[Long-term surgical results of microprolactinomas].

Sixty three patients with microprolactinoma were studied by long-term follow-up after surgical cure. Postoperative normalization of prolactin (PRL) was seen in 38 (60%) of the total 63 cases, and hyperprolactinemia recurred in 5 (13%) during the follow-up period. Spontaneous restoration of menstruation occurred in 45 (71%), and, in 41 (65%), regular menstrual cycle as continuing at the time of the last follow-up. 34 (72%) of 47 patients who wanted children became pregnant, and 31 of these had a subsequent successful delivery. Clinical cure by surgical treatment alone, followed by spontaneous restoration of menstruation and continuation of regular cycle during the follow-up period after pregnancy and delivery was seen in 41 (65%) of the total cases. From these results, it was concluded that transsphenoidal surgery should be the treatment of first choice for microprolactinoma when complete cure is required.

Immunohistochemical study of the relationship between extracellular matrix and root bifurcation in the mouse molar.

In order to investigate epithelial-mesenchymal interaction during root bifurcation, the distribution of type III and I collagen, fibronectin and laminin in the epithelial-mesenchymal junction between the dental epithelia (epithelial diaphragm and interradicular process) and the cells of the dental papilla (or pre-odontoblasts) was examined, using maxillary first molar tooth germs of CF1 mice from day 1-16 after birth. Three-dimensional reconstructions of the immunofluorescent patterns were made from serial sections of tooth germs from day 3-9, stained with the antibodies against the collagens. The findings were as follows. (1) Type III collagen was first seen in the epithelial-mesenchymal junction at the tip of the interradicular process, where it sprouted from the epithelial diaphragm, and spread along the interradicular process toward its base, accompanied its extension, and then disappeared on completion of root bifurcation. No staining was seen in the epithelial-mesenchymal junction at the epithelial diaphragm during and after root bifurcation. (2) Type I collagen appeared in the epithelial-mesenchymal junction at the base of the interradicular process, where it sprouted from the epithelial diaphragm and spread toward the tip of the interradicular process, following its extension, and increased on completion of the root bifurcation. No staining was seen in the epithelial-mesenchymal junction at the epithelial diaphragm during or after root bifurcation. (3) Fibronectin and laminin remained constant in the epithelial-mesenchymal junction, both at the interradicular process and the epithelial diaphragm, during and after root bifurcation. These findings suggest that type III collagen may play a significant role in the early stage of root bifurcation in the molar.

[An experience of single dose captopril renal scintigraphy in the diagnosis and management of bilateral renovascular hypertension].

A 41-year-old woman visited our hospital with chief complaint of hypertension which was refractory to multiple antihypertensive drugs. Her blood pressure was 200/140 mmHg and her plasma renin activity was as high as 3.1 ng/ml/hr. Angiography revealed bilateral renal artery stenosis. To determine the laterality of the kidney which were responsible for her hypertension, the 99mTc-DTPA renal scintigraphy with captopril was performed. The estimated GFR of the right kidney was lowered than that of the control, while there was no change in the left kidney. Subsequently Percutaneous Transluminal Angioplasty (PTA) was performed to the right kidney. Postoperatively blood pressure remained elevated as well as serum renin level. 99mTc-DTPA scintigraphy with captopril was repeated and revealed no decrease in the GFR of the right kidney this time, but significant reduction in the GFR of the left kidney. After the second PTA to the left kidney, her blood pressure was finally normalized. Postoperatively GFR of both kidneys was not affected by captopril on renal scintigraphy, and currently she has remained normotensive without medication. 99mTc-DTPA scintigraphy with captopril appears to be a useful method to diagnose the laterality and to evaluate in clinical response to therapeutic intervention.

[Difference in the therapeutic efficacy between anaplastic astrocytoma and glioblastoma multiforme types].

We have investigated the differences in therapeutic efficacy between 25 cases with an anaplastic astrocytoma (AA group) and 31 cases with a glioblastoma multiforme (GM group). A good response to induction therapy was achieved with radiation and nitrosourea after a (sub) total removal that led to a prolonged tumor-free interval that was remarkable in the AA group, especially in those with moderate anaplastic astrocytoma. On the other hand, in the GM group, early recurrence tended to occur with no correlation to the degree of the tumor resection, and this made the execution of the maintenance therapy difficult in spite of a similar response to the initial induction therapy as that seen in the AA group.

[Coronary artery disease and the results of coronary bypass surgery in diabetics].

