanism of the inhibition of growth of B104 cells by anti-IgM antibodies is blockage of the transition from the G2 to the M phase of the cell cycle. In contrast to anti-IgM antibodies, anti-IgD antibodies could not cause growth inhibition of B104 cells at all. B cell growth factors such as IL-4 and IL-6 had no effect on the inhibition of growth of B104 cells by anti-IgM antibody. IFN-alpha and -beta, which have no B cell growth factor activity, did increase the number of cells that survived the treatment with anti-IgM antibodies. B104 is an excellent experimental model for the study of the mechanism of signal transduction through sIg as well as the functional difference between sIgM and sIgD.

Regulation of Fc gamma receptor subtype expression on a human eosinophilic leukemia cell line EoL-3: participation of cAMP and protein kinase C in the effects of interferon-gamma and phorbol ester.

We investigated the effects of interferon-gamma (IFN-gamma), phorbol myristate acetate (PMA), and dibutyryl cAMP (Bt2cAMP) on Fc gamma R subtype expression on a human eosinophilic leukemia cell line, EoL-3. Unstimulated EoL-3 cells expressed Fc gamma RII as determined by monoclonal antibody (mAb) IV-3, whereas there was little or no Fc gamma RI and Fc gamma RIII expression as determined by mAbs 32.2 and 3G8, respectively. IFN-gamma induced Fc gamma RI expression, and Bt2 cAMP, which did not induce Fc gamma RI expression by itself, showed an additive effect on IFN-gamma-induced Fc gamma RI expression. Fc gamma RII expression was augmented by IFN-gamma, PMA, and Bt2 cAMP. Bt2 cAMP also showed an additive effect on IFN-gamma-augmented Fc gamma RII expression. Fc gamma RIII expression could be induced only by IFN-gamma plus Bt2 cAMP. H-7, a protein kinase C (PK-C) inhibitor, suppressed the enhancement of Fc gamma R subtype expression induced by these reagents. These results show that Fc gamma R subtype expression on EoL-3 cells is regulated differently in each subtype and that cAMP and PK-C play important roles in the regulation.

Collagen and elastin in the normal fetal bladder.

The development of muscle, collagen and elastic fibers was studied histologically in 15 human fetal bladder specimens. Muscle thickness progressively increased and the relative collagen content in the muscle decreased during gestation. The ratio of thick-to-thin collagen fibers also decreased, whereas elastic fibers increased. These findings document the normal process of maturation of the fetal bladder and serve as a basis for comparison with congenitally obstructed fetal bladders.

Collagen and elastin in the obstructed fetal bladder.

Histological findings of muscle, collagen and elastin in obstructed fetal bladders were compared with those of age-matched controls. Muscle thickness was markedly increased, however, the relative collagen content in the muscle was decreased. The ratio of thick-to-thin collagen fibers was markedly increased as was the amount of elastin. These findings suggest that the ratio of thick collagen to elastin has an important role in determining the compliance of the obstructed fetal bladder.

Dibutyryl cyclic AMP induces formyl peptide receptor expression and chemotactic responses in a human eosinophilic cell line, EoL-1.

We investigated actin polymerization and the increase of cytosolic free calcium concentration ([Ca2+]i) in a human eosinophilic leukemia cell line, EoL-1, in response to stimulation with chemotactic factors; we also investigated the effect of dibutyryl cyclic AMP (dbcAMP) on the responses. EoL-1 cells under normal culture conditions did not show either actin polymerization or an increase in [Ca2+]i when stimulated with formyl-methionyl-leucyl-phenylalanine (fMLP). Expression of formyl peptide receptors was not detectable on untreated EoL-1 cells, either. Dibutyryl cAMP induced the expression of formyl peptide receptors and the responsiveness to fMLP. The responsiveness of EoL-1 cells to the complement fragment C5a and platelet-activating factor (PAF) was also induced or enhanced by dbcAMP. The growth of EoL-1 cells was decreased and the proportion of cells in the G0/G1 phase of the cell cycle was increased by the treatment of EoL-1 cells with dbcAMP. The proportion of eosinophilic granule-containing cells and the content of eosinophil cationic protein (ECP) in EoL-1 cells was also increased when they were stimulated with dbcAMP for a longer period. The responsiveness of EoL-1 cells to fMLP, C5a, and PAF was shown to be regulated independently. EoL-1 cells and dbcAMP seem to be useful for examining chemotactic receptor expression and its signal transduction mechanisms.

