Effects of carnitine coingested caffeine on carnitine metabolism and endurance capacity in athletes.

The purpose of this study was to examine whether caffeine (CAF), carnitine (CAR), or CAF+CAR mixture administration affects exercise endurance time via carnitine metabolism. Water (CON), CAF, CAR, or CAF+CAR mixture was administered to five male rugby athletes participating in this study by a randomized double-blind fashion who were made to ride a cycle ergometer for exercise. The CAF effect on exercise endurance time was small, but the CAR trial significantly increased the exercise endurance time compared with CON trial; a further CAF+CAR mixture trial had greater effects on the exercise endurance time than those of a CON, CAF, or CAR trial. A CAR or CAF+CAR mixed trial increased urinary nonesterified carnitine (NEC) and total carnitine (TCAR), but no changes were observed in acid-soluble acylcarnitine (ASAC) and acid-insoluble acylcarnitine (AIAC) excretion. A CAR or CAF+CAR mixed trial resulted in higher levels of plasma NEC, ASAC, and TCAR fractions than the CON and CAF trials did on exhaustion time. Total cholesterol, triglyceride, and free fatty acid in blood were significantly increased at exhaustion time, but they were not affected in the CAF or the CAR trial. These results suggest that carnitine ingestion could promote fat oxidation, resulting in higher endurance performance in athletes, and especially these ergogenic effects of carnitine coingested with caffeine may be greater than those of carnitine alone.

[Isolation and characterization of the porcine glial fibrillary acidic protein (GFAP) gene by CATS].

Primers for the glial fibrillary acidic protein (GFAP) gene were designed from a human cDNA sequence aligned with the mouse GFAP gene on the principle of comparative anchor tagged sequence (CATS). The 412 bp PCR product isolated from Chinese Erhualian pig genome was characterized as the porcine GFAP gene by comparing the sequence with the GenBank database. The chromosomal location of the GFAP gene is on pig Chr: 12(p11-(2/3)p13) using pig x rodent somatic cell hybrid panel.

Expression and function of c-kit receptor in bone marrow mononuclear cells of patients with myelodysplastic syndromes.

OBJECTIVE: To determine the expression and function of the c-kit receptor in bone marrow mononuclear cells (BMMNC) of patients with myelodysplastic syndromes (MDS). METHODS: Direct immunofluorescence assay and reverse transcriptase-polymerase chain reaction (RT-PCR) were used to detect c-kit protein and c-kit mRNA expressions in the BMMNC of 29 MDS patients and 10 normal controls. Cell culture was used to detect the function of the c-kit receptor. RESULTS: c-kit protein expression in the MDS group was significantly higher than that in the control group (8.58% +/- 5.28% vs 3.04% +/- 1.49%, P < 0.05). c-kit protein expression in the refractory anemia (RA) group was significantly lower than that in the RA with an excess of blasts (RAEB)/RAEB in transformation (RAEB-t) group (5.12% +/- 2.13% vs 10.01% +/- 5.07%, P < 0.05). The rate of c-kit protein expression was 32.43% in acute myeloblastic leukemia (AML) cases transformed from MDS (t-AML). c-kit mRNA expression in the MDS group was correlated with c-kit protein expression. Interleukin-3 (IL-3) and erythropoietin (Epo), with or without stem cell factor (SCF), upregulated c-kit protein and its mRNA expression. In the presence of IL-3 and Epo, SCF showed significant stimulating effects on the formation of CFU-GM and BFU-E in semi-solid cultures of normal BMMNC, but had no effects on those of the MDS patients. CONCLUSION: The protein and mRNA expression of the c-kit receptor in the BMMNC of MDS patients were higher than those of normal controls, and the function of this receptor in MDS BMMNC was abnormal.

[Immunorelated pancytopenia].

