Fluoride profiles in different sites of approximal surfaces of second primary molars after topical application of acidulated phosphate fluoride gel in vivo.

Thirty-four (17 paired) extracted second primary molars were obtained from 17 individuals (9 boys and 8 girls) aged from 9 yr 2 months to 12 yr 7 months. A tooth on one side was extracted as a control, and an acidulated phosphate fluoride (APF) gel was then applied to the paired contralateral second primary molar. Three months later, the experimental tooth was extracted. Nine sites were assayed by a microsampling technique from small areas of the approximal enamel surface. The fluoride and phosphorus concentrations were determined by a fluoride electrode and by colorimetric procedure, respectively. Fluoride concentrations were higher in the teeth treated with the APF gel than in the control teeth. The highest fluoride uptake was observed in the central area of the approximal surfaces. Deeper areas (> 10 microns) had a marked uptake of fluoride as compared with surface areas (< 3 microns). It was concluded that the APF gel application increased the fluoride levels of approximal tooth surfaces, particularly the mid-central site, of second primary molars, even at 3 months after application.

Immunoglobulin gene analysis of cutaneous pseudolymphoma by polymerase chain reaction.

Polymerase chain reaction (PCR) was used to amplify the V-D-J region of the immunoglobulin heavy chain gene from DNA extracted from the formalin fixed, paraffin embedded skin of 3 cases of pseudolymphoma. The products were electrophoresed and observed under ultraviolet light after ethidium bromide staining. Specimens of two cases showed smears of polyclonal amplified DNA. The specimen of one case (Case 3), however, showed a single band with a smear. The presence of monoclonality in B lymphocyte populations may suggest the possibility of low grade malignancy of pseudolymphoma or transformation to malignant lymphoma in the future.

T cell-rich B cell lymphoma bearing Epstein-Barr virus in tumor cells: a case of IBL-T-like lesion following Lennert's lesion.

A case of T cell-rich B cell lymphoma (TCRBCL) with Epstein-Barr virus (EBV) infection in tumor cells is reported. A 50 year old male developed right cervical lymph node swelling in July 1988. Initial biopsy in April 1989 demonstrated many scattered Hodgkinoid atypical cells with Lennert's lesion. After partial remission following chemotherapy, the lymph nodes enlarged again, and a second biopsy in February 1991 showed an IBL-T-like lesion. Only a small number of Hodgkinoid atypical cells were still observed. After apparently, complete remission, the lesion soon recurred and the patient died in November 1992. Immunohistochemically the Hodgkinoid cells were positive for L26, but negative for LN2, LN3, UCHL-1, MT1, lysozyme, Ber-H2 and Leu-M1. Reactivity for immunoglobulins showed false-positive because of polyclonal staining. IgH monoclonality was detected by the polymerase chain reaction method in the first biopsied specimen, and by Southern blotting in the second biopsied snap-frozen specimen. Monoclonal TCR beta rearrangement was not detected. The Hodgkinoid atypical cells were positive for EBV-encoding RNA by in situ hybridization, and LMP-1 by immunostaining. Occasionally, EBV-bearing immunoblastic, medium sized, or small lymphocytic cells were also observed. This case indicates the possibility that EBV is related to the pathogenesis of TCRBCL.

Posterior bite raising effects on a primary anterior crossbite case.

Stainless steel crowns were cemented on the mandibular primary molars to make bite raising for an anterior reversed bite case. Chewing force and MKG were recorded before and after bite raising. Data from MKG revealed that co-ordination pattern of muscle activities after bite raising became higher in masseter muscles than those before anterior reversed bite was corrected easily without any incisal interferences by using this treatment method.

The sedative effects of intranasal midazolam administration in the dental treatment of patients with mental disabilities. Part 2: optimal concentration of intranasal midazolam.

