Independent nonframeshift deletions in the MC1R gene are not associated with melanistic coat coloration in three mustelid lineages.

Sequence variation within the 5' flanking (about 240 bp) and exon regions (426 bp) of the melanocortin-1 receptor (MC1R) gene was examined to determine the potential role of this protein in the melanistic coat coloration of 17 mustelid species in four genera: Gulo (wolverines), Martes (martens), Mustela (weasels), and Meles (badgers). Members of the genera Mustela and Meles, together with Martes flavigula and Martes pennanti, were shown to have intact gene sequences. However, several "in frame" deletions of the MC1R gene region implicated in melanism of other species were detected within members of the genera Martes and Gulo. For instance, Gulo gulo possessed a 15 bp deletion in the second transmembrane domain coding region, while Martes americana, Martes melampus, Martes zibellina, and Martes martes shared a 45 bp deletion overlapping this area. In addition, Martes foina was found to possess a 10 bp insertion followed closely by a 28 bp deletion immediately downstream of the deletion found in other martens. Notably, none of these indels was associated with a melanistic phenotype. Phylogenetic analysis revealed that each of these nonrandomly distributed deletions arose independently during the evolution of this family. Specific indel-neighboring motifs appear to largely account for the biased and repeated occurrence of deletion events in the Martes/Gulo clade.

Rubber band ligation versus excisional haemorrhoidectomy for haemorrhoids.

BACKGROUND: Traditional treatment methods for haemorrhoids fall into two broad groups: less invasive techniques including rubber band ligation (RBL), which tend to produce minimal pain, and the more radical techniques like excisional haemorrhoidectomy (EH), which are inherently more painful. For decades, innovations in the field of haemorrhoidal treatment have centred on modifying the traditional methods to achieve a minimally invasive, less painful procedure and yet with a more sustainable result. The availability of newer techniques has reopened debate on the roles of traditional treatment options for haemorrhoids. OBJECTIVES: To review the efficacy and safety of the two most popular conventional methods of haemorrhoidal treatment, rubber band ligation and excisional haemorrhoidectomy. SEARCH STRATEGY: We searched all the major electronic databases (MEDLINE, EMBASE, CENTRAL, CINAHL). SELECTION CRITERIA: Randomised controlled trials comparing rubber band ligation with excisional haemorrhoidectomy for symptomatic haemorrhoids in adult human patients were included. DATA COLLECTION AND ANALYSIS: We extracted data on to previously designed data extraction sheet. Dichtomous data were presented as relative risk and 95% confidence intervals, and continuous outcomes as weighted mean difference and 95% confidence intervals. MAIN RESULTS: Three trials (of poor methodological quality) met the inclusion criteria. Complete remission of haemorrhoidal symptom was better with EH (three studies, 202 patients, RR 1.68, 95% CI 1.00 to 2.83). There was significant heterogeneity between the studies (I2 = 90.5%; P = 0.0001). Similar analysis based on the grading of haemorrhoids revealed the superiority of EH over RBL for grade III haemorrhoids (prolapse that needs manual reduction) (two trials, 116 patients, RR 1.23, CI 1.04 to 1.45; P = 0.01). However, no significant difference was noticed in grade II haemorrhoids (prolapse that reduces spontaneously on cessation of straining) (one trial, 32 patients, RR 1.07, CI 0.94 to 1.21; P = 0.32) Fewer patients required re-treatment after EH (three trials, RR 0.20 CI 0.09 to 0.40; P < 0.00001). Patients undergoing EH were at significantly higher risk of postoperative pain (three trials, fixed effect; 212 patients, RR 1.94, 95% CI 1.62 to 2.33, P < 0.00001). The overall delayed complication rate showed significant difference (P = 0.03) (three trials, 204 patients, RR 6.32, CI 1.15 to 34.89) between the two interventions. AUTHORS' CONCLUSIONS: The present systematic review confirms the long-term efficacy of EH, at least for grade III haemorrhoids, compared to the less invasive technique of RBL but at the expense of increased pain, higher complications and more time off work. However, despite these disadvantages of EH, patient satisfaction and patient's acceptance of the treatment modalities seems to be similar following both the techniques implying patient's preference for complete long-term cure of symptoms and possibly less concern for minor complications. So, RBL can be adopted as the choice of treatment for grade II haemorrhoids with similar results but with out the side effects of EH while reserving EH for grade III haemorrhoids or recurrence after RBL. More robust study is required to make definitive conclusions.