De-epithelialized scrotal flap in repair of urethrocutaneous fistula and hypospadias.

Eight cases of closure of urethrocutaneous fistula and 15 cases of urethroplasty for hypospadias are reviewed. Most of the fistulas were present in the penoscrotal level. Fistula and hypospadias opening were closed by re-enforcement using a de-epithelialized scrotal skin flap. Of 8 patients with fistula, there was no recurrence, and 3 procedures were performed without urinary diversion or urethral stent. Of 15 patients with hypospadias, there was no fistula formation in the original urethral opening area but a fistula distal to the original urethral opening developed in 3 patients. This technique is satisfactory in repairing complicated as well as noncomplicated urethrocutaneous fistula and can be done without urinary diversion or urethral stent. It appears to be useful in reducing fistula formation following urethroplasty in hypospadias or urethral fistula repair.

Transforming growth factor beta and dexamethasone suppress the expression of Fc epsilon receptor 2 (CD23) on a human eosinophilic cell line EoL-3.

The effects of interferon-alpha (IFN-alpha), INF-gamma, transforming growth factor beta (TGF-beta) and dexamethasone on low-affinity Fc receptors for IgE (Fc epsilon R2/CD23) expression on a human eosinophilic leukemia cell line, Eol-3, were examined. Fc epsilon R2/CD23 expression was enhanced by both IFN-alpha and IFN-gamma, and suppressed by TGF-beta and dexamethasone. Northern blot analysis revealed that these reagents regulate the Fc epsilon R2/CD23 expression from mRNA level: both IFN-alpha and IFN-gamma increased the amount of Fc epsilon R2/CD23 mRNA, while both dexamethasone and TGF-beta decreased Fc epsilon R2/CD23 mRNA, where the effect of dexamethasone was much stronger than that of TGF-beta. In comparison with IFN-alpha, IFN-gamma seemed to enhance preferentially the release of surface Fc epsilon R2/CD23, which resulted in the increase of soluble Fc epsilon R2/CD23. These results suggest that these reagents may play important regulatory roles in allergy and in helminth infections via their effects on Fc epsilon R2/CD23 expression on eosinophils.

The effect of experimental varicocele on the testis of adolescent rats.

Experimental varicocele was created by partial ligation of the left renal vein in rats 4, 7 and 12 weeks old to compare the effects of varicocele on the testis at different times of onset: prepuberty, adolescence and adulthood. We examined the testes at 4 and 8 weeks after creation of the varicocele. The experimental groups of rats 4 and 12 weeks old showed slight testicular changes compared to control groups at 4 and 8 weeks postoperatively. In the 7-week group left testicular weight was significantly decreased (1.03 +/- 0.31 gm.) compared to that of the control group (1.51 +/- 0.12 gm.) at 4 weeks. The seminiferous tubular diameter in the left testis also was significantly reduced (219 +/- 37 microns) compared to that of the control group (270 +/- 14 microns). Histological changes, such as degeneration of germinal epithelium, tubular atrophy, Sertoli cell hyperplasia and interstitial edema, were observed more frequently in the 7-week group. At 8 weeks these changes were less significant. Our results suggest that varicocele during adolescence has the most damaging effects on the testis in rats.

[The effects of antibodies and complement in macrophage-mediated cytotoxicity on metacercariae of the lung fluke, Paragonimus westermani].

