Embolism from intracranial aneurysms.

Embolism from an aneurysm is one of the mechanisms involved in the pathogenesis of ischemic symptoms associated with intracranial aneurysms. Four cases are reported in which aneurysms of the internal carotid arteries and middle cerebral arteries were the source of emboli resulting in cerebral infarction. In the treatment of these aneurysms, it is best to clip the neck of the aneurysm with great care to avoid embolism due to extrusion of clot into the distal artery.

[A histopathological study of the brain of patients who died of cerebral vasospasm after rupture of cerebral aneurysm (author's transl)].

It is said that cerebral angiospasm affects very seriously the prognosis of the patient with ruptured aneurysm. Of 44 patients with ruptured aneurysm who died from cerebral ischemia due to cerebral angiospasm, we performed histopathological studies of brains of 18 patients without iatrogenic factors (radical surgical operation, prolonged artificial breathing and so on). In all of the cases, cerebral infarctions were observed in the area fed by the artery with angiospasm. The size and extent of the cerebral infarction were various. In 12 cases who died in the acute stage (within 3 weeks after subarachnoid hemorrhage), those who survived longer showed more severe and wide cerebral infarction, while those who died earlier showed more localized infarction in the basal ganglia and thalamus. In 6 cases who died in the chronic stage (more than 3 weeks after subarachnoid hemorrhage), the cerebral infarction was slight. No ischemic changes were observed in the brain stem, except for secondary petechial hemorrhage due to incisural herniation in cases with extensive cerebral infarction.

Neuroradiologic evaluation of the subdural pathogenesis in infants with small heads.

A marginal low density area is often revealed by CT scan in infants with microcephalia. However, it is difficult to assess precisely the main pathologic state of the marginal low density area and also whether such a lesion exists in the subdural-epiarachnoid space or in the subarachnoid space. A carotid angiographic evaluation of cortical vessels in the marginal avascular area was made and suggestive results were obtained. When there is acute subdural effusion or subdural hematoma, cortical arteries in the marginal avascular area will have a straightened and attenuated figure. But when such a pathologic state persists for a long period, it induces secondary cortical atrophy and the subarachnoid space becomes enlarged. The cortical artery appears as if it is flying in the enlarged subarachnoid space because it is detached from the gyrus and the sulcus following the cortical atrophy. This is why we named it the 'flying artery.' Angiographic findings allow more precise interpretation of the marginal low density area in the CT scan.

[Effects of ATP on cerebral circulation--morphological observation of cerebral circulation and cerebral vasculature].

Review was made on the effect of adenosine triphosphate, 1.0-3.0 mg/kg intravenously, on the cerebral circulation and the presence of the dilating action on the cerebral blood-vessels in cats. 1. The group of the normal intra-cranial pressure showed, as reported previously, the transient hypotension and its complicated decrease of cerebral blood-circulation at the initial period for 30-100 check, then demonstrating the increase of cerebral blood-flow for 200-1000 check. Its increased volume and duration were dose-dependent. 2. The group of the loaded intracranial pressure of 20-30 mmHg (slightly elevated group) showed the increase of cerebral blood-flow, like the group of the normal intracranial pressure. However, the effect of ATP showed the decrease or disappearance in accordance with the elevation of the loaded pressure. 3. Cerebral vasodilating effect of ATP was promptly observed after the administration, and its degree was 30-50% in photographical observation. 4. Increasing effect on cerebral blood-flow and cerebral vasodilating action of ATP were similar in the pattern to that of vasodilators such as papaverine hydrochloride.
