Cells isolated from endoscopical biopsy specimens.

For a better understanding of local immunological factors in the pathogenesis of ulcerative colitis (UC), we isolated colonic mucosal lymphocytes (CML) from endoscopical biopsy specimens and studied their immunological characteristics. CML were isolated by a DTT-collagenase-cotton column method. Viability of isolated cells exceeded 90%, and the average yield per biopsy specimen was 1.2 +/- 0.6 X 10(4) (mean +/- SD). A large proportion of the CML isolated in this way were small lymphocytes and the numbers were higher in UC patients than in the controls. The percentage of T cells among the CML was higher in UC than in the controls, but the percentages of B cells did not differ. Peripheral blood lymphocytes from healthy controls showed very low levels of spontaneous synthesis of IgG, IgA, and IgM, but when cultured with PWM, these cells showed a high level of IgG and IgM synthesis but only a moderately elevated IgA production. In contrast, CML from healthy controls had a high level of spontaneous synthesis of IgG, IgA, and IgM; after culture with PWM, IgG synthesis roughly doubled and IgA synthesis increased tenfold compared with that of PBL. IgM synthesis was on the same level as in PBL. Although both PBL and CML cultured with PWM showed similar synthesis of IgG, IgA, and IgM in UC and healthy controls, the spontaneous synthesis of IgA and IgM in CML from patients with UC was approximately double that in the controls. Furthermore, in UC both PBL and CML had decreased Con A-induced suppressor-cell activity compared with controls. A defect of suppressor-cell activity and an increased number of T cells may play a role in the pathogenesis of UC.

Experimental colitis in rabbits.

Experimental colitis was induced in rabbits either by immunizing the antigens which possess the cross-reacting antigenicity with colonic mucosa, and by infusing intravenously their own lymphocytes sensitized with E. coli 014 endotoxin which may contain a high concentration of common antigen (Kunin antigen). The hemorrhagic inflammatory changes were developed in the colon as follows; (1) three of fifteen rabbits immunized with rat colon, (2) one of three rabbits with their own E. coli, (3) six of fifteen rabbits with E. coli 014 and, (4) five of eight rabbits by infusing their own lymphocytes sensitized with E. coli 014 endotoxin. It was suggested that the cross-reacting antigenicity with the colonic mucosa and the sensitized lymphocytes implicate in the pathogenesis of chronic ulcerative colitis.

Development of a new transvenous patent ductus arteriosus occlusion technique using a shape memory polymer.

A novel percutaneous patent ductus arteriosus (PDA) occlusion technique, without the potential problems associated with conventional techniques, has been long awaited. The development of a novel transvenous PDA occlusion technique using a temperature-shape changeable occluder device, and the verification of its in vitro performance, were demonstrated in this study. The principles of this technique are: 1) the bar form device was inserted into the PDA transvenously by a guide wire and a pushing catheter, 2) the device was dashed with hot water through catheters, 3) the device was expanded by hot water in the PDA, and the PDA was occluded. The occluder device, made of a shape memory polymer (polynorbornene), was designed to have the monobloc configuration of a thin disk with a hole for a guide wire in the center and a cone. The barlike device by hot press was fully expanded within 10 sec upon immersion into 45 degrees C water. The mock-circulation test with the "great arteries" and a specially designed "PDA" showed 1) the introduction of hot water (45 degrees C) through the catheters caused expansion of the device which stayed in the "PDA" without any support, 2) the "aortic" pressure and the distal "aortic" flow increased rapidly, and the "pulmonary" flow reduced promptly. This resulted in a drastically reduced shunt ratio at the PDA from 68% to 30%. Thus, the novel PDA occlusion technique developed here functioned well to occlude the PDA in a great arteries model.