OBJECTIVE: A report of a group of patients with pancytopenia which might be related to abnormal immunity. METHODS: The clinical and laboratory features of 29 patients with positive bone marrow mononuclear cell (BMMNC) Coombs tests were analyzed. RESULTS: Most of these cases were pancytopenia with normal or decreased bone marrow cellularities and increased normoblasts. They were all found to have negative results of conventional hemolysis tests and hematopoietic nutrient determination and had no evidence of malignant clonal hematopoiesis. However, all of them showed positive BMMNC-Coombs test and good response to corticosteroids. CONCLUSION: Immunorelated pancytopenia might be caused by abnormal immunity (especially autoantibody) mediated destruction or abnormal function of blood cells. It is beneficial to differentiate immunorelated pancytopenia from aplastic anemia or myelodysplastic syndromes.

[Sequential intensified immunosuppressive therapy combining with hematopoietic growth factors in the treatment of severe aplastic anemia].

OBJECTIVE: To explore more effective regimen for reducing early mortality of severe aplastic anemia (SAA) and improving therapeutic effectiveness. METHODS: Antilymphocyte globulin/antithymocyte globulin (ALG/ATG) and cyclosporine A (CsA) (sequential intensified immunosuppressive therapy, SIIST), with or without hematopoietic growth factors (HGFs) were administered to 73 SAA patients in a prospective randomized clinical trial to test the effectiveness of the addition of HGFs for the patients. RESULTS: The response rate of SIIST with HGFs group was significantly higher than that of SIIST alone group (89.2% vs 63.9%), with lower rates of early infection (24.3% vs 55.3%) and mortality (4.0% vs 16.7%), shorter duration of cytopenia and blood transfusion dependence and faster recovery of bone marrow hematopoiesis. The addition of HGFs to SIIST was tolerated well in all patients. There was no difference in the treatment outcome of the two groups with GM-CSF plus Epo or G-CSF plus Epo. CONCLUSION: The use of HGFs in combination with SIIST could reduce early infection and mortality rates and, therefore, improve the response rates in SAA patients.

[Study on the transformation from myelodysplastic syndromes into acute leukemias].

OBJECTIVE: To study the patterns of transformation from myelodysplastic syndromes (MDS) into acute leukemias (AL). METHODS: Leukemic transformation of MDS patients was dynamically followed up and the clinical manifestations, peripheral blood and bone marrow pictures, karyotypes, immunophenotypes, response to treatment and prognosis of post MDS acute leukemia (postMDS-AL) were observed. RESULTS: During the past eight year and seven months, 21 (13.91%) of 151 MDS patients progressed to overt leukemia with a median interval of 5 (1 - 21) months. There were no significant differences among the rates of leukemia from RA, RAEB and RAEB-t groups. The transformation was developed either gradually or rapidly. There were five parameters related to the leukemic transformation: under 40 years of age, pancytopenia, more than 0.15 blasts in bone marrow, at least two types of abnormal karyotype and combined chemotherapy. All of the 21 post MDS-AL were acute myeloid leukemia (AML); and most of them were M(2), M(4) and M(5). Two (9.52%) post MDS-AML developed extramedullary infiltration. Leukopenia was found in 47.62% of patients. Two third of the patients, whose bone marrows were generally hypercellular, showed neutropenias. After evolving into AML, 8 (47.06%) patients developed abnormal karyotypes. High expression of immature myeloid antigens, including CD(33) (49.83 +/- 24.50)%, CD(13) (36.38 +/- 33.84)%, monocytic antigen CD(14) (38.50 +/- 24.60)%, and stem cell marker CD(34) (34.67 +/- 30.59)% were found on bone marrow mononuclear cells of post MDS-AML cases. In some cases, lymphoid antigens, such as CD(5), CD(7), CD(9) and CD(19) were coexisted with myeloid antigens. A low complete remission rate (31.25%) and short survival duration with median survival of 6 (1 - 28) months were found in patients with post MDS-AML treated by induction therapy. CONCLUSION: MDS was at high risk of evolving into AML, either gradually or rapidly. Patients with post MDS-AML had specific biologic features and worse prognoses.