In a previous paper, we reported on the effect of a 0.2 mg/kg dose of midazolam, administered intranasally, prior to performing various restorative dental procedures on a group of mentally disabled patients under local anesthesia and nitrous oxide/oxygen analgesia. The purpose of this study was to compare the clinical and possible adverse effect of doses of 0.2 mg/kg and 0.3 mg/kg midazolam, administered intranasally, and to determine the most appropriate concentration for the drug when administered by this route. Patients were assessed by a behavioral test which consisted of a scale from 1-7 with 3 ranges: markedly effective (1-3), effective (4-5) or ineffective (6-7). Forty-three mentally handicapped patients, aged 5 to 20 years, all of whom had previously exhibited highly combative and resistant behavior toward dental treatment under local anesthesia, were stratified by age and randomly assigned in a double blind manner to two groups, receiving either 0.2 mg/kg or 0.3 mg/kg midazolam administered intranasally. Group 1, consisting of 22 patients, average age 11 years 8 months, received 0.2 mg/kg. Group 2 consisted of 21 patients, average age 13 years 8 months, each of whom was administered 0.3 mg/kg intranasal midazolam. Only patients assessed as ASA anesthesia status I or II were admitted to the study. Subsequent to intranasal administration of midazolam, no patient rejected the nasal mask nor refused to inhale nitrous oxide/oxygen. The induction of nitrous oxide/oxygen sedation and oral examination were effected smoothly in every case in the two groups.(ABSTRACT TRUNCATED AT 250 WORDS)

[Peripheral T cell lymphoma (immunoblastic type, HTLV-1 negative) associated with aganglionosis of the intestine].

A 56-year-old female was hospitalized in 1992, because of fever and lymphadenopathy. She had ileus due to aganglionosis of the intestine diagnosed in 1990. Blood tests disclosed anemia and thrombocytopenia. Peripheral blood smear demonstrated 69.5% abnormal cells and a T-cell origin was demonstrated on cell marker study by flow cytometry. Immunological examinations, including serum HTLV-1 antibody were negative. Lymph node biopsy revealed malignant lymphoma; diffuse large, immunoblastic. The clonality of the T-cell malignancy was shown by detecting rearrangement of T-cell receptor beta gene. A diagnosis of PTCL, high grade, immunoblastic, was made. The patient received various chemotherapeutic agents and achieved a short remission, then died 5 months after the diagnosis. The classification of T-cell lymphoma was established by Suchi and the Kiel classification. Many ATL/L cases have been reported in Japan before the above classification was affirmed, although there have been few reports of ATL/L-like diseases in which serum HTLV-1 antibodies were negative. The case reported here was HTLV-1 negative, although the patient was diagnosed as ATL/L-like disease, because the results of various laboratory examinations were similar to these of HTLV-1 positive cases. Characteristics of this case included a female patient and leukemic clinical features, because the gender preponderance of PTCL, high grade, immunoblastic is male and the clinical features of the disease are not usually leukemic. It was also characteristic that PTCL coexisted with aganglionosis of the intestine. Basic disturbances in the latter condition would be similar to aganglionic megacolon in which a possible aberrant immune mechanism was shown.(ABSTRACT TRUNCATED AT 250 WORDS)

The sedative effect of intranasal midazolam administration in the dental treatment of patients with mental disabilities. Part 1. The effect of a 0.2 mg/kg dose.

The purpose of this study was to determine the sedative effect of a 0.2 mg/kg dose of midazolam, administered intranasally, prior to performing various restorative dental procedures on a group of mentally disabled patients under local anesthesia and nitrous oxide/oxygen analgesia. Twenty-one patients, aged 4 to 21 years, all of whom had previously exhibited highly combative and resistant behavior toward dental treatment under local anesthesia, were sedated with 0.2 mg/kg midazolam. Only patients assessed as ASA anesthesia status I or II were admitted to the study. After administering the midazolam, each patient was allowed to rest before initiating the dental procedures. Behavioral patterns during the various procedures were rated on a behavioral rating scale of 1-7. Each patient served as his or her own control, comparing behavior with or without intranasal midazolam. The results showed a marked improvement in behavioral patterns after administration of intranasal midazolam. Ratings on a scale of 1-7 were noted as "markedly effective" and "effective" for 69.2% of those patients who received infiltration injection anesthesia, 93.8% under rubber dam, 76.2% during cavity preparation, 84.2% for restoration placement and 87.5% during pulpotomy procedures. The majority of patients were discharged within 150 minutes of intranasal instillation. Further studies are indicated to ascertain the most appropriate dose of intranasally administered midazolam.

