Systolic and diastolic left ventricular and myocardial performance were analysed in ten patients with hypertrophic obstructive cardiomyopathy (HOCM) from biplane cineangiograms and simultaneous pressure recordings. In spite of significant geometrical abnormalities, pump function of the left ventricle on the average was normal. Only in two cases ejection fraction and isovolumic phase indices, respectively, were moderately reduced. On the contrary nine of ten patients had a considerable reduction of left ventricular distensibility, partly due to a decreased myocardial compliance. The symptoms of patients with HOCM cannot be explained by an impaired systolic pump function, they are primarily caused by a disturbed ventricular und myocardial function during diastole.
In 19 patients with healed myocardial infraction and normal right ventricular pressures, right and left ventricular volumes (biplane cineangio), ejection fractions and pressures were measured. Twelve of the patients had proximal severe stenosis of the right coronary artery (Group A), which was not diseased in seven patients (Group B). RV mean velocity of diameter shortening (VD) and LV mean velocity of circumferential fiber shortening Vcf) were calculated. Group A and B had similar RV systolic pressures (26.3 +/- 1.0 and 24.7 +/- 2.1 mm Hg) and LV hemodynamics (LVP: 127.5 +/- 5.9 and 122.1 +/- mm Hg, LVPED: 13.1 +/- 1.0 and 13.9 +/- 1.0 mm Hg, LVEF: 45.3 +/- 3.8 and 48.4 +/- 4.6%, Vcf: 0.768 +/- 0.114 and 0.861 +/- 0.123 circ/s). Only LVEDV was significantly higher in Group A (135.3 +/- 6.0 and 103.7 +/- 5.4 ml/m2). RVEDV (Group A: 131.4 +/- 4.5 and Group B: 87.9 +/- 4.8 ml/m2), RVEF (48.8 +/- 1.9 and 56.0 +/- 1.0%), RVPED (6.1 +/- 0.8 and 4.0 +/- 0.4 mm Hg) and VD (0.628 +/- 0.049 and 1.073 +/- 0.082 D/s) indicated significantly impaired RV function in Group A. It can be concluded that coronary artery disease of the right coronary causes significant functional impairment of the right ventricular myocardium.

The influence of contrast medium on systolic and diastolic left ventricular function was studied in eight patients with coronary heart disease and in five dogs during and five minutes after ventriculography. Two biplane ventriculograms were performed in 5-minute intervals, in the dogs an additional one after 10 minutes. 0.66 cc/kg BW of sodium methyl-glucamin-iothalamate were injected during diastole into the left ventricel. 1. During ventriculography there are only slight changes of left ventricular volumes, pressures and diastolic compliance. The first four opacified heart cycles can be evaluated for the analysis of left ventricular function. 2. In patients with coronary heart disease there are much more pronounced alterations of systolic ventricular function 5 minutes after Ventriculography than in the normal dog. The ischemic ventricle has a reduced tolerance for volume load and negative inotropic effect of contrast media. In patients and dogs diastolic compliance is significantly increased 5 minutes after the injection of contrast medium. After 10 minutes however in the dogs compliance is significantly decreased. Analysing left ventricular function by repeated ventriculographies one has to take into account that the function is altered by the first angiogram. The degree of the changes depends on the initial hemodynamic conditions and on the time interval between the ventriculograms.
The effects of nitroglycerin on left ventricular hemodynamics were determined in ten patients from biplane cineventriculograms and simultaneous pressure measurements. Before and after nitroglycerin (1.6 mg sublingually) the following parameters were measured: Systolic (PLV) and enddiastolic pressure (PLVED), enddiastolic (EDV) und endsystolic volume (ESV), cardiac output (SV), ejection fraction (EF) and ejection rate (ER), ejection time (tej), heart rate (HR), dp/dtmax, dp/dtmax Pi-Ped, maximal systolic wall stress (sigmamax), integrated systolic wall stress (sigma), outflow resistance (Rsys) and ventricular compliance (a). Nitroglycerin leads to a reduction of both, pre- and afterload, with a significant fall of PLV (10.8%), PLVED (21.8%), EDV (9.1%) ESV (13.3%) AND Rsys (14.4%). The changes of ventricular pressure and geometry result in a diminution of left ventricular wall stress (sigmamax- 11.8%, sigma - 26.4%), representing a decrease of calculated total oxygen-consumption of 11.5%. In six patients with reduced left ventricular diastolic compliance nitroglycerin leads to a remarkable increase of the compliance. This is probably due to the reversal of a latent ischemia by the decreased myocardial oxygen-consumption.
The results of intra-aortic balloon pumping in 7 patients with cardiogenic shock after cardiac surgery are reported. 2 patients were unresponsive to IABP, 4 patients showed initial stabilization of the circulation, but became IABP-dependent and died. One patient could be weaned from IABP, but died of uncontrollable cardiac arrhythmias. These results are discussed.
Left ventricular function in 8 patients with coronary heart disease, who died during or immediately after surgery, was compared with the corresponding data of 25 survivors. Both groups were similar in age and had about the same number of myocardial infarctions preoperatively. For LVEDP, EDV, EF and VCF critical values could be defined, which were found in none of the survivors. These values were: LVDEP approximately 20 mm Hg, EDV approximately 150 ml/m2, EF less than 30% and VCF less than 0,4 circ/s. The least prognostic significance was found for LVEDP; EDV and VCF correlated best with the surgical risk.