Reproducibility of a commercial in vitro allergen-specific assay for immunoglobulin E in dogs.

Samples of serum taken from 42 dogs with clinical signs and histories indicating pruritic skin disease and/or diseases of the ear were tested in a commercial allergen-specific assay for immunoglobulin E. Dogs which had been treated with glucocorticoids and/or antihistamines were not excluded. The samples were separated into two equal aliquots, given different randomised numbers, and analysed in two batches on two separate days. The laboratory was blinded to the identification numbers and the history of each dog, but knew the purpose of the study. The results for 48 allergens were expressed in modified absorbance units (MAU). The overall median MAU was 29. For each allergen the mean difference between the MAU values of the paired duplicates was determined and the difference was compared to zero by a paired t test. The number of means that were not 0 (P<0.05) in each allergen group were: seven of 10 grasses, seven of nine weeds, two of 13 trees, six of 10 fungi, and three of six environmental allergens. A single 2 x 2 table for the 48 allergens was created with MAU > or = 60 defined as 'positive' and < 60 as 'negative'. There were 116 of 188 (62 per cent) pairs that were reproducibly 'positive' and 1756 of 1828 (96 per cent) pairs that were reproducibly 'negative'.

Postdefaecation pain syndrome after circular stapled anopexy is abolished by oral nifedipine.

BACKGROUND: Controversy has surrounded the technique of circular stapled anopexy since an isolated report of a high incidence of persistent postdefaecation pain following the procedure. The characteristics, clinical course and management of this complication have not been described. METHODS: Within an ongoing multicentre randomized clinical trial comparing circular stapled anopexy with closed haemorrhoidectomy, 77 patients underwent circular stapled anopexy. Follow-up was at 6, 12, 24 and 48 weeks. Patients underwent transanal ultrasonography, anal electrosensitivity testing and manometry. RESULTS: Of the 77 patients who had circular stapled anopexy, three men reported new-onset postdefaecation pain that compromised lifestyle, including ability to return to work. All three had sphincter hypertonicity on digital and manometric examination but were refractory to topical 0.2 per cent glyceryl trinitrate ointment. The addition of oral nifedipine 20 mg twice daily did not alter anal sphincter pressures but rapidly abolished symptoms and restored quality of life. CONCLUSION: Postdefaecation pain is a specific complication of circular stapled anopexy, affecting a small percentage of patients. Men with a high anal sphincter pressure appear to be at risk. Although the exact aetiology remains unclear, it is likely that rectal rather than anal sphincter muscle is affected. Oral nifedipine represents an effective therapy.

Systematic review of randomized trials comparing rubber band ligation with excisional haemorrhoidectomy.

BACKGROUND AND METHOD: This review compares the two most popular treatments for haemorrhoids, namely rubber band ligation (RBL) and excisional haemorrhoidectomy. Randomized trials were identified from the major electronic databases. Symptom control, retreatment, postoperative pain, complications, time off work and patient satisfaction were assessed. Relative risk (RR) and weighted mean difference with 95 per cent confidence interval (c.i.) were estimated using a random-effects model for dichotomous and continuous outcomes respectively. RESULTS: Three trials met the inclusion criteria and all were of poor methodological quality. Complete remission of haemorrhoidal symptoms was better after haemorrhoidectomy (RR 1.68 (95 per cent c.i 1.00 to 2.83)). There was significant heterogeneity between the studies (I(2) = 90.5 per cent; P < 0.001). Fewer patients required retreatment after haemorrhoidectomy (RR 0.20 (95 per cent c.i 0.09 to 0.40)), but anal stenosis, postoperative haemorrhage and incontinence to flatus were more common with this operation. CONCLUSIONS: Haemorrhoidectomy produced better long-term symptom control in patients with grade III haemorrhoids, but was associated with more postoperative complications than RBL.