To evaluate the influence of diabetes mellitus on coronary artery disease and the results of coronary bypass surgery, a review was made of 63 consecutive patients undergoing isolated saphenous vein aortocoronary bypass, of whom 38 patients (G-1) were nondiabetic, 9 patients (G-2) had impaired glucose tolerance, and 16 patients (G-3) were diabetic. The severity of coronary artery disease was assessed using an angiographic grading system. Among three patient groups, there was no difference in total coronary score per patient reflecting total extent of disease (G-1; 15.7 +/- 5.5, G-2; 14.1 +/- 5.9, G-3; 17.1 +/- 6.3, mean +/- S.D.) and the incidence of diffusely diseased vessels. The mean number of diseased vessels (greater than 50% stenosis) per patient was 2.5 +/- 0.6 in G-1, 2.6 +/- 0.7 in G-2, and 2.5 +/- 0.7 in G-3, and the mean number of bypass grafts per patient was 2.3 +/- 0.9, 2.2 +/- 0.9, and 2.1 +/- 0.7, respectively. Coronary luminal diameters and the severity of atherosclerotic changes of coronary arteries at the site of graft anastomosis, studied intraoperatively, were similar in all groups. Vein graft blood flows and early graft patency in diabetics (112 +/- 55 ml/min, 96.9%, respectively) were also similarly good as those in the other groups (98 +/- 49 ml/min, 91.4% in G-1, and 92 +/- 39 ml/min, 94.1% in G-2). Overall hospital mortality was 1.6% (one of 63). The only death from meningoencephalitis occurred in the diabetic group.(ABSTRACT TRUNCATED AT 250 WORDS)

[Etoposide delivery to malignant brain tumors by induced hypertension with angiotensin II].

Etoposide delivery to 9L gliosarcoma model in Fischer 344 rats was evaluated. Etoposide 2 mg/body was given intravenously (IV) or intracarotidly with (IHIC) or without (IC) intravenously administrated angiotensin II. Mean etoposide concentration of tumors was 5.08 micrograms/g for IHIC group, 2.27 micrograms/g for IC group and 0.70 micrograms/g for IV group. The difference in etoposide concentration between IHIC group and IV group was statistically significant (p less than 0.05). In addition, etoposide concentrations of the normal brain tissues were lower than concurrent plasma concentrations in all groups. The etoposide concentration was increased in the tumor and very low in the normal brain tissues in Fischer 344 rats with 9L gliosarcoma by induced hypertension with angiotensin II. These studies might suggest that intraarterial chemotherapy by induced hypertension with angiotensin II was useful on treatment of malignant brain tumors.

[Randomized study of initial treatment with radiation.MCNU or radiation.MCNU.interferon-beta for malignant glioma. Hiroshima Brain Tumor Study Group].

The efficacy of radiation.MCNU (MR group) or radiation.MCNU.interferon-beta (IMR group) for malignant glioma was studied by a randomized trial at numerous medical facilities. MR group was irradiated with 50-60 Gy and intravenously injected with 2 mg/kg of MCNU on the initial day of irradiation and 6 weeks later. IMR group was also given intravenous administration of interferon-beta at the dose of 2 x 10(6) IU/m2 for 5 serial-days every eight weeks. There was no difference in background between the two groups. The response rate in MR group and IMR group was 44.4% (4/9) and 30.0% (3/10), respectively, showing no significant difference. The resected tumor volume before the start of these regimens seemed to correlate the response to the treatment in both groups. The major toxicity was myelosuppression, especially using MCNU with interferon-beta. These results indicated that this combined therapy is effective for malignant glioma, and should be executed further trials and follow up study.

Quadricuspid aortic valve--report of a case associated with severe aortic regurgitation and review of the literature.

A case of a 59-year-old woman who had a quadricuspid aortic valve associated with severe aortic regurgitation is reported herein. The anomaly was revealed by aortography, after which the incompetent valve was excised and replaced by a St. Jude Medical prosthesis. A quadricuspid aortic valve must be considered as a malformation which leads to severe valve failure in later life.

Small, superficial esophageal carcinoma treated with high-dose-rate intracavitary irradiation only.

Six patients with small, superficial carcinoma of the esophagus were treated with 18-24 Gy of high-dose-rate intracavitary irradiation only. All patients were examined with an endoscope within 1 month after therapy, and the tumor in all six had disappeared. However, erosion induced by high-dose-rate intracavitary irradiation was seen in five of the patients. Five of the patients have experienced no local recurrence; endoscopic biopsy showed local recurrence in the remaining patient 7 months after radiation therapy. All patients have survived 6-16 months. Esophageal ulceration induced by intracavitary irradiation occurred in three of the six patients; however, the ulcers have been healing with conservative treatment. This new method of using only high-dose-rate intracavitary irradiation appears to be effective in the patient with small, superficial esophageal carcinoma.

The relation between phenytoin-receptor and glucocorticoid in the induction of cleft palate with phenytoin in mice.

Both phenytoin (PHT) and glucocorticoid (GC) have been reported to be an effective teratogen for the production of cleft palate. The authors suggest the possibility that a part of GC-receptor to which PHT competitively bind (common receptor with PHT), participate in the production of cleft palate. In the present study, as one of the studies for the elucidation of the mechanism of cleft palate induction with PHT, we investigated the PHT-receptors and its relation with GC in the lung tissue of mice, which were either sensitive (CF1) or resistant (C57BL/6) to PHT or GC. Scatchard analysis of the specific binding of 14C-PHT to the cytosol protein revealed that the total capacity of PHT-receptor for CF1 (1,571 fmole/mg protein) was much greater than that for C57BL/6 (994 fmole/mg). The value of the Kd's for these strains, however, were 18.1 and 16.5 nM, respectively, indicating the absence of any great difference between both strains. The capacity of the common receptors with GC for CF1 (236 fmole/mg) was greater than that for C57BL/6 (126 fmole/mg). But, the value of the Kd's for these strains were 5.4 and 3.1 nM, respectively, indicating the absence of a great difference between the strains. The analysis indicated that the lung tissue cytosols of both strains also contain the proper receptor for PHT, which is in a much greater amount than that of the common receptor.(ABSTRACT TRUNCATED AT 250 WORDS)

Antitumor effect of recombinant human lymphotoxin on a tumor line of human malignant glioma.