Distribution of fluoride across human primary enamel.

The distribution of fluoride was described in detail from the incisal to the cervical region in 10 pairs of primary lower central incisors. Fluoride analyses were done on the right incisors; optical microscopic studies were done on the left. The distribution data obtained from fluoride analyses of the right incisors were superimposed on micrographs of sections through the left incisors. An abrasive microsampling technique (Weatherell et al., 1985) was used to determine the fluoride concentration and distribution. Fluoride concentration was the highest at the enamel surface and decreased from there to the interior. The fluoride distribution did not level off to an even plateau in the enamel interior, as suggested by earlier studies. Apart from the high concentration in the surface region, the fluoride levels in prenatal and postnatal enamel were rather similar. The fluoride concentration tended to be low at the neonatal line.

Psychological studies of emotional changes in children undergoing dental treatment. 1. Changes in plethysmogram.

This study was designed to examine the influence of children's personality traits and parents' child-rearing attitudes on children's internal behaviors before and after treatment. As an indicator of emotional changes, polygraph data and particularly plethysmograms were compared with psychological analysis data of children's personality traits and parental child-rearing attitudes. The subjects were 40 children with slightly carious primary molars, who were out-patients at Aichi-Gakuin University Dental Hospital. The results show that the emotional changes of children undergoing dental procedures are, in fact, observed at the resting pre-treatment stage, and that they are influenced by various factors, including parental child-rearing attitudes and children's personality traits. More over, in the resting post-treatment stage, these factors impinge upon children's emotions in a more complicated manner.

[Basic investigation using K6 diagnostic system. 3. Fabrication of the apparatus wearing a Sensor Alley and the evaluation of its accuracy].

UNLABELLED: An investigation was undertaken to evaluated the accuracy of Sensor Alley, a mandibular movement analyser: K6 Diagnostic System EX with a newly designed auxiliary apparatus. RESULTS: 1) The errors in the values of the standard Sensor Alley were 4.8 mm for the VER-coordinate, 5.0 mm for the AP-coordinate and 3.0 mm for LA-coordinate. 2) The errors of the AP-coordinate were bigger than those of the VER and LA-coordinates. 3) The errors in the values of the standard Sensor Alley fitted with a newly designed auxiliary apparatus were 0.9 mm for the VER-coordinate, 2.0 mm for the AP-coordinate and 1.1 mm for the LA-coordinate. 4) The results led to the conclusion that this newly designed auxiliary apparatus was useful for various studies on mandibular movement used with the K6 Diagnostic System EX.

[The image on dental environment in mothers. A comparison of handicapped and normal children's mother].

The purpose of the present study was to investigate the image on the dental environment of the mothers of handicapped and normal children. To investigate the image on dental environment, the Semantic Differential Method was applied to mothers of 42 handicapped and 35 normal children. They rated 4 items--'dentist', 'my child', 'dental treatment' and 'dental operating room'--on the adjective-scales 12 pairs. The results of this study were summarized as follows: 1. In comparing the mean of the rating score, it observed that the mothers of handicapped children have a more positive image than the mothers of normal children. This tendency appeared more clearly at 'dental treatment' and 'dental operating room'. 2. By principal factor analysis and varimax rotation, 2 factors were extracted from every 4 items. 3. Factor scores at factor 1 and factor 2 of the mothers of handicapped children were different from those of the mothers of normal children. The mothers of handicapped children had views that a dental treatment is ease and acceptable, and that a dental operating room is comfortable and safe.

[Numerical anomaly of teeth in mongrel juvenile dogs. Canis familiaris].

