[Onset of rheumatoid arthritis in the elderly].

The predominant age of manifestation of rheumatoid arthritis (chronic polyarthritis) is between the 35th and 45th year of life. Onset of disease in the higher age groups is, however, not infrequent. Comparison of two groups of patients with rheumatoid arthritis with disease onset before or after the age of 60 showed that a quarter of late-onset cases commence particularly acutely. These was a remarkably early involvement of shoulder joints with a frequency of up to 50%. The symptom triad of shoulder joint involvement, markedly raised ESR and severe generalised disease was characteristic. This combination of symptoms is not infrequently attributed erroneously to the "shoulder-arm syndrome or suspect malignant tumour". Differential diagnosis from rheumatic polymyalgia or true paraneoplastic arthropathy may be difficult. Therapeutic problems arise mainly due to the high number of other medical problems of the elderly.

Glucose absorption by in vitro perfused ileum of the fetal rat.

In vitro intraluminal perfusion of the fetal rat ileum at 19 and 20 days of gestation was employed to measure the rate of glucose absorption and the unidirectional fluxes of 3-O-methylglucose (3-O-MG). Fetal ileum was mounted on pipettes within an oxygenated bath and perfused with a solution containing glucose or 3-O-MG and polyethylene glycol with average molecular weight of 4,000 (PEG) as a marker substance. We verified that the PEG was not transported and did not diffuse across the fetal ileum. Scanning and electron microscopy before and after perfusion demonstrated preservation of mucosal anatomy. The rate of glucose absorption was 200 +/- 17 mumol.h-1.g-1 at 19 days and increased significantly to 378 +/- 12 mumol.h-1.g-1 at 20 days. The absence of sodium abolished this process. The flux of 3-O-MG from lumen to bath was 88 +/- 10 mumol.h-1.g-1 at 19 days and 160 +/- 16 mumol.h-1.g-1 at 20 days. Bath-to-lumen flux was -17 +/- 2 mumol.h-1.g-1 and did not vary with age. Active, rapidly developing transport of glucose and 3-O-MG is demonstrated by in vitro luminal perfusion of the fetal rat ileum at 19 days.

HLA-B27, antinuclear antibodies and drug-induced agranulocytosis.

Eight female patients with drug-induced agranulocytosis (five patients with definite seropositive rheumatoid arthritis (RA), three patients with upper respiratory infections) were studied for the presence of HLA-B27 and antinuclear antibodies (ANA). Five of eight patients were found to be HLA-B27 positive and all RA patients had ANA in their serum. The frequency of HLA-B27 and ANA was found to be significantly different from control groups. It is concluded that the occurrence of HLA-B27 and ANA was found to be significantly different from control groups. It is concluded that the occurrence of HLA-B27 in female patients with seropositive RA (especially in those with ANA) and of HLA-B27 alone in other individuals could reflect an increased risk for drug-induced agranulocytosis.

[Rehabilitation of rheumatoid arthritis and ankylosing spondylitis: a study of the pre-requisites (author's transl)].

Rheumatoid arthritis/chronic polyarthritis and ankylosing spondylitis belong to the most common inflammatory rheumatic diseases. Owing to their progressive character they can result in morphological and functional defective conditions making rehabilitation procedures necessary. In contrast to other diseases calling for rehabilitation treatment the rehabilitation of these two diseases is in the majority of cases a life-long process, during which a change in rehabilitation goals and procedures is not seldom. Hence, rehabilitation is a dynamic process which requires flexible adjustment. A positive rehabilitation outcome is dependent upon the following factors: (1) the patient's disease (duration, stage, type of affection, progression, inflammatory and immunological activity, general condition and drug tolerance), (2) the patient's personality (age, intelligence, motivation), (3) the patient's home and vocational environments. In addition to technical pre-requisites, which are also important, the following medical ones have to be outlined: early diagnosis and initiation of therapy and the availability of a comprehensive treatment programme embracing drug, surgical, physical and balneological therapies, also as an in-patient. A further pre-requisite is the high rheumatological qualification of the responsible physician, who must maintain good contacts with the other members of the rehabilitation team. In the long run, this qualification can only be achieved with the provision of adequate training programmes, and if rheumatology is accepted as an independent discipline.