ISIS-3521. Isis Pharmaceuticals.

ISIS-3521 is a 20-mer antisense phosphorothioate oligonucleotide PKCa expression inhibitor, under development by Isis (formerly in collaboration with Novartis) for the potential treatment of solid tumors that are refractory to, or recurrent with, standard treatment regimens [175741]. In November 1999, Novartis announced that it would end its codevelopment of ISIS-3521 [348221], [348222]. In August 2001, Eli Lilly in-licensed ISIS-3521 [420062]. In October 2000, phase III trials of ISIS-3521, in combination with carboplatin and paclitaxel, were initiated for the treatment of non-small cell lung cancer (NSCLC) [386128]. The FDA granted ISIS-3521 Fast Track review status for NSCLC in November 2000 [388930]. In April 2001, Bear Sterns & Co predicted US approval of ISIS-3521 in 2002 [411081]. In August 2001, Eli Lilly and Isis entered into a four-year strategic alliance that includes ISIS-3521. For the license of ISIS-3521, Isis will receive $25 million in upfront fees and will be reimbursed for remaining phase III development and registration costs [420062].

[Two terms tympanoplasty in chronic suppurative otitis media].

OBJECTIVE: To explore the clinical manifestations and therapeutic effects of treating chronic suppurative otitis media with two terms tympanoplasty. METHOD: 36 patients were retrospectively analyzed. The criterions of success are air room in middle ear, intact eardrum and decrease of A-B gape in audiometry. RESULT: 32 cases of all patients got successful effects, 4 cases failed. The main cause of failure is to fail forming air room in middle ear. CONCLUSION: The two term's tympanoplasty can save the residual hearing in most patient. But it is difficult to get successful result if more mucous membrane in the middle ear disappeared.

[Anatomy structures of nasal cavity and paranasal sinus on virtual endoscopy and coronal image].

OBJECTIVE: To evaluate normal and bony anatomy of nasal cavity and paranasal sinus on the spiral CT virtual endoscopy(VE) and coronal scanning image. METHOD: After patients were scanned on axial or coronal position by spiral CT, data were transferred to workstation. Structures such as ostium-meatal complex(OMC) were viewed by navigator software when threshold was changing. RESULT: Turbinates, meatus and ostium-meatal complex were better viewed by virtual endoscopy compared with by coronal scanning image. To some extent, bony anatomy was clearer. CONCLUSION: Virtual endoscopy is of value in realizing characteristics of nasal cavity and paranasal sinus anatomy.

[The location of new porcine microsatellites by means of PRINS and somatic cell hybrid panel].

In this study, two new microsatellites HAU02 and HAU06, which are screened and cloned. HUA02 and HAU06 are repectively mapped in lq23-27 and 6q11-21 by means of PRINS (primed in situ labeling) and somatic cell hybrid (SCH) panel techniques, we discussed their advantage and disadvantage in gene mapping. These two means provide us more techniques in gene mapping, and the locations of these new microsatellites accumulate more resource for Porcine Physical Map.

[Research on the dynamic variation and elimination of nitrite content in sauerkraut during pickling].

To investigate the dynamic variation and elimination of nitrite content in pickles and sauerkraut during pickling, green tuber growing in Fuling was selected to make semi-manufactured mustard according to the process technology. Green tuber, cabbage and cauliflower were selected as raw materials to make sauerkraut in family way. The nitrite content in pickles and sauerkraut was determined by spectrophotometry with 1-Amino-2-(a-naphthy lamine) ethoine dihydrochloride. The nitrite content in mustard was 258.8 mg/kg when 6% salt was used and pickling for 8 days; the nitrite content was 235.9 mg/kg when 15% salt was used and pickling for 15 days. There was no nitrite in mustard when pickling for 35 days. The nitrite content in the sauerkraut was 258.7 mg/kg when pickling for 6 days and no nitrite was found after 20 days. The higher nitrite content in pickle and sauerkraut could be eliminated by water. The elimination ratio was 91% for leafy sauerkraut and semi-manufactured cut mustard and only 72% for whole mustard. It was concluded that pickles and sauerkraut that pickled for enough time (for leafy vegetable more than 20 days, for tuber more than 35 days) had no harm for people to eat, The higher nitrite content in pickles and sauerkrauts could be reduced by 91% with water for several times.