Bioenergetics and thermal physiology of American water shrews (Sorex palustris).

Rates of O(2) consumption and CO(2) production, telemetered body temperature (T(b)) and activity level were recorded from adult and subadult water shrews (Sorex palustris) over an air temperature (T(a)) range of 3-32 degrees C. Digesta passage rate trials were conducted before metabolic testing to estimate the minimum fasting time required for water shrews to achieve a postabsorptive state. Of the 228 metabolic trials conducted on 15 water shrews, 146 (64%) were discarded because the criteria for inactivity were not met. Abdominal T(b) of S. palustris was independent of T(a) and averaged 38.64 +/- 0.07 degrees C. The thermoneutral zone extended from 21.2 degrees C to at least 32 degrees C. Our estimate of the basal metabolic rate for resting, postabsorptive water shrews (96.88 +/- 2.93 J g(-1) h(-1) or 4.84 +/- 0.14 ml O(2) g(-1) h(-1)) was three times the mass-predicted value, while their minimum thermal conductance in air (0.282 +/- 0.013 ml O(2) g(-1) h(-1)) concurred with allometric predictions. The mean digesta throughput time of water shrews fed mealworms (Tenebrio molitor) or ground meat was 50-55 min. The digestibility coefficients for metabolizable energy (ME) of water shrews fed stickleback minnows (Culaea inconstans) and dragonfly nymphs (Anax spp. and Libellula spp.) were 85.4 +/- 1.3% and 82.8 +/- 1.1%, respectively. The average metabolic rate (AMR) calculated from the gas exchange of six water shrews at 19-22 degrees C (208.0 +/- 17.0 J g(-1) h(-1)) was nearly identical to the estimate of energy intake (202.9 +/- 12.9 J g(-1) h(-1)) measured for these same animals during digestibility trials (20 degrees C). Based on 24-h activity trials and our derived ME coefficients, the minimum daily energy requirement of an adult (14.4 g) water shrew at T(a) = 20 degrees C is 54.0 kJ, or the energetic equivalent of 14.7 stickleback minnows.

Anastomotic recurrence of rectal adenoma after anterior resection.

Transanal local excision of posterior benign rectal tumors is usually safe. Here, we report a case of transanal excision of a posterior anastomotic recurrence of a rectal adenoma after a stapled anterior resection that resulted in perforation into the peritoneal cavity.

Variation in ovarian morphology in four species of New World moles with a peniform clitoris.

Female moles of the Old World genus Talpa display a curious suite of reproductive features that include a peniform clitoris and ovaries with a discrete interstitial gland or testis-like region (so-called 'ovotestes'). The masculinization of the female external genitalia in Talpa has accordingly been linked with secretion of androgens from the interstitial gland region of the fetal gonad. Although their ovarian morphology has received less attention, some species of New World moles also have ovaries with a pronounced interstitial gland (for example star-nosed mole, Condylura cristata), whereas females of other species do not (for example eastern mole, Scalopus aquaticus). Although it is difficult to determine the sex of both Old and New World moles, published accounts describing the external genitalia of female moles are available only for Talpa. The hypothesis that masculinization of the female external genitalia in moles is associated with the presence of an ovarian interstitial gland (OIG) was tested in the present study by using a comparative approach to determine whether these features are ever found in isolation of one another. Three genera of North American moles (Scapanus, Condylura and Neurotrichus) were studied and a peniform clitoris was found in all three species, but OIG were found in only two of three genera. The ovaries of S. latimanus and S. orarius were unremarkable, with no evidence of a discrete interstitial gland or testis-like region. Mapping these results onto recent talpid phylogenies indicates that loss of the bipolar ovarian morphology is a derived trait in Scapanus, and conclusively demonstrates that masculinization of the external genitalia in female moles can develop in the presence or absence of 'ovotestes'.

