Effect of alpha 1-adrenoceptor blockade on the left ventricular force-length relationship in dogs.

We used the left ventricular (LV) end-systolic force-diameter (Fes-Des) relation to evaluate the effect of an alpha1-adrenoceptor antagonist (bunazosin hydrochloride) on the contractility of the beta-blocked left ventricle. Nine adult mongrel dogs were instrumented with ultrasonic crystals to measure LV diameter and a micromanometer to measure LV pressure. Beta-adrenergic and vagal blockade was induced with intravenous propranolol (2 mg/kg) and atropine (0.2 mg/kg), respectively, and preload was decreased by inferior vena caval occlusion. The slope (Ec) and extrapolated diameter intercept (Do) of the LV Fes-Des relation were derived from end-systolic data obtained in the control state (after beta-blockade) and after bunazosin infusion (1 mg/kg). Ec was used as a new index of LV contractility. After bunazosin infusion, the heart rate and Ec were decreased by 7 and 22%, respectively, in comparison with the control state, whereas Do did not change. These results indicate that alpha1-adrenoceptor blockade significantly reduces myocardial contractility in the beta-blocked canine heart, perhaps by decreasing the intracellular calcium concentration and/or myosin ATPase activity.

A statistical study on clinical findings of solar keratosis.

Clinical findings were studied in 50 patients with 53 lesions of solar keratosis encountered during the 15 year period from 1977 to 1991. The majority of these cases were evident in the over-sixty age group (average 62.2 years). A greater proportion were females. Most lesions were observed on the face; especially on the cheek, and from the outer eyelid to the temple. The erythematous type (desquamative-keratotic) lesion was the most evident. With the exceptions of the years 1979 and 1991, no remarkable increase in the number of patients was noted during the 15 year period.

A statistical study on histopathologic findings of solar keratosis.

We examined histopathologic data from 50 patients with 53 lesions of solar keratosis encountered over the past 15 years (1977 to 1991). The epidermis showed acanthosis, uneven epidermal dyeing properties, disorderly arrangement of the basal cells, atypia of the nucleus, mitotic figures and hyperpigmentation of the basal layers. The dermis showed actinic elastosis, cell infiltration mainly consisting of the lymphoid cells, and incontinentia pigmenti histologica. Among histologic types, the hypertrophic type was the primary example.

[Perioperative management of a patient with transient polyuria].

Perioperative and anesthetic management of a patient with a diabetes insipidus is reported. A 33 year old man was followed by physical therapy after spinal cord injury. At that time polyuria (4300-8600 ml.day-1) and polydipsia developed. His urine output was controlled by DDAVP (desmopressin) preoperatively. As the result of water restriction test and Carter-Robbins test the diagnosis of complete central diabetes insipidus was doubted preoperatively. We investigated the changes of his perioperative body fluids and endocrine responses. The following conclusions are made: 1) The diagnosis of this case is not renal and true diabetes insipidus, but is probably partial or transient diabetes insipidus. 2) These results indicate that endocrine examinations related to AVP secretion are essential.

[Methicillin-resistant Staphylococcus aureus (MRSA) isolated in hospitals in Fukuoka City area].

Bacteriological and clinical studies were carried out on 280 strains of Staphylococcus aureus isolated in hospitals in Fukuoka city area from September 1990 to March 1991. Of all S. aureus strains studied 116 (41.4%) were methicillin-resistant. The proportion of MRSA in S. aureus isolates from outpatients was 10% (11/109), 69.2% (90/130) in those from inpatients. The average age of patients with isolated MRSA was 70.5 +/- 16.9 years and that of patients with isolated MSSA, 44.2 +/- 29.3. MRSA strains were recovered mainly from sputum and pus. The isolation rate did not vary significantly with hospitals of different size (number of beds). Of all MRSA strains 48 (41.3%) produced coagulase type VII. As for drug susceptibility, MRSA strains with coagulase type VII were more sensitive to clindamycin and more resistant to minocyclin compared to MRSA with other coagulase types.