Paragonimus westermani is a tissue migrating parasite in the early stage until arriving at lung, and most of the parasites spend their life spans there. Considerable immune responses including activation of macrophages are taken place during the residence of parasites in the host. However, concerning the immunologic defense mechanisms of the host against this parasite, only a few document is available so far. In this study, the cytotoxic effect of peritoneal macrophages under the presence of antibody and/or complement against metacercariae of P. westermani was investigated in vitro. Metacercariae were collected from the crayfish, Cambaroides similis and hatched out in Tyrode solution (pH 7.4). Plastic adherent cells from normal or infected rat (Wistar) peritoneal exudates were used as experimental macrophages. Polyclonal antibodies were obtained from infected rats and a cat. Cat IgG was fractioned with ion exchange chromatography. Fresh rabbit complement was used according to experimental scheme. Various combinations of peritoneal macrophages, normal or infected rat serum, complement and cat IgG were incubated at 36 degrees C in 5% CO2 incubator for 6, 14, 24 and 48 hours. The results obtained were as follows: 1. P. westermani infection activated peritoneal macrophages non-specifically and this activation induced increases of cell adherence and cytotoxicity on metacercariae. 2. In the presence of infected rat serum the antibody-dependent cell-mediated cytotoxicity of peritoneal macrophages on metacercariae was significantly increased and showed a peak at 6-hour incubation. But the cytotoxic effect was markedly reduced after inactivation of complement and heat-labile IgE antibody by the heating of infected serum at 56 degrees C for 30 minutes. 3. The highest cytotoxic effect (100%) of concomitant incubation with IgG and complement showed 24 hours after incubation, although cell adherence was relatively low at 6-hour incubation and 0% at 24-hour incubation. 4. Coordinative functions of complement with serum and IgG were effective in cell adherence and in cytotoxicity, but it is not clear the independent role of complement on the macrophage-mediated cytotoxicity in this study. With these results it is assumed that P. westermani infection can induce the non-specific activation of peritoneal macrophages, and serum antibodies including IgE antibody might enhance the cytotoxicity by macrophages.

[Predictors of sick role behavior in patients receiving radiotherapy for cancer].

Cancer is still a threat to human beings. The incidence and mortality rate of cancer have been gradually increasing as the life span has been lengthened. Radiotherapy is one of the most commonly used treatments for cancer. This study explored the influence of social support and stress on sick role behavior of patients receiving radiotherapy for cancer. The subjects for this study were 60 patients undergoing radiotherapy for cancer, selected from the radiotherapy treatment unit of the out patient departments of two major medical centers in Jeonju. Data were collected from February 1 to 28, 1990 by a Likert Scale Questionnaire and an interview schedule designed by the investigator. Data analysis included percentages, mean and standard deviation, t or F-test, Pearson Correlation Coefficient and stepwise multiple regression. Results included the following: 1. Support came primarily from sons and daughters (90.1%); the type of support was primarily emotional support from friends (60.0%); informational support came from health personnel (81.7%); and material support was sons and daughters (40.0%); satisfaction with support was highest for the spouse (4.02 +/- .52). 2. Among the patient's demographic status was occupation the was the only socioeconomic characteristic influencing sick role behavior (F = 2.91, p = .029). 3. Directly previewed support was positively correlated with sick role behavior (r = .2374, p = .034). 4. Stepwise multiple regression was used to determine the predictors of sick role behavior. Directly perceived support was the most significant predictor accounting for the highest contribution to sick role behavior (5.6%). Directly perceived support, socioeconomic status, perceived stress and indirectly perceived support variables together, accounted for only 6.8% of sick role behavior.

Regulation of Fc gamma receptor expression and phagocytosis of a human monoblast cell line U937. Participation of cAMP and protein kinase C in the effects of IFN-gamma and phorbol ester.