Nonenteric Escherichia coli isolates from dogs: 674 cases (1990-1998).

OBJECTIVE: To determine nonenteric sites associated with Escherichia coli isolates in dogs and the antimicrobial susceptibilities of the isolates. DESIGN: Retrospective study. SAMPLE POPULATION: 17,000 canine specimens. PROCEDURE: Medical records of 17,000 canine specimens submitted for bacteriologic culture were examined and the number of isolations of E coli was determined. For these cases, records were further examined with respect to body system involvement, sex, concurrent infection with other species of bacteria, and antimicrobial susceptibility. RESULTS: 674 E coli isolates (424 from urine, 62 from the skin, 52 from the respiratory tract, 45 from the ear, 43 from the female reproductive tract, 25 from the male reproductive tract, and 23 from other organ systems) were identified. There was a significantly higher proportion of isolates from urine specimens from spayed females than from sexually intact females or males. Escherichia coli was isolated in pure culture from 65.9% of the specimens. Most E coli isolates were susceptible to norfloxacin (90%), enrofloxacin (87.5%), gentamicin (90.7%), and amikacin (85.9%). CONCLUSIONS AND CLINICAL RELEVANCE: Most nonenteric E coli infections in dogs involve the urinary tract. Amikacin, gentamicin, norfloxacin, and enrofloxacin have the highest efficacy against canine E coli isolates. For E coli isolates from dogs, in vitro susceptibility to commonly used antimicrobial agents has remained fairly stable during the past decade.

Uses and indications for video-otoscopy in small animal practice.

Acute and chronic otitis externa and otitis media are common disorders in dogs and cats. In combination with other diagnostic and therapeutic procedures, the video-otoscope is a useful and effective tool in the management of clinical cases. The enhanced illumination and magnification provide the practitioner with more detailed information for diagnosis and prognosis, and the configuration of the working channel facilitates sampling, improves efficacy of cleaning procedures, and decreases risks of iatrogenic injury to structures of the middle and inner ear. Photographic documentation of clinical cases enhances the medical record, communication with colleagues, and client education. Although video-otoscopy facilitates diagnosis and therapy, it does not replace other important diagnostic tests such as evaluation for atopy, adverse food reactions, and immune-compromising disease. Failure to identify the underlying primary cause usually results in treatment failure regardless of the technology employed.

Postprandial heat increment does not substitute for active thermogenesis in cold-challenged star-nosed moles (Condylura cristata).

The postprandial increase in metabolic rate associated with consuming, assimilating and excreting a meal is often termed the heat increment of feeding (HIF). The metabolic heat production of star-nosed moles, Condylura cristata, held at thermoneutrality was monitored for 4 h following a single 10 min session of feeding on a ration consisting of 0 g (controls), 3.5 g or 10 g of earthworms. Coefficients for metabolizable energy digestibility and digesta passage rate of earthworms fed to C. cristata were also determined. We then tested whether feeding-induced thermogenesis substitutes partially or completely for thermoregulatory heat production in these animals exposed to sub-thermoneutral air temperatures (9-24 degrees C). A single feeding on earthworms had both short- and long-term effects on the metabolic rate and respiratory exchange ratio of C. cristata. The observed short-term (0-65 min) rise in metabolic rate, assumed to be associated primarily with the physical costs of nutrient digestion, absorption and excretion, was similar to the calculated mean retention time (66.7+/-7.8 min; mean +/- s.e. m., N=5) of this species. This component of the HIF represented 2.9 % of the food energy ingested by moles fed a single 3.5 g (13.21 kJ) meal of earthworms and 1.4 % of the food energy ingested by moles fed a single 7.5 g (28.09 kJ) meal of earthworms. At all test temperatures, resting metabolic rate typically remained above fasting levels for 1-4 h following ingestion of the high-protein earthworm diet. This protracted rise in metabolic rate, presumably associated with the biochemical costs of amino acid oxidation/gluconeogenesis and ureagenesis, averaged 12.8 % of the metabolizable energy and 8.7 % of the gross energy intake. Despite the potential thermoregulatory benefit, we found no evidence that biochemical HIF substitutes for facultative thermogenesis in star-nosed moles exposed to low air temperatures.

