Congenital anomalies of the upper limb among the Chinese population in Hong Kong.

Three hundred twenty-six patients with 396 congenital upper limb anomalies are classified according to the system adopted by the International Federation of Societies for Surgery of the Hands. The relative incidence of each separate anomaly is listed and compared with that of other series. Two hundred cases are also analyzed with respect to the family history, pregnancy history, neonatal history, and associated anomalies and the results are discussed. Significant racial differences were found in the incidence of polydactyly, syndactyly, anomalies associated with syndromes, and transverse arrests. The incidence of polydactyly is higher in this series than in comparable western series.

Finger tip injuries--a prospective study on seven methods of treatment on 200 cases.

7 common methods of treatment were selected at random for 200 cases of finger tip injuries. The methods included split skin grafting, full-thickness skin grafting, volar V-Y advancement, Kutler's bilateral V-Y advancement, revision amputation, cross-finger flap and simple dressing. These cases were seen weekly until healing was completed and then at 3 and 6 months. A proper functional assessment was given to each case at the last visit. The results were analysed along the parameters of wound healing, appearance, scar condition, sensory return, finger joint movements, motor-power and sick-leave benefits. The best results were obtained from the volar V-Y advancement method. Results were most unfavourable when cross-finger flaps were used. This latter method, therefore, was condemned. Simple dressing gave excellent results if one disregarded the lengthy period of wound healing, a somewhat tender scar and possible beaking of the nails. The other methods gave satisfactory results but had no outstanding advantage.

Complications in treatment of trochanteric fractures using Ender's nails--a review of 120 patients.

A prospective study of 120 Chinese patients with trochanteric fractures treated with the surgical method of Ender's Nailing was undertaken. The average age of the patients was 79 years (ranged from 57 to 105 years). The follow-up period ranged from 4 to 18 months, with an average of 8 1/2 months. The mortality rate was 6%. There was no incidence of non-union, delayed union or deep wound infection. Local complications included extrusion of the nails, perforation of the femoral head, supracondylar fracture of the femur, mild limb shortening and external rotational deformity. Hip and knee pain occurred in a small percentage of patients but these tended to improve with time. Weight bearing walking could be started in the first week after operation in most of the patients. The functional result at 6 months was good in 73% of the patients. This method had the advantage of a limited surgical approach, minimal blood loss and good functional result. Analysis of the causes of the local complications indicated that if operative precautions were taken, morbidity could be reduced.

Intravenous regional anaesthesia in hand surgery - experience with 632 cases.

Practical experience in the use of intravenous regional anaesthesia in hand surgery on 632 patients is reported. The safety of the anaesthesia, the efficacy of the procedure and the patient-comfort are well attested in this study. We recommend that its role should be established particularly in busy orthopaedic surgery departments where both elective and emergency work-loads on hand surgery are heavy.

Sample viscosity can be a source of analytical error when discrete sampler-dilutors are used.

Total protein concentration in the serum of a patient with hyperviscosity syndrome differed as measured by the biuret procedure in the DuPont aca (80 g/L) and the SMA 12/60 (105 g/L), owing to viscosity-dependent errors with the aca sampling system; the magnitude depended on sample temperature and volume of sample aspirated. This kind of error was not observed with the SMA 12/60 and was far less severe when a Micromedic sampler-dilutor was tested. It could be eliminated in the case of the aca by adding sample to test packs with a syringe rather than with the aca automated sampler-dilutor. We thus recommend use of the syringe method when unusually viscous samples (serum or other body fluids) are analyzed in the aca.

Charcoal hemoperfusion for treatment of carbamazepine poisoning.

We report a case of carbamazepine (Tegretol) poisoning that was treated with charcoal hemoperfusion. When admitted, the patient was comatose and required ventilatory support. Hemoperfusion with coated activated charcoal successfully decreased her serum carbamazepine concentration from 60 mg/L to 36 mg/L within 2 h, with subsequent clinical improvement. Approximately 39 h would have been required to obtain this decrease via the patient's inherent elimination rate. Activated charcoal hemoperfusion may be useful as an adjunctive therapy for patients in advanced comatose states with associated respiratory depression and potentially fatal outcome.

Sequential passage of alpha2mu-globulin through the hepatic endoplasmic reticulum and Golgi apparatus during secretion.

Studies on the synthesis and secretion of the sex-dependent urinary protein, alpha2mu-globulin, have been extended by establishing its sequential passage from the rough to the smooth endoplasmic reticulum and Golgi-rich fractions of the liver of adult male rats. After injection of 14C-labeled amino acids, the maximum radioactivity of alpha2mu occurred at 20 min in the rough, 25 min in the smooth microsomes and 30 or 35 min in the Golgi-rich fractions. Radioactive alpha2mu-globulin appeared in the bloodstream and kidneys after a lag of 20--25 min. Results indicate that alpha2mu-globulin follows a secretory pathway similar to that of serum albumin.

Glucocorticoid induction of hepatic alpha2u-globulin synthesis and messenger RNA level in castrated male rats in vivo.

alpha2u-Globulin is a male rat liver protein of Mr = 20,000 which is synthesized in the liver of adult male rats, secreted into the serum, and excreted in the urine. Its function is unknown. The hepatic synthesis of this protein is under complex hormonal control. We had previously shown that castration of male rats diminishes hepatic alpha2u-globulin synthesis and the level of its mRNA, and that administration of androgen to these castrated animals results in the reinduction of the synthesis of this protein and the level of its mRNA. We now report that alpha2u-globulin synthesis and the level of its mRNA can be fully reinduced in castrated males by administration of glucocorticoid alone. This induction is much more rapid than the androgenic induction and is inhibited by the glucocorticoid antagonist progesterone. Administration of glucocorticoid to intact male animals does not induce alpha2u-globulin synthesis above normal levels; however, if alpha2u-globulin synthesis has been depressed in intact male rats by pretreatment with estrogen or cyproterone acetate, the level of this protein can be reinduced by administration of glucocorticoids. The implications for the control of alpha2u-globulin gene expression are discussed.

Transcription of the alpha2u-globulin gene in male rat liver nuclei in vitro.

Alpha2u-globulin is a male rat liver protein under multihormonal control which represents approximately 1% of hepatic protein synthesis. We have measured the rate of transcription of the alpha2u-globulin gene using a nuclear cell-free transcriptional system with mercurated CTP as substrate for the endogenous RNA polymerases. The newly synthesized, mercurated RNA was purified free of endogenous RNA by chromatography on sulfhydryl agarose and hybridized to 3H-labeled alpha2u-globulin cDNA. It was found that, in male rat liver nuclei, alpha2u-globulin RNA sequences represent 0.005% of the total newly synthesized RNA. Actinomycin D or alpha-amanitin completely blocks the synthesis of alpha2u-globulin RNA in these nuclei. No alpha2u-globulin RNA synthesis was detectable in female rat liver nuclei. Thus, within the limits of our detection the absence of hepatic alpha2u-globulin mRNA in female rats appears to be due to a lack of transcription of the alpha2u-globulin gene in these animals.