[Effect of nafamostat mesilate on the renin-aldosterone system].

It has been reported that continuous intravenous infusion of nafamostat mesilate (FUT) produces hyperkalemia due to reduced urinary excretion of potassium. The present study was performed to see whether renin-aldosterone effect is involved in this inhibition of potassium excretion. Ten patients were studied who had been given this drug (4 mg.hr-1) to prevent postsurgical pancreatitis or DIC. Urine potassium output decreased significantly from 44 +/- 5 microEq.min-1 prior to administration of FUT to 18 +/- 4 microEq.min-1 in 3 hours, sodium/potassium ratio increased significantly from 1.7 +/- 0.7 prior to administration of FUT to 5.4 +/- 3.3 in 5 hours; and plasma aldosterone decreased significantly from 92 +/- 24 pg.ml-1 prior to administration of FUT to 63 +/- 22 pg.ml-1 in 6 hours. The results suggest that hyposecretion of aldosterone may be one of the main causes of hypokalemia. Reduced secretion of aldosterone may be due to other factors than the suppression of renin-angiotensin system.

Hemodynamics in the prone jackknife position during surgery.

We examined the hemodynamic changes occurring with prone jackknife positioning during colorectal surgery. The operative procedure was restorative proctocolectomy with ileal J-pouch anal anastomosis in five patients with adenomatosis coli and six patients with ulcerative colitis and anoabdominal resection of the rectum with colonic J-pouch anal anastomosis in eight patients with rectal cancer. Nineteen patients (10 men and 9 women aged 41 +/- 19 years) were monitored with arterial and Swan-Ganz catheters during positioning. Measurements were obtained in the supine and prone positions (1 minute, 3 minutes), and the jackknife position (1, 3, 5, and 10 minutes), as well as before and after adoption of the Lloyd-Davies position (1, 3, 5, and 10 minutes). Turning the patient from the supine position to the prone position resulted in a significant decrease in the cardiac index (CI). However, following head-down rotation, the CI increased and returned to the value seen in the supine position (p less than 0.05). Heart rate (HR) slowed and mean arterial pressure (MAP) increased in the prone jackknife position. We concluded that the extent of the changes in cardiac function presented no serious problems.

[Differences in ventricle dilation and clinical findings between the early- and late-onset subtypes of Alzheimer's dementia].

The authors compared the characteristics of the early-onset subtype of Alzheimer's dementia (AD) with those of the late-onset subtype (SDAT) to determine whether Alzheimer's dementia (AD/SDAT) can be divided into two different diagnostic categories. In this study, AD refers to Alzheimer's in which the onset of disease occurred before 65 years of age; and SDAT refers to cases in which onset of illness occurred at age 65 or more. The studied parameters were distribution of age at disease onset, progression of speed of ventricular dilation, speed of deterioration of Hasegawa's rating scale for dementia (HRSD) score, and lesional pattern on the Japanese revised version of the GBS scale (GBSS-JR). The distribution of age at onset in 258 patients with AD/SDAT showed one peak. These patients included both outpatients and inpatients. A 30-month follow-up study to determine the degree of ventricular dilation was performed in 20 AD and 24 SDAT patients. For CT measurement, the width of the third and lateral ventricle (cella media index) was measured every 6 months after admission. Average ventricle width at admission was matched between the two groups. Twelve and 24 months after admission, the mean cella media index of patients with AD was significantly wider than that of patients with SDAT. In addition a deterioration speed of HRSD scores of 16 AD and 14 SDAT patients was compared every month for 5 months after admission. The average duration of illness and HRSD score at admission were matched in the two groups.(ABSTRACT TRUNCATED AT 250 WORDS)

Histologic findings on the condylar process of the mandible in rats with reduced bone-mineral density.