Implicit power motivation moderates men's testosterone responses to imagined and real dominance success.

This study tested the hypothesis that implicit power motivation moderates individuals' testosterone responses to the anticipated success in and actual outcome of a dominance contest. Salivary testosterone levels were assessed in 42 male students at the beginning of the study, after they had imagined a success in an ensuing power contest, and immediately after the contest had taken place. Contest outcome (winning or losing against a competitor on a speed-based task) was varied experimentally. Participants' power motive was assessed with a picture-story exercise, in which an assertive, personalized (p Power) component was distinguished from an altruistic, socialized (s Power) component. In contrast to all other participants, individuals high only in p Power (a) had elevated testosterone after imagining a success in a subsequent dominance contest and (b) continued to have high testosterone levels after actually winning, but not after losing, the contest.

Fasting metabolism and thermoregulatory competence of the star-nosed mole, Condylura cristata (Talpidae: Condylurinae).

Metabolic and body temperature (Tb) responses of star-nosed moles (Condylura cristata) exposed to air temperatures ranging from 0 to 33 degrees C were investigated. The thermoneutral zone of this semi-aquatic mole extended from 24.5 to 33 degrees C, over which its basal rate of metabolism averaged 2.25 ml O2 g-1 h-1 (45.16 J g-1 h-1). This rate of metabolism is higher than predicted for terrestrial forms, and substantially higher than for other moles examined to date. Minimum thermal conductance was nearly identical to that predicted for similar-sized eutherians and may represent a compromise between the need to dissipate heat while digging and foraging in subterranean burrows, and the need to conserve heat and avoid hypothermia during exposure to cold. C. cristata precisely regulated Tb (mean +/- SE = 37.7 +/- 0.05 degrees C) over the entire range of test temperatures. Over three separate 24-h periods, Tb of a radio-implanted mole varied from 36.6 to 38.8 degrees C, and generally tracked level of activity. No obvious circadian variation in Tb and activity was apparent, although cyclic 2-4 h intervals of activity punctuated by periods of inactivity lasting 3-5 h were routinely observed. We suggest that the elevated basal metabolic rate and relatively high Tb of star-nosed moles may reflect the semi-aquatic habits of this unique talpid.

Methicillin resistance among staphylococci isolated from dogs.

OBJECTIVE: To determine whether methicillin-resistant staphylococci from dogs expressed the mecA gene and to determine what proportion of canine staphylococcal isolates positive for the mecA gene were resistant to oxacillin and other antibiotics. SAMPLE POPULATION: 25 methicillin-resistant (10 coagulase-positive and 15 coagulase-negative) and 15 methicillin-susceptible (8 coagulase-positive and 7 coagulase-negative) staphylococci isolated from dogs. PROCEDURE: All strains were tested for methicillin resistance by use of oxacillin agar screening and identified by use of standard techniques. Minimum inhibitory concentrations of 16 antibiotics were determined for all 40 isolates. A polymerase chain reaction method targeting a 533-basepair fragment of the mecA gene was used to detect mecA gene expression. RESULTS: 23 of the 25 methicillin-resistant isolates and none of the methicillin-susceptible isolates possessed the mecA gene. For 10 of 16 antibiotics, the proportion of mecA-positive isolates that were resistant or of intermediate susceptibility was significantly higher than the proportion of mecA-negative isolates that were resistant or of intermediate susceptibility. Only 1 methicillin-resistant coagulase-positive isolate was identified as Staphylococcus intermedius; the other 9 were identified as S. aureus. CONCLUSIONS AND CLINICAL RELEVANCE: Results confirm that staphylococci isolated from dogs may have methicillin resistance mediated by the mecA gene. Isolates positive for the mecA gene were more likely to be resistant to various antibiotics than were isolates negative for the mecA gene. Results suggest that in dogs, infections caused by staphylococci that have the mecA gene may be difficult to treat because of resistance to antibiotics.