[The photostability of hydrocortisone injection].

OBJECTIVE: This experiment was designed to make known wether the photostability of drugs in daylight can be obtained from lamplight exposure. METHODS: A self made cumulative illuminometer via pulse counting method was used for measuring cumulative illuminance of daylight to investige the photodegradation of Hydrocortisone injection in various lamp light and daylight; the equivalent influences of different light sources on the photostability of the injection were obtained, and the shelf-life in indoor daylight of the drug was predicted. RESULTS: The photodegradation of Hydrocortisone injection obeys zero order kinetics: C = C0-kEt; the shelf-life in indoor daylight of the injection in nude ampoules was predicted as 36 days and is comparable to 35 days in a long-term storage test. CONCLUSION: The photostability of drugs in daylight can be obtained from lamplight exposure experiment.

[Investigation on the value of applying interventional radiologic treatment in the management of advanced gynecological malignancies].

OBJECTIVE: To investigate the value of applying interventional radiologic treatment (IRT) in the management of advanced gynecological malignancy. METHODS: Twenty-four historically confirmed advanced gynecological malignancy patients from September 1997 to June 1998 were included. After receiving internal iliac arterial infusion chemotherapy with Seldinger, the patients' tumor tissues were pathologically studied at different times. The tumor size was measured by ultrasonography two weeks after IRT. RESULTS: Before internal iliac arterial infusion chemotherapy, internal iliac arteriography showed that uterus artery was thickened and tortuous, the new vessels appeared and the tumors were stained. In some cases, the image of blood vessel-lake and arteriovenous shunt were seen in tumor tissues. Twenty-four hours after internal iliac arterial infusion chemotherapy, focal necrosis was observed in tumor cells, seventy-two hours after, the necrosis became more thorough. In regard to the tumor size, two patients were CR, nineteen patients were PR, and two patients were NC. Only one patient failed to respond to IRT. Chemotherapy side effects were slight. Patients' WBC returned normal within two weeks after chemotherapy. No complication happened. CONCLUSION: 1. Internal iliac arteriography has practical significance in assessing the tumor size and evaluating the therapeutic effects; 2. the internal iliac arterial infusion chemotherapy can reduce the tumor size, which prepares the patient in a better condition for operation. The most appropriate time for operation is two weeks after internal iliac arterial infusion chemotherapy. Even for the patient who is not suitable for operation, the reduction of the tumor size still can enhance the therapeutic effects of radiotherapy. 3. IRT is easy to perform, effective and with few slight side effects and complication. It is an effective way to save the patient's life.

[Expression of bcl-2 in facial motoneurons and its ultrastructural localization following facial nerve injury].

OBJECTIVE: To explore the expression of bcl-2 in facial motoneurons and its subcellular distribution. METHODS: Wistar rats were used in this study. Facial nerve transection was performed at stylomastoid foramen or internal acoustic meatus. Facial nerve crush was made at stylomastiod foramen. The animal survived for 1, 3, 7, 15, 30 and 60 days respectively. Facial nucleus was treated with bcl-2 monoclonal antibody or bcl-2 DIG-labelling probe and studied with immunohistochemistry and in situ hybridization. The bcl-2 positive motoneuron was investigated with immuno-electron microscope. RESULTS: It was demonstrated that bcl-2 protein level was corresponded with bcl-2 mRNA expression. The level of bcl-2 expression in facial motoneurons was high in normal facial nerve. It increased on the first day and declined on the third day post-transection in facial motoneuron. It reached the lowest level on the 15th days following facial nerve injury (P < 0.05). The expression recovered to normal level in two months (P > 0.05). After facial nerve transected, the reduction of bcl-2 expression was more significant when facial nerve transected close to facial nucleus than that far from facial nucleus (P < 0.05). Comparing to facial nerve transection in stylomastoid foramen, there was more intensive bcl-2 expression following facial nerve crush (P < 0.05). Further study showed that bcl-2 primarily resided in the nuclear envelop, endoplasmic reticulum and mitochondrial membrane. CONCLUSIONS: These data indicated that high level bcl-2 protein may prevent facial motoneuron death following facial nerve injury. It is suggested that overexpression bcl-2 by transgene may prevent facial motoneurons death.