We studied the histology of the condylar process in rats on a low calcium diet. Female rats aged 15 weeks were given a low Ca diet for 14 weeks, which reduced bone-mineral density, i.e., induced osteoporosis. Histologic observation revealed tylosis of the fibrous layer of the condyle, shrinkage of the cartilaginous layer, enlargement of the marrow cavity, reduction in the number of osteoblasts in the condylar neck, and cellular disarrangement and other morphologic changes of both the fibrocartilaginous and cartilaginous layers.

An experimental study on mandibular movement and osteoporosis.

In order to clarify the relationship between mandibular movement and osteoporosis, the authors secured the temporo-mandibular joints of 8-week-old rats to immobilize their jaws; then conducted histologic studies chronologically, up to the fifth week. After three weeks, an expansion of the marrow cavity and a reduction of the trabecular were observed in the mandibular process and mandibular fossa. During the fifth week, an osteoporosis-like state was clearly observed. Further, an irregularity in the form of mandibular process was recognized, as well as a remarkable disturbance caused by cartilaginous ossification of the articular cartilage of the condyle.

pH-dependent interaction of amphiphilic polypeptide poly(Lys-Aib-Leu-Aib) with lipid bilayer membrane.

A sequential polypeptide, poly(Lys-Aib-Leu-Aib) (Aib represents 2-aminoisobutyric acid), was synthesized, and the interaction with lipid membrane was studied. Poly(Lys-Aib-Leu-Aib) was designed to take an amphiphilic structure upon the formation of alpha-helix. Circular dichroism of poly(Lys-Aib-Leu-Aib) in an aqueous solution showed a negative Cotton effect due to alpha-helix. The content of alpha-helix increased when the pH was raised above 7.5 or in the presence of small unilamellar vesicles composed of egg yolk lecithin. On the other hand, alpha-helical conformation was broken by increasing the ionic strength of solution. Carboxyfluorescein leakage from dipalmitoylphosphatidylcholine (DPPC) vesicles induced by binding of poly(Lys-Aib-Leu-Aib) to the lipid membrane was facilitated in an alkaline solution and/or in a solution of low ionic strength. These phenomena can be related to the alpha-helix content of the polypeptide. It was shown that poly(Lys-Aib-Leu-Aib) induced fusion of DPPC vesicles in an alkaline solution below the phase-transition temperature of the membrane. It was further shown that the aggregation and fusion of the neutral lipid membrane was regulated by changing the pH of solution.

Existence and unique N-terminal sequence of alpha II (omega) interferon in natural leukocyte interferon preparation.

Human alpha interferon produced in Sendai virus-stimulated leukocytes was purified by immunosorbent affinity chromatography and subjected to subtype analysis. All subtypes in leukocyte culture supernatant were confirmed in purified preparation. One of these subtypes was reactive in Western blotting with antibody specific to alpha II (omega). N-terminal amino acid sequence analysis of this particular subtype showed addition of two amino acids to the N-terminus of alpha II predicted through cDNA studies. The apparent cleavage site for leader sequence of alpha II was different from the site common to all other alpha subtypes. This is the first report on the isolation of natural alpha II and its unique N-terminal amino acid sequence.

Elemental analysis of hair among hydrofluoric acid exposed workers.

The concentrations of fluorine (F), sodium (Na), magnesium (Mg), aluminum (Al), potassium (K), phosphorus (P), calcium (Ca), iron (Fe), nickel (Ni), zinc (Zn) and iodine (I) in the hair of 142 hydrofluoric acid (HF) exposed workers and 273 control subjects were analyzed by wavelength dispersive X-ray fluorescence spectrometry. Postshift urine and serum specimens were also collected for the determination of fluoride. To determine whether external contamination influences hair analysis, the control hair samples were kept in the work environment for one week. With the exception of lower K and Fe values, the levels of F and other elements in the hair of HF workers were higher than in the control subjects. The concentrations of F in hair and in postshift urine were in good correlation (r = 0.65). In the hair of HF workers F and Ca, F and Mg, F and P, F and Al were well correlated with each other (r greater than 0.6). F concentration in the hair increased after the sample was retained, however, it decreased to the reference value after washing. The levels of Ca, Mg and other elements did not change during the experiment.