We examined the antitumor effect of recombinant human lymphotoxin (rHuLT) on a xenotransplantable human malignant glioma line. Tumor-bearing nude mice were treated with rHuLT for three weeks following four schedules: intratumoral injection of rHuLT 20,000 units once a week, twice a week, intravenous injection once a week and twice a week. The inhibition rate of tumor growth was 98.8%, 99.1%, 92.1% and 98.8%, respectively. Histologically, necrotic lesions were observed in the tumors of all treated mice. Thrombo-obstructive changes of tumor vessels were also seen in the tumors of mice after intravenous injection of rHuLT. None of the mice died as a result of this treatment in spite of significant body weight loss. These results indicate that rHuLT has a strong antitumor effect on a xenotransplantable human malignant glioma line.

[Coronary artery bypass grafting in a patient with Sjögren's syndrome and other autoimmune diseases].

Coronary arterial involvement in collagen disease is a very rare complication. Particularly in Sjögren's syndrome is there no report of coronary artery bypass grafting (CABG). We report a 68-year-old woman with various autoimmune diseases who underwent successful CABG for unstable angina pectoris. She had a history of Sjögren's syndrome, hypothyroidism, thrombocytopenia and lupoid hepatitis, and had been on the regimens of prednisolone and levothyroxine sodium during the last 4 years. Two months prior to admission when the doses of these drugs were increased, she began to complain of angina on mild exertion and occasionally even at rest. Selective coronary angiograms revealed 75% stenosis at Seg. 2 and 99% stenosis at Seg. 6. Based on these findings and severity of the angina, saphenous vein CABG to the posterior descending and the left anterior descending branches was performed. Despite the associated autoimmune diseases, the patient's postoperative course was uneventful and she is now doing well with no attack of angina, 12 months following the operation.

[PTCA and coronary artery bypass operation prior to subsequent surgery of gastric cancer in patients with coronary artery disease].

Two patients with coronary artery disease required operation for gastric cancer. In one patient with single vessel disease, 37 days later after successful PTCA, gastric resection for early gastric cancer was performed without any cardiac complication. The other patient with severe three vessel disease initially underwent coronary artery bypass operation, and 13 days later gastric resection for advanced cancer was performed. This patient developed supraventricular arrhythmia as a sole complication which was easily controlled by medical treatment. In both cases, fentanyl was used as an anesthetic agent and nitroglycerin was continuously administered intravenously during the perioperative period. ECG, central venous pressure, and direct arterial pressure were also carefully monitored. We feel that noncardiac operation such as gastric resection can be performed without major cardiac complications even shortly after coronary artery bypass operation.

[Production of monoclonal antibodies against tooth germs of hamster or rat by in vitro immunization].

The in vitro immunization procedure for the production of monoclonal antibodies has several advantages over the in vivo procedure; e.g. it requires a much smaller amount of immunogen and only a few days are required for immunization. However, no in vitro immunization procedure for the production of monoclonal antibodies against tooth germs has been reported. By means of in vitro immunization, we tried to produce monoclonal antibodies against the components of tooth germs which would be useful for immunohistochemical investigation of the development of tooth germs. Spleen cells of mice were immunized with a homogenate of molar tooth germs from hamsters or rats. The hybridomas produced were screened by immunohistochemical examination of paraffin sections of tooth germs. We obtained five monoclonal antibodies reacting with the dental tissues; ameloblasts, odontoblasts, enamel, dentin, predentin, and dental sac. One of them reacted specifically with only ameloblasts and one with ameloblasts and enamel, but the others are not specific for individual dental tissues. The results demonstrate that in vitro immunization is a satisfactory method for producing monoclonal antibodies useful for immunohistochemical investigation of tooth germs.

[Surgical treatment of coarctation of the aorta and interrupted aortic arch].

From December, 1984, through April, 1987, ten infants with coarctation of the aorta and six with interrupted aortic arch underwent staged repair of aorta and other cardiac lesions. Simultaneous pulmonary artery banding was performed in six of 8 patients with ventricular septal defect (VSD) and in all of seven patients with complex cardiac lesions. With first operation, there were no operative deaths and two late deaths. Eight of 14 survivors underwent total correction of associated lesions at three to 17 months after initial operation. VSD was closed in five patients with one operative death. One patient required pulmonary artery debanding alone because of decrease of VSD size. The Damus-Kaye-Stansel operation was performed successfully in one patient with Taussig-Bing anomaly and the Jatene operation was done in one with transposition of the great arteries. Based on these results, we prefer staged repair with pulmonary artery banding for coarctation or interruption of the aorta associated with complex cardiac lesions.