As this study subjects, upper and lower dentitions of 49 mongrel dogs were used. X-ray films of the upper and lower dentitions of the dogs were taken. These films were from the collection of the Department of Pedodontics, School of Dentistry, Aichi-Gakuin University. Although the data on the sex and age of the dogs were not available, the dental ages were judged to range from the deciduous dentition to the early mixed dentition. Molars were not dealt with in this study for two reasons: 1) the molars of many of the subjects had not yet been calcified, and 2) the upper second and/or lower third molars, if present, had frequently been sacrificed when the jaws were separated. Numerical anomalies were observed in 16 dogs (32.7%). And infranumerary teeth were observed in 14 dogs (26.8%). Incidences of infranumerary teeth were higher in the secondary dentitions than in the primary ones. Most of the infranumerary teeth were found in both the upper and lower premolars, but not in the upper incisors and the upper and lower canines. Infranumerary teeth often occurred symmetrically whereas supernumerary teeth occurred asymmetrically. This investigation suggests that tendency of the reduction of the upper third premolar was considered to be caused by the reduction of the maxilla through the process of domestication. These results are inconsistent with Fujita's terminal reduction theory.

[Survey of what dental students feel regarding their profession (3). The relation with their self concept].

The present survey was undertaken to find out how dental student feel about their profession and what type of the self concept they have. The subjects were 263 students in the 5th and 6th grades of dental school, Aichi-Gakuin University. The pattern of self concept as dental students or dentists was classified into three types, based on a survey of twenty statements test. These types were positive, negative and neutral. The results were as follows: 1. In both the 5th and 6th grades, statements about "present satisfaction as a dental student", in the 5th grade, "progress of university entrance examination", and in the 6th grade, "feelings about the scholastic side of dentistry", were strongly related to their type of self concept. But in both the 5th and 6th grades, there were few related statements about "the motive for entering the dental school" and "occupational choice after graduation". 2. In the result of the multivariate analysis Discriminant Analysis by the Second Class of Hayashi's Quantifying Theory, those students who were graded as "positive" had the following characteristics: (1) Regarding entrance examinations, they took only one examination for dental school when they entered. (2) When choosing dentistry as their occupation, they had not thought of it as a "self supporting occupation" or "occupation of high social standing". (3) Their present interest is learning about dentistry. (4) After their graduation from dental school, they would like to be professional dentists or, a practicing dentists or students of clinical dentistry in the hospital of the dental school.

[Dental treatment and the prognosis for handicapped children].

We analysed a survey made on oral examinations at the first visit, the kind of dental treatment and the prognosis. The subjects were 77 handicapped children who received dental treatment at the pedodontic clinic of the Aichi-Gakuin University Dental Hospital and periodic examinations after the completion of dental treatment. The average age of the subjects was 7.4 (2.11 to 16.7 years old). (1) At the first visit, the mean number of untreated decayed teeth was 7.1 for the group of children who received dental treatment without general anesthesia, and 12.0 for the group of children who received dental treatment with general anesthesia. And the rate of untreated decayed teeth was 40.1% of the deciduous and 21.0% of the permanent teeth for the group of children who had received dental treatment without general anesthesia, and 65.5% of the deciduous and 34.2% of the permanent teeth for the group of children who had received dental treatment with general anesthesia. (2) For the group of children who received dental treatment without general anesthesia, restoration was performed on 62.5% of the deciduous and 87.5% of the permanent teeth; the remaining 37.5% of the deciduous and 12.5% of the permanent teeth were extracted. Endodontic treatment was performed on 20.6% of the deciduous and 8.9% of the permanent teeth. (3) For the group of children who received dental treatment with general anesthesia, restoration was performed on 57.0% of the deciduous and 95.1% of the permanent teeth; the remaining 42.9% of the deciduous and 4.9% of the permanent teeth were extracted. Endodontic treatment was performed on 25.3% of the deciduous teeth and 12.2% of the permanent teeth. (4) In the case of restorative treatment, the highest incidence was in metal crowns on the deciduous teeth, and amalgam fillings in the permanent teeth for the groups of children who received dental treatment with or without general anesthesia. In the case of endodontic treatment, the highest incidence was in pulp amputation for the groups of children who received dental treatment with or without general anesthesia. (5) During the past year of completed dental treatment, unsuccessful cases were 5.4% for the restorative treatment and 0% for the endodontic treatment for the group of children who received dental treatment without general anesthesia; unsuccessful cases were 14.1% for the restorative treatment and 4.1% for the endodontic treatment for the group of children who received dental treatment with general

[The behavior of mothers during dental treatment for their handicapped children. 1. Relationship between mothers' behavior and child patients' factors].