Hemodynamic effects of thyroidectomy in sedentary horses.

OBJECTIVE: To investigate hemodynamic effects of thyroidectomy in horses at rest. ANIMALS: 6 healthy aged Quarter Horse mares. PROCEDURE: Horses were monitored for 5 months before and 4 weeks after thyroidectomy and for an additional 4 weeks after administration of thyroid hormone supplement (2.5 microg of thyroxine/kg of body weight, PO, q 12 h, and 0.6 microg of triiodothyronine/kg, PO, q 12 h). Responses to thyroid-stimulating hormone (TSH) were measured before and 4 weeks after thyroidectomy. Other variables monitored daily were resting rectal temperature (T), heart rate (HR), respiratory rate (RR), and body weight (BW). Monthly cardiac output (Q), blood volume (BV), plasma volume (PV), standard electrocardiographic measures, systolic and right ventricular blood pressure, and HR responses were determined after IV administration of isoproterenol and phenylephrine. Variables were analyzed by use of repeated-measures ANOVA. RESULTS: Complete thyroidectomy was confirmed by minimal response to TSH 4 weeks after surgery. Resting HR, RR, T, Q, and beta-adrenergic responsiveness to isoproterenol decreased significantly after thyroidectomy. Resting T, Q, and beta-adrenergic responsiveness increased after administration of supplement and was not significantly different from euthyroid values. Blood volume and PV increased significantly after thyroidectomy but did not return to euthyroid values despite administration of supplement. Response to phenylephrine was minimally different between treatments. CONCLUSIONS AND CLINICAL RELEVANCE: Thyroidectomy in horses caused decreased resting HR, RR, T, Q, and isoproterenol responsiveness and increased BV, PV, PQ interval, and QT interval corrected for HR. Some of these surgically induced changes appeared to be partially reversed by administration of thyroid hormone supplement.

Cytologic examination of exfoliative specimens obtained during endoscopy for diagnosis of gastrointestinal tract disease in dogs and cats.

OBJECTIVE: To determine whether cytologic examination of exfoliative specimens obtained during endoscopy was as useful as histologic examination of mucosal biopsy specimens for the diagnosis of gastrointestinal tract disease in dogs and cats and to compare the diagnostic accuracy of 2 techniques (brush or touch) in preparing specimens for cytologic examination. DESIGN: Prospective case series. ANIMALS: 85 dogs and 23 cats. PROCEDURE: Specimens for cytologic and histologic examination were obtained during routine endoscopic examination of the stomach, small intestine, and colon. A diagnosis was made on the basis of cytologic findings (graded objectively) and compared with the diagnosis on the basis of histologic findings. RESULTS: The diagnostic accuracy of cytologic examination was high for all 3 organs. Sensitivities, specificities, and predictive values of positive and negative results were > 90% in most instances. The diagnostic accuracy of the brush technique was equal or superior to that of the touch technique for 84% of specimens. The brush technique was most useful in detecting cellular infiltrates in the lamina propria, whereas the touch technique was more likely to detect acute mucosal inflammation. Percentages of false-positive (3.2%) and false-negative (6.9%) cytologic interpretations were low. CLINICAL IMPLICATIONS: Endoscopy is safe and requires little time to procure specimens for cytologic examination, which can be obtained concurrently with mucosal biopsy specimens. Cytologic examination of exfoliative specimens obtained during endoscopy is a useful and reliable adjunct to histologic examination of biopsy specimens in the diagnosis of gastrointestinal tract disease in dogs and cats.