[Maxillofacial vascular malformation associated with abnormal communication between external carotid and cranial arteries].

OBJECTIVE: To introduce the clinical characteristics of maxillofacial vascular malformation with abnormal communication between external carotid and cranial arteries. METHODS: One hundred and twenty patients with maxillofacial vascular malformations had been studied by arteriography of internal and external carotid, and vertebral arteries before embolization of tumor supplying artery. Cases found to have communications between extra- and intra-cranial arteries were analyzed. RESULTS: Fourteen patients (11.67%, 14/120) were found to have abnormal communications between external carotid and cranial arteries. Among them, 11 patients demonstrated communications between occipital and vertebral arteries, 1 patient showed ascending pharyngeal artery and vertebral artery communication, and 2 patients showed maxillary artery-ophthalmic artery communication. CONCLUSION: Embolization of tumor supplying artery is a safe and practical method for the treatment of maxillofacial vascular malformation when done under digital subtraction angiography and superselective catheterization to avoid the abnormal communicant branches.

Stabilization of bound polycyclic aromatic hydrocarbons by a pi-cation interaction.

Proteins can use aromatic side-chains to stabilize bound cationic ligands through cation-pi interactions. Here, we report the first example of the reciprocal process, termed pi-cation, in which a cationic protein side-chain stabilizes a neutral aromatic ligand. Site-directed mutagenesis revealed that an arginine side-chain located in the deep binding pocket of a monoclonal antibody (4D5) is essential for binding the neutral polynuclear aromatic hydrocarbon benzo[a]pyrene. This Arg was very likely selected for in the primary response, further underscoring the importance of the pi-cation interaction for ligand binding, which should be considered in protein analysis and design when ligands include aromatic groups.

Upper airway resistance syndrome and its treatment.

Although upper airway resistance syndrome (UARS) is being recognized by a growing number of specialists, its prevalence remains unknown. UARS is associated with nocturnal and daytime complaints and oro-naso-maxillo-mandibular signs. Spectrum analysis of the nocturnal sleep EEG from the central leads indicates significant differences in absolute power in the 12-14 Hz and the 7-9 Hz bands of UARS patients compared to controls. The 0.5-2.0 Hz band also appears to be involved when analyses performed on matched controls are compared to results obtained in subjects treated with nasal CPAP. Several treatment avenues--nasal CPAP, dental devices, surgical procedures, and radiofrequency thermal ablation--have been used for the treatment of UARS. The number of subjects treated have been too low and the protocols too limited to arrive at appropriate outcome recommendations, but many of the approaches have shown positive results, suggesting the possibility of several treatment avenues.

Cytochrome c deficiency causes embryonic lethality and attenuates stress-induced apoptosis.

Cytochrome c released from mitochondria has been proposed to be an essential component of an apoptotic pathway responsive to DNA damage and other forms of cell stress. Murine embryos devoid of cytochrome c die in utero by midgestation, but cell lines established from early cytochrome c null embryos are viable under conditions that compensate for defective oxidative phosphorylation. As compared to cell lines established from wild-type embryos, cells lacking cytochrome c show reduced caspase-3 activation and are resistant to the proapoptotic effects of UV irradiation, serum withdrawal, or staurosporine. In contrast, cells lacking cytochrome c demonstrate increased sensitivity to cell death signals triggered by TNFalpha. These results define the role of cytochrome c in different apoptotic signaling cascades.