We investigated the positive and negative effects of IFN-gamma, PMA, dibutyryl cAMP (Bt2cAMP), dexamethasone and transforming growth factor-beta (TGF-beta) on Fc gamma R subtype expression and phagocytosis of a human monoblast cell line, U937. IFN-gamma increased and Bt2cAMP decreased Fc gamma RI expression determined by a mAb 32.2, whereas PMA and Bt2cAMP increased Fc gamma RII expression determined by a mAb IV-3. Phagocytosis was measured microscopically by counting ingested aggregated human IgG- or BSA-treated ox E (Eo'-IgG or Eo'-BSA). IFN-gamma increased the phagocytosis of Eo'-IgG but not that of Eo'-BSA, and PMA increased the phagocytosis of both Eo'-IgG and Eo'-BSA. Bt2cAMP decreased both basal and IFN-gamma- and PMA-augmented phagocytosis of U937 cells. Dexamethasone also inhibited both basal and IFN-gamma-augmented Fc gamma RI expression and PMA-augmented Fc gamma RII expression and phagocytosis, but did not affect IFN-gamma-augmented phagocytosis of Eo'-IgG. The augmentation of phagocytosis of Eo'-IgG by IFN-gamma thus seems to be due mainly to the increased internalizing process rather than to increased Fc gamma RI expression. TGF-beta slightly decreased Fc gamma R expression. In a study of the participation of protein kinase C (PK-C), it was found that H-7, a PK-C inhibitor, did not inhibit either IFN-gamma- or PMA-enhanced Fc gamma RI and Fc gamma RII expression, respectively, and 1-oleoyl-2-acetylglycerol and N-(6-phenylhexyl)-5-chloro-1-naphthalenesulfonamide, both PK-C activators, did not show any apparent increase in Fc gamma R expression and phagocytosis. These results show that Fc gamma RI and Fc gamma RII expression on U937 cells is regulated by different mechanisms and that IFN-gamma and PMA play their roles in Fc gamma R expression and phagocytosis by different pathways. It is possible that cAMP but not PK-C plays an important role in the regulation of Fc gamma R expression and phagocytosis.

Regulation of Fc epsilon receptor 2 (CD23) expression on a human eosinophilic cell line EoL3 and a human monocytic cell line U937 by transforming growth factor beta.

Regulation of low-affinity receptors for IgE (Fc epsilon R2/CD23) on a human eosinophilic cell line EoL3 and a human monocytic cell line U937 was studied using an anti-Fc,R2/CD23 monoclonal antibody H107 by flow cytometry. While platelet-activating factor, interleukin 4, and interferon gamma significantly augmented Fc epsilon R2/CD23 expression on both cell lines, transforming growth factor beta (TGF beta) inhibited both the basal level of Fc epsilon R2/CD23 expression and the enhanced Fc epsilon R2/CD23 expression induced by these reagents in dose- and time-dependent manners. However, TGF beta did not significantly suppress the high basal level of Fc epsilon R2/CD23 expression on RPMI 8866 cells. These results suggest that Fc epsilon R2/CD23 expression on EoL3 and U937 cells is regulated by various cytokines and growth factors, and that TGF beta plays an important regulatory role in IgE-mediated immune responses.

Interferon and (2'-5')oligoadenylate enhance the expression of low affinity receptors for IgE (Fc epsilon R2/CD23) on the human monoblast cell line U937.

The regulation of low affinity IgE receptor (Fc epsilon R2/CD23) expression on the human monoblast cell line U937 was examined by an anti-Fc epsilon R2/CD23 monoclonal antibody (H107) and the cDNA probe for Fc epsilon R2/CD23. Alpha interferon (IFN-alpha) and its intracellular mediator, (2'-5')oligoadenylate (2, 5-A), induced Fc epsilon R2/CD23 expression on U937 with no significant increase of the Fc epsilon R2/CD23 mRNA. PMA and IFN-gamma increased both surface Fc epsilon R2/CD23 expression and the Fc epsilon R2/CD23 mRNA levels. IFN-alpha effectively induced 2, 5-A synthetase activity in U937 cells, whereas IFN-gamma induced little. The results suggest that the mechanisms of enhancement of Fc epsilon R2/CD23 expression on U937 cells by IFN-alpha and IFN-gamma are different and that 2, 5-A may play an important role in the Fc epsilon R2/CD23 expression on U937 cells induced by IFN-alpha.

Magnetic control for the vocal cord adduction in the canine.