The purpose of this study was to find out factors which determine whether or not mothers of handicapped children chose to be with their children while receiving dental care. The subjects were 46 pairs consisting of a handicapped child and his or her mother who came to the pedodontic clinic of Aichi-Gakuin University Dental School. Such factors of the patients as the various abilities of the patients, their behavior and other items were surveyed with a behavior evaluation. These factors are considered to be related to determining the behavior of the mothers. The results of the survey are summarized as follows: 1. Patients whose mothers chose to be with them tended to be slightly inferior in basic language use and mouth rinsing ability, when compared with patients whose mothers chose to stay in the waiting room. 2. Patients who were autistic more often had mothers who chose to be with them than patients with other disabilities. 3. Patients whose mothers chose to be with them got higher Behavior Evaluation Scores in all dental care situations so that they tended not to adapt to dental care, when compared with patients whose mothers chose to stay in waiting room. 4. From the analyses of Quantification Type II, a height correlation ratio was obtained in the distinction efficiency of the mothers' behavior. Also the behavior of the mothers could be predicted by the factors of the children. 5. There were four factors present in those handicapped children which are considered to have determined their behavior of the mothers: the kind of disability, verbal expression ability, mouth rinsing ability and adaptation to dental care.

[Clinical effects of training in behavior management on handicapped patients in dentistry].

Behavior-therapeutic training was applied to patients who were not cooperative in dental surroundings and could not be managed for dental treatment. The training was continued from the first to the final visit and also at periodical checkups. The clinical effects of training were evaluated. The subjects were 150 handicapped child patients who visited the pedodontic clinic of Aichi-Gakuin University. The patients were classified into two groups according to the degree of cooperation regarding dental treatment during the first visit; 1) the slightly uncooperative group and 2) the uncooperative group. The results were as follows: 1. Training applied prior to actual dental treatment caused 48.7 per cent of the patients to accept routine dental treatment. 2. When N2O/O2 inhalation sedation was added to the training, 79.0 per cent of the slightly uncooperative group were able to accept dental treatment under conseious situation, and 46.0 per cent of the uncooperative group were able to accept it. 3. Continuous training caused 60.7 per cent of all patients to become cooperative. 4. Continuous training was effective with 95.3 per cent of the patients who experienced N2O/O2 sedation, 76.8 per cent of the patients who experienced general anesthesia and 71.2 per cent of the patients who experienced routine dental treatment.

[Behavior of mothers during dental treatment for their handicapped children. 1. Relationship between the mothers' behavior and the image to dental environment and self-consciousness].

The purpose of this study was to find out factors by which mothers of handicapped children chose whether to be with their children while receiving dental treatment or not. The questionnaires from mothers of 42 handicapped children were examined. Factors on the part of the mothers such as their views on the dental environment, their egogram, and the reasons of the mothers' behavior were surveyed. The results of this study were summarised as follows: 1. Both the group of the mothers with their children in the operating room and the group of the mothers not with their children had views of slightly good for a dentist, her child, and a dental operating room. These views were examined by the Semantic Differential Method. 2. Both types of mothers had views of slightly bad for dental treatment. 3. The group of mothers not with their children tended to have a good view of her child and a dental operating room, when compared with the group of mothers with their children. 4. From the analyses of the egogram of the Tokyo university' test, an AC (Adapted Child) was drawed as the factor, in relation to mothers' behavior. 5. The reasons why the mothers chose to be with their children were as follows: the children would have no anxiety, the mothers were apprehensive for their children, the children were not able to speak, and so forth. 6. The reasons why the mothers chose to stay in waiting room were as follows: there will be no reliance on the mothers and child will not be become spoiled, and they should do everything by themselves, children can manage to be treated alone, and so forth.