An entirely satisfactory and physiologic solution to adduct paralyzed vocal cord during phonation, coughing, and swallowing has not yet been achieved. The authors noticed that velopharyngeal closure takes place simultaneously with adduction of vocal cords in order to perform phonation, coughing, and swallowing. We devised a new laryngeal pacing system to adduct the paralyzed vocal cord, utilizing velopharyngeal closure under magnetic control. Two mongrel dogs were anesthetized and the interior of the larynx was exposed using a Lynch suspension laryngoscope. A small magnet wrapped by thin Silastic was inserted into the nasal side of the soft palate via a small incision. After making a vertical midline neck incision, the pharynx was dissected and the Gaussmeter probe was inserted into the retropharyngeal space. The Gaussmeter probe was connected to the Gaussmeter and finally to the pacemaker. Electrodes were inserted into the paralyzed adductor laryngeal intrinsic muscles via punctures of the cricothyroid membrane. When the pacing system operated, arbitrarily elevated soft palate to the posterior pharyngeal wall brought about an abrupt increase in magnetic force and thus obvious adduction of the paralyzed vocal cords could be seen.

Soluble low affinity Fc receptors for IgE in the serum of allergic and nonallergic children.

IgE-binding factors are thought to have regulatory activity in in vitro IgE synthesis. To obtain evidence of the participation of IgE-binding factors in in vivo IgE synthesis, the serum level of low affinity Fc receptors for IgE (sFc epsilon RII) (IgE-BFs) was examined in 41 nonallergic children and in 37 allergic children whose serum IgE levels were significantly higher than those of nonallergic children. The serum level of sFc epsilon RII showed a marked age-dependent variation. It was highest in infants and then decreased gradually with age. The serum level of sFc epsilon RII in allergic children was significantly higher than that of nonallergic children in early childhood (1128.0 +/- 323.8 vs 777.3 +/- 227.0 pg/ml, p less than 0.01 in infants (less than 1 y) and 851.8 +/- 270.0 vs 579.4 +/- 197.1 pg/ml, p less than 0.05 in children aged 1-2 y) but not in older children (3-15 y). Three allergic infants (less than 1 y) with serum sFc epsilon RII levels higher than the mean + 1 SD (1451.8 pg/ml) of all allergic infants (less than 1 y) had serum IgE levels (geometric mean 125.9 IU/ml) significantly higher than the other seven allergic infants (less than 1 y) (geometric mean 5.6 IU/ml, p less than 0.05). A close positive correlation between the serum level of sFc epsilon RII and the absolute number of Fc epsilon RII(+) peripheral blood lymphocytes was observed (Spearman's rank correlation coefficient = 0.79, p less than 0.001 in 27 allergic and Spearman's rank correlation coefficient = 0.72, p less than 0.001 in 19 nonallergic children). In conclusion, serum sFc epsilon RII may be derived mainly from Fc epsilon RII(+) lymphocytes, and may have relationship to the increased production of IgE in early childhood (0-2 y).

A survey of externally recognizable genitourinary anomalies in Korean newborns. Korean Urological Association.

To estimate the incidence of externally recognizable genitourinary anomalies and associated anomalies in the newborns in Korea, retrospective and prospective studies have been performed. Thirty eight of 48 urology training hospitals participated in this nationwide survey. In this study we have included minor defects or variations in the anomaly to evaluate the incidence of the recognizable genitourinary conditions in the newborns. The incidence of genitourinary anomaly in 1,000 newborn delivery in a year was 11.0 in the prospective study and this figure is about three times higher than the retrospective study and is considered to be close to the true incidence. The incidence of genitourinary anomaly in 1,000 male newborn was 20.4. Hydrocele, cryptorchidism and hypospadias were most commonly observed. The incidence of hydrocele in 1,000 male newborn was 9.89 and the incidence of cryptorchidism was 7.26 and the incidence of hypospadias was 2.13. The incidence of associated anomaly in 100 genitourinary anomaly was 7.9. Congenital heart diseases and anorectal anomalies were commonly associated anomalies. In the newborns with genitourinary anomalies, premature infants account larger portion than is usually reported in total delivery.
